There is a growing demand for engineered bone tissues custom-designed to match the patient-specific defect size and in vitro models for studying bone diseases and/or drug screening. Herein, we propose a bioprinted bone tissue construct using SaOs-2 cells within alginate/gellan gum/hydroxyapatite inks. Different ink formulations were developed with varying hydroxyapatite content and then evaluated for viscoelasticity, printability, biomineralization properties, post-printing viability, proliferation, metabolic activity, and osteogenic phenotype of SaOs-2-encapsulated cells. Results indicate that ink formulations exhibit non-Newtonian shear-thinning behaviour, maintaining shape integrity and structural stability post-printing. Ink mineralization rates increase with the hydroxyapatite content, rendering them suitable for bone defect strategies. Post-printed cells in the developed constructs remain live, spreading, and metabolically active but do not proliferate. Osteogenic gene and protein expression, both early and late, show upregulation at day 7 relative to day 1, followed by downregulation at day 14. Lower hydroxyapatite content inks demonstrate up to fourfold upregulation in genes and proteins at most time points. Additionally, these constructs release calcium and phosphate at levels conducive to mineralization. Overall, the tissue-engineered miniaturized constructs not only meet the criteria for early-stage bone defect/fracture regeneration but also serve as a promising platform for drug screening and evaluating potential therapeutic treatments.

Screen printing is one of the most used techniques for developing printed electronics. It stands out for its simplicity, scalability, and effectivity. Specifically, the manufacturing of hybrid integrated circuits has promoted the development of the technique, and the photovoltaic industry has enhanced the printing process by developing high-performance metallization pastes and high-end screens. In recent years, fine lines of 50 µm or smaller are about to be adopted in mass production, and screen printing has to compete with digital printing techniques such as inkjet printing, which can reach narrower lines. In this sense, this work is focused on testing the printing resolution of a high-performance stainless-steel screen with commercial conductive inks and functional lab-made inks based on reduced graphene oxide using an interdigitated structure. We achieved electrically conductive functional patterns with a minimum printing resolution of 40 µm for all inks.

BACKGROUND: Healthy literacy is a determinant key children/teenager's health and health outcomes. The aim of this study to identify the parents' assessment about Health Education practice to children and teenagers. METHODS: We opted for a descriptive, quantitative and cross-sectional research, with a non-probabilistic convenience sample. The inclusion criteria were: being a parent who uses attending children health appointments in primary health care; being a parent who has a child hospitalized and is accompanying him/her in the pediatric hospital inpatient ward. A questionnaire survey was built with three sections: sample characterization, Health Education practices performed by nurses (5 questions) and a scale that measured Health Education Assessment Scale (HEAS), which contained 48 items and was validated. It was applied from September to December 2018. RESULTS: The survey was filled in by 113 parents. The results showed that 100% (n = 113) of the parents feel comfortable to talk with nurses about children/teenagers health; 79.6% (n = 90) consider that nurses have time availability for the doubts clarification; 61.9% (n = 70) point out that nurses identify child/teenager needs; Healthy eating" (60.2%; n = 68), the "National Vaccination Plan" (53.1%; n = 60) and "Harmful behaviors prevention" (46.9%; n = 53) are the most important topics; 56.6% (n = 64) of the parents, when in doubt, turn first to the pediatrician, and 66.4% (n = 75) considered that this practice was equal important, compared with other nursing interventions. DISCUSSION: This study shows that Health Education provided by nurses is based on the need's identification, with a perspective of involvement and participation, promoting health and conscious changes which reinforces the nurses' position as health educators.

