RESUMO

This study presents a quantitative approach to support policy decision making for the preservation of riverscapes, taking into account the EC Water Framework Directive (2000/60/EC) and the EC Nitrates Directive (91/676/EEC) concerning the protection of waters against nitrate pollution from agricultural sources. A choice experiment was applied to evaluate the benefits, as perceived by inhabitants, of the implementation of policies aiming to reduce the concentration of nitrates in groundwater, preserve the riverscape by maintaining a minimum water flow and increasing hedges and woods along the Serio River in central northern Italy. Findings suggested that people were particularly concerned about groundwater quality, probably because it is strongly linked to human health. Nevertheless, it was interesting to observe that people expressed a high willingness to pay for actions that affect the riverscape as a whole (such as the minimum water flow maintenance plus reforestation). This is probably due to the close connection between the riverscape and the functions of the river area for recreation, health purposes, and biodiversity preservation.

The introduction of new products on the market poses several challenges; in particular, whether the characteristics of the proposed product will be judged positively by potential consumers. This paper analyses the preferences of consumers regarding the introduction on the Italian market of a new product: organic Mediterranean sea bass. The aim of this study is to assess the importance given by consumers to four main characteristics of sea bass (country of origin, size, production method - organic or conventional - and price) so as to be able to formulate marketing strategies. We applied a choice experiment (CE) in order to define not only the ordinal ranking of preferences but also the willingness to pay (WTP) for the key characteristics of the newly-introduced product. We found that consumers show a higher WTP for the sea bass country of origin than for the breeding method used. Our results suggest that while organic aquaculture might be a new and important strategy for diversification, if suitable communication, either from a public policy or commercial perspective, and labelling/certification are not taken into consideration, the added value of the production method might not be perceived by the final consumers.

OBJECTIVES: Data of Literature suggest that age is not an independent risk factor for adverse drug reactions (ADR), while there is evidence of a positive correlation between ADR and the number of drug taken. To investigate if that is true, we have examined the occurrence of ADR in elderly patients admitted to our Geriatric Department in the first nine months of 1999 for acute illness. MATERIALS AND METHODS: We have evaluated the occurrence of ADR in 510 patients more than 65 years of age (80 +/- 7.35 years). The adverse drug effect was worth considering when symptomatical or caused a change in the management of the patient (suspension or change of therapy, prolongation of staying in hospital). We used the algorithm of Naranjo (19) to estimate the probability that a drug caused an ADR. RESULTS AND CONCLUSIONS: In our study the frequency of ADR was 12.5%. Furosemide was responsible for 50% and digoxin for 8% of ADR. Among the patients taking furosemide, 12.5% had an ADR (generally low potassium). The frequency of ADR was correlated neither with the age of the subjects nor with the number of drug taken. It can be possible that the main cause of ADR is the type of drugs assumed by elderly.

RESUMO

The best chance for a patient with a bleeding gastroduodenal ulcer is: "a diligent doctor and a reluctant surgeon". It is necessary to evaluate clinical and endoscopic elements and to check haemodynamic and general conditions of the patient. A better prognosis is ensured by prompt initiation of therapy. All patients with a bleeding since 12 hours or rebleeding must be selected for urgent surgery. The success of surgical intervention is the result of an exact indication and the most correct surgical procedure. Selection of patients for surgery must ensure careful local haemostasis and control of hyperchlorhydria.

Using survival corrected for intercurrent deaths as an endpoint, the prognostic significance of age, sex, histology, clinical extent of disease, size of primary tumor, type and result of treatment, was assessed in a series of 133 patients with differentiated thyroid carcinomas treated over a 16-year period. The following parameters were shown to be unfavourably related to survival: old age, follicular moderately-differentiated histological type, extrathyroid growth of the primary tumor and/or presence of distant metastases at diagnosis. In patients not cured after surgery, postoperative radioiodine therapy was correlated with, better survival rates. A multivariate statistical analysis (Cox model) showed that tumor stage and age at diagnosis were the major determinants of prognosis. Based on this data a risk index was worked out and three low/intermediate/high risk subgroups, characterized by significantly different survival rates, were identified in the study population.
