BACKGROUND: Musculoskeletal injuries (MSI) in racehorses are commonly due to bone fatigue, a function of the number of cycles (strides) and the magnitude of load applied to the limb. These parameters can be estimated using speed and distance, with greater than 6000 m/month at a gallop (>14 m/s), in combination with canter distances greater than 44,000 m/month, reported to increase fracture risk. Despite their importance, there are limited data on the distances and speeds horses are exposed to during training. OBJECTIVES: Estimate training volume at different speeds undertaken by Australian Thoroughbred racehorses. STUDY DESIGN: Cross-sectional study. METHODS: Registered trainers (n = 66) in Victoria, Australia were surveyed. Questions were designed to assess the full training workload from initial pre-training to training performed to achieve and maintain race fitness, as well as information on rest periods. Descriptive analyses were stratified by trainer- and horse-level factors, with assessment of variance within and between groups. Long bone fractures in racehorses may present as stress fractures which have a good prognosis, or complete fractures, which often result in a fatal outcome. In order to identify differences in modifiable management practices that may contribute to these outcomes, racing histories of horses with humeral or tibial fractures and of matched controls were examined. A retrospective case-control study of Australian Thoroughbred racehorses diagnosed with a fracture of the humerus or tibia by scintigraphy or at post-mortem between 2002 and 2016 was undertaken. Control horses were matched from the same race or trial on age and sex. Statistical analysis was performed using conditional logistic regression, χ2 and Mann-Whitney U tests. More humeral fractures than tibial fractures were fatal (12/47, 26% vs. 3/35, 8.6%, P = 0.049). No differences in pre-injury racing histories were observed between cases and controls for humeral and tibial fractures. Both humeral and tibial fracture case horses were younger than the registered Thoroughbred racing population (P < 0.001), but horses sustaining humeral fractures were older than those with tibial fractures (3.3 ± 0.9 vs. 2.8 ± 0.8 years, P = 0.005) yet raced fewer times prior to the injury (0.5 ± 1.1 vs. 1.3 ± 1.7 races, P = 0.009). Horses with fatal humeral fractures were less likely to have raced than those with non-fatal humeral fractures (16.7% vs. 55.6%, P = 0.02). In conclusion, tibial and humeral fractures occur in young racehorses, and humeral fractures are more likely to be fatal in those with the least exposure to trialling and racing.

Studies of racehorse injury or fatality in various countries have identified common, and in some cases conflicting, risk factors. We conducted a systematic search of the relevant literature published from 1990 to 2017. Peer-reviewed articles were included if they reported the incidence of fatal or catastrophic musculoskeletal injury (CMI) in Thoroughbred flat races (n=21) or risk factors for CMI (n=65). Pooled effect sizes were estimated using the random-effects DerSimonian-Laird model. The pooled incidence of CMI was 1.17 (95% confidence interval 0.90, 1.44) per 1000 race starts. Almost 300 factors have been investigated in epidemiological studies for potential associations with CMI. Factors found to have consistent evidence of increasing risk of CMI are: (1) Horse-level factors such as older horse age and age at first start, male sex, and higher race class or lower claiming price; (2) Race-level factors such as firmer track conditions on turf and wetter conditions on dirt, longer race distance, and a greater number of starters; and (3) Management-related factors including more time since previous start, greater number of starts, longer career length, issues identified at pre-race examination, previous injury, and recent administration of medication or injections. Studies investigating recent cumulative distance of high-speed exercise were conflicting and suggestive of at least two mechanisms of injury related to the accumulation of bone damage: (1) In well adapted bone following a period of intense training; and (2) In poorly adapted bone at relatively low levels of training intensity. Future studies should evaluate success of interventions and mechanisms for injury.

