RESUMO

There is little published information on the health of young doctors, apart from a number of studies which show increased rates of psychiatric symptoms. Nor is there much known of their health behaviour. Anecdotal accounts suggest that doctors' own health care is poor, especially in terms of their willingness to consult other doctors. This paper presents data from a longitudinal study of a class cohort of young doctors first interviewed when they were students. Data show that they suffer from frequent minor physical ailments, with women reporting more ailments than men. Despite this, they took less sick leave. Overall, the doctors took very little time off work. Using the GHQ-28, with a threshold of 5/6, 30% of doctors fell into the "caseness" category for psychiatric symptoms. This is in keeping with findings elsewhere. From the doctors' own reported health behaviour, both in terms of their response to illness over the past year, as well as their predicted response to hypothetical illness, they have developed maladaptive patterns. These include continuing to go to work when unfit, self-prescribing, and consulting friends and colleagues rather than going for a formal consultation. This is seen as inappropriate, especially in cases of mental illness. A third of the young doctors are not registered with a local general practitioner and the majority have no clear idea of the role of the Occupational Health Service. The results are discussed in terms of the need to change attitudes to health care and to develop guidelines, staffing and services to enable doctors to take better care of themselves.

In a prospective Multi-Centre research study involving four British Adolescent Psychiatry Inpatient Units, all 276 adolescent admissions were diagnosed using both ICD-9 and DSM-III. Ratings of diagnostic confidence for each diagnostic group varied considerably. Clinicians recorded high levels of confidence in the diagnosis of both Schizophrenia and Anorexia nervosa, while considerably lower scores were noted for Adjustment Disorders (ICD-9 3-digit code 309.), and for Emotional Disorders (ICD-9 3-digit code 313.). Exact concordance in diagnoses made for each case using both ICD-9 and DSM-III occurred in 72% of the entire cohort. The dissimilar concordance rates comparing ICD-9 and DSM-III diagnoses for the same patient admissions suggest differences in validity measures, such as face validity and content validity, in the reliability of diagnostic criteria between similarly named disorders across the two systems, and in the clinician's training and attitude to diagnosis. With the recent introduction of DSM-IV following upon ICD-10, further comparative studies are needed to examine both reliability and validity issues into diagnosis and classification in child and adolescent psychiatry.

Body mass index (BMI) was compared with percentage body fat (%Fat) measured by dual energy X-ray absorptiometry (DXA) in 233 adolescent schoolgirl volunteers and 179 adult female patients. Repeat measurements were made on 67 of the adolescents and 51 of the adults. The correlations between BMI and %Fat were established from the 300 adolescent measurements and the 230 adult measurements. Although highly significant relationships were found between BMI and %Fat, only 58% of the variance in %Fat in adolescents and 66% in adults could be predicted by BMI. At the 95% confidence levels, a BMI of 20 kg m-2 can correspond to a range of 18-33% body fat in adolescents and 13-32% in adults. Without any change in BMI, an adolescent's percentage fat can change by as much as -3% to +7%. For an individual adult the same BMI can correspond to changes in fat of +/-5%. Since the strength of prediction of percentage body fat from BMI is poor, caution should be exercised in its use for eating disorders research.

To establish improved predictive values for normal bone mineral content (BMC) and density (BMD) in adolescent girls a community based study of schoolgirl volunteers was carried out by dual energy x ray absorptimetry. Measurements were performed on 216 subjects aged 11.0 to 17.9 years; measurements were repeated one year later on 84 of the girls, providing a total of 300 studies. For total body BMC the standard error of the prediction was improved from 290.9 g to 134.1 g when weight, height, and shoulder width were added to the normal variable of age. For spine BMD the standard error of the prediction was improved from 0.105 g/cm2 to 0.066 g/cm2 when height, weight, and shoulder width were added to the normal variable of age. Significant improvements were also obtained for total BMD and spine BMC. Despite the normal practice of predicting bone density from age alone this was not selected as the first variable in a multiple stepwise regression for either total body or spine. The prediction of BMC and BMD can be significantly improved by the inclusion of simple body habitus parameters in the prediction equations. As the mean (SE) z score derived from the manufacturer's normal data was - 0.36 (0.02) these American data are probably not appropriate for use in adolescents in the UK.

Sixteen Axis 5 ICD-10 codes were applied to a prospective multicentre study of 276 adolescent psychiatric inpatients. Frequency distributions were obtained for the whole cohort and separately for each of the seven main ICD-9 psychiatric diagnoses. Certain abnormal psychosocial situations were associated with particular diagnoses, and many significant correlations were found between Axis 5 codes. Factor analysis with varimax rotation produced four factors accounting for 46% of the total variance.

Prediction equations for percentage body fat have been derived for a group of 93 female patients being treated for eating disorders, using fat measured by dual-energy X-ray absorptiometry (DXA) as the reference method. Separate bioelectrical impedance analysis (BIA) prediction equations were derived for patients with a body mass index above and below 16 kg/m2. This provided an improvement compared with the equations supplied by the manufacturer. The standard error of the estimate (SEE) for the entire group was 3.2% (r = 0.922). However, an alternative prediction equation based on anthropometry alone gave an SEE of 4.2% (r = 0.861). The ability of BIA and anthropometry alone to measure changes in percentage fat was assessed in a separate group of 24 females during treatment for eating disorders. The SEEs using the BIA and anthropometry prediction equations were 2.6% (r = 0.931) and 2.7% (r = 0.901) respectively. We conclude that in patients with eating disorders BIA does not provide any significant improvement over simple anthropometry prediction equations for the assessment of percentage fat or its changes. The apparently reasonable agreement between fat predicted by BIA and that measured by DXA is largely due to the inclusion of body habitus parameters in the BIA prediction equations.

RESUMO

This paper describes the treatment of a boy with transsexual behaviour by means of family therapy supplemented by group therapy. It argues that a family systems approach can grasp dimensions of the problems of transsexualism that are missed if an exclusively individual treatment approach is adopted, and concludes that a family systems approach should be part of the assessment and treatment of all children and adolescents presenting with effeminacy or transsexual problems.

This study investigates whether three-year-old children whose mothers had been depressed after their birth showed more behaviour disturbance than children of mothers who were not depressed at that time. Ninety-one of 103 mothers who took part in an earlier prospective study of postnatal depression were reinterviewed three years later to determine their present mental state, and to assess their child's behaviour, using Richman's Behavioural Screening Questionnaire. No relationship was found between a prolonged postnatal depression and behaviour disturbance in the child, but children whose mothers had brief postnatal depressive episodes showed more behaviour disturbance than those whose mothers had not been depressed since childbirth.

Difficulties arising during the establishment of a clinical unit students' clerkship programme are presented. These are reviewed to establish general principles important for the successful introduction of such clerkships. Particular emphasis is laid on continual evaluation, and the results of one such evaluation are discussed.

A review of referrals to the University department of child psychiatry in Newcastle upon Tyne demonstrated active consultation and collaboration between the department and paediatric departments. Important differences in sex, distribution of psychiatric disorders and presence of associated physical and social handicap were found between patients referred from different sources. The presence of some psychological disturbance in those children referred from hospital wards was evident to most paediatricians, and constituted the most frequent reason for referral. Recommendation for psychiatric treatment was made for the majority of patients, but subsequent parental co-operation was often poor. In the authors' view the greatest value of this research is that it highlights atrange of disorders of common concern to paediatricians and children's psychiatrists, using not only the terminology traditional in child psychiatry but also employing a descriptive terminology. They believe it is essential that trainees of both disciplines should have adequate experience in this wide range of disorders.
