RESUMO

Objective:To investigate the effects of repetitive transcranial magnetic stimulation (rTMS) on behavioral and psychological symptoms and quality of life in patients with mild to moderate Alzheimer disease (AD).Methods:Forty mild and moderate patients who met the diagnostic criteria of AD in the tenth edition of the International Classification of Diseases from May 2017 to December 2018 in the Seventh People′s Hospital of Wenzhou City were selected and divided into the control group (20 cases) and the rTMS group (20 cases) according to random number list. The control group was treated with basic treatment while the rTMS group was treated with rTMS on the basis of the basic treatment. The scores of AD assessment scale-cognitive section (ADAS-cog), mini mental state examination (MMSE), neuropsychiatric inventory (NPI), activity daily living (ADL), and quality of life in AD (QOL-AD) were observed and compared between the two groups before and after treatment.Results:There were no significant differences in ADAS-cog, MMSE, NPI, ADL, and QOL-AD scores between the rTMS group and the control group before treatment ( P>0.05). After treatment, the MMSE, ADL, and QOL-AD scores of the rTMS group were significantly higher than those of the control group: (22.80 ± 3.83) scores vs.(20.30 ± 5.49) scores, (63.05 ± 17.24) scores vs. (54.15 ± 9.20) scpres, (37.55 ± 7.94) scores vs. (31.00 ± 11.45) scores; the ADAS-cog and NPI scores were significantly lower: (18.45 ± 4.16) scores vs.(22.15 ± 5.01) scores, (57.2 ± 16.25) scores vs. (72.65 ± 39.37) scores, (27.15 ± 7.53) scores vs. (34.65 ± 14.91) scores, and there were significantly differences ( P<0.05). At the same time, the MMSE, ADL, and QOL-AD scores of the rTMS group were significantly higher than those before treatment, the ADAS-cog and NPI scores were significantly lower, and the difference was statistically significant ( P<0.05). Conclusions:rTMS can effectively improve the behavioral and psychological symptoms of patients with mild to moderate AD and significantly improve the quality of life.

Objective@#To understand the characteristics of HIV/AIDS epidemic in Guangxi Zhuang Autonomous Region (Guangxi) with a purpose to accurately provide scientific basis for prevention and control measures, 2010-2017.@*Methods@#Data were retrieved from case reporting cards of Guangxi during 2010 to 2017 through National HIV/AIDS Comprehensive Response Information Management System. Data was analyzed using epidemiological methods such number of cases, proportion and rate. χ2 test was used for statistical analysis.@*Results@#The HIV positive rate was 12.53 per ten thousand (85 182/67 959 000) in Guangxi during 2010 to 2017. The number of newly diagnosed HIV/AIDS cases and the number of death yearly respectively increased by 22.34%(2 602/11 648) and 32.83% (952/2 900) in 2011 compared with 2010, and both showed a six-year continuous downward trend (the number of newly diagnosed cases respectively 12 229 cases, 10 877 cases, 9 460 cases, 9 190 cases, 8 848 cases, 8 680 cases, and the number of death respectively 3 888 cases, 3 316 cases, 2 914 cases, 2 717 cases, 2 595 cases, 2 600 cases) from 2012 to 2017. But proportion of late discovery remained above 50.00% (50.53%-57.06%) for eight-years continuously. The ratio of male and female was 2.47 ∶ 1 (60 639/24 543). The ratio of males and females aged 50 and over was 2.71∶1 (28 654/10 557). Proportion of the cases in 25-49 years old group and 50 years old group accounting for 47.40%(40 377/85 182) and 46.03% (39 211/85 182) respectively. The occupation was farmers accounting for 68.40% (58 262/85 182), housekeeping, housework and unemployment accounting for 11.21% (9 546/85 182), student accounting for 0.86% (729/85 182). Heterosexual transmission accounted for 90.60% (77 171/85 182, homosexual transmission accounted for 3.13% (2 669/85 182), injection drug use transmission accounted for 4.60%(3 924/85 182) and mother-to-child transmission accounted for 0.73% (619/85 182).@*Conclusions@#The number of newly diagnosed cases and the number of death yearly showed a continuous downtrend for six-years from 2012 to 2017. However, proportion of late discovery remained above 50.00% for eight-years. The major route of infection was heterosexual transmission. With the change of HIV/AIDS newly epidemic mode in Guangxi, there are many new challenges for HIV/AIDS prevention and control work. Strategy of targeted intervention modes should be innovated for a new breakthrough.