Our study provides valuable insights into the relationship between artificial intelligence (AI) and Human Resource Management (HRM). We have minimised bias and ensured reliable findings by employing a systematic literature review and the PRISMA statement. Our comprehensive synthesis of the studies included in this research, along with a bibliometric analysis of articles, journals, indexes, authors' affiliations, citations, keyword co-occurrences, and co-authorship analysis, has produced robust results. The discussion of our findings focuses on critical areas of interest, such as AI and Talent, AI Bias, Ethics and Law, and their impact on Human Resource (HR) management. Our research highlights the recognition by organisations of the importance of talent management in achieving a competitive advantage as higher-level skills become increasingly necessary. Although some HR managers have adopted AI technology for talent acquisition, our study reveals that there is still room for improvement. Our study is in line with previous research that acknowledges the potential for AI to revolutionise HR management and the future of work. Our findings emphasise the need for HR managers to be proactive in embracing technology and bridging the technological, human, societal, and governmental gaps. Our study contributes to the growing body of AI and HR management knowledge, providing essential insights and recommendations for future research. The importance of our study lies in its focus on the role of HR in promoting the benefits of AI-based applications, thereby creating a larger body of knowledge from an organisational perspective.

Some of the species of the genus Bacopa have been used in Pharmacopoeia worldwide. However, in Mexico, Bacopa monnieri has neither been extensively cultivated nor studied, nor has their use in traditional medicine been reported. The aim of this work was to assess the taxonomic verification of the four wild populations of B. monnieri, the chemical content of their pigments and phenols and to provide an analysis of their potential bioactivity. B. monnieri wild populations from Mexico were validated using molecular markers. Chromatographic profiling using HPLC-PDA revealed 21 compounds comprising 12 chlorophylls and nine carotenoids; of the latter, the major ones were lutein (0.921 ± 0.031 µg/mg of dry extract) and ß-carotene (0.095 ± 0.003 µg/mg of dry extract). The total phenolic content, determined using the Folin-Ciocalteu assay, ranged from 54.8 ± 5.8 to 70.3 ± 2.2 µg of gallic acid equivalents (GAE)/mg. Plant extracts scavenged from the free radical DPPH in IC50 ranged from 130.6 ± 3.0 to 249.9 ± 12.1 µg dry extract/mL. In terms of the anti-inflammatory potential, the most effective extract was from a soil-based plant from Jalisco (BS), reduced from nitric oxide in a RAW 264.7 culture medium, with an IC50 value of 134 µg of dry extract/mL. The BS extract showed a significant neutral lipid-reducing activity in the zebrafish model, ranging from 3.13 µg/mL p < 0.05 to 100 µg/mL p < 0.0001. Overall, the extracts analyzed here for the first time seem promising for future use because of their antioxidant, anti-inflammatory and anti-obesity potential.

Objetivo: O presente estudo busca descrever o relato de experiência do clube de leitura "Por uma Prescrição Segura" de estudantes de medicina como estratégia para desenvolver habilidades de pesquisa na literatura científica e estudo crítico de evidências, focando em uma prescrição segura. Método: Desenvolveu-se o clube de leitura com nove estudantes de medicina, os quais participaram de reuniões dirigidas por líder estudantil e uma professora. As reuniões ocorreram quinzenalmente, ao vivo, via Google Meet. Os estudantes desenvolveram questões sobre diferentes temas, pesquisaram a literatura, localizaram e estudaram criticamente os artigos. Resultados: os dados revelaram a participação dos estudantes no Clube de Leitura nas reuniões planejadas coletivamente e com os temas: "Medicina Baseada em Evidências"; "Revisão Integrativa"; "Segurança da prescrição, paciente e medicamento"; "Polifarmácia"; "Resistência aos antimicrobianos"; "Prescrição de psicofármacos"; "Incorporação de tecnologias"; "Práticas integrativas e complementares, Cuidados paliativos". Conclusão: O clube de leitura proporcionou o desenvolvimento do trabalho acadêmico de pesquisa e estudo da literatura científica sobre prescrição segura. Além disso, trouxe compreensão dos conceitos e modelos de práticas de medicina baseada em evidência no estágio pré-clínico. Foi uma estratégia-modelo liderada por estudante e orientada pela professora com resultado favorável para integrar as atividades curriculares.


