RESUMO

PURPOSE: To analyze the results of round 1 of the population-based Valencia Breast Cancer Screening Program. MATERIALS AND METHODS: In this program, 78,224 (72.98%) of the 107,178 women invited (aged 45-65 years) underwent screening. Complementary views were obtained in 5,771 women (7.38%). Among the total population studied, 3,502 (4.48%) underwent short-term mammographic follow-up studies; 3,898 (4.98%) underwent additional studies and treatment at hospitals. Five hundred eighty-seven women (0.75%) underwent biopsy. RESULTS: Cancer was detected in 334 patients (4.27 cancers per 1,000 women [3.24 per 1,000 women aged 45-49 years, 6.30 per 1,000 women aged 60-65 years]; six patients with lobular carcinoma in situ excluded). The estimated sensitivity was 89%; specificity, 99%. The positive predictive value of mammography was 8.56%; of mammography with additional examinations, 26.82%; and of biopsy, 56.89%. Forty-one patients (12.28%) had ductal carcinoma in situ; 284 (85.03%) had infiltrating carcinoma. In 73 (25.70%) of the 284 patients, infiltrating carcinomas were smaller than 1 cm. Two hundred twenty-five patients (76.27%) had no lymph node involvement. One hundred seventy-nine (61.09%) had stage 0 or 1 cancer. CONCLUSION: Results are consistent with other published results; differences are due to methods and patient population characteristics.

To characterize the relationship between findings on magnetic resonance (MR) images and histologic changes in chronic liver disease, a prospective study was performed in 100 patients with chronic hepatitis and cirrhosis and 28 healthy subjects. Biopsy specimens, obtained in all patients before MR imaging, were evaluated with the histologic activity (HA) index; MR images were obtained with short inversion time inversion-recovery (STIR) and spin-echo sequences. On STIR images, normal livers were iso-intense to fat. Significant differences (P < .001) existed between signal intensity of normal livers and that of diseased livers, which were brighter than normal livers on STIR images. The ratio of signal intensity of liver to that of fat on STIR images was associated with an HA index grouped by severity (P < .05): Patients with higher HA scores had a brighter liver. Signal intensity ratios on MR images were statistically significantly associated with periportal and lobular necrosis and portal inflammation. The signal intensity of liver on STIR images is associated with the degree of histologic severity in patients with chronic liver disease.

We have correlated the angiographic and pathological findings in renal and peri-renal inflammatory disease in acute, sub-acute and chronic phases. In all stages of renal infection, the renal vasculature is attenuated. This corresponds in acute inflammation to vasospasm, acute vascular necrosis and post glomerular peritubular capillary congestion. In sub-acute inflammation, the reduction in vascularity is due to peri-vascular fibrosis and intimal and medial hyperplasia. In chronic infections, the renal vessels are completely destroyed and replaced by fibrotic tissue. In contrast to this, peri-renal inflammatory disease leads to vaso-dilatation and enlargement of the renal capsular arterial complex, and of retroperitoneal arteries. If both renal and peri-renal inflammation occurs simultaneously, a discordance between the renal cortical and capsular arterial filling results: this striking phenomenon is highly suggestive of inflammation, and extremely rare in tumours. Three types of inflammatory vascularity are described, two of which can be easily distinguished from tumour vascularity. An inflammatory blush, slow emptying of vessels and a mottled nephrogram with loss of cortical definition are highly suggestive signs of renal inflammation. It is concluded that angiography is a sensitive and accurate method for the early diagnoses of renal and peri-renal flammatory conditions.
