RESUMO

Mitochondrial dysfunction is a hallmark of cellular senescence and many age-related neurodegenerative diseases. We therefore investigated the relationship between mitochondrial function in peripheral blood cells and cerebral energy metabolites in young and older sex-matched, physically and mentally healthy volunteers. Cross-sectional observational study involving 65 young (26.0 ± 0.49 years) and 65 older (71.7 ± 0.71 years) women and men recruited. Cognitive health was evaluated using established psychometric methods (MMSE, CERAD). Blood samples were collected and analyzed, and fresh peripheral blood mononuclear cells (PBMCs) were isolated. Mitochondrial respiratory complex activity was measured using a Clarke electrode. Adenosine triphosphate (ATP) and citrate synthase activity (CS) were determined by bioluminescence and photometrically. N-aspartyl-aspartate (tNAA), ATP, creatine (Cr), and phosphocreatine (PCr) were quantified in brains using 1H- and 31P-magnetic resonance spectroscopic imaging (MRSI). Levels of insulin-like growth factor 1 (IGF-1) were determined using a radio-immune assay (RIA). Complex IV activity (CIV) (- 15%) and ATP levels (- 11%) were reduced in PBMCs isolated from older participants. Serum levels of IGF-1 were significantly reduced (- 34%) in older participants. Genes involved in mitochondrial activity, antioxidant mechanisms, and autophagy were unaffected by age. tNAA levels were reduced (- 5%), Cr (+ 11%), and PCr (+ 14%) levels were increased, and ATP levels were unchanged in the brains of older participants. Markers of energy metabolism in blood cells did not significantly correlate with energy metabolites in the brain. Age-related bioenergetic changes were detected in peripheral blood cells and the brains of healthy older people. However, mitochondrial function in peripheral blood cells does not reflect energy related metabolites in the brain. While ATP levels in PBMCs may be be a valid marker for age-related mitochondrial dysfunction in humans, cerebral ATP remained constant.

Introduction: Youth soccer academies are challenged with the constant recruitment process of young talented players to select those who will achieve long-term success as an athlete. Youth soccer academies strive to enhance the physical and technical skill development as well as personality development of talented players because psychological characteristics play a crucial role in players' future success in their transition to professional soccer. The least mature players and relatively young players may have a greater need to possess superior technical/tactical or psycho-behavioral skills than those relatively older counterparts because of the higher selection rates of early maturing players. Due to RAEs, a significant decrease in the overall quality of professional soccer teams might be observed because of the loss of talent of physically smaller, but psychologically stronger and more versatile relatively young players who possess proper technical and tactical attributes at an early age. The first objective of this study was to examine any possible relationship between RAE and personality constructs. A second objective was to identify factors and effects that can help in the further improvement of talent selection and equal opportunities for elite youth soccer players based on their underlying RAE. The third objective was to consider the impact of RAE on long-term career development. Methods: In this retrospective observational study, 151 elite youth soccer players between 15 and 18 years of age were first grouped in birth quartiles. Personality constructs were then assessed, using a combination of observations, interviews, and a self-assessment questionnaire. Next competition level after 8 years was evaluated to identify RAEs, differences in personality characteristics and opportunities to reach professional soccer player status between relatively older vs. younger players. Results: A clear significant RAE was observed for the whole database (Q1 = 38.4% vs. Q4 = 13.9%) with OR of 2.61 (χ2 = 19.46, p < 0.01, r = -0.85). Relatively young players had higher median scores on personality constructs such as self-confidence (p = 0.04), while relatively old players had higher median scores on personality constructs such as team orientation (p = 0.03). In the long term, more players of the youngest birth quartile were signed as professional players (76.2%), compared with relatively old players (46.6%). 65.0% of the 20 players had the highest total score on personality constructs developed as a professional soccer player, vs. 35.0% of the 20 players with the lowest scores. Discussion: In conclusion, this study showed not only further evidence of the RAE but also provided evidence supporting "the underdog hypothesis" in national elite youth teams. Relatively young players were also more likely to get higher value senior professional contracts in the long term. We propose that this may be due to the relatively young players developing superior psychological skills and technical expertise to compensate for their early physical disadvantage. This in turn suggests the need for greater awareness of the importance of personality constructs in the future development of youth elite soccer players. Therefore, the crux of the issue is how youth soccer academies elicit the "best of both worlds" ie. moderating RAE whilst also gaining the benefits of the underdog hypothesis by creating the right environment for every player to develop to their full potential in elite youth soccer academies.