Objective: The present study seeks to describe the experience report of the medical students' jornal club "For a Sefety Prescription" as strategy to develop skills on searching in cientific literature and critically study of the evidences, focusing on safe prescription. Method: The journal club was developed with nine medical students, who participated in meetings led by a student leader and a teacher. The meetings ocorred biweekly, live, via Google Meet. The students developed questions on different topics, searched the literature, located and critically studied the articles. Results: the data revealed the participation of students in the jornal club in meetings planned collectively and with the themes: "Evidence-Based Medicine"; "Integrative Review"; "Prescription, patient and medication safety"; "Polypharmacy"; "Antimicrobial resistance"; "Prescription of psychotropic drugs"; "Incorporation of technologies"; "Integrative and complementary practices, Palliative care". Conclusion: The article-centered journal club provided the development of academic work on research and study of the scientific literature on safe prescribing. In addition, broght understanding of the concepts and models of evidence-based medicine practices in the preclinical stage. It is a student-led and teacher-led model strategy with favorable outcome for integrating curricular activities.

Método: se desarrolló un club de lectura con nueve estudiantes de medicina que participaron en reuniones dirigidas por un líder estudiantil y un maestro. Las reuniones se realizaron quincenalmente, en vivo, a través de Google Meet. Los estudiantes desarrollaron preguntas sobre diferentes temas, investigaron la literatura, localizaron y estudiaron críticamente los artículos. Resultados: los datos revelaron la participación de los estudiantes del club de lectura en encuentros planificados colectivamente y con los temas: "Medicina Basada en la Evidencia"; "Revisión Integrativa"; "Seguridad de la prescripción, del paciente y de los medicamentos"; "Polifarmacia"; "Resistencia antimicrobiana"; "Prescripción de psicofármacos"; "Incorporación de tecnologías"; "Prácticas integradoras y complementarias, cuidados paliativos". Conclusión: El club de lectura centrado en el artículo proporcionó el trabajo académico de investigar y estudiar la literatura científica sobre la prescripción segura. Además, comprensión de los conceptos y modelos de las prácticas de la medicina basada en la evidencia en la etapa preclínica. Es una estrategia modelo dirigida por el estudiante y el docente con resultados favorables para la integración de actividades curriculares.

Abstract Background In tetralogy of Fallot, correction surgery is a priority choice, seeing that it is desirable to minimize pulmonary hypoflow and severe hypoxemia, which result in hypoxemic crises, with sudden worsening of cyanosis, tachypnea, and, in some cases, loss of consciousness, seizures, and even death. Objective To evaluate the predicting factors of surgical mortality in children and adolescents undergoing correction of tetralogy of Fallot. Methods Retrospective cross-sectional study carried out by consulting all surgical records of the Child and Adolescent Cardiology Service of the Brazilian National Institute of Cardiology, during the period from 2007 to 2010. Results with p values < 0.05 were considered significant. Results The study evaluated 93 medical records. In relation to the characteristics of the population at the time of surgery, the median age was 3.69 (2.13 to 5.79) years, and 58.06% (n = 54) were male. White was the most common skin color, accounting for 55.43% (n = 51) of cases. In relation to assessment of nutritional status, median weight was 13.25 (10.10 to 17.60) kg, and body mass index was 14.49 (13.44 to 16.28) kg/m2. Down syndrome was present in 11.83% (n = 11) of the patients. All patients underwent correction surgery (n = 93, 100%). Prior cyanotic crisis was found in 53.85% (n = 49) with p = 0.013; surgical procedure duration was 218.83 ± 60.63 minutes, with p = 0.003, and lactate was 1.88 ± 1.33 mg/dL during the immediate postoperative period, with p = 0.009. Regarding the outcome of surgical death, it was found in 15.05% (n = 14) of patients. Mean follow-up lasted 5.68 ± 3.76 years. Conclusions According to the factors analyzed, the duration of the surgical procedure, prior cyanotic crisis, and blood lactate level may be relevant to surgical mortality.

