RESUMO

Heterotopic pregnancies (simultaneous ectopic and intrauterine pregnancies) can occur spontaneously or associated with assisted reproductive technology. It is infrequent in Mexico. We present a case of heterotopic pregnancy in a 32 year old woman submitted to surgery due to rupture of the ectopic pregnancy located in the right fallopian tube. The evolution was satisfactory for the woman, but the newborn died shortly after delivery because of meconium aspiration. We review the literature on this disease.

RESUMO

Six cases of biliary ascariasis were treated at Dr. Manuel Gea González General Hospital in the last sixteen years. The patient population consisted of five female and one male subjects. With a median age of 39 years. Main clinical manifestations were abdominal pain mimicking biliary colic, nausea, vomiting and jaundice. Diagnosis was established during surgery in four patients and by means of abdominal sonogram and endoscopic retrograde cholangiopancreatogram in two cases. Cholecystitis diagnosis was considered preoperatively in three patients and cholangitis in another one. Five patients were operated on and in one case the worm was successfully removed with the endoscope. Only one patient was complicated with biliary peritonitis, hepatic abscess and reinfestation. No deaths occurred. Mexican literature published in the lasts 25 years is reviewed and the current state in diagnosis and treatment concerning this entity is discussed in this paper.

RESUMO

Fournier gangrene is a disease which primarily affects adults. This finding makes it rarely seen within the pediatric population. Therefore, we feel it convenient to inform of a case of Fournier gangrene which occurred in an eight day old neonate, successfully treated with surgical debridement, the administering of solutions and antibiotics. This is the youngest patient ever described in the medical literature. Neonate; Fournier; gangrene.

RESUMO

Four cases of familial polyposis coli (FPC), seen at Dr. Manuel Gea González General Hospital from 1985 to 1987, are presented. Three females and one male, with ages ranging between 23 and 52 years, are presented. The clinical picture included bloody feces, polyp prolapse, and a maxillary osteoma. In 3 cases the correct preoperative diagnosis was made by rectosigmoidoscopy, biopsy and barium enema. In one patient, urgently operated on, the diagnosis was postoperatively made by colonoscopy. The adenocarcinoma of the colon appeared associated with FPC in three cases (one of the mucinous variety). One total coloproctectomy and two abdominal colectomies with ileorectal anastomosis were performed. One patient operated on because of bowel obstruction was treated by right hemicolectomy. This case developed abdominal carcinomatosis afterwards. One woman died; another one was lost during follow-up, and the other one had no complications. The association of FPC and mucinous adenocarcinoma of the colon is reported for the first time.

RESUMO

This is a case report of an ileosigmoid volvulus in a 68 year old man. The patient had emergency surgery for an acute abdominal condition. Intraoperative diagnosis of an ileosigmoid knot was made and it was necessary to resect the necrotic portion of the ilium, with ilioiliostomy, and to resect the gangrenous section of the sigmoid colon, end colostomy, and Hartmann pouch. Eleven weeks later, a colosigmoidostomy was realized. The evolution of the patient was satisfactory. The literature about ileosigmoid volvulus is reviewed.

RESUMO

A case of hepatic rupture in 32 years old woman with severe preeclampsia is presented. She was definitively treated by right hepatic arterial occlusion and right porta branch occlusion. There were no postoperative complications, and the patient is still alive. This is the first case report of right hepatic arterial occlusion and right porta branch in severe preeclampsia.
