RESUMO

The rugate filter employs a sinusoidal refractive index depth profile to produce high reflection in a narrow band of wavelengths. Fabrication relies on a continuously variable index of refraction in the wavelength regime of interest. The near IR refractive index of amorphous silicon-nitrogen films decreases continuously as the composition varies from pure silicon to stoichiometric silicon nitride (Si(3)N(4)). Ion implantation was found unsuitable as a fabrication method for rugate filters. Homogeneous and inhomogeneous films up to 5 microm in thickness have been produced by simultaneous deposition of electron beam evaporated silicon and of energetic nitrogen particles arising from an ion beam. The relative fluxes of beam and evaporant are found to determine the ratio of nitrogen to silicon in the films and therefore to determine the index. Single-band reflection filters of the rugate design of high peak optical density were fabricated under computer control using a quartz crystal oscillator shielded from the beam to monitor the silicon evaporation and three suppressed Faraday cups to monitor the ion beam current.

RESUMO

The care of forty-four children with chronic arthropathies (usually juvenile rheumatoid arthritis) was studied by parental interview. The goal was to determine parents' perceptions of how management responsibilities are shared between primary physicians and specialists. Responses to questions about each of nine specific areas of care, ranging from diagnosis and treatment of the chronic disorder to the care of minor, acute illnesses, enabled the investigators to determine which physician had assumed major responsibility for each area. The results suggest a pattern whereby basic care is either divided or duplicated, but with many of the supportive aspects of care neglected in a high proportion of families. A comparison of these results with those of a similar study of children with meningomyelocele provides support for the view that such patterns are typical of the care of most children with chronic disorders.

RESUMO

Parents, for many reasons, are not always able to understand the nature of their child's handicap. Many physicians recognize that they are not always as effective as they might wish to be in enabling parents to comprehend handicaps. Written hand-out materials as aids to better understanding can be helpful to both physicians and families as supplements to verbal descriptions of the disease and instructions for care. This compilation of currently available literature, grouped by category of disease and with addresses where they may be ordered, should offer the pediatrician and other health personnel a ready source of assistance.

RESUMO

Patterns of care provided by primary physicians for children with chronic physician illnesses are examined. Pratices of pediatricians are compared with those of general practitioners in a ten-county region in Upstate New York. Data collected from 82 physicians surveyed in nine rural counties and one urban area indicate that, on the average, 7.4% of all children seen annually have one or more chronic conditions. Most such children are referred to subspecialists for part or all of their care; the frequency of referral, however, depends chiefly on the nature of the disorder. Primary responsiblity for the broader aspects of care is more often assumed by rural than urban physicians and more often by general practitioners than pediatricians. Nevertheless, mental, social, and some technical services are generally underutilized by groups of primary physicians.