A morte faz parte do quotidiano da vida dos enfermeiros, principalmente quando lidam com pacientes em situação crítica. A forma como percecionam a morte pode ser relevante para uma prestação de cuidados humanizados. Objetivo: identificar quais os sentimentos de estudantes de um programa de mestrado perante a morte e a sua influência na prestação de cuidados a pacientes críticos. Método: Trata-se de uma pesquisa de abordagem qualitativa, aprovada pela comissão de ética da Universidade (Doc11/CE/2018 de 09/04/2018). Realizaram-se entrevistas a 11 estudantes, processadas por análise de conteúdo. Resultados: os principais sentimentos associados à morte dos pacientes foram: impotência, frustração/revolta, tristeza/angústia, aceitação, alívio e distanciamento. Conclusão: os estudantes criam barreiras emocionais para gerir o sofrimento dos pacientes com que lidam, sendo importante que estas não comprometam a qualidade dos cuidados. Os sentimentos dos estudantes perante a morte influenciam positivamente o cuidado ao paciente crítico, pois as vivências e experiência de vida permitem-lhes prestá-los com mais serenidade


Death is part of nurses' daily lives, especially when they deal with patients in critical situations. The way they perceive death may be relevant for providing humanized care. Objective: to identify the feelings of students of a master's program towards death and its influence on care provided to critical patients. Method: This is a study with a qualitative approach, approved by the ethics committee of the University (Doc11/CE/2018 of 09/04/2018). Interviews were carried out with 11 students, processed by content analysis. Results: the main feelings associated with the death of patients were: impotence, frustration/revolt, sadness/anguish, acceptance, relief, and distancing. Conclusion: students create emotional barriers to manage the suffering of the patients they deal with, and it is important that these do not compromise the quality of care. Students' feelings towards death positively influence critical patient care, as their experiences and life experience allow them to provide care more calmly.

Objective: to know the perception of nurses at work regarding the areas of knowledge and intervention. Method: quantitative, descriptive and cross-sectional study. It involved a sample of 472 occupational nurses. A questionnaire was used as a data collection instrument, which took place between May and September 2017, based on the core areas of knowledge and competencies of the occupational nurse and on the scale of perception of the areas of knowledge and intervention of the occupational nurse. Results: Occupational Nursing was understood by the majority as a transitory professional activity, because it was exercised as a financial complement to the main activity or because they could not be placed in the area of professional practice of their preference. Conclusion: the professional practice in the areas of knowledge and intervention was devalued or not recognized and seen by the majority of nurses as transitory, because almost half aspire to work in the future in this area, but only on a part-time basis.


Objetivo: conocer la percepción de las enfermeras en el trabajo con respecto a las áreas de conocimiento e intervención. Método: estudio cuantitativo, descriptivo y transversal. Involucró una muestra de 472 enfermeras de trabajo. Se utilizó un cuestionario como instrumento de recolección de datos, que tuvo lugar entre mayo y septiembre de 2017, basado en las áreas nucleares de conocimiento y competencias de la enfermera de trabajo y en la escala de percepción de las áreas de conocimiento e intervención de la enfermera de trabajo. Resultados: la Enfermería Ocupacional fue entendida por la mayoría como una actividad profesional transitoria, porque se ejercía como complemento económico de la actividad principal o porque no podían ubicarse en el área de práctica profesional de su preferencia. Conclusión: la práctica profesional en las áreas de conocimiento e intervención fue devaluada o no reconocida y vista por la mayoría de las enfermeras como transitoria, porque casi la mitad aspira a trabajar en el futuro en esta área, pero solo a tiempo parcial.