The purpose of this study was to understand the contribution of each performance indicator to evaluate match performance of football referees. Thirty-four elite Referee Match Observers (RMOs) from the Portuguese FA participated voluntarily in the study. From the official assessment sheet of each game, the referee's game score was categorized in two groups according to referee's game score: i) Referees with a Low Score (LFS) and ii) Referees with High Score (HFS). A multivariable binary logistic regression model was used to assess the relationship between the Performance Indicators (PIs) of the dimensions i) game difficulty and game intervention and ii) disciplinary actions and game management in relation to the referee's match assessment score. The model proposed revealed that only the PIs negative application of the laws of the game and referee teamwork, from the dimension disciplinary actions and game management, significantly defined the final game referee's assessment score. This study suggests that the PIs scored by the RMOs contributed in a different way to the referee's game score. The model explains 60.4% of the referee's game score based on the variables referee teamwork and negative application of the laws of the game.

This study investigated the effects of 10 weeks of recreational football training on the leg-extensor force-velocity (F-V) profile in 55- to 70-year-old adults. Simultaneous effects on functional capacity, body composition and endurance exercise capacity were examined. Forty participants (age 63.5 ± 3.9 years; 36♂ 4♀) were randomized in a football training (FOOT, n = 20) and a control (CON, n = 20) group. FOOT performed 45-min to 1-h of football training sessions with small-sided games twice a week. Pre- and post-intervention assessments were performed. The results revealed a greater increase in maximal velocity (d = 0.62, pint = 0.043) in FOOT compared to CON. No interaction effects were found for maximal power and force (pint > 0.05). 10-m fast walk improved more (d = 1.39, pint < 0.001), 3-step stair ascent power (d = 0.73, pint = 0.053) and body fat percentage (d = 0.61, pint = 0.083) tended to improve more in FOOT than in CON. RPE and HR values at the highest speed level during a submaximal graded treadmill test decreased more in FOOT compared to CON (RPE: d = 0.96, pint = 0.005; HR: d = 1.07, pint = 0.004). Both the number of accelerations and decelerations as well as the distance spent in moderate- and high-speed zones increased markedly throughout the 10-week period (p < 0.05). Participants perceived the sessions as very enjoyable and feasible. In conclusion, recreational football training resulted in improved leg-extensor velocity production, which translated to a better performance on functional capacity tests that rely on a high execution velocity. Simultaneously, exercise tolerance was improved and body fat percentage tended to reduce. It appears that short-term recreational football training can induce broad-spectrum health benefits in 55- to 70-year-old adults with only 2 hours of training per week.

Feed additives used in finishing diets improve energy efficiency in ruminal fermentation, resulting in increased animal performance. However, there is no report evaluating the effect of BEO associated with exogenous α-amylase in response to increased starch content in feedlot diets. Our objective was to evaluate increasing levels of starch in the diet associated with a blend of essential oils plus amylase or sodium Monensin on performance, carcass characteristics, and ruminal and cecal morphometry of feedlot cattle. 210 Nellore bulls were used (initial body weight of 375 ± 13.25), where they were blocked and randomly allocated in 30 pens. The experiment was designed in completely randomized blocks in a 3 × 2 factorial arrangement: three starch levels (25, 35, and 45%), and two additives: a blend of essential oils plus α-amylase (BEO, 90 and 560 mg/kg of DM, respectively) or sodium Monensin (MON, 26 mg/kg DM). The animals were fed once a day at 08:00 ad libitum and underwent an adaptation period of 14 days. The diets consisted of sugarcane bagasse, ground corn, soybean hulls, cottonseed, soybean meal, mineral-vitamin core, and additives. The animals fed BEO35 had higher dry matter intake (P = 0.02) and daily weight gain (P = 0.02). The MON treatment improved feed efficiency (P = 0.02). The treatments BEO35 and BEO45 increased hot carcass weight (P < 0.01). Animals fed BEO presented greater carcass yield (P = 0.01), carcass gain (P < 0.01), rib eye area gain (P = 0.01), and final rib eye area (P = 0.02) when compared to MON. The MON25 treatment improved carcass gain efficiency (P = 0.01), final marbling (P = 0.04), and final subcutaneous fat thickness (P < 0.01). The use of MON reduced the fecal starch% (P < 0.01). Cattle-fed BEO increased rumen absorptive surface area (P = 0.05) and % ASA papilla area (P < 0.01). The MON treatment reduced the cecum lesions score (P = 0.02). Therefore, the use of BEO with 35 and 45% starch increases carcass production with similar biological efficiency as MON; and animals consuming MON25 improve feed efficiency and reduce lesions in the rumen and cecum.

