BACKGROUND: Recombinant protein vaccines are vital for broad protection against SARS-CoV-2 variants. This study assessed ReCOV as a booster in two Phase 2 trials. RESEARCH DESIGN AND METHODS: Study-1 involved subjects were randomized (1:1:1) to receive 20 µg ReCOV, 40 µg ReCOV, or an inactivated vaccine (COVILO®) in the United Arab Emirates. Study-2 participating individuals were randomized (1:1:1) to receive 20 µg ReCOV (pilot batch, ReCOV HA), 20 µg ReCOV (commercial batch, ReCOV TC), or 30 µg BNT162b2 (COMIRNATY®) in the Philippines. The primary immunogenicity objectives was to compare the geometric mean titer (GMT) and seroconversion rate (SCR) of neutralizing antibodies induced by one ReCOV booster dose with those of inactivated vaccine and BNT162b2, respectively, at 14 days post-booster. RESULTS: Heterologous ReCOV booster doses were safe and induced comparable immune responses to inactivated vaccines and BNT162b2 against Omicron variants and the prototype. They showed significant advantages in cross-neutralization against multiple SARS-CoV-2 variants, surpassing inactivated vaccines and BNT162b2, with good immune persistence. CONCLUSIONS: Heterologous ReCOV boosting was safe and effective, showing promise in combating COVID-19. The study highlights ReCOV's potential for enhanced protection, supported by strong cross-neutralization and immune persistence. Hole mobility is critical to the power conversion efficiencies of perovskite solar cells (PSCs). Organic small-molecule hole-transporting materials (HTMs) have attracted considerable interest in PSCs due to their structural flexibility and operational durability, but they suffer from modest hole mobility. On the other hand, inorganic HTMs with good hole mobility are inflexible in structural variation and exhibit unsatisfactory cell efficiency. In this study, a ligand BT28 and its zinc-based coordination complex BTZ30 were synthesized, characterized, and investigated as HTMs for PSC applications. The mixed-halide perovskites can be grown uniformly with large crystalline grains on both HTMs, which exhibit similar optical and electrochemical properties. However, it was discovered that the BTZ30-based solar cell exhibited an open-circuit voltage of 1.0817 V and a high short-circuit current density of 23.1392 mA cm-2 with a champion power conversion efficiency of close to 20 %. The performance difference between the two HTMs can be attributed to the difference in their hole mobilities, which is 63.31 % higher for BTZ30 than BT28. The comparison of non-metal and metal HTMs revealed the importance of considering hybrid structures to overcome some shortcomings associated with organic and inorganic HTMs and achieve high-performance PSCs.

Hole transport materials (HTMs) with appropriate energy levels and comprehensive passivation effects help to obtain highly efficient and stable perovskite solar cells (PSCs). Electron-deficient character-induced HTMs can generate varying energy level alignments near the HTM/perovskite interface. Herein, we report the synthesis and investigation of two new dipolar HTMs, WWC103 and WWC105, based on 2-(1,1-dicyanomethylene)rhodamine and 4-cynophenylacetonitrile acceptors, enabling high-efficiency mixed-cation mixed-halide perovskite solar cells. Apart from having different acceptors, these HTMs are built on a heterocyclic frame, which can provide passivation effects and improve the morphology of the perovskite layer. As a result, these dopant-free HTM-based solar cells show a high open-circuit voltage and good power conversion efficiency. Among both, the solar cell based on the HTM with 2-(1,1-dicyanomethylene)rhodamine exhibits a high open-circuit voltage of 1.09 V with a champion power conversion efficiency of over 20.51%. The improved performance of WWC103 over WWC105 (19.74%) is attributed to the new acceptor, which, in addition to providing good energy-level alignments and hole mobility, also holds the ability to passivate the defects. The findings suggest a new acceptor unit for constructing dopant-free HTMs for efficient PSCs.

Pancreatic cancer is a highly malignant tumor with a poor prognosis. It is very hard to treat pancreatic cancers for their high heterogeneity, complex tumor microenvironment, and drug resistance. Currently, gemcitabine plus nab-paclitaxel, capecitabine and FOLFIRINOX are standard chemotherapy for resectable or advanced metastatic pancreatic cancer. Considering the limited efficacy and toxic side effects of chemotherapy, targeted and immune drugs have gradually attracted attention and made some progress. In this article, we systematically reviewed the chemotherapeutic drugs, targets and related targeted drugs, and immunotherapy drugs for pancreatic cancer.

