PURPOSE: This study evaluated the long-term outcomes of endoscopic suturing and knotting-dacryocystorhinostomy (eSK-DCR) without the use of a stent or mitomycin C. METHODS: A prospective interventional case series was performed on patients with nasolacrimal duct obstruction (NLDO) who underwent eSK-DCR at Zhongshan Ophthalmic Center from October 2019 to December 2019. The surgeon sutured the lacrimal sac mucosa with the nasal mucosa by tying knots under endoscopic DCR. Subject demographics, preoperative data and postoperative data were collected, including clinical presentation, Munk score for epiphora, surgical indications, operation time, duration of knotting, number of knots, endoscopic ostium size, complications, and anatomical and functional success. Anatomic success was defined as patent ostium on lacrimal irrigation, and functional success was defined as subjective improvement in symptoms. Statistical analysis was performed by IBM SPSS software (Version 20.0; SPSS Inc., Chicago, IL, USA). RESULTS: A total of 60 patients (71 eyes) underwent pure eSK-DCR. Of these, 95.0% (57/60) were females. The mean age of the patients was 54.7 years. The mean surgical time was 37.60 min, and the average time for each knotting was 2.86 min. Endoscopic evidence found that all patients showed patent ostium and normal healing of the flaps after 4 weeks. The Munk scores dropped significantly at 6 months postoperatively compared to preoperative scores (P < 0.0001). Although 4 patients (7 eyes) were lost to follow-up at the end of the 2-year period, the anatomical and functional success remained stable during the 2-year follow-up period (anatomical, 100%; functional, 87.5%). No serious complications were detected during the follow-up period. CONCLUSION: Pure eSK-DCR is a simple and reliable therapeutic method for the management of NLDO. The surgical outcomes were good and remained stable beyond 6 months postoperatively.

BACKGROUND: The therapeutic effect of adriamycin (ADM) has been limited by chemoresistance in breast cancer (BC). Circular RNAs are involved in resistance regulation by mediating the miRNA/mRNA axis. Circ_0001667 enhanced ADM resistance via the miR-4458/NCOA3 axis in BC. This study was to investigate the other miRNA/mRNA network for circ_0001667. METHODS: The level detection of circ_0001667, microRNA-193a-5p (miR-193a-5p) or Ras-Related Protein 2a (Rap2A) was conducted by reverse transcription-quantitative polymerase chain reaction (RT-qPCR). Half inhibitory concentration (IC50) of ADM was detected through cell counting kit-8 (CCK-8) assay. The proliferation analysis was performed by colony formation assay and EdU assay. Flow cytometry was used for assessing apoptosis. Transwell assay was applied for examining cell migration and invasion. The protein detection was carried out by western blot. In vivo assay was performed using xenograft tumor model. Dual-luciferase reporter and RNA immunoprecipitation (RIP) assays were implemented to validate the target interaction. RESULTS: Circ_0001667 was highly expressed in ADM-resistant BC tissues and cells. Downregulation of circ_0001667 reduced ADM resistance and inhibited proliferation, migration, invasion in ADM-resistant BC cells. Tumor growth was repressed by circ_0001667 knockdown in ADM-resistant xenograft model. Circ_0001667 has induced the sponge effect on miR-193a-5p. The circ_0001667 function was partly achieved by targeting miR-193a-5p. Rap2A expression was positively regulated by circ_0001667 through sponging miR-193a-5p. The miR-193a-5p upregulation restrained chemoresistance and BC progression by the downregulation of Rap2A. CONCLUSION: All results unraveled that circ_0001667 contributed to ADM resistance and tumor development in BC via the miR-193a-5p-mediated Rap2A expression change, providing a novel regulatory mechanism for circ_0001667.

The presence of strong ambient vibrations could have a negative impact on applications such as high precision inertial navigation and tilt measurement due to the vibration rectification error (VRE) of the accelerometer. In this paper, we investigate the origins of the VRE using a self-developed MEMS accelerometer equipped with an area-variation-based capacitive displacement transducer. Our findings indicate that the second-order nonlinearity coefficient is dependent on the frequency but the VRE remains constant when the displacement amplitude of the excitation is maintained at a constant level. This frequency dependence of nonlinearity is a result of several factors coupling with each other during signal conversion from acceleration to electrical output signal. These factors include the amplification of the proof mass's amplitude as the excitation frequency approaches resonance, the nonlinearity in capacitance-displacement conversion at larger displacements caused by the fringing effect, and the offset of the mechanical suspension's equilibrium point from the null position of the differential capacitance electrodes. Through displacement transducer and damping optimization, the second-order nonlinearity coefficient is greatly reduced from mg/g2 to µg/g2.