The rating of perceived exertion method (RPE) allows to describe training intensity in a single value. To better understand the underlying components, the separate rating of perceived breathlessness (RPE-B) and leg-muscle exertion (RPE-L) has been proposed. Here we hypothesised that the separation between the two components may (partly) be determined by the impacts on the lower extremities. In this study, we aimed to experimentally evaluate the differential effect of high versus low impact running and jumping on RPE-B and RPE-L in team sport activities by manipulating the movement strategy (heel strike and passive landing pattern versus forefoot strike and active landing pattern). Eighteen recreational team sport players participated in two submaximal tests consisting of a sequence of running and jumping bouts, whilst ground reaction forces (GRF) were collected. RPE-B and RPE-L data were collected after each bout using the CR100 scale. Paired-samples t-tests were used to analyse between-session differences in these variables. GRF analysis showed that absorption mechanics differed considerably between the two sessions. RPE-L was on average 6.50 AU higher in the low impact session (p = 0.006). However, RPE-B was also increased by 4.96 AU with low impact (p = 0.009). We conclude that the extent to which the lower extremities are being exposed to high or low impacts does not explain a possible separation between the two RPE types.HighlightsThe separate rating of the different underlying components of RPE (e.g. variables related to the cardiorespiratory and the muscular system) may provide more insight in the relationship between training load and training outcomes, which likely differs between these components.The findings of this study do not support the idea that the separation in rating between perceived breathlessness (RPE-B, cardiorespiratory) and leg-muscle exertion (RPE-L, muscular) is also rooted in the extent to which musculoskeletal structures in the lower extremities are being exposed to high or low impacts.

Following the 15 January 2022 Hunga Tonga-Hunga Ha'apai eruption, several trace gases measured by the Aura Microwave Limb Sounder (MLS) displayed anomalous stratospheric values. Trajectories and radiance simulations confirm that the H2O, SO2, and HCl enhancements were injected by the eruption. In comparison with those from previous eruptions, the SO2 and HCl mass injections were unexceptional, although they reached higher altitudes. In contrast, the H2O injection was unprecedented in both magnitude (far exceeding any previous values in the 17-year MLS record) and altitude (penetrating into the mesosphere). We estimate the mass of H2O injected into the stratosphere to be 146 ± 5 Tg, or ∼10% of the stratospheric burden. It may take several years for the H2O plume to dissipate. This eruption could impact climate not through surface cooling due to sulfate aerosols, but rather through surface warming due to the radiative forcing from the excess stratospheric H2O.

PURPOSE: To examine the utility of a standardized small-sided game (SSG) for monitoring within-player changes in mean exercise heart rate (HRex) when compared with a submaximal interval shuttle-run test (ISRT). METHODS: Thirty-six elite youth football players (17 [1] y) took part in 6 test sessions across an in-season period (every 4 wk). Sessions consisted of the ISRT (20-m shuttles, 30″:15″ work:rest ratio, 70% maximal ISRT) followed by an SSG (7v7, 80 × 56 m, 6 min). HRex was collected during both protocols, with SSG external load measured as high-speed running distance (>19.8 km·h-1) and acceleration distance (>2 m·s-2). Data were analyzed using linear mixed-effect models. RESULTS: Controlling for SSG external load improved the model fit describing the SSG-ISRT HRex relationship (χ2 = 12.6, P = .002). When SSG high-speed running distance and SSG acceleration distance were held constant, a 1% point change in SSG HRex was associated with a 0.5% point change in ISRT HRex (90% CI: 0.4 to 0.6). Inversely, when SSG HRex was held constant, the effects of a 100-m change in SSG high-speed running distance and a 21-m change in SSG acceleration distance on ISRT HRex were -1.0% (-1.5 to -0.4) and -0.6% points (-1.1 to 0.0), respectively. CONCLUSIONS: An SSG can be used to track within-player changes in HRex for monitoring physiological state. Given the uncertainty in estimates, we advise to only give meaning to changes in SSG HRex >2% points. Additionally, we highlight the importance of considering external load when monitoring SSG HRex.