Tumor brings great threat to human public health. In recent years, incidence rate and mortality of tumor were rapidly increased in the world. Anti-tumor therapies have undergone the development of cytotoxic therapy, targeted therapy, and immunotherapy. Among them, tumor immunotherapy is rapidly developed and becomes an important anti-tumor therapy in recent years, although it also brings some related side effects. Tumor microenvironment (TME) is composed of immune cells, vascular vessels, fibroblasts, the extracellular matrix, etc. TME significantly affects the efficacy of immunotherapy. Macrophages in the TME are named as tumor associated macrophages (TAMs). Recently, increasing studies have shown that TAMs play an important role in the regulation of tumor immunity, especially in tumor immune surveillance and immune escape. Currently, more and more anti-tumor immunotherapy strategies targeting TAMs are at the development stage. Based on the important role of TAMs in the TME and their potential as therapeutic targets in tumor immunotherapy, we first reviewed the subtypes and functions of TAMs, as well as the roles of TAMs in tumors. Furthermore, we summarized the research progress on anti-tumor strategies targeting TAMs and the current status of drug targeting TAMs. The current review will provide new ideas and novel insights for tumor immunotherapy.

Objective:To construct a training system of the professional postgraduate degree of general practice using the Delphi method.Methods:On the basis of consulting relevant policies and literature, and drawing lessons from foreign general practice training systems, the index system of training professional postgraduate medical students of general practice was preliminarily developed. After two rounds of Delphi expert consultation, using the threshold method to select evaluation indexes, the training system was finally established. Excel 2010 and SPSS 19.0 were used to make data entry and statistical analysis.Results:The active coefficients of experts in the two rounds of Delphi consultation were 75.0% (12/16) and 91.7% (11/12) respectively, both the two rounds of the degree of specialist authority were 0.87, and the final coordination coefficient of the specialists' opinion was 0.452. The index system was composed of 5 first-level indexes including training objectives, theoretical curriculum setting, clinical practice setting, teaching staff and graduation requirements and 31 second-level indexes.Conclusion:The experts have great initiative and authority on research subjects. The opinions of all experts tend to be uniform after two rounds of consultation, and the results are reliable. A relatively complete and credible training index system of professional postgraduate degree of general medicine has been built.

Objective:To build a system for evaluation on post competency of general practitioners.Methods:From April to October 2020, the Delphi method was used to consult on the importance of 4 first-level indicators and 48 second-level indicators for evaluation on post competency of general practitioners. Excel 2016 and SPSS 22.0 was used for data entry and statistical analysis.Results:There were 16 consulting experts, and the positive coefficients of the two rounds were 100% and 75%, respectively. The overall authority coefficient Cr of experts was 0.85. For the first-level indicators, the mean values of the importance of the two rounds were 8.07-9.00 and 8.67-9.00, and the coefficient of variation were 0-0.16 and 0-0.09, respectively. For the second-level indicators, the mean values of the importance of the two rounds were 6.79-9.00 and 8.17-9.00, and the coefficient of variation were 0-0.38 and 0-0.16, respectively. All the coordination coefficient tests were statistically significant ( P<0.001). Finally, the evaluation system was established including 4 first-level indicators and 32 second-level indicators. Conclusion:The constructed post competency evaluation system for general practitioners has a relatively high coordination and credibility, which can provide a reference for the post competency evaluation of general practitioners.

This study addresses some issues regarding the problems of applying CAE to the injection molding production process where quite complex factors inhibit its effective utilization. In this study, an artificial neural network, namely a backpropagation neural network (BPNN), is utilized to render results predictions for the injection molding process. By inputting the plastic temperature, mold temperature, injection speed, holding pressure, and holding time in the molding parameters, these five results are more accurately predicted: EOF pressure, maximum cooling time, warpage along the Z-axis, shrinkage along the X-axis, and shrinkage along the Y-axis. This study first uses CAE analysis data as training data and reduces the error value to less than 5% through the Taguchi method and the random shuffle method, which we introduce herein, and then successfully transfers the network, which CAE data analysis has predicted to the actual machine for verification with the use of transfer learning. This study uses a backpropagation neural network (BPNN) to train a dedicated prediction network using different, large amounts of data for training the network, which has proved fast and can predict results accurately using our optimized model.