BACKGROUND: Subchondral bone injury at the palmar/plantar aspect of the condyles of the third metacarpal/metatarsal bone (MC/MT3) commonly causes lameness and poor performance in racehorses. Injury occurs due to repetitive loading, the magnitude of which may be influenced by the position of the distal phalanx relative to the ground surface, i.e. the solar angle. The association of solar angle and injury at the palmar/plantar condyles of distal MC/MT3 therefore warrants investigation. OBJECTIVES: Investigate the relationship between solar angle and radiopharmaceutical uptake at the palmar/plantar aspect of distal MC/MT3 on scintigraphic images of racehorses. STUDY DESIGN: Retrospective case-controlled study. METHODS: Scintigraphic images of Thoroughbred racehorses presented for poor performance or lameness were graded for intensity of radiopharmaceutical uptake in the palmar/plantar aspect of distal MC/MT3. Solar angle was graded (positive, neutral or negative), referring to the angle of the solar plane of the distal phalanx relative to the ground surface. Repeatability of solar angle (n = 1226 limbs) and agreement with objective radiographic evaluation (n = 52 limbs) were evaluated. Prescintigraphy performance data were collected from race records. Associations between solar angle, performance and radiopharmaceutical uptake were investigated using multivariable logistic regression. RESULTS: Repeatability of scintigraphic solar angle grading (κ = 0.89, 95% CI 0.87-0.91) and agreement of scintigraphic and radiographic solar angle (κ = 0.88, 95% CI 0.75-0.97) were excellent. Horses that performed best prior to presentation were more likely to have both greater radiopharmaceutical uptake and a neutral/negative solar angle. When controlling for prior performance, horses with neutral/negative forelimb solar angle were twice as likely to have moderate/marked radiopharmaceutical uptake than horses with positive solar angle (P<0.02). Horses with negative hindlimb solar angle were six times more likely to have moderate/marked radiopharmaceutical uptake than horses with positive/neutral solar angle (P<0.001). MAIN LIMITATIONS: Population bias due to preselected hospital population. CONCLUSIONS: Both solar angle and race performance are independently associated with increased bone activity in the palmar/plantar aspect of the third metacarpal/tarsal condyles. BACKGROUND: Microdamage accumulation leads to subchondral bone injury and/or fracture in racehorses. An understanding of this process is essential for developing strategies for injury prevention. OBJECTIVES: To quantify subchondral bone microdamage in the third metacarpal bone of Thoroughbred racehorses at different stages of the training cycle. STUDY DESIGN: Cross-sectional. METHODS: Bone blocks from the palmar aspect of the medial condyles of third metacarpal bones from 46 racing Thoroughbred horses undergoing post-mortem were examined with micro computed tomography (microCT) to detect calcified microcracks, and light microscopy to quantify bulk stained microcracks. Racing and training histories were obtained for comparison with microdamage data using regression modelling. RESULTS: Subchondral bone microcracks were observed in all bones with at least one method. Microdamage grade was greater in older horses, levelling-off for horses 5 years and older (quadratic term P = 0.01), and with lower bone material density in the parasagittal groove (P = 0.02). Microcrack density was higher in older horses (P = 0.004), and with higher bone volume fraction (BV/TV) in the parasagittal groove in horses in training (interaction effect, P = 0.01) and lower in horses resting from training (P = 0.02). MAIN LIMITATIONS: Cross-sectional data only. Incomplete detection of microdamage due to the limits of resolution of microCT and lack of three-dimensional imaging with microscopy. Multicollinearity between variables that indicated career progression (e.g. age, number of career starts, duration of training period) was detected. CONCLUSIONS: Fatigue damage in the distal metacarpal subchondral bone is common in Thoroughbred racehorses undergoing post-mortem and appears to accumulate throughout a racing career. Reduced intensity or duration of training and racing and/or increased duration of rest periods may limit microdamage accumulation. Focal subchondral bone sclerosis indicates the presence of microdamage.