Context: Prediction of adult height (AH) is important in clinical management of short children. The conventional methods of Bayley-Pinneau (BP) or Roche-Wainer-Thissen (RWT) have limitations. Objective: We aimed to develop a set of algorithms for AH prediction in patients with idiopathic short stature (ISS) which are specific for combinations of predicting variables. Methods: Demographic and auxologic data were collected in childhood (1980s) and at AH (1990s). Data were collected by Dutch and German referral centers for pediatric endocrinology. A total of 292 subjects with ISS (219 male, 73 female) were enrolled. The population was randomly split into modeling (n = 235) and validation (n = 57) cohorts. Linear multi-regression analysis was performed with predicted AH (PAH) as response variable and combinations of chronological age (CA), baseline height, parental heights, relative bone age (BA/CA), birth weight, and sex as exploratory variables. Results: Ten models including different exploratory variables were selected with adjusted R² ranging from 0.84 to 0.78 and prediction errors from 3.16 to 3.68 cm. Applied to the validation cohort, mean residuals (PAH minus observed AH) ranged from -0.29 to -0.82 cm, while the conventional methods showed some overprediction (BP: +0.53 cm; RWT: +1.33 cm; projected AH: +3.81 cm). There was no significant trend of residuals with PAH or any exploratory variables, in contrast to BP and projected AH. Conclusion: This set of 10 multi-regression algorithms, developed specifically for children with ISS, provides a flexible tool for AH prediction with better accuracy than the conventional methods.

PURPOSE: To examine the utility of differential ratings of perceived exertion (dRPE) for monitoring internal intensity and load in association football. METHODS: Data were collected from 2 elite senior male football teams during 1 season (N = 55). External intensity and load data (duration × intensity) were collected during each training and match session using electronic performance and tracking systems. After each session, players rated their perceived breathlessness and leg-muscle exertion. Descriptive statistics were calculated to quantify how often players rated the 2 types of rating of perceived exertion differently (dRPEDIFF). In addition, the association between dRPEDIFF and external intensity and load was examined. First, the associations between single external variables and dRPEDIFF were analyzed using a mixed-effects logistic regression model. Second, the link between dRPEDIFF and session types with distinctive external profiles was examined using the Pearson chi-square test of independence. RESULTS: On average, players rated their session perceived breathlessness and leg-muscle exertion differently in 22% of the sessions (range: 0%-64%). Confidence limits for the effect of single external variables on dRPEDIFF spanned across largely positive and negative values for all variables, indicating no conclusive findings. The analysis based on session type indicated that players differentiated more often in matches and intense training sessions, but there was no pattern in the direction of differentiation. CONCLUSIONS: The findings of this study provide no evidence supporting the utility of dRPE for monitoring internal intensity and load in football.

The objective of this study was to examine the relationships among ruminal microbial community, rumen morphometrics, feeding behavior, feedlot performance, and carcass characteristics of Nellore cattle, classified by residual feed intake (RFI). Twenty-seven Nellore yearling bulls with an initial body weight (BW) of 423.84 ± 21.81 kg were fed in feedlot for 107 d in individual pens to determine the RFI phenotype. Bulls were categorized as high RFI (>0.5 SD above the mean, n = 8), medium RFI (±0.5 SD from the mean, n = 9), and low RFI (<0.5 SD below the mean, n = 10). At harvest, whole rumen content samples were collected from each bull to evaluate ruminal microbial community, including bacteria and protozoa. The carcass characteristics were determined by ultrasonography at the beginning and at the end of the experimental period, and behavior data were collected on d 88. As a result of ranking Nellore bulls by RFI, cattle from low-RFI group presented lesser daily dry matter intake (DMI), either in kilograms (p < 0.01) or as percentage of BW (p < 0.01) than high-RFI yearling bulls, resulting in improved gain:feed (G:F). However, variables, such as average daily gain (ADG), final BW, hot carcass weight (HCW) and other carcass characteristics did not differ (p > 0.05) across RFI groups. The eating rate of either dry matter (DM )(p = 0.04) or neutral detergent fiber (NDF) (p < 0.01) was slower in medium-RFI yearling bulls. For ruminal morphometrics an RFI effect was observed only on keratinized layer thickness, in which a thinner layer (p = 0.04) was observed in low-RFI Nellore yearling bulls. Likewise, Nellore yearling bulls classified by the RFI did not differ in terms of Shannon's diversity (p = 0.57) and Chao richness (p = 0.98). Our results suggest that the differences in feed efficiency of Nellore bulls differing in phenotypic RFI should be attributed to metabolic variables other than ruminal microorganisms and epithelium, and deserves further investigation.