OBJECTIVE: The aim of this study was to analyze the pharmacokinetics/pharmacodynamics (PK/PD) of higher-dose tigecycline (100 mg q12h) in severely infected intensive care unit (ICU) patients receiving continuous renal replacement therapy (CRRT). MATERIALS AND METHODS: In this prospective single-center observational study, severely infected patients receiving intravenous tigecycline were enrolled. They were divided into a CRRT group (7 cases) and a non-CRRT group (9 cases). The blood samples and CRRT ultrafiltrate were collected. The drug concentration in each sample was determined by a HPLC-UV method. The pharmacokinetic parameters were simulated and calculated with DAS 2.0. The PK/PD parameters were evaluated according to published data. The registration number of this study is NCT02931526 in ClinicalTrials.gov. RESULTS: In the non-CRRT group, Cmax, Cmin, and AUC0-24 were 1.00 ± 0.66 µg×mL-1, 0.20 ± 0.12 µg×mL-1, and 22.12 ± 14.46 µg×h×mL-1, respectively. The clinical efficiency was 55.6%, and the bacterial clearance rate was 77.8%. In the CRRT group, Cmax, Cmin, and AUC0-24 were 0.96 ± 0.31 µg×mL-1, 0.22 ± 0.12 µg×mL-1, and 19.90 ± 8.14 µg×h×mL-1, respectively. The clinical efficiency was 28.6%, and the bacterial clearance rate was 28.6%. The individual differences of tigecycline plasma concentrations in our study were widely variable, and the differences of the two groups' PK/PD parameters had no statistical significance (p < 0.05). CONCLUSION: CRRT may have had little influence in tigecycline metabolism in our study, and therapeutic drug monitoring needs to be introduced for critically ill patients because of various pharmacokinetic parameters.

PURPOSE: To investigate whether subconjunctival bevacizumab help prevent corneal graft neovascularization and prolong the graft survival of patients with chemical burns. METHODS: We performed a prospective nonrandomized comparative case series study. Twenty-six eyes received subconjunctival bevacizumab (10 mg/0.4 mL) once and topical immunosuppressive agents after sclerocorneal lamellar keratoplasty as the treatment, and 13 eyes received a topical immunosuppressant alone and served as the control group. The main outcomes were a cumulative probability of graft survival, development of corneal neovascularization, and complications. RESULTS: The postoperative follow-up time was 14.3 months (range, 2-62 mo). The cumulative graft survival time was significantly longer in the treatment group than that in the control group (42.9 ± 5.9 vs. 4.8 ± 0.7 mo; log rank < 0.001). In the treatment group, 19 of the 26 grafts (73.1%) survived as transparent with a mean follow-up of 18.7 ± 3.0 months. At the end of the follow-up, 4 grafts remained free of neovascularization, 2 developed edema without neovascularization, and 15 remained transparent with a stable ocular surface and some neovascular vessels in the peripheral transplant interface. The other 5 grafts became opaque and neovascularized. In the control group, all grafts became opaque and neovascularized within the follow-up period (5.5 ± 0.7 mo). During the follow-up, a corneal epithelial defect developed in 9 eyes in the treatment group and 7 in the control group. CONCLUSIONS: Early application of subconjunctival bevacizumab after sclerocorneal lamellar keratoplasty can significantly prevent corneal neovascularization and promote graft survival for severe late-stage ocular chemical burns.

This paper reports the performance of a torsion-type thrust stand suitable for studies of micro-Newton thrusters, which is developed for ground testing the micro-Newton thruster in Chinese Test of the Equivalence Principle with Optical readout space mission. By virtue of specially suspending design and precise assembly of torsion balance configuration, the thrust stand with load capacity up to several kilograms is able to measure the impulse bit up to 1350 µNs with a resolution of 0.47 µNs, and the average thrust up to 264 µN with a resolution of 0.09 µN in both open and close loop operation. A pulsed plasma thruster, the preliminary prototype developed for Chinese TEPO space mission, is tested by the thrust stand, and the results reveal that the average impulse bit per pulse is measured to be 58.4 µNs with a repeatability of about 5%.
