RESUMO

Pancreatic cancer is among the most malignant cancers, and thus early intervention is the key to better survival outcomes. However, no methods have been derived that can reliably identify early precursors of development into malignancy. Therefore, it is urgent to discover early molecular changes during pancreatic tumorigenesis. As aberrant glycosylation is closely associated with cancer progression, numerous efforts have been made to mine glycosylation changes as biomarkers for diagnosis; however, detailed glycoproteomic information, especially site-specific N-glycosylation changes in pancreatic cancer with and without drug treatment, needs to be further explored. Herein, we used comprehensive solid-phase chemoenzymatic glycoproteomics to analyze glycans, glycosites, and intact glycopeptides in pancreatic cancer cells and patient sera. The profiling of N-glycans in cancer cells revealed an increase in the secreted glycoproteins from the primary tumor of MIA PaCa-2 cells, whereas human sera, which contain many secreted glycoproteins, had significant changes of glycans at their specific glycosites. These results indicated the potential role for tumor-specific glycosylation as disease biomarkers. We also found that AMG-510, a small molecule inhibitor against Kirsten rat sarcoma viral oncogene homolog (KRAS) G12C mutation, profoundly reduced the glycosylation level in MIA PaCa-2 cells, suggesting that KRAS plays a role in the cellular glycosylation process, and thus glycosylation inhibition contributes to the anti-tumor effect of AMG-510.

RESUMO

Objective To investigate the effect of three-dimensional conformal radiotherapy (3D-CRT)combined with PC chemotherapy (paclitaxel + carboplatin)on non-small cell lung cancer (NSCLC) patients and the serum levels of CA125,tissue inhibitor of metalloproteinase-1 (TIMP-1),serum amyloid A (SAA)and T-lymphocyte subsets. Methods A total of 100 patients with NSCLC treated in Affiliated Hospital of Guangdong Medical University from May 2015 to December 2017 were selected as the study subjects. They were divided into control group and observation group according to random number table method,with 50 cases in each group. The observation group was treated with 3D-CRT combined with PC chemotherapy,while the con-trol group was treated with PC chemotherapy. The two groups were treated for 4 cycles. The therapeutic effect, serum CA125,TIMP-1,SAA,T-lymphocyte subsets and adverse reactions were compared between the two groups. Results Four cases were lost to follow-up both in the two groups. The overall response rate in the observation group (43. 48%,20 / 46)was higher than that in the control group (23. 91%,11 / 46;χ2 = 3. 941, P = 0. 047). The serum levels of CA125,TIMP-1 and SAA of the two groups had no significant difference be-fore treatment,and the levels of these indexes decreased after treatment. The serum levels of CA125,TIMP-1 and SAA in the observation group after treatment were (12. 31 ± 1. 13)U/ ml,(275. 31 ± 13. 69)pg/ ml and (47. 21 ± 7. 21)mg/ L,which were lower than those in the control group [(30. 36 ± 1. 98)U/ ml,(320. 36 ± 17. 23)pg/ ml,(65. 92 ± 8. 36)mg/ L],with significant differences (t = 53. 699,P < 0. 001;t = 13. 884, P < 0. 001;t = 11. 495,P < 0. 001). The levels of CD3 +,CD4 +,CD8 + and CD4 + / CD8 + of the two groups had no significant difference before treatment,and the levels of these indexes decreased after treatment. The levels of CD3 +,CD4 +,CD8 + and CD4 + / CD8 + in the observation group were (35. 27 ± 10. 31 )%, (20. 27 ± 6. 72)%,(15. 89 ± 3. 37)% and 0. 91 ± 0. 37,which were higher than those in the control group [(30. 77 ± 9. 27)%,(15. 27 ± 5. 73)%,(12. 02 ± 2. 69)% and 0. 75 ± 0. 39],with significant differences (t = 2. 201,P = 0. 030;t = 3. 840,P < 0. 001;t = 6. 087,P < 0. 001;t = 2. 019,P = 0. 047). There were no significant differences in the adverse reactions such as nausea and vomiting [63. 04% (29 / 46)vs. 43. 48%(20 / 46);χ2 = 3. 537,P = 0. 060],phlebitis [6. 52% (3 / 46)vs. 4. 35% (2 / 46);χ2 = 0. 000,P >0. 999],abnormal liver function [6. 52% (3 / 46)vs. 2. 17% (1 / 46);χ2 = 0. 261,P = 0. 609]and myelo-suppression [8. 70% (4 / 46)vs. 6. 52% (3 / 46);χ2 = 0. 000,P > 0. 999]between the observation group and the control group. Conclusion For patients with NSCLC,3D-CRT combined with PC chemotherapy can im-prove the overall response rate,decrease the levels of serum CA125,TIMP-1 and SAA,and improve the im-mune function of patients. The therapeutic effect is remarkable and the safety is good. The therapeutic scheme is suitable for the treatment of NSCLC.