Relative age selection bias persists within all major soccer leagues and youth soccer academies across the globe, with the relative age effect (RAE) being typically characterized as the over selection of relatively older players (who have sometimes also been shown to be early maturing). The aim of this study was to examine if a new allocation method (i) eliminates the RAE, and (ii) reduces the presence of any additional maturity-related differences in anthropometric and physical fitness characteristics which may exist between players within the same selection category. In the first phase, 1,003 academy soccer players [under (U) 9-16] from 23 UK professional soccer clubs were sampled and a clear RAE per birth quarter (Q) was observed for the overall sample (Q1 = 45.0% vs. Q4 = 9.8%) as well as for the different age categories. Using the newly suggested reallocation method, youth players were divided by allocation date which was defined as the midway point between the chronological age and the estimated developmental (ED) birthdate. Stature was used as an anthropometric reference point on the P50 of the growth curve to determine the developmental birthdate for this new method. After the reallocation of the players using ED, the distribution of players was more equally spread (Q1 = 25.3%, Q2 = 25.6%, Q3 = 22.4%, Q4 = 26.7%). After reallocation, the mean delta stature was reduced by 16.6 cm (from 40.3 ± 7.08 to 23.7 ± 4.09 cm, d = 2.87). The mean delta body mass difference after allocation was reduced by 6.7kg (from 33.2 ± 6.39 to 26.5 ± 4.85 kg, d = 1.18). The mean age difference increased from 1.8 to 3.9 years. A total of 42.7% of the sample would have been reallocated to a different age group compared to the current one. After reallocation, 45% of the anthropometric and physical fitness comparisons showed reductions in the within-group variation expressed as a percentage of coefficient of variation (CV%). The U10 players demonstrated the largest reduction in CV% (-7.6%) of the anthropometric characteristics. The U10 squad also showed the largest reduction in CV% for various physical fitness characteristics (-7.5%). By both eliminating the RAE and reducing temporary maturity-related anthropometric and physical fitness differences, soccer academies across the world may diversify and increase the size of the talent pool both for clubs and national youth teams. In conclusion, this study provides further evidence that the newly proposed allocation method shows the potential to remove the RAE and to create a more "leveled playing field" by reducing the within-group variation of anthropometric and physical fitness characteristics affording relatively younger, and eventually, late-maturing players the opportunity to develop their talent fairly.

Introduction: The ability to perform optimally under pressure is critical across many occupations, including the military, first responders, and competitive sport. Despite recognition that such performance depends on a range of cognitive factors, how common these factors are across performance domains remains unclear. The current study sought to integrate existing knowledge in the performance field in the form of a transdisciplinary expert consensus on the cognitive mechanisms that underlie performance under pressure. Methods: International experts were recruited from four performance domains [(i) Defense; (ii) Competitive Sport; (iii) Civilian High-stakes; and (iv) Performance Neuroscience]. Experts rated constructs from the Research Domain Criteria (RDoC) framework (and several expert-suggested constructs) across successive rounds, until all constructs reached consensus for inclusion or were eliminated. Finally, included constructs were ranked for their relative importance. Results: Sixty-eight experts completed the first Delphi round, with 94% of experts retained by the end of the Delphi process. The following 10 constructs reached consensus across all four panels (in order of overall ranking): (1) Attention; (2) Cognitive Control-Performance Monitoring; (3) Arousal and Regulatory Systems-Arousal; (4) Cognitive Control-Goal Selection, Updating, Representation, and Maintenance; (5) Cognitive Control-Response Selection and Inhibition/Suppression; (6) Working memory-Flexible Updating; (7) Working memory-Active Maintenance; (8) Perception and Understanding of Self-Self-knowledge; (9) Working memory-Interference Control, and (10) Expert-suggested-Shifting. Discussion: Our results identify a set of transdisciplinary neuroscience-informed constructs, validated through expert consensus. This expert consensus is critical to standardizing cognitive assessment and informing mechanism-targeted interventions in the broader field of human performance optimization.