In this study, the assembly behavior for two injected components made by a family mold system were investigated. Specifically, a feasible method was proposed to evaluate the characteristic length of two components within a family mold system using numerical simulation and experimental validation. Results show that as the packing pressure increases, the product index (characteristic length) becomes worse. This tendency was consistent for both the simulation prediction and experimental observation. However, for the same operation condition setting through a basic test, there were some differences in the product index between the simulation prediction and experimental observation. Specifically, the product index difference of the experimental observation was 1.65 times over that of the simulation prediction. To realize that difference between simulation and experiment, a driving force index (DFI) based on the injection pressure history curve was proposed. Through the DFI investigation, the internal driving force of the experimental system was shown to be 1.59 times over that of the simulation. The DFI was further used as the basis for machine calibration. Furthermore, after finishing machine calibration, the integrated CAE and DOE (called CAE-DOE) strategy can optimize the ease of assembly up to 20%. The result was validated by experimental observation.

Objective:To observe short-term and long-term effect of modified Shenfuwan on lumbar disc herniation due to kidney deficiency and cold coagulation. Method:100 cases were randomly divided into the control group and observation group,50 cases each. The control group were taken meloxicam tablets,7.5 mg each time,once a day and the observation group were given modified Shenfuwan,300 mL each time,twice a day. The treatment course was 3 months. The short form McGill pain questionnaire(SF-MPQ),oswestry disability index(ODI),and lumbar intervertebral disc herniation due to kidney deficiency and cold coagulation by traditional Chinese medicine (TCM ) syndrome were observed before and after treatment. Whole blood viscosity (high,medium,low),serum inflammatory factors [tumor necrosis factor-alpha(TNF-<italic>α</italic>),interleukin-1<italic>β</italic>(IL-1<italic>β</italic>),interleukin-6(IL-6),pain mediators[endothelin-1(ET-1),prostaglandin E<sub>2</sub>(PGE<sub>2</sub>),cyclooxygenase-2(COX-2)] were detected. The clinical efficacyand,recurrence rate followed up for 12 and 24 months was was observed. The type and time of adverse reactions were recorded during the study. Result:After treatment,the total effective rate in the observation group was 98.0%(49/50) higher than that 82.0%(41/50) in the control group(<italic>P</italic><0.05). After 12 months of follow-up,the recurrence rate in the observation group was 23.9%(11/46) lower than that 52.5%(21/40) in the control group (<italic>χ</italic><sup>2</sup>=5.193,<italic>P</italic><0.05). After 24 months of follow-up,the recurrence rate in the observation group was 43.9%(18/41) lower than that that 80.6%(29/36) in the control group(<italic>P</italic><0.05). Compared with the control group after treatment,SF-MPQ,ODI,TCM symptoms,whole blood viscosity,TNF-<italic>α</italic>,IL-1<italic>β</italic>,IL-6,ET-1,PGE<sub>2</sub> and COX-2 were significantly reduced in the observation group(<italic>P</italic><0.05). The incidence of adverse reactions in the observation group was 2.0%(1/50) lower than that 52.0%(26/50) in the control group(<italic>χ</italic><sup>2</sup>=6.264,<italic>P</italic><0.05). Conclusion:Modified Shenfuwan can significantly improve the short-term and long-term clinical efficacy of patients with lumbar disc herniation due to kidney deficiency and cold coagulation syndrome,and the incidence of adverse reactions is low.