Objetivo: conhecer a perceção dos enfermeiros do trabalho no que respeita às áreas de conhecimento e intervenção. Método: estudo quantitativo, descritivo e transversal. Envolveu amostra de 472 enfermeiros do trabalho. Utilizou-se como instrumento de recolha de dados, que decorreu entre maio e setembro de 2017, questionário, alicerçado nas áreas nucleares de conhecimentos e competências do enfermeiro do trabalho e na escala de perceção das áreas de conhecimento e intervenção do enfermeiro do trabalho. Resultados: a Enfermagem do Trabalho foi entendida pela maioria como atividade profissional transitória, pelo facto de ser exercida como complemento financeiro à atividade principal ou porque não conseguiram colocação na área de exercício profissional da sua preferência. Conclusão: o exercício profissional nas áreas de conhecimento e intervenção foi desvalorizado ou não reconhecido e encarado pela maioria dos enfermeiros como transitório, pelo facto de quase metade ambicionar trabalhar no futuro nessa área, mas apenas em regime de tempo parcial.

The scientific community has been doing significant efforts towards engineering new 3D bone models in recent years. Osteocytes are mechanosensitive cells that play significant roles in the maintenance of bone homeostasis. Currently, as far as we know, there are no 3D models that faithfully recapitulate a bone microenvironment capable of promoting the differentiation of osteoblasts towards osteocytes. Besides, in the existing models, the use of human cells does not prevail over the animal cell lines. For so, we propose a 3D model that may have important implications for ongoing efforts towards a better understanding of bone physiology and disease. The main aim of the current work was the promotion of an effective differentiation of osteoblasts into osteocytes by mean of using a 3D model composed of primary human osteoblasts (hOBs) cultured on Gellan Gum-Hydroxyapatite (GG-HAp) matrix under a long-term osteogenic culture. The results revealed that GG-HAp matrix stimulated a fast cell migration/entrapment, attachment, spreading, and mineralization. Moreover, the transition process from osteoblasts to osteocytes was confirmed by the expression of the osteogenic-related (ALP, Runx2, COL I, OC, OPN and OSX) and osteocyte-related (hPDPN) marker throughout the culture time. Overall, the developed 3D model holds a great promise for the treatment of various bone diseases, namely on diagnostic applications and for bone regeneration purposes.

It is a fact that no one expected what took place in the world as of the end of 2019. And yes, I am talking about the ill-fated, damned and inconvenient COVID-19, which quickly spread around the globe, having been declared a pandemic by the World Health Organization in March 2020.(1) In the face of such a threat, not virtual, but very real, the need existed to undertake various measures that could interrupt the transmission of the infection by implementing, for example, restrictions on the movement of people, social distancing, use of personal protective equipment, measures of respiratory protocol, social confinement, among others.(2) Education, namely, higher education, also suffered immensely the consequences of COVID-19, right from the start, because there was a cessation of all classroom teaching activity, practically all over the world, which caused major modifications/alterations in the teaching-learning process.(3) From the outset, closing of Higher Education Institutions, globally, caused severe changes in teaching-learning processes because measures of social distancing and circulation restrictions were imposed.(4,5)

There are cultural and educational barriers that hinder the adoption of technologies in educational practice. This is mainly owed to how the majority of teachers are still digital immigrants lacking adequate preparation to work in an environment of fast-paced innovation and without the corresponding updates of cognitive technology. In order to mitigate this problem, in this paper we present an alternative framework that intends to increase the effectiveness of technological application in broader educational contexts bringing together three constructs causally relating the domains of Cognitive and Motivational Competencies (CMC) - relating to people; Organisational Institutional Complexity (OIC) - educational systems and Behavioural Intentional Use of Technology (BIUT) relating to adoption of attitudes. Measurement indicators were applied to evaluate each construct in order to better predict teaching behaviour in a broader educational context. The framework evaluation was performed by two focus groups composed of experienced, highly specialised education consultants who assessed the pedagogical, technological, teacher's behaviour and overall institutional/organisational environment involving the proposed alternative solutions. The discussions and results show that participants agreed on the proposed framework's usefulness and its contributions to help teachers mediate the gaps between school and the outside world, all the while considering political and administrative barriers. The evaluators also highlight the structured motivational and self-efficacy aspects that must involve teachers and other stakeholders concerned with adopting a new social action profile for the benefit of collectives. The next work to follow this paper will be a dissemination proposal concerning the Brazilian National Educational Plan, a technological adoption that effectively meets Education's institutional principles in this country.