BACKGROUND: Palmar/plantar osteochondral disease (POD) and third metacarpal/-tarsal condylar fractures are considered fatigue injuries of subchondral bone (SCB) and calcified cartilage due to repetitive high loads in racehorses. In combination with adaptive changes in SCB in response to race training, the accumulation of SCB fatigue is likely to result in changes of joint surface mechanical properties. OBJECTIVES: To determine the spatial relationship and correlation of calcified articular surface biomechanical properties with SCB microstructure and training history in the distal palmar metacarpal condyle of Thoroughbred racehorses. STUDY DESIGN: Cross-sectional study. METHODS: Third metacarpal condyles were examined from 31 Thoroughbred horses with micro-computed tomography (microCT). Hyaline cartilage was removed and reference point indentation (RPI) mechanical testing of the calcified articular surface was performed. Training histories were obtained from trainers. The association among indentation distance increase (IDI, an inverse RPI measure of toughness), and microCT and training variables was assessed using a mixed-effects generalised linear model. RESULTS: Untrained horses had higher IDI than horses that had commenced training (P<0.001). Death as a result of musculoskeletal bone fatigue injury (P = 0.044) and presence of POD (P = 0.05) were associated with higher IDI. The microCT variables connectivity density and trabecular pattern factor were positively (P = 0.002) and negatively (P<0.001) correlated with IDI respectively. MAIN LIMITATIONS: The application of RPI to the calcified articular surface is novel and there is a potential for measurement variability with surface unevenness. CONCLUSION: Commencement of race training is associated with altered material properties of the calcified articular surface in horses. Reduced articular surface material properties can also be detected in horses that have fatigue injuries of the distal metacarpus and at other sites in the skeleton. Measures of SCB connectivity and trabecular surface shape may be more important determinants of resistance to failure of the calcified articular surface than traditional measures such as SCB volume and density.

OBJECTIVES: To investigate the prevalence of microscopic subchondral bone injury in the distal metacarpi/tarsi of Thoroughbred racehorses and associations with recent and cumulative training history. METHODS: Metacarpi/metatarsi were obtained from postmortem examination of Thoroughbred racehorses. The severity of palmar/plantar osteochondral disease (POD) was graded in forelimbs from 38 horses and in hindlimbs from a separate cohort of 45 horses. Forelimb samples were embedded in methyl methacrylate and examined using backscattered scanning electron microscopy. Microfracture density in the condylar subchondral bone was determined. Horizontal subchondral bone fractures were identified in hindlimb samples using sections of demineralised tissue. Empty osteocyte lacunae were quantified in hindlimb samples using sections of demineralised tissue. RESULTS: The prevalence of gross POD was 65.8% (95% confidence interval (CI) 48.7-80.4%) in the forelimb and 57.8% (95% CI 42.2-72.3%) in the hindlimb cohort of horses. Microfractures occurred in the forelimbs of 97.4% (95% CI 86.2-99.9%) of horses. Microfracture density in forelimbs increased with age (rs = 0.50, P = 0.001), the number of race starts (rs = 0.47, P = 0.003) and was greater in the medial condyles of horses in training than in those not in training (n = 21, median: 3.1/mm; range: 0.8-10.0 vs n = 17, 1.4/mm; 0-4.5, P = 0.008). Empty osteocyte lacunae were observed in the subchondral bone of hindlimbs in 97.7% (95% CI 88.0-99.9%) of 44 horses. CONCLUSIONS: Subchondral bone pathology occurs with a high prevalence in Thoroughbred racehorses presented for postmortem examination. The accumulation of subchondral bone damage with longer career duration is consistent with bone fatigue.

