RESUMO

Time-varying metasurfaces are emerging as a powerful instrument for the dynamical control of the electromagnetic properties of a propagating wave. Here we demonstrate an efficient time-varying metasurface based on plasmonic nano-antennas strongly coupled to an epsilon-near-zero (ENZ) deeply subwavelength film. The plasmonic resonance of the metal resonators strongly interacts with the optical ENZ modes, providing a Rabi level spitting of ∼30%. Optical pumping at frequency ω induces a nonlinear polarization oscillating at 2ω responsible for an efficient generation of a phase conjugate and a negative refracted beam with a conversion efficiency that is more than 4 orders of magnitude greater compared to the bare ENZ film. The introduction of a strongly coupled plasmonic system therefore provides a simple and effective route towards the implementation of ENZ physics at the nanoscale.

Analyzing the autocorrelation function of the fluorescence intensity, we demonstrate that these nanoemitters are characterized by a short value of the mean duration of bright periods (ten to a few hundreds of microseconds). The comparison of the results obtained for samples with different geometries shows that not only the shell thickness is crucial but also the shape of the dot-in-rods. Increasing the shell aspect ratio results in shorter bright periods suggesting that surface traps impact the stability of the fluorescence intensity.

Quantum field theory predicts that a spatially homogeneous but temporally varying medium will excite photon pairs out of the vacuum state. However, this important theoretical prediction lacks experimental verification due to the difficulty in attaining the required nonadiabatic and large amplitude changes in the medium. Recent work has shown that in epsilon-near-zero (ENZ) materials it is possible to optically induce changes of the refractive index of the order of unity, in femtosecond time scales. By studying the quantum field theory of a spatially homogeneous, time-varying ENZ medium, we theoretically predict photon-pair production that is up to several orders of magnitude larger than in non-ENZ time-varying materials. We also find that while in standard materials the emission spectrum depends on the time scale of the perturbation, in ENZ materials the emission is always peaked at the ENZ wavelength. These studies pave the way to technologically feasible observation of photon-pair emission from a time-varying background with implications for quantum field theories beyond condensed matter systems and with potential applications as a new source of entangled light.

The FORWARD study is a randomised, double-blind trial that compares the efficacy and safety of 48 weeks treatment with extrafine beclomethasone dipropionate/formoterol fumarate (BDP/FOR), 100/6 µg pMDI, 2 inhalations BID, vs. FOR 12 µg pMDI, 1 inhalation BID, in severe COPD patients with a history of exacerbations. Co-primary endpoints were exacerbation rate over 48 weeks and pre-dose morning FEV(1) at 12 weeks. The ITT population included 1186 patients (69% males, mean age 64 years) with severe airflow limitation (mean post-bronchodilator FEV(1) 42% predicted). Salbutamol as rescue therapy, theophylline and tiotropium (if stable regimen prior to screening) were allowed. Compared to FOR, BDP/FOR: (1) reduced the exacerbation rate (rate ratio: 0.72 [95% confidence interval 0.62-0.84], p < 0.001); (2) improved pre-dose morning FEV(1) (mean difference: 0.069 L [0.043-0.095] p < 0.001); (3) prolonged the time to first exacerbation; (4) improved the SGRQ total score. The percentage of patients with adverse events was similar (52.1% with BDP/FOR and 49.2% with FOR). Pneumonia incidence was low, slightly higher with BDP/FOR (3.8%) than with FOR (1.8%). No difference for laboratory values, ECG or vital signs. Extrafine BDP/FOR significantly reduces the exacerbation rate and improves lung function of patients with severe COPD and history of exacerbations as compared to FOR alone.

Images of semiconductor "dot-in-rods" and their small clusters are studied by measuring the second-order correlation function with a spatially resolving intensified CCD camera. This measurement allows one to distinguish between a single dot and a cluster and, to a certain extent, to estimate the number of dots in a cluster. A more advanced measurement is proposed, based on higher-order correlations, enabling more accurate determination of the number of dots in a small cluster. Nonclassical features of the light emitted by such a cluster are analyzed.

We studied 177 patients with pneumonia admitted to an internal medicine department over a period of 3 years to determine the incidence of two emerging pathogens, Legionella pneumophila and Mycoplasma pneumoniae. Clinical, radiological and laboratory tests were performed and included blood cultures, serology, gram staining and sputum cultures. L. pneumophila was the agent involved in 9 patients (5.1%) and M. pneumoniae in 12 (6.8%). These prevalences were about in the middle of the range of previously published figures. Legionella pneumonia is a rare illness, which even in the absence of suggestive clinical signs must be considered because of its possibly serious course and to allow appropriate therapeutic decisions to be made.

We studied soluble immune complexes in sera and synovial fluids of ten patients suffering from Rheumatoid Arthritis (RA), six seropositive and four seronegative. Isolation of the immune complexes was obtained by a combination of gel filtration and affinity chromatography from biological fluids. The first step involved preparation of a globulin-enriched fraction by precipitation of either serum or synovial fluid with 5% polyethylene glycol 6000. The precipitate was solubilized in Borate NaCl buffer and fractioned by gel filtration in Ultrogel AcA 34. Eluted peaks were submitted to affinity chromatography on a Protein A-Sepharose CL 4B column. The recovered material was analyzed by double immunodiffusion and electrophoresis in 6-20% polyacrylamide gel containing sodium dodecylsulphate (SDS-PAGE) and was shown to contain IgG, IgM, IgA with small amounts of C1q, C3 and C4. Observation occurred that there is a different distribution of macromolecular and intermediate complexes in seropositive and seronegative samples respectively, total C1q binding activity being equal.
