RESUMO

In order to understand nurses’ willingness to participate in humanistic nursing training and its influencing factors, and provide reference for managers to understand the current situation and improve nurses’ enthusiasm for humanistic nursing training. The convenience sampling method was used to investigate 23 707 nurses in 28 provinces (autonomous regions and municipalities directly under the central government) through a self-designed questionnaire distributed on the Internet. The results showed that 98.1% of nurses thought that participating in humanistic nursing related training was helpful to clinical work, but only 88.6% of the respondents were willing to participate in humanistic nursing training. Thirty factors were analyzed from four aspects of basic characteristics of individuals, cognitive relevant experience and organizational atmosphere. Fifteen factors had significant significance in binary Logistic regression analysis (P<0.05). Among them, the factors that had a positive impact on training willingness were: marriage, education, professional title, post establishment, agree with humanistic care is the basic duty of a nurse praised, experience of being praised at work, family support, rapport with patients, passion of colleagues to participate in training, sustained high-quality care demonstration activities, join the humanistic care related organization, hospital reimbursement of training expenses (OR value of 6.559~1.113, P<0.001). The OR value of humanistic nursing as a nurse’s responsibility was 6.559 and the 95%CI was 5.585~7.702. The factors that hindered nurses from participating in training were: work occupied most of time and energy, think humanistic nursing is abstract and difficult to understand, think the mastered humanistic knowledge and skills meet the needs of work (OR value of 0.657~0.722, P<0.001). Through the analysis, it is considered that nurses have a extremely consistent high recognition of the significance of humanistic nursing training, but their willingness to receive training is affected by many factors such as individual experience, cognitive attitude and organizational atmosphere. In order to realize nurses’ high recognition of humanistic nursing training to high enthusiasm of behavior, the aspects of individual cognition and organizational atmosphere must be discussed.

RESUMO

Resistance to radiotherapy is frequently observed in the clinic and leads to poor prognosis of non-small cell lung cancer (NSCLC). How to overcome resistance to radiotherapy is a challenge in the treatment of NSCLC. In this study, PPDPF was found to be upregulated in NSCLC tissues and cell lines, and its expression negatively correlated with the overall survival of patients with NSCLC. PPDPF promoted the growth, colony formation and invasion of lung cancer cells. Moreover, knockout of PPDPF inhibited tumorigenesis in the KL (KrasG12D; LKB1f/f) mouse model of lung cancer. Additionally, overexpression of PPDPF led to radioresistance in lung cancer cells, and knockdown of PPDPF sensitized lung cancer cells to radiotherapy. Mechanistically, PPDPF interacted with BABAM2 (an antiapoptotic protein) and blocked its ubiquitination by MDM2, thus stabilizing BABAM2 and promoting the radioresistance of lung cancer cells. Our present study suggested PPDPF as a therapeutic target in NSCLC.

RESUMO

Objective:To observe the short-term efficacy of sub-Tenon’s triamcinolone acetonide (TA) in the treatment of cystoid macular edema (CME) after cataract surgery. Methods:A retrospective study. From October 1, 2013 to October 1, 2018, 21 patients (21 eyes) with CME after cataract surgery diagnosed at The Affiliated Eye Hospital of Wenzhou Medical University were included in this study. All eyes underwent BCVA, intraocular pressure, frequency domain OCT examination. The standard logarithmic visual acuity chart was used for BCVA examination, which was converted into logMAR visual acuity when recorded. A frequency domain OCT instrument was used to measure the foveal retinal thickness (CRT). Among 21 eyes of 21 patients, 9 eyes were male and 12 eyes were female; the average age was 66.38±10.88 years. The average logMAR BCVA was 0.46±0.23; the average CRT was 519.90±131.59 μm; the average intraocular pressure was 11.01±3.97 mmHg (1 mmHg=0.133 kPa). All eyes underwent a single treatment of 100 mg/ml TA suspension 0.4 ml (containing TA 40 mg) under Tenon's capsule. The average follow-up time after treatment was 32.86±20.2 days. The changes of BCVA, CRT and intraocular pressure were observed 1 month after treatment. The BCVA, CRT and intraocular pressure before and after treatment were compared by paired t test. Results:One month after treatment, the average logMAR BCVA, CRT, and intraocular pressure of the CME eyes were 0.29±0.22, 307.71±35.82 μm and 14.19±6.30 mmHg, respectively. Compared with those before treatment, the differences were statistically significant ( t=5.252, 8.166, 4.128; P=0.000, 0.000, 0.001). Among 21 eyes, 13 eyes (61.90%) of the retinal cysts in the macular area completely disappeared; 8 eyes (38.1%) were still visible between the retinal layers. Three eyes with intraocular pressure over than 21 mmHg were treated with lowering intraocular pressure and returned to normal. Conclusion:Sub-Tenon’s capsular injection of TA to treat CME after cataract surgery can reduce CRT and improve vision in a short time.