Objective:This study aims to investigate the clinical efficacy of modified Anshentang on the treatment of ankylosing spondylitis in early and middle stages with kidney deficiency and cold-governing syndrome and its effect on serum inflammatory factors, immune function, and bone metabolism indexes of patients. Method:In this study, 120 patients were randomly divided into control group and observation group, 60 cases in each group. On the basis of ethotrexate treatment, patients in control group took Bushen Shuji granule orally, while patients in observation group took modified Anshentang orally for 8 weeks. Before and after treatment, patients in two groups were observed for clinical symptoms [ bath ankylosing spondylitis patient global score (BAS-G), bath ankylosing spondylitis disease activity index (BASDAI), spondyloarthritis research consortium of Canada (SPARCC), traditional Chinese medicine symptoms (TCM symptoms)<italic> </italic>], serum inflammatory factors [ tumor necrosis factor-<italic>α </italic>(TNF-<italic>α</italic>), macrophage migration inhibitory factor (MIF), interleukin-23 (IL-23)], immune function [ immunoglobulin A(IgA), immunoglobulin G(IgG), immunoglobulin M(IgM)], bone metabolic indicators [osteocalcin (BGP), bone morphogenetic protein-2 (BMP-2), bonespecific alkaline phosphatase (BALP)]. The clinical efficacy, adverse reactions and recurrence rates of 12 months in two groups were observed. Result:During the study, 4 cases dropped out from control group and 2 cases from observation group. The total effective rate of 96.55% (56/58) in observation group was higher than 80.36% (45/56) in control group (<italic>χ</italic><sup>2</sup>=4.827,<italic>P</italic><0.05). The recurrence rate of 5.17% (3/58) in observation group was lower than 19.64% (11/56) in control group (<italic>χ</italic><sup>2</sup>=5.187, <italic>P</italic><0.05). Compare with control group after treatment, BAS-G,BASDAI, SPARCC, TCM symptoms, TNF-<italic>α</italic>, MIF and IL-23 in observation group were significantly decreased (<italic>P</italic><0.05), while BGP, BMP-2, BALP, IgA, IgG and IgM were significantly increased (<italic>P</italic><0.05). The incidence of adverse reactions was 12.07%(7/58) in observation group, which was lower than 32.14%(18/56) in control group (<italic>χ</italic><sup>2</sup>=4.826,<italic>P</italic><0.05). Conclusion:Modified Anshentang is effective in the treatment of ankylosing spondylitis in early and middle stages with kidney deficiency and cold-governing syndrome, and the incidence of adverse reactions is low.

Objective:To develop a monitoring tool for evaluating health literacy level of general practitioners.Methods:The health literacy monitoring questionnaire for general practitioners was preliminarily developed by referring to the National Residents′ Health Literacy Monitoring Questionnaire. The Delphi method was used to conduct two rounds of consultation among 15 experts to improve the questionnaire.Results:The effective response rates of the two rounds of expert consultation were 100.0%, the expert authority coefficients of the two rounds of consultation were 0.87 and 0.86, the mean scores of the two rounds of expert questions were 3.93 -5.00 and 4.47-5.00. The variance coefficients of the two rounds of expert scores were 0-0.32 and 0-0.20, and the Kendall harmony coefficients of the two rounds of expert scores were 0.167 and 0.102, respectively (all P<0.05). Finally, the basic theory literacy dimension (including 8 chronic disease prevention items, 5 maternal and child health items and 5 infectious disease prevention items), and the basic skill literacy dimension (including 6 safety and first aid items, 8 health information items), and the healthy lifestyle and behavior dimension (7 items, including regular physical examination, exercise, diet and so on) were established. A total of 39 items and 3 dimensions were used to monitor the health literacy level of general practitioners. Conclusion:The monitoring tool of health literacy level for general practitioners based on Delphi method has been preliminarily constructed, which can be used to evaluate the health literacy level of general practitioners.

In recent ten years, general practitioner (GP) training and practice develops rapidly in China. It has established a relatively complete GP training system, including medical college education and standardized residency training. However, the path of continuing professional development and clinical skills improvement for GP after standardized residency training is still unclear. The concept of GP with special interest (GPSI) has been proposed these days, and it has put forward a new development direction for the career path of GP. In this article, on the basis of understanding the current status of GPSI in China, we try to discuss its advantages and problems, and then propose our suggestions for its future development.