REASONS FOR PERFORMING STUDY: To gain a better understanding of the aetiology of articular surface collapse in horses with palmar osteochondral disease. OBJECTIVES: To determine whether acceleration of focal bone resorption associated with reduced physical activity contributes to articular surface collapse in racehorses with palmar osteochondral disease. STUDY DESIGN: Cross-sectional study comparing metacarpal bones from horses at varying stages of race training. METHODS: Metacarpal bones from 36 racing Thoroughbred horses were examined with high-resolution peripheral quantitative computed tomography to determine the proportion of the articular surface that had collapsed and with backscattered scanning electron microscopy to quantify porosity and eroded bone surface. Racing and training histories were obtained for comparison with imaging data. RESULTS: In 21 cases, inward collapse of the calcified cartilage layer was observed on backscattered scanning electron microscopy. An increased extent of articular surface collapse was associated with greater numbers of microfractures in the calcified cartilage and superficial subchondral bone (Spearman's correlation [rs ] = 0.62, P<0.001). In the deeper bone (6-10 mm), porosity was lower with a greater extent of articular surface collapse (rs = -0.38, P = 0.023), whereas in the superficial bone (0-4 mm) there was no association between articular surface collapse and porosity (rs = 0.19, P = 0.26). Both porosity (median 14, range 3.8-26 vs. 3.8, 1.6-17%, P = 0.008) and eroded surface (1.1, 0.74-4.5 vs. 0.64, 0.11-4.7 mm(-1) , P = 0.016) of the superficial subchondral bone were higher in resting than in training horses, and in some resting horses subchondral bone voids were highly concentrated, resulting in an apparent loss of support for the overlying calcified cartilage layer. CONCLUSIONS: Articular surface collapse is common in cases of palmar osteochondral disease and is likely to be a sequel to fatigue injury of subchondral bone. Focal subchondral bone resorption appears to contribute to collapse of the calcified cartilage and is potentiated by a reduced-intensity exercise regimen.

REASONS FOR PERFORMING STUDY: High-resolution 3D imaging may improve the prediction and/or early identification of condylar fractures of the distal metacarpus/tarsus and reduce the frequency of breakdown injury in racehorses. OBJECTIVES: To test the hypotheses that horses suffering condylar fractures have higher bone volume fraction (BV/TV) of the distal metacarpal epiphysis, greater subchondral bone thickness at the fracture site and higher second moment of inertia in the metacarpal midshaft as identified with high-resolution 3D imaging. STUDY DESIGN: Cross-sectional study using cadaver material. METHODS: Thoroughbreds that died on racetracks were grouped as: 1) horses with third metacarpal (McIII) fractures with a condylar component (cases, n = 13); 2) horses with no limb fracture (controls, n = 8); 3) horses with fractures in other bones or suspensory apparatus disruption (other fatal injuries, n = 16). The palmar condyles of McIII and the midshaft were examined with high resolution peripheral quantitative computed tomography (HR-pQCT). Statistical analysis included logistic regression and Spearman's correlation. RESULTS: There were no significant differences in BV/TV of distal McIII and second moment of inertia of the midshaft between cases and controls. Epiphyseal bone BV/TV was greater in injured limbs of horses with any fatal limb injury (Groups 1 and 3 combined) compared with controls (odds ratio = 1.20, 95% confidence interval 1.01-1.42, P = 0.034). An epiphyseal BV/TV>0.742 resulted in a sensitivity of 82.8% and specificity of 62.5% in identifying horses with fatal limb injury. In horses without condylar fracture, increased subchondral bone thickness was associated with palmar osteochondral disease lesions in the adjacent condyle (rs = 0.65, P<0.001). CONCLUSIONS: Increased BV/TV of the distal metacarpus may have some value for identifying horses at risk of any fatal breakdown injury but not metacarpal condylar fractures. Measurement of parasagittal groove subchondral bone thickness is complicated by adjacent palmar osteochondral disease lesions. Thus, high-resolution imaging of the distal metacarpus appears to have limited ability to identify horses at risk of condylar fractures.