The serum levels of CA125, TIMP-1 and SAA in the observation group after treatment were (12.31±1.13) U/ml, (275.31±13.69) pg/ml and (47.21±7.21) mg/L, which were lower than those in the control group [(30.36±1.98) U/ml, (320.36±17.23) pg/ml, (65.92±8.36) mg/L], with significant differences (t=53.699, P<0.001; t=13.884, P<0.001; t=11.495, P<0.001). The levels of CD3+ , CD4+ , CD8+ and CD4+ /CD8+ of the two groups had no significant difference before treatment, and the levels of these indexes decreased after treatment. The levels of CD3+ , CD4+ , CD8+ and CD4+ /CD8+ in the observation group were (35.27±10.31)%, (20.27±6.72)%, (15.89±3.37)% and 0.91±0.37, which were higher than those in the control group [(30.77±9.27)%, (15.27±5.73)%, (12.02±2.69)% and 0.75±0.39], with significant differences (t=2.201, P=0.030; t=3.840, P<0.001; t=6.087, P<0.001; t=2.019, P=0.047). There were no significant differences in the adverse reactions such as nausea and vomiting [63.04% (29/46) vs. 43.48% (20/46); χ2=3.537, P=0.060], phlebitis [6.52% (3/46) vs. 4.35% (2/46); χ2=0.000, P>0.999], abnormal liver function [6.52% (3/46) vs. 2.17% (1/46); χ2=0.261, P=0.609] and myelosuppression [8.70% (4/46) vs. 6.52% (3/46); χ2=0.000, P>0.999] between the observation group and the control group.@*Conclusion@#For patients with NSCLC, 3D-CRT combined with PC chemotherapy can improve the overall response rate, decrease the levels of serum CA125, TIMP-1 and SAA, and improve the immune function of patients. The therapeutic effect is remarkable and the safety is good. The therapeutic scheme is suitable for the treatment of NSCLC.

Objective To evaluate the diagnostic value of endoscopic ultrasonography (EUS), and explore the efficacy of endoscopic treatment in patients with esophageal submucosal tumor. Methods Sixty-eight patients with esophageal submucosal tumor were selected, and the tumor was derived from the muscularis mucosa and submucosa according to the common endoscope and endoscopic ultrasonography detection. Endoscopic mucosal resection (EMR) was applied to remove submucosal tumor with diameter less than 1.0 cm, endoscopic piecemeal mucosal resection (EPMR) or endoscopic submucosal dissection (ESD) was applied to remove submucosal tumor with diameter 1.1 - 1.5 cm, and ESD was applied to remove submucosal tumor bigger than 1.5 cm. Samples were examined by pathology after treatment. Results Tumors in all the patients were completely removed, and the tumor diameter was 0.6-2.3 cm. Forty-one cases were treated with EMR, 9 cases were treated with EPMR and 18 cases were treated with ESD. Four patients had intra-operative bleeding that was stopped by electrocoagulation hemostasis. No perforation occurred in all cases. Postoperative pathology revealed 43 cases had leiomyoma, 23 cases had interstitialoma, and 2 cases had lipoma. Patients were reviewed by gastroscope 3 months after operation. The white scars formed in all patients, and there was no residue or recurrence. Conclusions Different origin layers and property of esophageal submucosal tumor can be diagnosed accurately by EUS, and endoscopic therapy (EMR, EPMR and ESD) is an effective treatment for submucosal tumor from muscularis mucosa and submucosa. Endoscopic therapy is safe and effective. It provides sufficient pathological information.