Objective To investigate the effects of concomitant administration oftryptophan on depression-related and anxiety-like behaviors of fluoxetine treated adult rats.Methods Sixty male Wistar rats were divided into the following groups by randomized block design:control group,L-tryptophan group,fluoxetine group,and experimental group (n=15);9 g/L sodium chloride injection (5 ml/kg),50 mg/kg L-tryptophan,10 mg/kg fluoxetine and both 50 mg/kg L-tryptophan and 10 mg/kg fluoxetine were,respectively,given to the rats in the above four groups via intragastric administration;depression models were established by 21 d-long-term chronic,medium and unpredictable stress stimulation.The modified forced swimming test (FST) was used to detect the times spent in immobility behavior,swimming behavior and climbing behavior;and elevated plus-maze (EPM) was employed to detect the times spent in open arms,closed arms and center area,and ratio of entries into open arms to the total entries;while tryptophan levels in cerebrospinal fluid were measured by high performance liquid chromatography.Results The times spent in immobility behavior,swimming behavior and climbing behavior showed significant differences among the four groups (P<0.05);both fluoxetine and tryptophan had significant influences in FST results.The times spent in open arms,closed arms and center area showed significant differences among the four groups (P<0.05);fluoxetine had significant influences in EPM results,and tryptophan had no significant influences in times spent in open arms,closed arms and center area;moreover,there was no significant interaction between tryptophan and fluoxetine treatments in parameters of FST and EPM.Significant difference of tryptophan level was noted among the four groups (F=6.874,P=0.002);that in the experimental group was significantly higher than that in the fluoxetine and control group (P<0.05).Conclusions Tryptophan can increase depressive-related behavior,but not alter anxiety-like behavior.Fluoxetine can decrease depression-related behavior,but induce anxiety.Concomitant use oftryptophan and fluoxetine does not alter anxiety-like behavior and the antidepressant effect offluoxetine is not enhanced.

Objective To investigate the expressions of CD133 and CD44 and their prognostic significance in gastrointestinal stromal tumors (GIST).Methods Streptavidin perosidase (SP) method of immunohistochemistry was used to detect expressions of CD133 and CD44 proteins in 42 cases of GIST, and the relationship between their expressions and tumor size, mitotic count were analyzed by univariate and multivariate factor analyses.Results The expressions of CD133 and CD44 proteins in GIST were 21.4% (9/42) and 78.6% (33/42), respectively.The expressions of CD133 and CD44 proteins were significantly correlated with tumor size and mitotic count (P < 0.05).Univariate factor analysis showed that the overall survival of GIST patients with positive CD133 protein (23.2 months) was shorter than that of patients with negative CD133 protein(63.1 months) (P < 0.05).The overall survival of GIST patients with negative CD44 protein (23.2 months) was shorter than that of patients with positive CD44 protein (63.3 months) (P < 0.05).Multivariate factor analysis showed that tumor size, mitotic count and CD44 protein were independent prognostic indicators for survival time after operation.Conclusions The positive expressions of CD133 and CD44 proteins might be the prognostic factors of GIST patients.