Bone is repaired by remodelling, a process influenced by its loading environment. The aim of this study was to investigate the effect of a change in loading environment on bone remodelling by quantifying bone resorption and formation activity in the metacarpal subchondral bone in Thoroughbred racehorses. Sections of the palmar metacarpal condyles of horses in race training (n = 24) or resting from training (n = 24) were examined with light microscopy and back scattered scanning electron microscopy (BSEM). Bone area fraction, osteoid perimeter and eroded bone surface were measured within two regions of interest: (1) the lateral parasagittal groove (PS); (2) the lateral condylar subchondral bone (LC). BSEM variables were analysed for the effect of group, region and interaction with time since change in work status. The means ± SE are reported. For both regions of interest in the training compared to the resting group, eroded bone surface was lower (PS: 0.39 ± 0.06 vs. 0.65 ± 0.07 per mm, P = 0.010; LC: 0.24 ± 0.04 vs. 0.85 ± 0.10 per mm, P < 0.001) and in the parasagittal groove osteoid perimeter was higher (0.23 ± 0.04% vs. 0.12 ± 0.02%). Lower porosity was observed in the subchondral bone, reflected by a higher bone area fraction in the LC of the training group (90.8 ± 0.6%) compared to the resting group (85.3 ± 1.4%, P = 0.0010). Race training was associated with less bone resorption and more bone formation in the subchondral bone of highly loaded areas of the distal metacarpus limiting the replacement of fatigued bone. Periods of reduced intensity loading are important for facilitating subchondral bone repair in Thoroughbred racehorses.

A retrospective cohort study was performed to compare the rates of musculoskeletal injury (MSI) in horses receiving local corticosteroid injection (LCI) with those that were untreated and those prior to treatment. Of the 1911 study horses, 392 had been treated. A LCI was defined as any injection of corticosteroid into or adjacent to a synovial structure, muscle, or tendon/ligament. A MSI was defined as any limb injury identified by a veterinarian, following which the horse did not race for at least 6 months, or was retired. Hazard ratios (HR) comparing hazard of injury following injection to that in non-injected horses and prior to injection were calculated using Cox proportional hazards models. At least one LCI was administered to 392 horses (20.5%; median 2, range 1-16). Most LCIs were performed bilaterally (70.9%) and intra-articularly into the carpal (49.7%) or fore fetlock (29.3%) joints. There were 219 MSIs of which carpal injuries (47%), fore fetlock (22%) and forelimb tendon injuries (16%) were the most common. The incidence rate of MSI in untreated horses and those prior to injection was 1.22 (95% CI 1.04-1.44) injuries/100 horse-months, and following LCI the hazard of MSI was greater (HR 4.83, 3.54-6.61, P<0.001). The hazard ratio returned to levels indistinguishable from before treatment after 49 days. The hazard of MSI in horses following second and subsequent LCIs in the data collection period was greater than in horses following their first LCI (HR 2.10, 1.31-3.36, P=0.002). There was a positive association between LCI and subsequent musculoskeletal injury rates which was most likely due to progression of the musculoskeletal condition which prompted treatment. Assuming horses that received LCI were at increased risk of MSI subsequently, any beneficial effects of the LCI were insufficient to counter this increased risk for at least 49 days after the injection.

Musculoskeletal injuries are a common cause of lost training days and wastage in racehorses. Many bone injuries are a consequence of repeated high loading during fast work, resulting in chronic damage accumulation and material fatigue of bone. The highest joint loads occur in the fetlock, which is also the most common site of subchondral bone injury in racehorses. Microcracks in the subchondral bone at sites where intra-articular fractures and palmar osteochondral disease occur are similar to the fatigue damage detected experimentally after repeated loading of bone. Fatigue is a process that has undergone much study in material science in order to avoid catastrophic failure of engineering structures. The term 'fatigue life' refers to the numbers of cycles of loading that can be sustained before failure occurs. Fatigue life decreases exponentially with increasing load. This is important in horses as loads within the limb increase with increasing speed. Bone adapts to increased loading by modelling to maintain the strains within the bone at a safe level. Bone also repairs fatigued matrix through remodelling. Fatigue injuries develop when microdamage accumulates faster than remodelling can repair. Remodelling of the equine metacarpus is reduced during race training and accelerated during rest periods. The first phase of remodelling is bone resorption, which weakens the bone through increased porosity. A bone that is porous following a rest period may fail earlier than a fully adapted bone. Maximising bone adaptation is an important part of training young racehorses. However, even well-adapted bones accumulate microdamage and require ongoing remodelling. If remodelling inhibition at the extremes of training is unavoidable then the duration of exposure to high-speed work needs to be limited and appropriate rest periods instituted. Further research is warranted to elucidate the effect of fast-speed work and rest on bone damage accumulation and repair.

