RESUMO

The authors describe a tonsillolith detected by a 70-year-old man with odynophagy and a history of often tonsillar infections. A diagnosis of peritonsillar abscess was first made. The large concrement was yellowish-gray, measured 41 x 21 x 19 mm, which was one of the largest reported case in the world.

RESUMO

On the basis of recent literature review the authors consider pharmacological polypectomy, concentrating on the reasons for high efficacy of steroids. The facts presented prove that in the case of nasal polyps steroids should be used not only as a supplement of surgical procedures. Steroid-therapy a simple, not invasive and effective management procedure should be used regularly reserving surgical procedures for certain specific cases only.

RESUMO

Sarcoidosis is a disease of unknown etiology characterized by non-caseating granulomatous inflammation of various organs, but most frequently involving the lungs of young adults. Sarcoidosis is seldom seen by ear, nose, throat and larynx. The isolated case of sarcoidosis of the larynx is very rare. We present a case of histologically confirmed sarcoidosis of the vocal cord. The clinical aspects, diagnostic problems and therapeutical possibilities of this rare disease are described.

RESUMO

The authors describe a case of a man with a large choanal polyp descending into the hypopharynx. The man presented additional symptoms of full aspirin intolerance and pollinosis caused with grass pollen antigens. The Caldwell-Luc procedure was performed and the choanal polyp was successfully removed through the mouth. Postoperatively treatment with leukotriene antagonists and with an intranasal steroids with positive clinical effect was begun. However the patient discontinued the therapeutic process causing polyp recurrence within a short time.

RESUMO

Branchiogenic carcinoma is a rare condition and should not be diagnosed in the absence of the criteria laid down by Dr Hayes Martin. The authors describe a 65 year-old woman with a cystic mass on the side of the neck. The woman was admitted to the hospital for possible branchial cyst. The histopathological analysis showed a carcinoma developed on a branchial cyst. The authors stress the importance of careful and repeated clinical examinations, which exclude the possibility of a primary tumor of other localization.

RESUMO

Granular cell tumor is an unusual growth of probably neuroectodermal histogenesis, first reported by Abrikossoff in 1926 with the name of myoblastenmyoma. Authors described a case of a 54 year man with laryngeal seat of granular-cell myoblastoma. In this case Abrikossoff tumor was located in the right vocal chord. The tumor was treated successfully surgically by microlaryngoscopy. The etiology, clinical features and diagnostic difficulties are discussed.

RESUMO

The authors describe a rare case of hamartoma in the larynx of a 31 years old man. The egg sized tumor was situated in the left aryepiglottic fold filling the lower pharyngeal space. The unfortunate location, patient disdain as well as late diagnosis and proper management caused the histologically benign tumor to enlarge and block the laryngeal entrance that endangering life. After initial tracheotomy the tumor was successfully removed through the mouth.

UNLABELLED: The aim of the study was to estimate by means of acoustic rhinometry (AR) the nasal blockade in 37 patients (5-73 years old, mean 32) with upper airways infections treated by fusafungine. The examinations were carried out on the first (D1) and seventh day (D7) of observation. Cross-sectional area (CA) mean values of 3 cm sector laid back to C notch of AR curves (CA-C3) were selected to the analysis. RESULTS: The mean value of CA-C3: in all patients it was 2.61 cm2 on D1 and 2.8 cm2 on D7; in rhinitis and sinusitis patients--2.23 cm2, in others--3.16 cm2; without nasal discharge--3.48 cm2, in others (with severe or medium)--2.53 cm2 and 2.48 cm2 respectively. 29 patients had nasal blockade (78.4%) with mean CA-C3 = 2.34 cm2 (others--3.56, p < 0.05). During seven days the following enlargement of CA-C3 was reported: 14.9% in all patients (p < 0.01); in viral infections 24% (n = 14, p < 0.01), bacterial 14.3% (n = 11, NI); in nasal blockade patients--15.22%, others--13.55% (p < 0.01). Mean value of CA-C3 increased by 37.7% in patients who demonstrated, according to the physicians, "very good improvement", 5.7%--"good", 1.5%--"weak" (p < 0.05). There was also enlargement of nasal cavities in CA-C3 in non-sneezers (p < 0.05) and medium discharge symptom patients (p < 0.05). There was no statistical difference in clinical and acoustic rhinometry results between patients treated with fusafungine together with non-steroid anti-inflammatory drugs and patients treated with fusafungine. CONCLUSION: AR is a good instrument to be used in the objective assessment of the nasal blockade changes in people with infection of the nasal mucosa and showed positive efficacy of fusafungine in the treatment of upper airway infection.