Objective To study the relationship between serum carbohydrate antigen 19-9 (CA19-9) and blood glucose level in patients with type 2 diabetes (T2DM).Methods Totally 784 T2DM patients and 197 healthy controls were enrolled in this study.Age,duration,body mass index (BMI),systolic blood pressure (SBP),diastolic blood pressure (DBP),levels of fasting blood glucose (FBG),postprandial blood glucose (PBG),hemoglobin A1c (HbA1c),total cholesterol (TC),triglyceride (TG),low-density lipoprotein cholesterol (LDL-C),high density lipoprotein cholesterol (HDL-C),blood uric acid,C peptide and CA 19-9 were recorded.Patients were divided into different groups according to the levels of HbA1 c and CA 19-9.The relationship between CA19-9 and influencing factors was analyzed.Results CA19-9 level was increased in T2DM patients compared with healthy controls [(22.08±14.94) U/mlvs.(10.24±6.31) U/ml,t=12.10,P<0.001].The levels of SBP,DBP,FBG,PBG,C peptide,TC,TG,LDL and UA were higher and HDL C level was lower in T2DM patients than in healthy controls (all P<0.05).The levels of FBG,PBG and CA19-9 were increased along with the increase of HbA1c level in T2MD patients with HbA1c level under 6.5%,T2MD patients with HbA1c level from 6.5% to 7.5%,T2MD patients with HbA1c level over 7.5% [FBG:(6.36±3.70) mmol/L,(9.01±4.01) mmol/L,(11.96±4.73) mmol/L;PBG:(9.69±3.21) mmol/L,(12.18±5.54) mmol/L,(15.27±5.08) mmol/L; CA19 9 (14.33±11.72) U/ml,(18.76±14.39) U/ml,(25.96±17.52) U/ml,respectively,all P<0.05].The levels of FBG,HbA1c and pancreatic cancer incidence were higher in patients with elevated CA19-9 than with normal CA19-9 [(11.71±5.48)mmol/L vs.(9.78±4.38)mmol/L,(7.49±4.12)% vs.(6.33±3.70)%,0% vs.7.8%,t=-3.51,2.55,x2=83.58,allP<0.05].Pearson correlation analysis showed that CA19-9 level was positively correlated with the levels of HbA 1 c and FBG in T2DM patients (r=0.281,0.134,both P<0.05).Multiple Logistic regression analysis showed that the levels of FBG and HbA1c were correlated with the increase of CA19 9 in T2DM patients(OR=0.801,0.947,P=0.019,0.026).Conclusions Serum CA19-9 level is significantly increased in T2DM patients compared with the healthy people.Serum CA19-9 level is correlated with blood glucose control.

ObjectiveTo investigate the relationship of lymphotoxin β receptor (LTβR) and classical nuclear factor-κB (NF-κB) activation pathway in the pathogenesis and progress of cystitis and bladder cancer.MethodsThe LTβR and P65 mRNA expression were detected by Real-time quantitative PCR in 108 cases of fresh bladder tissue specimens (75 cases of bladder cancer,10 cases of inflammation and 23 normal bladder mucosa cases grouped by the tissue classification ),and protein expression were analyzed by immunohistochemistry assay in 118 cases of paraffin-embedded biopsy specimens (73 cases of bladder cancer,30 cases of cysitis and 15 normal bladder mucosa cases).The correlation analysis between the expressions of LTβR and P65 with clinical pathological data was then performed.Differences between LTβR and P65 mRNA and protein expression level were compared in different groups of bladder tissues using Kruskal-Wallis H test and the Chi-square test.Results( 1 )The mRNA expressions of LTβR and NF-κB/P65were higher in bladder cancer than those in normal group ( LTβR:29.4 ( 14.2 - 46.7 ) × 10 - 3/1.2 ( 0.3 -7.0) ×10-3,Z=-5.508; P65:9.7 (2.7 -21.1) ×10-3/1.0(0.8 ~1.8) ×10-3,Z=-5.030,P<0.05 ).There were significantly differences between bladder cancer with different histological grades ( LTβR:18.2(2.1-31.3) × 10-3/ 28.4(16.6-36.2) × 10-3/47.9(34.3 -70.5) ×10-3,x2K-W=20.378;P65:4.9(1.3 - 12.0) × 10-3/7.4(3.0-21.9) × 10-3/17.0(10.0 ~28.3)× 10-3 ,x2K-W2 =15.219,P all <0.05) and lymph node metastasis (LTβR:27.2(9.7-40.1) ×10-3/39.4(26.7 -52.6) ×10-3,Z=-2.552; P65:7.4(2.3-15.6) ×10-3/13.4(6.7-23.3) ×10-3,Z=-2.026,P<0.05).(2)The positive rates of LTβR and phosphorylated P65 ( p-P65 ) protein in cancer were higber than those of normal group (LTβR:69.8%/13.3%,x2 =16.600 ; p-P65:56.2%/6.7%,x2 =12.220,P < 0.05 ).Upregulation of LTβR and p-P65 were associated with the histological grade (LTβR:56.3%/70.0%/90.4%,x2 =7.055; p-P65:40.6% /60.0%/76.2%,x2 =6.679,P <0.05) and with lymph node metastasis (LTβR:58.3%/92.0%,x2 =8.849; p-P65:52.1%/64.0%,x2 =5.088,P <0.05).(3)There was a positive correlation between LTβR and P65 expression ( mRNA:r =0.654,P < 0.05,protein:r =0.399,P < 0.05 )in the bladder cancer and cystitis (r =0.521,P<0.05).ConclusionsThe activation of LTβR and P65 was associated with progression and metastasis of bladder cancer.The activation of classical NF-κB pathway by LTβR may be achieved by P65.