REASONS FOR PERFORMING STUDY: Radiography is commonly used in clinical practice but agreement of reporting of radiographically detected orthopaedic findings in horses has rarely been studied. OBJECTIVES: To assess agreement within and between observers for reporting of orthopaedic findings on presale radiographs of Thoroughbred yearlings. STUDY DESIGN: Retrospective analysis of archived radiographs. METHODS: Four veterinary radiology specialists each twice examined 167 sets of radiographs for orthopaedic findings in the fore feet, fore and hind fetlocks, carpi, tarsi and stifles. There were 27 findings analysed for agreement. Kappa statistic (κ), percentage of positive agreement (Ppos) and percentage of negative agreement are reported. RESULTS: An excellent percentage of negative agreement was observed for all findings, with the exception of regular vascular channels of the proximal sesamoid bones. Ppos and κ results were variable. The presence of extra carpal bones, osseous cyst-like lesions of the ulnar carpal bone, sagittal ridge defects of the third metacarpus, fracture of the fore and hind proximal sesamoid bones, regular vascular channels in the hind proximal sesamoids, osteochondrosis lesions of the distal intermediate ridge and/or medial malleoli of the tibia, and osseous cyst-like lesions in the medial femoral condyle was consistently observed with an intra- and interagreement κ≥0.5 and Ppos≥50%. Lucency within the proximal sesamoids consistently had an observed intra- and interagreement κ<0.4 and Ppos<40%. CONCLUSIONS: Observation of orthopaedic findings on yearling repository radiographs showed generally excellent agreement on the absence of findings, but variable agreement on the presence of findings. Agreement was good for larger and easy to categorise radiographic findings. More accurate definitions and training need to be developed to improve agreement within and between observers for orthopaedic findings with poor or fair to good agreement.

OBJECTIVE: To evaluate the long-term racing prognosis for Thoroughbred racehorses with displaced versus non-displaced fractures of the pelvis identified by scintigraphy. DESIGN: Retrospective case analysis. METHODS: Medical records of 31 Thoroughbred racehorses presenting to the University of Melbourne Equine Centre with fractures of the pelvis that were identified by scintigraphy were reviewed. Pelvic fracture site was determined and defined as displaced or non-displaced based on ultrasound and/or radiographic findings. Race records were analysed for each horse, with a minimum of 24 months' follow-up, and correlated with fracture type to determine long-term prognosis for racing. Results are expressed as median and range. RESULTS: Fractures at a single site were more common (n = 22) than fractures involving two sites (n = 9) and the ilial wing was the most commonly affected (n = 12). Thoroughbred racehorses with displaced pelvic fractures at any site (n = 12) raced fewer times within 24 months of diagnosis than horses with non-displaced fractures (n = 19) (median 0.5, range 0-13 vs 7, 0-24; P = 0.037), but there was no clear statistical difference in race earnings between the two groups (median A$0, range A$0-$123,250 vs A$14,440, A$0-$325,500, respectively; P = 0.080). Four horses with displaced fractures (33%) were euthanased on humane grounds because of persistent severe pain. When these horses were excluded from the analysis, there were no differences in performance variables between horses with a displaced or non-displaced pelvic fracture. CONCLUSION: Thoroughbred racehorses with a displaced or non-displaced pelvic fracture that survive the initial post-injury period have a good prognosis for racing.