While previous studies described these practices in elite men's football, no study has provided an overview of load monitoring practices in elite women's football. Given the clear organizational differences (i.e., professionalization and infrastructure) between men's and women's clubs, making inferences based on men's data is not appropriate. Therefore, this study aims to provide a first overview of the current load monitoring practices in elite women's football. Twenty-two elite European women's football clubs participated in a closed online survey (40% response rate). The survey consisted of 33 questions using multiple choice or Likert scales. The questions covered three topics; type of data collected and collection purpose, analysis methods, and staff member involvement. All 22 clubs collected data related to different load monitoring purposes, with 18 (82%), 21 (95%), and 22 (100%) clubs collecting external load, internal load, and training outcome data, respectively. Most respondents indicated that their club use training models and take into account multiple indicators to analyse and interpret the data. While sports-science staff members were most involved in the monitoring process, coaching, and sports-medicine staff members also contributed to the discussion of the data. Overall, the results of this study show that most elite women's clubs apply load monitoring practices extensively. Despite the organizational challenges compared to men's football, these observations indicate that women's clubs have a vested interest in load monitoring. We hope these findings encourage future developments within women's football.

Load monitoring is considered important to manage the physical training process in team sports such as Association Football. Previous studies have described the load monitoring practices of elite English football clubs and clubs with an established sports-science department. An examination of a broader international sample is currently not available. In addition, previous research has suggested factors that may improve the implementation of load monitoring practices, such as a strong club belief on the benefit of evidence-based practice (EBP) and high club financial resources. However, no study has examined yet the actual impact of these factors on the monitoring practices. Therefore, this study aims (1) to provide an overview of load monitoring practices in European elite football and (2) to provide insight into the differences in implementation between clubs by examining the impact of the club beliefs on the benefit of EBP and the club financial resources. An online survey, consisting of multiple choice and Likert scale questions, was distributed among sports-science and sports-medicine staff (n = 99, 50% response rate). Information was asked about the types of data collected, collection purposes, analysis methods, and staff involvement. The results indicated that external load data (e.g., global navigation satellite system, accelerometer…) was collected the most whilst respondents also indicated to collect internal load (e.g., heart rate, rating of perceived exertion…) and training outcome data (e.g., aerobic fitness, neuromuscular fatigue…) for multiple purposes. Considerable diversity in data analysis was observed suggesting that analysis is often limited to reporting the gathered data. Sports-science staff were responsible for data collection and analysis. Other staff were involved in data discussion to share decision-making. These practices were positively impacted by a stronger club belief on the benefit of EBP and greater financial resources. Creating an organizational culture, characterized by a strong belief on the benefit of EBP, is important to increase the impact of load monitoring. However, the actual potential may still be largely determined by financial resources. High-level clubs could therefore play a leading role in generating and sharing knowledge to improve training practices and player health.

CONTEXT: Recombinant human growth hormone (rhGH) is approved for treatment of pediatric growth hormone deficiency (GHD), with greatest growth responses observed in those with severe GHD. Orally administered GH secretagogues (GHS) may be useful treatment in patients with moderate GHD. Distinguishing children with severe vs moderate GHD could identify children who would be better treated with rhGH or GHS. OBJECTIVES: Evaluate baseline insulin-like growth factor-I (IGF-I) and stimulated peak GH response as predictors of 12-month height velocity (HV) in children with GHD. DESIGN: Data on children with GHD were analyzed in a legacy data base (GeNeSIS data). PARTICIPANTS: 514 naïve to rhGH-treatment, prepubertal children with idiopathic isolated GHD for whom stimulated GH, baseline serum IGF-I, and first-year HV during rhGH treatment data are available. OUTCOME MEASURES: Children with severe or moderate GHD were categorized based on GH and IGF-I data and evaluated based on baseline auxologic and hormone profiles and first-year growth response to rhGH. RESULTS: Cohorts of severe and moderate GHD were 81/514 (15.8%) and 433/514 (84.2%). Cohorts differed significantly with regard to indicators of GHD [eg, baseline height SD score (SDS), height SDS minus target height SDS, HV, HV SDS, and change in height SDS during rhGH treatment]. Multiple regression analysis showed IGF-I and stimulated GH were significant predictors of HV independent of other known variables. Expected first-year HV in moderate GHD was 8.3 cm/y. CONCLUSIONS: The combination of peak GH to GH stimulation testing and baseline IGF-I concentration are predictive enrichment markers for annualized HV responses to rhGH therapy.

