RESUMO
The authors report a clinical case of the fracture of the base of the skull with the concomitant damage to the sigmoid sinus, jugular vein, and internal carotid artery in the temporal bone pyramid. The injury was associated with massive bleeding from the external acoustic canal that resulted in the death of the patient.

The analysis of primary and general morbidity revealed at annual inspection within 20 years of workers of X-ray rooms in St. Petersburg and for control--the group of medical workers not connected with radiation was carried out. It is shown that work of radiologists is considerably connected with alarm for the health at radiating influence. Radio uneasiness doesn't decrease with increase in length of service, but, on the contrary, accrues. Increase of primary and general morbidity in workers of x-ray offices is revealed.

Results of sociological investigation of the staff members and patients of radiodiagnosis departments of treatment and prophylaxis institutions of St. Petersburg are analyzed. Anonymous questionnaires were distributed in 50 institutions of different profiles among 200 staff members (physicians--roentgenologists and radiologists, and x-ray laboratory assistants) and 300 patients (from x-ray rooms and radiodiagnosis departments). The study showed an objective picture of the state of radiological care rendered to the population viewed from two sides, by those rendering this care and those rendered it. The results helped develop recommendations for public health organization.

A total of 132 patients with generalized sarcoidosis (GS) who amounted to 2.8% out of 4700 examinees and followed-up patients with respiratory sarcoidosis were studied. The pathomorphological study of biopsy specimens, X-ray functional, and immunological studies are of great importance in the diagnosis of GS. The latter in the examinees was characterized by the most common lesions to the skin, subcutaneous fat, peripheral lymph nodes, liver, spleen, central and peripheral nervous systems and eyes. The combined pathogenetic therapy developed, which involves corticosteroidal hormones and anabolic drugs, immunomodulators, antioxidants and antihypoxants has been shown to be effective in 62% of the patients.

The aim of the work was to study the role of a scintigraphic examination in the complex diagnosis of disseminated pulmonary tuberculosis. Study was carried out by a computer-assisted gamma-camera with 99mTc pharmaceutical preparation. Combined examination included 55 patients among whom most patients (26 or 65.5%) had a disseminated lung process and 19 (34.5%) had changes circumscribed by 2-3 bilateral segments. The method was proved to be of high informative value. The microcirculatory changes in 47 (85.5%) patients with obvious signs of active pulmonary tuberculosis were more pronounced than those detected by X-ray. Dynamic scintigraphy in 8 (14q.5%) patients with a dubious process activity proved to be an advantageous supplementary method of study.

Comprehensive investigations including clinical examination, x-ray and scintigraphy of the lungs were conducted in 163 patients with sarcoidosis, Stages I, II, III, disseminated tuberculosis and tuberculosis of the intrathoracic lymph nodes. The scintigraphic method is more sensitive to initial pulmonary changes in the process of treatment than the x-ray one. Earlier changes in the course of disease are recorded by pulmonary circulation changes (microcirculation) revealed at pulmonoscintigraphy.

The paper is concerned with an algorithm of a program for pulmonoscintigram processing. The program developed in the algorithmic languages Basic and Assembler on the basis of this algorithm, made it possible to facilitate the data processing, to lower a physician's competence as a computer operator, to raise the objectivity of diagnosis of pulmonary diseases, etc. The program is intended for computers Vip-450, Vip-550 and MCS-560, Technicare, USA.
