RESUMO

Objective:To investigate the mechanism of Wnt5a in lentoid body (LB) induction from human embryonic stem cells (hESCs).Methods:A "three-stage" protocol was used for LB differentiation from hESCs in vitro, and Wnt5a level was modified by adding exogenous 500 ng/ml Wnt5a on day 18 as Wnt5a treatment group.Cells of control group and Wnt5a treatment group were collected on day 35.Cells were photographed by using the Zeiss Axio Observer Z1 microscope.Transcriptome sequencing was applied by Illumina.Genes with P value≤0.05 and fold change≥1.5 were identified as differentially expressed genes (DEGs). Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analysis were used to determine the biological functions of DEGs. Results:Compared with the control group, larger lentoid bodies were obtained in the Wnt5a treatment group.Transcriptome sequencing result showed that 478 genes were down-regulated and 201 genes were up-regulated in the Wnt5a treatment group compared with the control group, and Wnt5a up-regulated both lens cell differentiation and lens specific gene expression.Bioinformatics analysis result showed that most DEGs were involved in extracellular matrix remodeling, suggesting that Wnt5a regulated extracellular matrix remodeling during lens cell differentiation.The enrichment analysis result also showed that epithelial-to-mesenchymal transformation related processes were inhibited after Wnt5a treatment, suggesting that Wnt5a inhibited the abnormal differentiation of lens cells (especially lens epithelial cells) during lens cell differentiation.Wnt5a influenced the processes related to cytoskeleton remodeling.Conclusions:Wnt5a may act in lens cells through MAPK/ERK signaling pathways to affect ECM and cell cytoskeletal organization, which provides a new direction for studying lens development.

Objective@#To investigate the relationship between duration of sleep and all-cause mortality in patients with type 2 diabetes.@*Methods@#A total of 17 452 type 2 diabetic patients who were under the National Basic Public Health Service Project, in Changshu county, Qinghe district and Huai’an district in Huai’an city of Jiangsu province, were recruited as participants. Cox proportional hazards regression models were used to estimate the associations between different levels on the duration of sleep and all-cause mortality. Stratified analysis was performed according to factors as gender, age, and lifestyle.@*Results@#Among the 67 912 person-years of observation, from the fo1low- up program, the median time of follow-up was 4 years, with 1 057 deaths occurred during the follow-up period. Taking patients with duration of sleep as 7 h/d for reference, the multivariate adjusted hazard ratios (95%CI) of all-cause mortality appeared as ≤6, 8, 9 and ≥10 h/d were 1.14 (0.94-1.37), 1.10 (0.91-1.32), 1.33 (1.05-1.70), and 1.52 (1.24-1.87), respectively which were associated with the duration of sleep. Data from the subgroup analysis showed that this was consistent with the whole population. Longer duration of sleep was associated with the all-cause mortality but no significant association was found between shorter duration of sleep and the all-cause mortality.@*Conclusion@#Longer duration of sleep seemed to have increased the risk of all-cause mortality in type 2 diabetic patients.

Objective@#To analyze the attributable burden of smoking on the deaths of cardio-cerebral vascular diseases (CCVD) in inhabitants aged 30 years and above in Jiangsu Province.@*Methods@#Comparative risk assessment approach in Global Burden of Disease (GBD) was used with the data from Jiangsu Non-communicable Disease and Behavioral Risk Factor Surveillance System and Death Monitoring Surveillance System in 2013, and population attributable fraction (PAF), years of life lost (YLL), work of potential years of life lost, index of life lost and life loss attributed to smoking were calculated.@*Results@#In 2013, there were 162 158 CCVD deaths aged 30 years and above in the surveillance areas of Jiangsu Province, of which 25 102 deaths were attributed to smoking (PAF: 15.48%, attributed mortality rate: 55.13/100 000). The YLL, work of potential years of life lost and index of life lost were 532 494 person years, 78 024 person years and 9.15 years respectively. The decline of life expectancy was 0.58 years. The PAF of CCVD attributed to smoking in male and female were 27.97% and 3.18%, respectively, and the mortality rate of cardio-cerebral diseases attributed to smoking in male and female were 100.13/100 000 and 11.27/100 000, respectively. The burden of ischemic heart disease and hemorrhagic stroke were most severe, with standardized YLL rate were 3.65‰ and 3.20‰, respectively.@*Conclusion@#Smoking caused a great burden of cardio-cerebral disease deaths in inhabitants in Jiangsu province.