CONTEXT: We hypothesize, based on the degree of residual hypothalamic-pituitary function, that some, but not all, children with growth hormone deficiency (GHD) may have beneficial growth responses to the orally administered growth hormone (GH) secretagogue LUM-201. OBJECTIVE: To determine if pretreatment testing can identify predictive enrichment markers (PEM) for subjects with adequate residual function who are responsive to LUM-201. METHODS: We performed an analysis of a completed, randomized, placebo-controlled trial of LUM-201, a GH secretagogue receptor agonist, in which all randomized subjects had pretreatment testing. This international multicenter study conducted in pediatric endocrinology clinics included 68 naïve-to-treatment, prepubertal children with established diagnoses of GHD. Outcome measures included the sensitivity, specificity, and predictive accuracy of potential markers to predict 6-month growth responses to oral LUM-201 and daily rhGH. RESULTS: Two PEM were identified for use in defining PEM-positive status: (1) baseline insulin-like growth factor I (IGF-I) concentration >30 ng/mL and (2) peak GH response of ≥5 ng/mL upon administration of single-dose LUM-201. PEM-positive status enriches a population for better growth responses to LUM-201. PEM-negative status enriches a population for better growth responses to rhGH. CONCLUSION: Combined, the peak GH response to single-dose LUM-201 and the baseline IGF-I concentration are effective PEMs for 6-month growth responses to LUM-201 and rhGH in prepubertal children with GHD.

BACKGROUND: Evidence from in vitro and rodent studies suggests that leptin, a key signal of long-term energy reserves, promotes IGF1 synthesis and linear growth. This effect of leptin has not been fully investigated in humans. The aim of our study was to investigate the effect of leptin substitution on growth factors and linear growth in children with congenital leptin deficiency (CLD). METHODS: In this cohort study we included eight pediatric patients (six males), age 0.9-14.8 years, who were diagnosed with CLD and received leptin substitution at our University Medical Center. We calculated standard deviation scores (SDS) for serum levels of IGF1 and IGFBP3, IGF1/IGFBP3 molar ratio, and height at baseline (T0) and 12 months (T12) after the initiation of substitution with metreleptin. RESULTS: All patients had severe obesity (BMI-SDS mean ± SD: 4.14 ± 1.51) at T0 and significant BMI-SDS reduction to 2.47 ± 1.05 at T12. At T0, all patients were taller than the mid-parental median, yet had low IGF1 and IGF1/IGFBP3 molar ratios (IGF1-SDS[Formula: see text]T0: -1.58 ± 0.92, IGF1/IGFBP3 molar ratio-SDS[Formula: see text]T0: -1.58 ± 0.88). At T12, IGF1-SDS increased significantly (∆T0-12: 1.63 ± 1.40, p = 0.01), and IGFBP3-SDS and IGF1/IGFBP3 molar ratio-SDS showed a trend toward an increase. In the three children within the childhood growth period (post-infancy, pre-puberty) height-SDS increased (∆height-SDST0-12: 0.57 ± 0.06, p = 0.003) despite substantial weight loss. CONCLUSIONS: These results in CLD patients are contrary to observations in children with idiopathic obesity who typically have above-mean IGF1 levels that decrease with weight loss, and therefore suggest that leptin increases IGF1 levels and promotes linear growth.