Objective To investigate the effects and mechanism of Cordyceps sinensis on transplant arteriosclerosis in a rat model Methods Brown Norway aortic allografts were transplanted into Lewis recipient rats,and the recipient rats were randomly divided into four groups:group A,isograft control group,Lewis-Lewis (n =10); group B,allograft control group,BN-Lewis (n =10); groups C and D,allograft experimental groups,BN-Lewis (n =10).Rats were given normal saline via intragastric injection,once every day for 60 days in groups A and group B,and received different doses (1.5 and 3 g·kg-1 ·d 1) of Cordyceps sinensis in groups C and group D respectively.Grafts were harvested on the day 60 after transplantation. Intimal thickness was detected by HE staining.Protein was extracted from the abdominal aortas for Western blotting.Cellular localization was assessed by histology and immunohistochemistry.The serum was analyzed by an enzyme-linked immunosorbent assay (ELISA). Results Transplanted arteries were normal in group A.Transplanted arteries in group A had allograft vasculopathy,and intimal thickness was significantly increased.Transplanted arteries in allograft experimental groups had endometritis changes,and intimal thickness was significantly decreased as compared with that in group B (P < 0.05).Immunohistochemistry and Western blotting revealed that the expression levels of VEGF and PDGF-BB proteins in group A were significantly higher than in group B,and those in groups C and D were significantly reduced as compared with group B (P<0.05).ELISA showed that serum VEGF and PDGF BB concentrations in group B were significantly increased as compared with group A (P<0.05).Serum VEGF and PDGF-BB concentrations were significantly reduced in groups C and D as compared with group B (P<0.05).Conclusion Cordyceps sinensis could significantly inhibit the intimal hyperplasia,and delayed transplant arteriosclerosis caused by chronic rejection,which may be related to the down-regulated expression of VEGF and PDGF-BB.