UNLABELLED: Bone remodelling is inhibited by high repetitive loading. However, in subchondral bone of racehorses in training, eroded surface doubled in association with fatigue fracture and there was greater surrounding trabecular bone volume suggesting trabecular modelling unloads the bone focally, allowing damage repair by remodelling. INTRODUCTION: Remodelling replaces damaged bone with new bone but is suppressed during high magnitude repetitive loading when damage is most likely. However, in cortical bone of racehorses, at sites of fatigue fracture, focal porosity, consistent with remodelling, is observed in proportion to the extent of surrounding callus. Focal areas of porosity are also observed at sites of fatigue damage in subchondral bone. We hypothesised that fatigued subchondral bone, like damaged cortical bone, is remodelled focally in proportion to the modelling of surrounding trabecular bone. METHODS: Eroded and mineralizing surfaces and bone area were measured using backscattered scanning electron microscopy of post-mortem specimens of the distal third metacarpal bone in 11 racehorses with condylar fractures (cases) and eight racehorses in training without fractures (controls). RESULTS: Cases had a two-fold greater eroded surface per unit area at the fracture site than controls (0.81 ± 0.10 vs. 0.40 ± 0.12 mm(-1), P = 0.021) but not at an adjacent site (0.22 ± 0.09 vs. 0.30 ± 0.11 mm(-1), P = 0.59). Area fraction of surrounding trabecular bone was higher in cases than controls (81 ± 2 vs. 72 ± 2 %, P = 0.0020) and the eroded surface at the fracture site correlated with the surrounding trabecular area (adjusted R (2) = 0.63, P = 0.0010). CONCLUSION: In conclusion, exercise-induced inhibition of remodelling is offset at sites of fatigue fracture. Modelling of trabecular bone may contribute to unloading these regions, allowing repair by remodelling.

OBJECTIVES: To investigate associations between race performance and hoof-ground angle (HG), pastern-hoof angle (PH) and mediolateral hoof angle (ML) of the left forelimb. METHODS: A total of 1570 starts were recorded from 636 horses, and meetings were equally divided between sand and turf tracks. HG, PH and ML were measured on each starter in a race, over a 3-month period, using a Finnegan hoof gauge (Tallahesse Asia Holdings Ltd, Hong Kong) and digital photographic images. Race performance was recorded as win or place (1st-3rd). RESULTS: There was no association between the probability of winning or placing and gauge HG, photographic HG or photographic ML. There was a significant negative association between PH and the probability of winning on the turf track. On the turf surface, the odds ratio of winning for each 1° decrease in PH was 1.06 (95% confidence interval 1.01-1. 12). There was no association between the probability of winning or placing on the sand track and PH. CONCLUSIONS AND CLINICAL RELEVANCE: Direct measures of hoof conformation (HG and ML) were not related to racehorse performance in Macau. Lower PH was associated with better performance on a turf track. Further investigation into factors affecting PH and why it is related to performance are warranted. This study demonstrated that PH was associated with performance on a turf track. This information may be useful in the management and selection of Thoroughbred racehorses.

OBJECTIVE: To investigate predictors of survival and athletic function in adult horses with infection of a synovial structure. HYPOTHESES: Increasing duration from contamination to referral, bone or tendon involvement and positive microbial culture decreases short-term survival. Synovitis and/or sepsis at 5 days post-admission and involvement of Staphylococcus spp. decreases long-term athletic function. DESIGN: Retrospective study. METHODS: Records over 4 years of adult horses with synovial sepsis were reviewed. A two-tailed Fisher's exact test, Mann-Whitney U test or t-test was used to examine whether variables were predictive of short-term survival and long-term athletic function. RESULTS: During the study period 75 horses underwent treatment for infection of 93 synovial structures. Short-term survival was 84% (63/75) and 54% (30/56) of horses returned to athletic function. Of the variables measured at admission, duration from contamination to referral did not affect survival, whereas evidence of bone or tendon involvement decreased survival and athletic function. Of the variables available during treatment, abnormal synovial fluid at 4-6 days post-admission and positive microbial culture reduced athletic function. Staphylococcal infection was associated with persistent sepsis. CONCLUSIONS: Of the variables available at presentation, only evidence of bone or tendon involvement negatively affected survival and athletic function. During treatment of synovial sepsis, analysis of synovial fluid at 4-6 days and bacterial culture results have prognostic value.