BACKGROUND: The present study compared the effects of antifibrotic medications, pirfenidone, and nintedanib, with transplantation of human umbilical mesenchymal stem cells (HUMSCs) in restoring rat pulmonary fibrosis (PF). METHODS: A stable animal model was established via an intratracheal injection of 5 mg bleomycin (BLM). One single transplantation of 2.5× 107 HUMSCs or initiation of daily oral nintedanib/pirfenidone administration was performed on day 21 following BLM damage. RESULTS: Pulmonary function examination revealed that BLM rats exhibited a significant decrease in blood oxygen saturation and an increase in respiratory rates. While no significant improvements were found in BLM rats receiving nintedanib or pirfenidone, those who transplanted with HUMSCs showed a statistical amelioration in blood oxygen saturation and significant alleviation in respiratory rates. Quantification results revealed that a significant reduction in alveolar space and marked increases in substantial cell infiltration and collagen deposition in the left lungs of BLM rats. No significant alteration was observed in BLM rats administered nintedanib or pirfenidone. However, BLM rats transplanted with HUMSCs had a significant recovery in alveolar space and noticeable decreases in cell infiltration and collagen deposition. The inflammatory cell numbers in the bronchoalveolar lavage was increased in the BLM group. While the rats treated with nintedanib or pirfenidone had a lower cell number than the BLM group, a higher cell number was found as compared with the Normal group. In rats transplanted with HUMSCs, the cell number did not differ from the Normal group. CONCLUSIONS: Transplantation of HUMSCs could effectively treat PF as opposed to the administration of anti-fibrotic drugs with nintedanib or pirfenidone with a significant better result in lung volume, pathological changes, lung function, and blood oxygen saturation.

Age-related myocardial dysfunction is a very large healthcare burden. Here, we aimed to investigate whether ginsenoside Rb1 (Rb1) improves age-related myocardial dysfunction and to identify the relevant molecular mechanism. Young mice and aged mice were injected with Rb1 or vehicle for 3 months. Then, their cardiac function was inspected by transthoracic echocardiography. Serum and myocardium tissue were collected from all mice for histological or molecular expression analyses, including aging-related proteins, markers relevant to fibrosis and inflammation, and markers indicating the activation of the nuclear factor-kappa B (NF-[Formula: see text]B) pathway. Compared with the control condition, Rb1 treatment significantly increased the ejection fraction percentage and significantly decreased the internal diameter and volume of the left ventricle at the end-systolic and end-diastolic phases in aged mice. Rb1 treatment reduced collagen deposition and collagen I, collagen III, and transforming growth factor-[Formula: see text]1 protein expression levels in aged hearts. Rb1 also decreased the aging-induced myocardial inflammatory response, as measured by serum or myocardial interleukin-6 and tumor necrosis factor-[Formula: see text] levels. Furthermore, Rb1 treatment in aged mice increased cytoplasmic NF-[Formula: see text]B but decreased nuclear NF-[Formula: see text]B, which indicated the suppression of the NF-[Formula: see text]B signaling pathway by regulating the translocation of NF-[Formula: see text]B. Rb1 could alleviate aging-related myocardial dysfunction by suppressing fibrosis and inflammation, which is potentially associated with regulation of the NF-[Formula: see text]B signaling pathway.

RESUMO

New D-π-A configured organic sensitizers featuring halogen-substituted oxindole-bridged acceptor units have been synthesized for dye-sensitized solar cells applications. Among fluorine, bromine, and iodine substitution, the cell based on bromine incorporated dye exhibited the highest efficiency. The oxindoles in these sensitizers were found to assist the electron injection through the chelation of their amide carbonyl groups to the TiO2 surface. This study provides an alternate approach for future rational dye design to gain excellent DSSC performance.