RESUMO

Objective To study the effect of different insulin administration methods on glucose control of gestational diabetes mellitus ( GDM) .Methods Sixty-four cases with GDM were randomly divided into observation group and control group.The observation group received continuous subcutaneous insulin infusion insulin pump therapy, the control group received multiple subcutaneous insulin therapy.Then,the glucose control effect,SAS and SDS score were compared between two groups.Results After 4 weeks,the FBG,HbA1c,2h PG of observation group were significantly lower than the control group[(5.89 ±0.66)mmol/L vs (7.13 ±0.74)mmol/L,(7.14 ±0.82)% vs (8.92 ±0.95)%,(8.87 ±0.92)mmol/L vs (10.13 ±1.32)mmol/L](t=7.074,4.430,8.024,all P<0.05). The glucose fluctuations,insulin amount,hypoglycemia incidence of the observation group were significantly lower than the control group,blood sugar standard time was shorter than the control group[(5.64 ±0.65)mmol/L vs (8.44 ± 0.92)mmol/L,(0.52 ±0.06)U· kg-1· d-1 vs (0.73 ±0.09)U· kg-1· d-1,6.25% vs 28.13%,(4.76 ± 0.55)d vs (7.41 ±0.94)d,t/χ2 =14.061,10.983,5.379,13.764,all P<0.05)];SAS score,SDS score of the observation group were significantly lower than the control group[(34.95 ±4.2) points vs (40.23 ±3.98) points, (35.12 ±3.95)points vs (38.36 ±3.88)points,t=5.162,3.310,all P<0.05)].Conclusion Continuous subcu-taneous insulin infusion insulin pump therapy is helpful to control glucose levels,reduce blood sugar fluctuations and mitigate negative sentiment during treatment,it is an ideal insulin administration method for the treatment of GDM.

Objective To investigate the clinical effect of gastric bypass operation on treating type 2diabetes,and analyze the management status.Methods Fifty-seven patients wiith type 2 diabetes treated by gastric bypass operation were divided by different body mass index(BMI) into 4 groups:group A( BMI < 20kg/m2) with 8 cases,group B (20 kg/m2≤BMI <24 kg/m2) with 21 cases,group C (24 kg/m2≤BMI <27kg/m2) with 17 cases and group D (BMI ≥27 kg/m2) with 11 cases.The control of blood glucose and glycosylated hemoglobin,changes of diets and body weight,diabetes complications,adverse reactions after operation and postoperative management were followed up or reviewed.The clinical data were analyzed comprehensively.Results The cure rate of group C was the highest [ 70.6%( 12/17 ) ],while the cure rate of group A was the lowest (0).The overall cure rate was 43.9%(25/57) and the overall improvement rate was 28.1%(16/57).The overall efficient rate was 71.9%(41/57) and the overall inefficient rate was 28.1%(16/57).Apart from a few patients had malnutrition,amenorrhea,bad stomach,diarrhea and poor diet after leaving the hospital,other patients had no surgery complications and no patients died.There was no clinician to follow up and did education to these patients after operation.Conchusions Gastric bypass operation is safe and effective for obese and non-obese patients with type 2 diabetes.It is a new method to weat type 2diabetes at present,but the indications should be grasped and operation enlargement must he prevented.The management of teams of physicians,surgeons,dietitians,nurses should be established afler operation.

Objective To investigate the incidence and the possible risk factors of hepatic dysfunction in patients with Graves' disease (GD). Methods Totally 280 outpatients with hyperthyroidism (caused by GD) were divided in two groups: untreated group (131 patients) and treated group (149 patients). The hepatic function, serum thyroid hormone, TGAb and TMAb were detected for analysis of the incidence, clinical characteristics and correlative factors of the hepatic dysfunction. Results Patients who had at least one abnormal result of liver function test accounted for 70.99% in untreated group and 61.07% in treated group. The most common abnormal hepatic function parameter in patients with thyrotoxic hepatic lesion was increased alkaline phosphate enzyme (ALP), which was 57.3% in untreated group and 51.0% in treated group. Abnormalities of alanine transaminase (ALT), aspartate transaminase(AST), gamma-glutamyl tyanspeptidase (GGT) and total bilirubin (TBIL) were 28.2% (37/131), 18.3% (24/131), 10.7% (14/131) and 21.4% (28/131) in untreated group and 21.5%(32/149), 15.4%(23/148), 4.7% (7/149) and 20.1% (30/149) in treated group, respectively. The possibility of thyrotoxic hepatic dysfunction was correlated with the patients' age and FT3 and FT4 levels, but not with the genesis and course of disease, sex, family history of hyperthyroidism, or levels of TGAb and TMAb. Conclusion Abnormal results of liver function test are commom in patients with hyperthyroidism. Increased alkaline phosphate enzyme and alanine transaminase are the most common abnormal hepatic function parameters; the possibility of thyrotoxic hepatic dysfunction is correlated with patients' age and the level of thyroid hormones.