Despite various solutions proposed to solve the relative age effect (RAE), it is still a major problem confounding talent identification and selection processes. In the first phase, we sampled 302 under 7-21 academy soccer players from two Belgian professional soccer clubs to explore the potential of a new approach to solve the inequalities resulting from relative age- and maturity-related bias. This approach allocates players into four discrete quartile groups based on the midway point of their chronological and estimated developmental (ED) birth dates (calculated using the growth curves for stature of Belgian youth). With the use of chi square analyses, a RAE was found (p < 0.01) for the overall sample (Q1 = 41.4% vs. Q4 = 14.9%) that completely disappeared after reallocation (Q1 = 26.5%; Q2 = 21.9%; Q3 = 27.5%; Q4 = 24.2%). According to the new allocation method, the stature difference was reduced, on average, by 11.6 cm (from 24.0 ± 9.9 to 12.4 ± 3.4 cm, d = 1.57). Body mass difference between the two methods was 1.9 kg (20.1 ± 11.3-18.2 ± 13.1 kg, respectively, d = 0.15). The new method created a maximum chronological age difference of 1.9 vs. 0.8 years for the current method. With the use of this method, 47% of the players would be reallocated. Twenty-three percent would be moved up one age category, and 21% would be moved down. In the second phase, we also examined 80 UK academy soccer players to explore if reallocating players reduces the within-playing group variation of somatic and physical fitness characteristics. The percentage coefficient of variation (%CV) was reduced (0.2-10.1%) in 15 out of 20 metrics across U11-U16 age categories, with the U13 age category demonstrating the largest reductions (0.9-10.1%) in CV. The U12 and U13 age categories and associated reallocation groupings showed trivial to small (ES = 0.0-0.5) between-method differences and trivial to moderate (ES = 0.0-1.1) differences within the U14-U16 age categories. A reduction in RAE may lead to fewer dropouts and thus a larger player pool, which benefits, in turn, talent identification, selection, and development.

BACKGROUND: Perinatal stress has adverse effects on fetal outcome, yet the effect of early maternal trauma on fetal outcome has scarcely been studied. We investigated effects of maternal childhood trauma and current environment on important regulators of prenatal growth, fetal insulin-like growth factor (IGF)-1 and IGF-2 in amniotic fluid and assessed the impact of IGFs on newborn anthropometrics. METHODS: 79 pregnant women in their second trimester who underwent amniocentesis (15.9 ± 0.9 weeks of gestational age) and their newborns at birth were analyzed. Maternal childhood trauma was assessed using the childhood trauma questionnaire (CTQ) and current environment was operationalized by assessing maternal psychosocial, physical health and endocrine measurements in amniotic fluid. RESULTS: In this exploratory analysis of 79 pregnant women, maternal childhood trauma, defined as reporting at least low scores on any of the CTQ subscales, negatively correlated with fetal IGF-1 (Mln = 3.48 vs. 2.98; p = 0.012) and IGF-2 (Mdnln = 4.99 vs. 4.70; p = 0.002). Trauma severity, defined as the overall trauma score, negatively correlated with fetal IGF-2 (r = -0.24; p = 0.037). From trauma subscales, maternal sexual abuse correlated with fetal IGF-1 (r = -0.32; p = 0.006) and IGF-2 (r = -0.39; p = 0.001). Maternal BMI negatively correlated with fetal IGF-1 (r = -0.26; p = 0.023) and IGF-2 (r = -0.29; p = 0.011). Newborn anthropometrics were operationalized by length, weight, sex, gestational age, length/gestational age and weight/gestational age at birth. Fetal weight at birth associated with a trend with fetal IGF-1 when controlling for BMI. Maternal hypothalamus-pituitary-adrenal axis activity and maternal exercise did not contribute significantly to predicting fetal IGFs. Maternal childhood trauma (ß = -0.27; p = 0.011) and BMI (ß = -0.24; p = 0.026) remained significantly associated with fetal IGF-1. Maternal childhood trauma (ß = -0.32; p = 0.003), maternal BMI (ß = -0.30; p = 0.005) and maternal sexual abuse (ß = -0.22; p = 0.049) remained significantly associated with fetal IGF-2 and with a trend with fetal IGF-1 (ß = -0.21; p = 0.076) when excluding women with gestational diabetes. CONCLUSION: Maternal childhood trauma and BMI associate negatively with fetal IGF-1 and IGF-2 in amniotic fluid. Controlling for maternal BMI, fetal weight at birth remains associated with a trend with fetal IGF-1. The presented data suggests that childhood trauma can affect endocrine measurements of the developing next generation, providing a mechanism by which adverse maternal life events are transmitted to the next generation.
