RESUMO

The aim of this work was to study clinical and functional symptoms of funicular myelosis (FM) and chronic gastritis (CG) in patients with B12-deficiency anemia. A total of 77 patients with diagnosis verified by finding megaloblasts in bone marrow biopsies were examined. Clinical symptoms of FM were found in 57% of the patients. The most characteristic subjective ones were paresthesia (83.3%) and limb weakness (60%) while objective symptoms included impaired limb sensitivity (100%). All patients suffered CG with lesions in both antrum and fundus.

RESUMO

The article presents the clinical features of stroke-induced nosocomial pneumonia and interleukin-1alpha level monitoring in serum and cerebrospinal fluid of 100 patients with cerebral hemorrhage on the 1st, 3rd and 10th day. The authors show that 66% of patients with cerebral hemorrhage develop nosocomial pneumonia since the end of 2nd up to 5th day of conservative hospital treatment, more frequently in the serious cases with high level of neurological deficiency. The most important risk factors of stroke-induced nosocomial pneumonia are chronic focal infection, diabetes mellitus, cardiac failure, smoking, obesity. Since the first day of stroke the interleukin-1alpha level both in serum and cerebrospinal fluid exceeds 25-30 times its content in healthy people and increases more in the presence of nosocomial pneumonia. Interleukin-1alpha level can serve as an early risk marker of lethal outcome in patients with cerebral hemorrhage.

RESUMO

Results of operative treatment of 93 patients with tubular small and large intestine fistulas were analyzed depending on the operative access and volume of interventions. In the postoperative period incompetence of the intestinal anastomosis and recurrent intestine fistulas are found more often in the group of patients with not removed causes of maintenance of the fistula persistence. These complications are rarer in patients operated from the bordering access with liquidation of the cause of long existence of the fistula. The liquidation of the persistence causes and operative access some distance away from the external opening of the fistula resulted in the absence of incompetence of the intestinal anastomosis and recurrent fistulas.

RESUMO

Results of staged treatment of small bowel fistulas in 115 patients were analyzed. Staged treatment of 21.7% of patients resulted in healing the fistulas without operations. Modern methods of nutritional maintenance were shown to allow surgical restorative treatment to be performed in the optimal period in 82.2% of patients. The number of postoperative complications and lethality rate were reduced to 5.6%, general lethality to 7.8%.

As many as 45 patients with chronic atrophic gastritis against the background of B12 and iron-deficiency anemia were examined. Chronic atrophic gastritis combined with B12 deficiency anemia was found to damage in most cases the antral and fundic parts of the stomach simultaneously, while the gastric process combined with iron-deficiency anemia involved the antral part only in most cases. The disease is accompanied by a considerable reduction of acid, pepsinogen and biermerin formation, especially in cases of chronic atrophic gastritis associated with B12 deficiency anemia.

RESUMO

There was a study of 49 patients with iron- and B12-deficiency anemia. Morphological symptoms of chronic gastritis were revealed in 100% of patients. Chronic gastritis against the background of iron-deficiency anemia was characterized by superficial and focal atrophic lesions of the antral mucous coat, frequently--by erosions, and clinical manifestations in the form of intestinal indigestion and abdominal pains. Chronic gastritis against the background of B12-deficiency anemia always had an atrophic nature, was localized in the body and in the antral part of the stomach, and had clinical manifestations in the form of intestinal indigestion.

The paper deals with the use of a laser flow-type fluorescence aerosol particle counter to evaluate the concentrations of microbes in the surface air under high dust content. Various circuits of flow-type optic aerosol recorders are analyzed. Flow spectral luminescence analysis of some particles flow while exciting the fourth harmonics of a pulse laser on yttrium-aluminium garnet with neodymium by ultraviolet radiation is shown to be the most optimum method for indication of individual aerosol particles. Experiments were conducted on the authors' model of a pilot plant based on this method. The model of a laser flow-type optic analyzer was developed for experimental studies that give a clear display of biological aerosols in complex aerosols. The laser flow-type analyzer-based unit developed may provide a fluorescence signal of aerosol particles in the flow of a sample and that light diffusion signal from them at an exciting light wavelength of 266 nm. Experiments with BVC aerosols and soil dust particles were conducted in different regions of Russia. They showed it possible to detect and to rapidly calculate soil microorganisms by laser flow-type fluorescence assay of individual particles when excited by ultraviolet radiation.