REASONS FOR PERFORMING STUDY: The effect of palmar osteochondral disease lesions on performance of Thoroughbred (TB) racehorses is unclear. There is a need to describe patterns of radiopharmaceutical uptake (IRU) in fetlock joints of TB racehorses and to evaluate post scintigraphy performance. HYPOTHESES: IRU in the metacarpal (MC) and metatarsal (MT) condyles is more common than IRU in the parasagittal grooves and is associated with poorer post diagnosis performance than controls. METHODS: Location of IRU within the fetlock region was identified and graded subjectively in TB racehorses. Performance variables were determined from race records for horses with moderate/marked MC/MT condylar IRU (cases), other horses undergoing scintigraphy (scintigraphy controls) and age/sex matched controls from the last race in which a case participated (controls). Statistical analyses included quantile regression, Fisher's exact test, Kaplan-Meier survival curves, log-rank test and Cox regression. RESULTS: Metacarpal/MT condylar IRU was identified in 103/220 horses with only 3/220 having parasagittal IRU. Moderate/marked IRU was identified in the MC and MT condylar regions in 62 and 39 horses, respectively, with palmaromedial and plantarolateral IRU most common. Fore- and hindlimb cases had fewer starts, reduced earnings (P<0.001) and reduced earnings/start than controls respectively. Cases were more likely to return to racing later than controls following a rest period. Cases were older than scintigraphy controls. CONCLUSIONS: In TB racehorses presenting for lameness MC/MT condylar IRU is the most common abnormality identified within the fetlock joint. Racehorses with moderate/marked condylar IRU have a shortened racing career, are less successful than age/sex matched controls and are older than other racehorses presented for scintigraphy. POTENTIAL RELEVANCE: Overload of the MC/MT condyles is a common and significant problem in TB racehorses that is readily identified with scintigraphy. Scintigraphy of horses that are lame or performing poorly is less useful for screening for potential condylar fractures.

OBJECTIVE: To describe the clinical presentation, ultrasound findings, management and outcome in horses with muscle tear injuries of the hindlimbs. DESIGN: Retrospective case series Procedure Medical records of eight horses were reviewed and information on signalment, history, presenting complaint, physical examination findings and further diagnostic tests were recorded. Diagnosis of muscle injury was determined by the presence of abnormal ultrasound findings, compared with the contralateral limb, and, when required, nuclear scintigraphy. Follow-up information was obtained via telephone interviews with owners, trainers and referring veterinarians. RESULTS: Muscle tears causing lameness were identified in the middle gluteal (3), semitendinosus (1), semimembranosus (2) and gracilis (2) muscles. Tears were classified by ultrasound imaging as partial (6) or complete (2). The degree of lameness did not appear to be indicative of the extent of injury or of completion of healing. Long-term follow-up was available for seven horses and the outcome was favourable in six cases. CONCLUSIONS AND CLINICAL RELEVANCE: Ultrasonography is useful in the diagnosis and assessment of moderate to severe muscle strain injuries. The prognosis appears to be favourable in most cases, although recurrence of injury and lameness can delay the return to athletic activity and an inferior outcome with persistent gait abnormality may occur.

A 12-year-old show-jumping mare was presented for investigation of a chronic hindlimb lameness of 16 weeks duration. Perineural anaesthesia and ultrasonography localised the lesion to the medial collateral ligament of the distal interphalangeal joint of the left hindlimb. Treatment consisted of a heavily padded distal limb cast for 6 weeks, strict box rest and a strictly regulated hand walking program. Serial ultrasonographic examinations were performed throughout the rehabilitation period. Collateral ligament desmitis of the distal interphalangeal joint is a commonly diagnosed condition of the forelimb; however, hindlimb collateral ligament desmitis has been rarely reported. Ultrasonographic examination in this case was an invaluable diagnostic and follow-up tool. Immobilisation of the foot by means of a heavily padded distal limb cast allowed excellent healing of the ligament and is presented as a novel treatment for collateral ligament desmitis. At the time of writing the mare had returned to her previous level of exercise and was showing no signs of lameness.