Objective@#To evaluate the risk of 10-year risk of ischemic cardiovascular disease (ICVD) in patients with type 2 diabetes aged 35 years old and above in two cities of Jiangsu province.@*Methods@#From December 2013 to January 2014, a total of 15 624 patients with type 2 diabetes aged 35 years old and above, who received national basic public health service in Changshu county of Suzhou city, Huai′an and Qinghe districts of Huai′an city, Jiangsu province, were recruited by cluster sampling method. Face-to-face questionnaire survey, anthropometric and laboratory measurements were conducted to collect exposures to ICVD risk factors. Improved Ten Year Risk Assessment Table of ICVD in Chinese was used to assess the risk score and the absolute risk of developing ICVD.@*Results@#The participants were (62.2±9.7) years old, of which 6 137 were men (39.3%). Among the participants, the highest rate of exposure to ICVD risk factors was high systolic blood pressure (74.8%, n=11 685), followed by high total cholesterol (70.7%, n= 11 051).The score of 10-year risk for ICVD was (10.4±3.3) and the median (P25-P75) value of absolute risk was 15.6% (6.8%-32.7%). 16.7% (n=2 602) participants were under extremely high risk of 10-year risk for ICVD, 23.8% (n=3 714) under high-risk and 24.0% (n=3 746) under middle-risk. Among the total risk score of ICVD, age (49.1%), hypertension (17.7%) and diabetes (15.5%) accounted for relatively high proportion, however, smoking (11.0%) was the most important risk factor except for age (47.4%) and systolic blood pressure (20.5%) in male participants.@*Conclusion@#Patients with type 2 diabetes aged 35 years old and above in two cities of Jiangsu Province have a high risk of developing ICVD for 10 years, especially in elderly, female, hypertension patients and male smokers.

Objective To estimate the dose-response relationship between sedentary behavior with mortality in patients with type 2 diabetes. Methods A total of 17786 type 2 diabetic patients were recruited as participants, who were included in National Basic Public Health Service in Changshu County of Suzhou City, Qinghe District and Huai'an District in Huai'an City of Jiangsu Province. Cox proportional hazards regression model and restricted cubic spline model were employed to estimate the dose-response relationship between sedentary behavior with all-cause and cause specific mortality in patients with type 2 diabetes. Results Among 78114.34 person-years of the fo1low-up, the median of follow-up time was 4 years, and 1285 deaths occurred during that period. Compared to patients with sedentary behavior≤2 h/d, the multivariate adjusted hazard ratios of all-cause death associated with sedentary behavior levels of 3-4 h/d, 5-6 h/d, and≥7 h/d were 1.05(95%CI 0.92-1.20), 1.20(95%CI 1.03-1.42), and 1.39 (95%CI 1.16-1.65), respectively. Eevry increase of 1 h/d in sedentary behavior was associated with an increased hazard of death from cardiovascular disease(CVD) of 4%(HR=1.04, 95%CI 1.01-1.07) and from other causes of 6%( HR=1.06, 95%CI 1.03-1.09) . However, no significant association between sedentary behavior and malignant tumor death was found. The multivariable restrictive cubic spline regression indicated that the linear dose-response relationships were found between sedentary time with the all-cause, CVD cause, and other cause of mortality ( Non-linear test, P>0.05) . Conclusion Longer sedentary behavior could increase the risk of mortality in patients with type 2 diabetes.

Objective To detect the early diversification of the bone mineral density in skeletal fluorosis of rabbits by quantitative computed tomography (QCT),and analyze the possible relationship between bone mineral density and bone injury in rabbits with fluorosis.Methods A total of 16 rabbits,half male and half female,were randomly divided into control and experimental groups according to body weight.The two groups of rabbits were given drinking water containing NaF 0 or 300 mg/L,respectively,for 90 days.After the experiment,their bone fluoride content was determined via the fluorine ion-selective electrode method.The alkaline phosphatase (ALP) and osteocalcin (BGP) in serum were measured using enzyme-linked immunosorbent assay (ELISA).Femurl bone mineral density was detected with QCT in vivo.Histopathological changes of femur were observed under light microscope and trabecular acreage was calculated.The results were analyzed with independent-samples t test(t') and partial correlations.Results The bone fluoride content [(3 232.16 ± 927.85) mg/kg],ALP [(42.69 ± 3.28) U/L],BGP concentration [(2 504.19 ± 276.79) μg/L],bone density [(653.49 ± 167.81) g/cm3] and trabecular number [(39.02 ± 3.33)Tb.Ar] of the experimental group were higher than those of control group [(554.01 ± 376.51)mg/kg,(20.50 ± 4.90)U/L,(1 294.60 ± 191.86)μg/L,(540.40 ± 41.99)g/cm3,(8.15 ± 2.34)Tb.Ar],and the differences were statistically significant (t'=7.565,10.641,10.158,2.615,14.494,all P < 0.05).The tissue sclerosis,bone sclerosis and bone texture coarsening were observed through bone mineral density imaging taken by QCT in experimental group.The number of trabeculae increased and the arrangement of tra bec ulae was disorganized.Bone mineral density was positively correlated with bone fluoride,trabeculae,BGP and ALP (r =0.702,0.627,0.614,0.567,all P < 0.05).Conclusions QCT bone density measurement in skeletal fluorosis of rabbits can be used to compute the threedimensional bone density.And it has a good correlation with bone fluoride content,bone histopathological changes and index of bone metabolism in skeletal fluorosis,which suggests that QCT may provide a useful reference for application in patients with skeletal fluorosis.