RESUMO

Objective:To analyze the diagnosis and treatment of neonatal neuroblastoma (NB) by summarizing its clinical characteristics. Methods:This study retrospectively recruited 14 neonates with NB in Beijing Children's Hospital (National Center for Children's Health) from February 1, 2015, to February 1, 2020. Medical records and follow-up data as of February 29, 2020, were collected, and clinical staging based on International Neuroblastoma Staging System, risk grouping based on American Childhood Oncology Group risk grouping system, diagnosis, treatment, and prognosis were analyzed. According to whether surgical treatment was performed in the neonatal period or not, these subjects were divided into surgical and non-surgical groups. A descriptive statistical analysis was used for data analysis.Results:(1) Neonates with NB accounted for 0.063% (14/22 006) of the total number of newborns admitted to the hospital during the same period. The 14 cases were all full-term aged 15 d (8 h-23 d) at admission. Tumors were found in seven cases in prenatal examinations, while others presented with shortness of breath (three cases), abdominal distension (two cases), fever (one case), and dysuria and difficult defecation after birth (one case). (2) The primary tumor sites included the adrenal gland (eight cases), posterior mediastinum (three cases), retroperitoneum (two cases), and sacrococcygeal (one case). Three cases had extensive diffuse liver metastasis at admission. (3) Except for two cases who refused to examine, the serum neuron-specific enolase of 12 cases was 57.2 ng/ml (35.9-158.3 ng/ml) during hospitalization, and the urine vanillyl-mandelic acid creatinine of four cases was 2 304.940 (685.748-9 595.314). (4) Primary tumor sites were found in 14 cases by imaging examination. Bone scanning was performed in three cases, including one with a concentrated shadow of the right sacroiliac joint and two with no abnormalities. Ten cases underwent bone marrow aspiration and all with normal results. (5) Of the ten neonates received surgery (the surgical group), nine had the primary tumor wholly removed, without chemotherapy after the operation, and the tumor-free survival period was 19 months (1-45 months). One case (case 5) had a substantial primary tumor that could not be completely resected. The patient underwent a second surgery five months after the first operation due to disease progression and received postoperative chemotherapy. The child had stopped chemotherapy for 24 months and survived without a tumor. (6) In the non-surgical group (cases 11 to 14), the tumor in case 11 who refused chemotherapy shrank spontaneously after discharge, and the patient survived for 20 months with the tumor. The parents of the case 12 withdrew treatment during hospitalization, while the primary tumor and metastases disappeared after discharge, and the specific tumor markers gradually decreased to normal levels. The patient has been tumor-free survived for 25 months. Case 13 received mediastinal tumor resection and chemotherapy during infancy. At the end of the follow-up, chemotherapy had been stopped for 12 months, and the patient survived without a tumor. Case 14 withdrew treatment and died. (7) Among the ten cases in the surgical group, one patient's pathological result indicated a composite tumor, while the others were low differentiated neuroblastoma. There was no MYCN gene amplification, 1p36 deletion, or 11q23 deletion in the ten cases. (8) Among the ten children in the surgical group, nine were in stage 1, and one was in stage 4S (case 5). Nine cases were classified into extremely low-risk groups, while the other was in the low-risk group. The four cases in the non-surgical group could not be grouped by risk. Conclusions:Clinical manifestations of neonatal NB are often atypical. NB in stage 4S might resolve spontaneously, and expectant observation may be considered. The overall prognosis of neonatal NB is generally good, but further researches are needed.