Objective To evaluate the application value of combined detection of PCA3 and PSA mRNA in peripheral blood of patients with prostate cancer(PCa) for evaluation of mien)metastasis. Methods PCA3 and PSA mRNA were detected by duplex real time quantitative RT-PCR in a total of 49 PCa and 71 benign protatic hyperplasia (BPH) patients' peripheral blood. The diagnostic value was analyzed by receiver operative characteristic(ROC) curve. Results The levels of PCA3 mRNA in PCa patients were significantly higher than those in BPH patients [2 362( <30-7 421 ) copies/ml vs <30 copies/M, Z = -6. 66, P < 0. 01 ], and the same to PSA mRNA [3 425 ( 908-36 639 ) copies/ml vs < 200 copies/ml, Z = - 6. 40, P<0. 01 ]. The positive rate of PCA3 and PSA mRNA in peripheral blood was positively correlated with clinical stage[clinical stage B: 30.0% (3/10), C: 60.0% (9/15) and 86.7% (13/15), D: 91.7% (22/24) and 91.7% (22/24) ,Chi-square = 13. 534 and 16. 541, P <0. 01, respectively]. Meanwhile, the positive rate of PCA3 mRNA and PSA mRNA was also increased with the increase of Gleason score[ Gleason score of 2 to 4 : 20.0% (1/5) and 40. 0% (2/5) ;5 to 7 : 66.7% (12/18) and 72. 2% ( 13/18 ) ;8 to 10 : 84. 6% (22/26) and 92.3% ( 24/26 ) ;Chi-square = 8. 895 and 8. 015, P < 0. 05, respectively ]. ROC analysis showed that the sensitivities for PCA3 and PSA mRNA were 69. 4% (34/49) and 81.7% (40/49) and the specificities was 90. 1% (64/71) and 77.5% (55/71), respectively, when the cut-off value was 846 copies/ml for PCA3 mRNA and 280 copies/ml for PSA mRNA. Meanwhile, the sensitivity can reach to 85.7% (42/49) when the detection of PCA3 and PSA mRNA were combined. However, the specificity was decreased to 76. 1% (54/71). For the diagnosis of PCa micrometastasis, the sensitivity and specificity for PCA3 mRNA was 90.9% (20/22) and 84.7% (11/13), respectively. Conclusions PCA3 and PSA mRNA in peripheral blood are useful markers for PCa diagnosis. Simultaneous detection for PCA3 and PSA mRNA is more helpful for PCa diagnosis. Meanwhile, detection of PCA3 mRNA is a useful marker for diagnosing PCa micrometastasis.

Objective To explore hypermethylation of RARβ2, GSTP1 and DAPK gene in prostate cancer tissues, and to explore its correlation with clinicopathological features of prostate cancer and its diagnostic value. Methods Hypermethylation of RARe2, GSTP1 and DAPK gene was detected by nested methylation-specific PCR (NMSP) in 57 prostate cancer (PCa) tissues and 35 benign prostate hyperplasia (BPH) tissues. The correlation between hypermethylation and clinicopathological features of prostate cancer and its diagnostic value were analyzed. Results The hypermethylation frequencies of RARβ2, GSTP1 and DAPK gene in PCa were significantly higher than those in BPH (RARβ2: 52.6% vs. 0% GSTP1: 61.4% vs. 2.9%;DAPK: 43.9% vs. 8.6%;all P<0.01). The methylation rate of RARβ2 gene was directly correlated with distinct Gleason scores and clinical stage (4~7 score vs. 8~10 score: 34.8% vs. 64.7%;stage B, C vs. stage D: 37.0% vs. 66.7%;x2=4.927 and 5.004, P=0.026 and 0.025). The methylation rate of GSTP1 gene was significantly different in patients with different Gleason scores (4~7 score vs. 8~10 score: 43.5% vs. 73.5%;x2 =11.530, P=0.001), but had no difference in patients with distinct clinical stage (stage B, C vs. stage D: 51.9% vs. 70.0%;x2=1.975, P=0.16) . There was no difference in DAPK gene methylation rate among patients with different Gleason scores and distinct clinical stage (4 ~7 score vs. 8~10 score: 39.1% vs. 50.0%;stage B, C vs. stage D: 33.3% vs. 53.3%;x2= 1.290 and 2.309, both P~0.05). GSTP1 gene showed the highest sensitivity of 61.4% (35/57)with specificity of 97.1%(34/35), while RARβ2 gene had the highest specificity of 100% (35/35) with the sensitivity of 52.6% (30/57). The sensitivity and specificity of DAPK gene were 43.9% and 91.4% (25/57 and 32/35), respeetively. When the hypermethylation of RARβ2, GSTP1 and DAPK gene were detected together, the diagnostic sensitivity was increased, but the specificity was decreased. Conclusions The aberrant methylation of RARβ2, GSTP1 and DAPK gene is correlated with tumorigenesis and progression of prostate cancer, which may be used as an effective diagnostic marker for prostate cancer.