The aim of the study was to assess efficacy and safety of 5 mg loratadine/120 mg pseudoephedrine combination drug in patients with seasonal allergic rhinitis. 30 patients allergic to grass pollen were treated with the new drug (Clarinase) twice a day in 15-day study during grass pollen seasonal. Nasal an non-nasal symptoms were evaluated for efficacy. Loratadine/pseudoephedrine combination effected a significant decrease in total symptoms score as well as individual evaluated symptoms score: nasal stuffiness, itching and discharge, sneezing, eye itching, tearing and redness of the eyes. The treatments was well tolerated. No serious side effects were noticed. The incidence of mild sedation, dry mouth, insomnia and nervousness was only 3 to 7 percent. 5 mg loratadine plus 120 mg pseudoephedrine was safe and effective in relieving the symptoms of allergic rhinitis.

The authors have studied the effect of azelastine nasal spray in the treatment of seasonal allergic rhinitis. The trial was a double-blind, placebo controlled study, with 33 allergic subjects (17 female, 16 male), 15-40 years old. The patients were randomized into two parallel groups, to receive daily 0.56 mg of azelastine intranasally or placebo during two weeks. Before and after treatment severity of nasal symptoms such as: sneezing, itching, nasal blockage, nasal discharge and general feeling were evaluated by patients according to VAS (visual analogue scale). At the same time the physician's evaluation of nasal oedema, nasal discharge and general patient's condition were performed (VAS). Additionally during the treatment patients noted possible adverse events. Patient's and physician's evaluations showed clinical efficacy in the azelastine group and no evident efficacy in the placebo group--between those two groups statistical significances were noted for all evaluated parameters (p < 0.001). The bitter taste in the mouth was the only serious side effect in some patients in azelastine group, although this was quite well accepted. Generally the treatment was well-tolerated. Azelastine nasal spray is an effective and well-tolerated drug in the treatment of seasonal allergic rhinitis.

Allergy is an increasing problem therapeutic. The development of IgE-mediated allergy is a result of genetic and environmental factors. The severity of disease seems to be increasing. Immunotherapy offers a specific treatment, and a long-term effect and method that can change the development of the atopic state. The treatment allergy should always be carried out in accordance with a careful judgement of related costs: price, efforts, risk and benefits: preventive long-term effect, decreased symptoms and the use of medication.

RESUMO

Studies were performed on 21 industry-products sellers suffering to recurrent infections of upper respiratory tract. Immunostimulating treatment was performed using Broncho-vaxom administered during 6 months. Drug was applied by 10 consecutive days every month, 1 tablet at the morning on empty stomach (before breakfast). During immunotherapy the decrease in work-absences and diminution of some pathological symptoms were noted. No additional chemotherapeutics were taken during Broncho-vaxom. Although the efficacy of immunostimulatory therapy with Broncho-vaxom is not certainly proved, treatment with this drug seems to present be an alternative for chemotherapeutics in recurrent infections of upper respiratory tract. That treatment is probably like the psychotherapy.

The last ten or fifteen years have provided truly novel insight into the molecular and cellular pathobiology of allergic diseases. Whether cytokine regulation of mast cells is autocrine in part or entirely the purview of the T cell, remains to be determined, but still emphasizes the complex cellular interaction in sustained reactions falling within the clinical definition of allergy. The rapid advances being made in understanding the biochemistry and function of the three categories of adhesion molecule involved in allergic reaction. Cell adhesion molecules are glycoproteins, identified within the last decade, expressed on all surfaces that allow: cell-to-cell contact, control immune surveillance and effector functions. The three families of adhesion molecules that participate in this vital processes are the selectins, namely L-selectin, E-selectin and P-selectin, the integrins: LFA-1, CR-3, p 150.95 and very late antigen 4, and the immunoglobulin (Ig) superfamily which includes intercellular adhesion molecule 1 (ICAM-1), ICAM-2 and vascular cell adhesion molecule 1 (VCAM-1). These adhesion molecules play an essential role in the pathogenesis of allergic rhinitis.

RESUMO

Cetirizine and loratadine-two new antihistaminic drugs were evaluated in 56 patients with seasonal rhinitis in cross-over open study. I our study, no difference between loratadine and cetirizine has been seen. Both evaluated drugs significantly inhibited the symptoms of allergic rhinitis and conjunctivitis. Adverse reaction and inhibition of histamine and codeine skin tests were similar.