Objetivos: a acupuntura é, cada vez mais, uma técnica utilizada, em usuários portadores de várias patologias, e, em especial, aquelas que são acompanhadas de dor crónica. Esta pesquisa teve como objetivo geral analisar o efeito da terapêutica não farmacológica de acupuntura e eletroacupuntura para o controle da dor crónica, em usuários com diagnóstico médico de síndrome vertebral com irradiação. Métodos: foi realizada uma pesquisa com dois momentos de avaliação ­ antes e após sessões de terapêutica não farmacológica de acupuntura e eletroacupuntura. Utilizou-se um questionário com variáveis de caracterização sociodemográfica e clínica, o Brief Pain Inventory (BPI)e Short Form 6 Dimensions (SF-6D). Resultados: participaram no estudo 43 usuários, tendo sido encontradas diferenças estaticamente significativas, do momento 1 para o momento 2 de avaliação, quer no domínio da severidade da dor (p<0,01), quer no domínio interferência da dor no funcionamento diário (p<0,01), assim como nas diferentes dimensões do SF-6D foram encontradas diferenças estatisticamente significativas. Conclusões: salienta-se que a terapêutica não farmacológica de acupuntura e eletroacupuntura em usuários com diagnóstico de síndrome vertebral com irradiação e dor crónica contribuiu para a diminuição da severidade da dor, da interferência da dor no funcionamento diário e melhoria da qualidade de vida relacionada com a saúde.


Aims: acupuncture is increasingly a technique used in users with various pathologies, and especially those who are accompanied by chronic pain. This research aimed to analyze the effect of non-pharmacological acupuncture and electroacupuncture therapy for the control of chronic pain in users with medical diagnosis of irradiation vertebral syndrome. Methods: a research was conducted with two evaluation moments: before and after non-pharmacological acupuncture and electroacupuncture therapy sessions. A questionnaire with variables of sociodemographic and clinical characterization, the Brief Pain Inventory and Short Form 6 Dimensions (SF-6D) was used. Results: forty-three users participated in the study, and statistically significant differences were found from moment 1 to moment 2 of evaluation, either in the pain severity domain (p<0.01) or in the pain interference domain in daily functioning (p <0.01), as well as in the different dimensions of SF-6D, statistically significant differences were found. Conclusions: non-pharmacological acupuncture and electroacupuncture therapy in users diagnosed with irradiation and chronic pain have contributed to decrease pain severity, pain interference in daily functioning and improvement of health-related quality of life.

RESUMO

Um dos objetivos relacionado com segurança do doente, passa pela manutenção de uma comunicação eficaz na prestação de cuidados de enfermagem. O presente Relatório surge no âmbito dos estágios realizados e tem o objetivo de descrever de forma refletida, fundamentada e pormenorizada o processo formativo. É um documento que segue uma metodologia descritiva, refletindo o percurso formativo realizado ao longo dos estágios 1 e Final, sendo que neste último foi desenvolvido um trabalho que segue a metodologia de projeto, designado de intervenção major, que dá nome ao relatório. Foi utilizada uma escala de observação e um questionário na recolha de dados aplicado a 20 enfermeiros. Na atividade major foi evidenciada a necessidade da adequação de algumas estratégias de forma a otimizar o processo de comunicação entre os enfermeiros e a família/pessoa em situação crítica. Foram evidenciadas as competências comuns de enfermeiro especialista em enfermagem médico cirúrgica, assim como na vertente da pessoa em situação crítica e de mestre. Os objetivos definidos ao longo do documento foram alcançados, consta-se que há um maior enriquecimento pessoal e profissional ao nível técnico e científico.