<p><b>OBJECTIVE</b>To investigate the role of apoptosis in radiation-induced mouse thymus lymphocyte damage and repair and provide the basis for understanding the molecular mechanism of radiation-induced lymphocyte damage and repair as well as the prevention and treatment of acute radiation sickness.</p><p><b>METHODS</b>We studied the dynamic changes of apoptosis of mouse thymus lymphocytes and the expression of bax and bcl-2 gene products after 2, 4, 6 and 8 Gy of whole body gamma-irradiation using in situ terminal labeling, DNA electrophoresis and immunohistochemical techniques.</p><p><b>RESULTS</b>At the early stage after irradiation, the percentage of apoptotic lymphocytes increased rapidly in accordance with the increasing of radiation doses, while the counts of the thymus and peripheral lymphocytes decreased sharply, showing an opposite change to lymphocyte apoptosis. After 6 Gy gamma-irradiation, typical morphological characteristics of thymus apoptotic lymphocytes in early, middle and late stages were found by transmission electron microscopy. The thymus lymphocytes displayed characteristic DNA ladders 4 hr and 8 hr after 2-6 Gy gamma-irradiation,using DNA gel electrophoresis techniques. Abnormal expression of bcl-2 and bax gene products were shown in irradiated lymphocytes.</p><p><b>CONCLUSIONS</b>Apoptosis plays an important role in the process of radiation-induced mouse thymus lymphocyte damage and repair. Bcl-2 and Bax proteins may regulate the process of lymphocyte apoptosis.</p>

<p><b>OBJECTIVE</b>To study the effect of human calcyclin binding protein (CacyBP) encoding gene on the development of multiple drug resistance in gastric cancer.</p><p><b>METHODS</b>hCacyBP sense nucleic acid eukaryotic expression vector (pcDNA3.1/hCacyBP +) was constructed and then transfected steadily into the gastric cancer drug sensitive cell (SGC7901) mediated by lipofectamine ( trade mark ) 2000. RT-PCR was used to measure the CacyBP mRNA expression level. MTT was used to measure the adriamycin (ADR) drug sensitivity of SGC7901 and SGC7901 after transfection. FCM was used to measure the average ADR accumulation concentration and cell cycle of SGC7901 and SGC7901 after transfection.</p><p><b>RESULTS</b>The hCacyBP mRNA expression level of SGC7901 transfected with pcDNA3.1/hCacyBP + was higher than SGC7901 transfected with pcDNA3.1 or SGC7901, with the higher survival rate in the former. The average ADR accumulation concentration in SGC7901 and SGC7901 transfected with pcDNA3.1 or pcDNA3.1/hCacyBP + was 5.64, 5.49 and 5.17, respectively. The G(1) phase cell proportion of SGC7901 transfected with pcDNA3.1/hCacyBP + or pcDNA3.1 was reduced slightly but G(2) and S phases increased slightly as compared with SGC7901.</p><p><b>CONCLUSION</b>Calcyclin binding protein may play a certain role in gastric cancer drug resistance.</p>

Objective To study changes and significance of endothelin(ET) in rat cerebral concussion.Methods 80 Wistar male rats were used for animal model of cerebral concussion,which were sacrificed on 1,3,7,14 and 30 days after injury and the brain tissue were taken off. The expression of ET was studied in the course of cerebral concussion by means of immunohistochemistry.Results Typical clinical manifestation was observed in the 100 g group in which the pathological changes included cerebral vascular constriction and dilatation,congestion and edema of cerebral tissue,neuronal degeneration,necrosis,and obviously decreased even disappeared Nissl bodies.Increased expression of ET was observed on the first day,the positive area was seen in the plasma of endothelial cells in cerebral cortex,hippocampus,cerebellum and thalamus.ET expression peak occurred on the 7th day,the positive area was also found in the plasma of Purkinje cells in the cerebellum.Decreased ET expression was found on 14th day and returned to normal level on the 30th day.Conclusion The main pathological changes of cerebral concussion contained blood circulation disorder,and degeneration and necrosis of substantial cells.ET was involved in the brain tissue injury during the pathological process of cerebral concussion and might be related to regulation of cerebral vascular reaction,and neuron degeneration and necrosis.

The basic nature of medical ethics education is the cultivation of morality for medical students,in which humanistic spirit is the fundamental element.Humanistic spirit is in great need for the development of medical ethics,the transformation of medical mode,the harmonious development of physician-patient relationship,and the healthy growth of medical students.Based on present realistic conditions,humanistic spirit should be developed in the fields of classroom training,medical professional education,and other aspects of campus culture.It would be necessary to find multi-path training modes of humanistic spirit.