RESUMO

Compared with surgery, radiotherapy, chemotherapy and other conventional cancer treatments, near-infrared (NIR) photothermal therapy mediated by gold nanoparticles had advantages of non-invasive, safe, non-drug resistance and small side effects. Especially for tongue cancer and other head and neck tumors, NIR gold nanoparticle-mediated photothermal therapy is more conducive to preserve the anatomic and functional integrity of stomatognathic system, improving the life quality of patients. However, the toxic organic phase substances are commonly used in conventional gold nanoparticles preparation process in order to increase the stability of the material which limited its application in the biomedical field. In this study, a new type of gold nanoflower (GNF) particles were synthesized by seeding approach in aqueous phase, with better biocompatibility and free of organic substances. Then, the photo-thermal conversion curve under the irradiation of NIR laser ray were investigated. The absorption peak of prepared GNF particles was 719 nm and they had high efficient photothermal conversion in near-infrared region. Furthermore, we investigated the inhibition effect of NIR photothermal therapy with different laser power mediated by the GNF particles on human tongue carcinoma cells by detecting the relative growth rate, cell cycle, and apoptosis and necrosis. The results showed that NIR photothermal therapy mediated by prepared GNF particles could inhibit proliferation of tongue carcinoma Tca-8113 cells obviously, and the relative growth rate of Tca-8113 cells decreased along with the increasing of GNF concentration and laser power. At last, we validated the expression stabilities of 12 commonly used reference genes (18S, ACTB, ALAS1, GAPDH, TBP, HPRT1, RPL29, HMBS, PPIA, PUM1, GUSB, and B2M) after the above treatment through real-time quantitative polymerase chain reaction (RT-qPCR). Three commonly used software, geNorm, NormFinder, and BestKeeper are used to compare stability between reference genes. The final ranking of the candidate reference genes expression stability was TBP > PPIA > RPL29 > B2M > 18S rRNA > GAPDH > GUSB > ACTB > HPRT1 > ALAS1 > HMBS > PUM1. The combination of TBP + PPIA is best for normalizing the relative quantitation. The results suggested that GNF particles had high photothermal conversion in NIR region and was ideal mediated material for tumor NIR photothermal therapy. NIR photothermal therapy mediated by GNFs could inhibit the growth rate of tongue carcinoma Tca-8113 cells obviously. The recommend reference genes can be used for improving the accuracy of gene expression studies in the molecular mechanism of near-infrared (NIR) photothermal therapy mediated by the gold nanoflower (GNF) particles.

RESUMO

Objective To assess the clinical value of MSCT in congenital esophageal atresia (EA) and tracheoesophageal fistula (TEF) of newborns. Methods Twenty neonates (17 boys and 3 girls) with a mean age of 4.6 days (1 day to 16 days) diagnosed EA and distal TEF underwent MSCT, and multiple planar volume reconstruction (MPVR) and three-dimensional transparency lung volume rendering (TL-VR) imaging were used. The initial diagnosis was made on esophagram by showing the catheter into a blind-ended esophageal pouch. The MSCT manifestations were compared with the surgical findings. Statistical analysis was performed by using SPSS 10.0. Paired-Samples t test and Pearson correlation analysis were used. Results MSCT clearly showed the distal esophageal pouches in all EA patients. The distance between the proximal and distal esophageal pouches determined by MPVR (0.15--3.10 cm, median 0.70 cm) and TL-VR (0.10--3.10 cm, median 0.82 cm) had no remarkable differences and correlated well with the surgical findings (r=0.87, P<0.01). MPVR revealed the orifice of the fistula in 13 TEF cases, while TL-VR only in 4. Conclusion MSCT is an useful and noninvasive imaging method for demonstrating congenital EA and distal TEF, and is highly valuable for surgical planning.

RESUMO

To mitigate penumbra injury is the key factor in the treatment ofischemic stroke.Blood reflow and neuroprotection against time are premise to achieve this.Due to time window, thrombolysis rate is low(less than 5% in developed country). However, all neuroprotectants haven't been demonstrated to be effective in human, till now.Based on guidance ofwholism concept ofTCM and analysis ofrelative studies in western medicine. We found that functional status ofnon-ischemic areas can impact on injury degree offocal ischemia. One ofthe reasons that lead to failure in neuroprotective studies may be without considering this impaction. We put forward that non-ischemic areas can impact on the injury degree ofischemic area through the complex neuronal network, and effective regulating the functional status ofnon-ischemic areas is one ofthe key factors which can ameliorate focal ischemic injury.