One of the objectives related to patient safety is to maintain effective communication in the provision of nursing care. This Report comes within the scope of the internships carried out and aims to describe the training process in a reflected, reasoned and detailed manner. It is a document that follows a descriptive methodology, reflecting the formative path carried out during stages 1 and Final, with the latter developing a work that follows the project methodology, called major intervention, which gives the report its name. An observation scale and questionnaire were used to collect data applied to 20 nurses. In the major activity, the need to adapt some strategies in order to optimize the communication process between nurses and the family/ person in critical situation was highlighted. The skills of nurse specialist in medical surgical surgery, person in critical situation and master were evidenced. The objectives defined throughout the document were achieved, it is said that there is greater personal and professional enrichment at the technical and scientific level.

A emergência hipertensiva (EH) está integrada em um quadro nosológico mais geral denominado crise hipertensiva (CH). A CH representa situações clínicas que cursam com elevação aguda da pressão arterial (PA), geralmente níveis de PA sistólica (PAS) ≥ 180 mmHg e diastólica (PAD) ≥ 120 mmHg, que podem resultar ou não em lesões de órgãos-alvo (LOA) (coração, cérebro, rins e artérias).1-5 A CH pode se apresentar sob duas formas distintas em relação à gravidade e ao prognóstico: a urgência hipertensiva (UH) e a EH. Casos de EH cursam com elevação acentuada da PA associada a LOA e risco imediato de morte, fato que requer redução rápida e gradual dos níveis tensionais em minutos a horas, com monitoramento intensivo e uso de fármacos por via endovenosa (EV).1-5 Ela pode se manifestar como um evento cardiovascular, cerebrovascular, renal ou na gestação, na forma de pré-eclâmpsia ou eclâmpsia. Embora a definição clássica das duas apresentações da CH a descreva com valores acima dos 180/120 mmHg, atualmente o maior consenso se estabelece no conceito de que mais do que os valores é o dano ou o risco iminente de acometimento de órgãos-alvo que distingue a EH da UH. Assim, a UH caracteriza-se por elevações da PA, sem LOA e sem risco de morte iminente, fato que permite redução mais lenta dos níveis de PA em período de 24 a 48 horas. Atualmente, existe uma ampla discussão sobre a real existência do diagnóstico "urgência hipertensiva".6 Muitos preconizam que esta classificação necessita ser atualizada (se não abandonada) e que a maior importância diagnóstica é a observação dos sinais/sintomas e da disfunção aguda dos órgãos-alvo, mais do que no valor da PA. Outros acreditam que o termo correto deveria ser "elevação da PA sem LOA em evolução".5,7 Como visto, embora o nível de PA seja frequentemente muito elevado (≥ 180/120 mmHg), não é isso que define EH, mas o comprometimento dos órgãos-alvo. Portanto, o padrão numérico que define a CH é conceitual e serve como parâmetro de conduta, mas não deve ser usado como critério absoluto. Se a definição de CH hoje está mais universalmente aceita, a epidemiologia e prevalência desta condição são ainda questões de baixo conhecimento da comunidade científica. Na literatura, existem poucos estudos sobre o tema e todos eles com número limitado de participantes. Atualmente, discute-se a hipótese de a não adesão ao tratamento ser um dos fatores mais prevalentes na etiologia da CH, sem especificações quanto à separação entre UH e EH. Nos EUA, nos maiores estudos seriados a incidência de CH é de cerca de 4,8%, sendo 0,8% atribuída às EH.8,9 Outros centros mostram que a CH responde por uma taxa variável de 0,45 a 0,59% de todos os atendimentos de emergência hospitalar e 1,7% das emergências clínicas, sendo a UH mais comum do que a EH.10-12 Acidente vascular cerebral (AVC) isquêmico e edema agudo de pulmão (EAP) são as situações clínicas mais encontradas nas EH.10,11 Estima-se que cerca de 1% dos indivíduos hipertensos possa vir a apresentar uma CH ao longo da sua vida.1,2 As situações clínicas envolvidas em uma EH, de acordo com as LOA, são mostradas na Tabela 1. A Tabela 2 mostra as principais situações relacionadas à UH.
