RESUMO

Climate change has led to negative health outcomes and significant challenges in healthcare delivery, calling for a transformative approach to nursing curricula. To effectively address the adverse health impacts of climate change, it is imperative to equip future nurses with the necessary knowledge and competency. This can be accomplished by enhancing awareness among nurse educators, integrating climate change contents into nursing curricula, adopting inter- and multi-disciplinary approaches, nurturing nursing practice skills, and cultivating advocacy and leadership competencies. Implementation of these strategies in nursing education can nurture future nurses who can confront the health challenges associated with climate change, empowering them to advocate for sustainable nursing practice and public health policies related to mitigating the impact of climate change on health. This comprehensive, practical, and leadership-focused strategy in nursing education ensures that future nurses are well-prepared to effectively address health issues caused by climate change.

OBJECTIVES: To analyse the effectiveness of virtual reality (VR) distraction intervention for the management of dental anxiety in paediatric patients. DATA: Randomised controlled trials (RCTs) of VR distraction interventions for reducing anxiety in paediatric patients, published in English were included. SOURCES: Seven databases, including PubMed, Web of Science, Scopus, MEDLINE via ProQuest, EMBASE, CINAHL and Cochrane Central Register of Controlled Trials, covering the period between January 2000 and September 2022 were searched. STUDY SELECTION: A total of 12 RCTs involving 818 participants were included. Quality appraisal was undertaken using the Cochrane risk-of-bias tool for randomised trials by two authors independently. Random-effects model was used to summarise the effects of the interventions and pool data. CONCLUSIONS: Results showed that VR distraction interventions were effective in reducing the dental anxiety of paediatric patients. In meta-analysis, the VR distraction interventions had a significant effect on reducing paediatric patients' anxiety (SMD = -1.74, 95%CI = -2.46, -1.02, p < 0.001, I² = 95%), pain (SMD = -1.57, 95%CI = -2.22, -0.91, p < 0.001, I² = 91%) and heart rate (MD = -10.54, 95%CI = -20.26, -0.81, p = 0.03, I² = 99%) during dental treatment. However, the evidence of VR in managing dental anxiety would become weak because of the publication bias. CLINICAL SIGNIFICANCE: VR distraction interventions could be an effective approach to alleviate the dental anxiety of paediatric patients. Additional well-designed and high-quality RCTs with larger sample sizes are needed to determine the optimal way to deliver VR interventions in paediatric dental clinics.

BACKGROUND: Artificial Intelligence (AI) techniques are being applied in nursing and midwifery to improve decision-making, patient care and service delivery. However, an understanding of the real-world applications of AI across all domains of both professions is limited. OBJECTIVES: To synthesise literature on AI in nursing and midwifery. METHODS: CINAHL, Embase, PubMed and Scopus were searched using relevant terms. Titles, abstracts and full texts were screened against eligibility criteria. Data were extracted, analysed, and findings were presented in a descriptive summary. The PRISMA checklist guided the review conduct and reporting. RESULTS: One hundred and forty articles were included. Nurses' and midwives' involvement in AI varied, with some taking an active role in testing, using or evaluating AI-based technologies; however, many studies did not include either profession. AI was mainly applied in clinical practice to direct patient care (n = 115, 82.14%), with fewer studies focusing on administration and management (n = 21, 15.00%), or education (n = 4, 2.85%). Benefits reported were primarily potential as most studies trained and tested AI algorithms. Only a handful (n = 8, 7.14%) reported actual benefits when AI techniques were applied in real-world settings. Risks and limitations included poor quality datasets that could introduce bias, the need for clinical interpretation of AI-based results, privacy and trust issues, and inadequate AI expertise among the professions. CONCLUSION: Digital health datasets should be put in place to support the testing, use, and evaluation of AI in nursing and midwifery. Curricula need to be developed to educate the professions about AI, so they can lead and participate in these digital initiatives in healthcare. RELEVANCE FOR CLINICAL PRACTICE: Adult, paediatric, mental health and learning disability nurses, along with midwives should have a more active role in rigorous, interdisciplinary research evaluating AI-based technologies in professional practice to determine their clinical efficacy as well as their ethical, legal and social implications in healthcare.

With increasing air pollution, silica nanoparticles (SiNPs), as a main inorganic member of PM2.5, have gained increasing attention to its reproductive toxicity. Most existing studies focused on the acute exposure, while data regarding the chronic effect of SiNPs on reproduction is limited. Therefore, this study was designed to evaluate the chronic toxicity of SiNPs on spermatocyte cells. The cells were continuously exposed to SiNPs for 1, 10, 20 and 30 generations at dose of 5 µg/ml SiNPs for 24 h per generation after attachment. The results showed that with the increasing generations of the exposure, SiNPs decreased the viability of spermatocyte cells, induced apoptosis and increased the level of reactive oxygen species in spermatocyte cells. Moreover, SiNPs increased the protein expression of GRP-78, p-PERK, IRE1α, ATF6 and Cleaved caspase-3 in spermatocyte cells, suggesting that SiNPs improved unfolded protein response (UPR) and apoptosis. The present results indicated that the long-term and low-dose exposure to SiNPs could induce apoptosis by triggering ROS-mediated UPR in spermatocyte cells.