Muscle-invasive urinary bladder urothelial carcinoma (UBUC) is a lethal disease for which effective prognostic markers and potential therapy targets are still lacking. Previous array comparative genomic hybridization identified that 3q27 is frequently amplified in muscle-invasive UBUCs, one candidate proto-oncogene, B-cell CLL/lymphoma 6 (BCL6), mapped to this region. We therefore aimed to explore its downstream targets and physiological roles in UBUC progression. Methods: Specimens from UBUC patients, NOD/SCID mice and several UBUC-derived cell lines were used to perform quantitative RT-PCR, fluorescence in situ hybridization immunohistochemistry, xenograft, gene stable overexpression/knockdown and a series of in vitro experiments. Results: Amplification of the BCL6 gene lead to upregulation of BCL6 mRNA and protein levels in a substantial set of advanced UBUCs. High BCL6 protein level significantly predicted poor disease-specific and metastasis-free survivals. Knockdown of the BCL6 gene in J82 cells inhibited tumor growth and enhanced apoptosis in the NOD/SCID xenograft model. In vitro experiments demonstrated that BCL6 inhibited cytostasis, induced cell migration, invasion along with alteration of the expression levels of several related regulators. At molecular level, BCL6 inhibited forkhead box O3 (FOXO3) transcription, subsequent translation and upregulation of phosphorylated/inactive FOXO3 through phosphoinositide 3-kinase (PI3K)/AKT serine/threonine kinase (AKT) and/or epidermal growth factor receptor (EGFR)/mitogen-activated protein kinase 1/2 (MAP2K1/2) signaling pathway(s). Two BCL6 binding sites on the proximal promoter region of the FOXO3 gene were confirmed. Conclusion: Overexpression of BCL6 served a poor prognostic factor in UBUC patients. In vivo and in vitro studies suggested that BCL6 functions as an oncogene through direct transrepression of the FOXO3 gene, downregulation and phosphorylation of the FOXO3 protein.

BACKGROUND@#Gastric cancer (GC) is one of the most globally prevalent cancers in the world. The pathogenesis of GC has not been fully elucidated, and there still lacks effective targeted therapeutics. The influence of altered kinesin superfamily protein 22 (KIF22) expression in GC progression is still unclearly. The aim of this study was to investigate the KIF22 effects on GC and related mechanisms.@*METHODS@#Gastric carcinoma tissues and matching non-cancerous tissues were collected from patients with GC who have accepted a radical gastrectomy in Lanzhou University Second Hospital from May 2013 to December 2014. The expression of KIF22 was examined in GC of 67 patients and 20 para-carcinoma tissues by immunochemical staining. The relationship between the expression of KIF22 and clinicopathologic characteristics was next investigated in the remaining 52 patients except for 15 patients who did not complete follow-up for 5 years. Cell viability was performed via 3-(4,5-dimethyl-2-thiazolyl)-2,5-diphenyl-2-H-tetrazolium bromide (MTT) test and colony formation assay in the MGC-803 and BGC-823 GC cells. Cell scratch and trans-well invasion assay was performed to assess migration ability in the MGC-803 and BGC-823 GC cells. Gene set enrichment analysis (GSEA) pathway enrichment analysis was performed to explore the potential functions. Cell cycle was detected by flow cytometry. In addition, the two GC cell lines were used to elucidate the underlying mechanism of KIF22 in GC in vitro via assessing the effects on mitogen-activated protein kinase and extracellular regulated protein kinases (MAPK/ERK) signal transduction pathway-related expressions by Western blotting assays. The differences were compared by t tests, one-way analysis of variance, and Chi-squared tests.@*RESULTS@#The study showed that KIF22 was up-regulated in GC, and KIF22 high expression was significantly related to differentiation degree (χ = 12.842, P = 0.002) and poorly overall survivals. GSEA pathway enrichment analysis showed that KIF22 was correlated with the cell cycle. Silence of KIF22 decreased the ability of the proliferation and migration in gastric cells, induced G1/S phase cell cycle arrest via regulating the MAPK-ERK pathways.@*CONCLUSIONS@#KIF22 protein level was negatively correlated with prognosis. KIF22 knockdown might inhibit proliferation and metastasis of GC cells via the MAPK-ERK signaling pathway.

In order to alleviate the "difficulty and annoyance in dispensing medicine" of patients with chronic diseases and promote hierarchical healthcare system, the policy of long-term prescription for chronic diseases has been implemented in the country. This article summarizes the current status and effectiveness of implementation of the long-term prescription, and explores the existing problems. The improvement of prescription formulation, review, deployment, distribution of drugs, health education and other issues to ensure the safety of treatment are discussed. Meanwhile, we suggest that the relevant supporting policies of medical insurance should be improved to further promote the long-term prescription.