RESUMO

Gastrointestinal benign stricture is one of common diseases in clinics , endoscopic dilation is its preferred treatment . However , for the dilation ineffective refractory strictures , it represents a difficult management problem given the limited therapeutic in-terventions available .Mitomycin-C is an antibiotic which has the dual role of anti-tumor and anti-proliferation.As an anti-proliferative drug, it was first applied in the field of ophthalmology .Local auxiliary application of MMC can be a feasible and safe measurement to improve the efficacy of refractory esophageal strictures .The aim of this study is to perform a systematic review which employs mitomycin C in the treatment of refractory esophageal strictures caused by corrosion , endoscopic submucosal dissection , esophageal atresia repara-tion and radiotherapy .

BACKGROUND:Procolagen type 1 N-terminal propeptide (P1NP) and β-colagen special sequence(β-CrossLaps) are two bone metabolic markers that are closely related to osteoporosis. Combined detection of bone metabolic markers and bone mineral density is of clinical significance for the diagnosis of osteoporosis. Bone metabolic markers are ideal indicators to predict fractures, which can compensate for the lack of bone density test. OBJECTIVE:To introduce the application of bone metabolic markers in the monitoring of drug efficacy on the treatment of osteoporosis as wel as in the prediction of fracture risks in recent 20 years and to explore the clinical values of P1NP and β-CrossLaps to assess the therapeutic efficacy on osteoporosis and risks for osteoporotic fractures. METHODS:A computer-based search of CNKI and SCI databases were performed for relevant articles published from 2000 to 2014 using the keywords of “serum bone metabolic markers; osteoporosis; bone mineral density” in Chinese and English, respectively. Finaly, 44 articles meeting the inclusive criteria were reviewed. RESULTS AND CONCLUSION:This paper analyzes the source and detection mechanisms of P1NPand β-CrossLaps and then compares their advantages in the therapeutic effect assessment of osteoporosis. Serum bone metabolic markers cannot only reflect the dynamic changes of bone metabolism, but also have earlier changes than the bone mineral density. Both P1NPand β-CrossLaps are very important for assessing the early diagnosis of osteoporosis as wel as anti-osteoporosis drug efficacy.

<p><b>OBJECTIVE</b>To investigate the regulatory effect of central synaptic plasticity on pain induced by experimental tooth movement and to analyzethe expression of protein kinases Mζ (PKMζ) in the anterior cingulate cortex (ACC) after applying different magnitude of orthodontic force.</p><p><b>METHODS</b>One hundred and thirty-six male Sprague-Dawley (SD) rats (200-250 g) were used in this study. Orthodontic tooth movement devices were placed on the teeth in the experimental group, and different orthodontic forces (0.39, 0.78, 1.17 N) were applied to move the maxillary first molars, respectively. The same mechanical devices were placed on the teeth in sham-treated group and no orthodontic force was applied. No orthodontic procedure was applied in blank control group. The average time spent on mouth- wiping behavior in each group was recorded after experimental tooth movement. Brain tissue of the anterior cingulate cortex was isolated on day 3 after experiment, and the expression level of PKMζ was analyzed with the method of immunohistochemistry and enzyme-linked immune sorbent assay. ζ-pseudosubstrate inhibitory peptide (ZIP), a selective inhibitor for PKMζ, was injected into ACC on day 3 after experimental tooth movement, and the effects of ZIP on mouth-wiping behavior were evaluated.</p><p><b>RESULTS</b>No statistical difference was found between the blank control group and the sham- treated group in the average time spent on mouth-wiping, value of A and expression level of PKMζ (P > 0.05). Compared with the sham-treated group and blank control group, the average time of mouth-wiping behavior [(58.6±6.9), (66.3±7.8), (78.9±8.7) s], value of A (4 569±454, 6 850±365, 8 294±558) and expression level of PKMζ [(0.32±0.02), (0.34±0.02), (0.36±0.02) mg/L] in 0.39, 0.78, 1.17 N force group were found to be up-regulated with the increase of orthodontic force (P < 0.05). LSD test in three experimental sub-group showed statistical difference (P < 0.05). After microinjection of ZIP, the average time spent on mouth-wiping behavior significantly decreased (P < 0.01), while microinjecting saline did not change rats' mouth-wiping behavior (P > 0.05).</p><p><b>CONCLUSIONS</b>More pain caused by increased orthodontic force maybe due to the up-regulation of PKMζ in the anterior cingulate cortex.</p>
