RESUMO

Bone fractures are a medical condition where the continuity of the bone is broken due to a fall or accident. The fracture may also be the result of medical conditions such as osteoporosis, cancers of bone or osteogenesis imperfect. During the bone fracture healing process, the mesenchymal stem cells (undifferentiated connective tissue cells) are recruited from local and systemic sources. The modulation of mesenchymal cell migration to the fractured site is the desired goal. Still, there are many processes that are still required to be studied and analyzed. We aimed to consolidate and review the available information on this topic.

With the increase of the average age of our population, the incidence of diseases specific for older adults has been increasing. One of such diseases is osteoporosis. The true incidence of osteoporosis is unknown. But the estimates indicate that this disease affects wide proportions of the population, ranging in millions or even ten millions in large countries like the United States. As this poses a significant burden on the health care system, interventions that could prevent or treat this condition are in the focus of clinical research. Vitamin D, the determinant of bone health, has been tested in clinical studies as the agent to treat osteoporosis. Despite the progress, there is still some controversy about the targeted blood levels of vitamin D, most efficient way to supplement this vitamin, and clinical efficacy of this supplementation in the elderly.In the present review, we will highlight the metabolism of vitamin D and the aforementioned unresolved issues, as well as review the recent interventional studies on vitamin D supplementation. OBJECTIVES: Icariin, a prenylated flavonol glycoside isolated from traditional Chinese medicinal herb of the genus Epimedium, has been demonstrated to be a potential alternative therapy for osteoporosis, and its action mechanism so far has been mainly attributed to its phytoestrogenic property. As blood supply to bone is considerably reduced with ageing and by the menopause, we hypothesized that icariin treatment would reduce bone loss by preventing ischaemia-induced hypoxic damages to bone. MATERIALS AND METHODS: To investigate effects of icariin treatment on cultured rat calvarial osteoblasts exposed to hypoxic conditions (2% oxygen). RESULTS: Compared to normoxic control, cell viability decreased with time to 50% by 48 h in the hypoxic group, and icariin attenuated the reduction, dose dependently, with 10(-6) and 10(-5)  m concentrations showing significant protective effects. Icariin also inhibited increase of lactate dehydrogenase activity in culture media. Measurements on oxidative stress, cell cycling and cell survival indicated that icariin protected osteoblasts by reducing production of reactive oxygen species and malondialdehyde, increasing superoxide dismutase activity, arresting the cell cycle and inhibiting apoptosis. Icariin also preserved osteogenic differentiation potential of the hypoxic cells in a dose-dependent manner, compared to the hypoxia alone group, as revealed by increased levels of RUNX-2, OSX and BMP-2 gene expression, alkaline phosphatase activity, and formation of mineralized nodules. CONCLUSIONS: Our results demonstrated that icariin attenuated oxidative stress and apoptosis and preserved viability and osteogenic potential of osteoblasts exposed to hypoxia in vitro, and suggested that its anti-osteoporotic effect may be attributed to its anti-hypoxic activity and phytoestrogenic properties.

The infrared (3,200-30 cm(-1) spectra of gaseous and solid 1-bromosilacyclobutane, c-C3H6SiBrH, have been recorded. Additionally, the Raman spectra of the liquid (3,200- 30 cm(-1) with quantitative depolarization values and the solid have been recorded. Both the equatorial and the axial conformers have been identified in the fluid phases, Variable temperature ( - 105 to - 150 degrees C) studies of the infrared spectra of the sample dissolved in liquid krypton have been carried out. From these data the enthalpy difference has been determined to be 182 +/- 18 cm(-1) (2.18 +/- 0.22 kJ/mol) with the equatorial conformer the more stable rotamer and only conformer remaining in the annealing solid. At ambient temperature there is approximately 22% of the axial conformer present in the vapor phase. A complete vibrational assignment is proposed for both conformers based on infrared contours, relative intensities, depolarization values and group frequencies. The vibrational assignments are supported by normal coordinate calculations utilizing the force constants from ab initio MP2/6-31G(d) calculations. From the frequencies of the Si-H stretch, the Si-H bond distance of 1.483 A has been determined for both the equatorial and the axial conformers. Complete equilibrium geometries have been determined for both rotamers by ab initio calculations employing the 6-31G(d) and 6-311 +/- G(d,p) basis sets at levels of Hartree Fock (RHF) and/or Moller- Plesset with full electron correlation by the perturbation method to the second order (MP2). The results are discussed and compared to those obtained for some similar molecules.

The infrared (3200-30 cm(-1) spectra of gaseous and solid and the Raman spectra of liquid (3200-30 cm(-1), with quantitative depolarization values, and solid vinyldichlorosilane, CH2=CHSiHCl2, have been recorded. Both the gauche and the cis conformers have been identified in the fluid phases. Variable temperature (105-150 degrees C) studies of the infrared spectra of the sample dissolved in liquid krypton have been carried out. From these data the enthalpy difference has been determined to be 20 +/- 5 cm(-1) (235 +/- 59 J mol(-1) with the gauche conformer the more stable rotamer. It was not possible to obtain a single conformer in the solid even with repeated annealing of the sample. The experimental enthalpy difference is in agreement with the prediction from MP2/6-311 + G(2d,2p) ab initio calculations with full electron correlation. However, when smaller basis sets, i.e. 6-31G(d) and 6-311 + G(d,p) were utilized the cis conformer was predicted to be the more stable form. Complete vibrational assignments are proposed for both conformers based on infrared contours, relative infrared and Raman intensities, depolarization values and group frequencies, which are supported by normal coordinate calculations utilizing the force constants from ab initio MP2/6-31G(d) calculations. From the frequencies of the Si-H stretches, the Si-H bond distance of 1.474 A has been determined for both the gauche and the cis conformers. Complete equilibrium geometries have been determined for both rotamers by ab initio calculations employing the 6-31G(d), 6-311 + G(d,p) and 6-311 + (2d,2p) basis sets at level of Hartree-Fock (RHF) and/or Moller Plesset to the second order (MP2) with full electron correlation. The potential energy terms for the conformer interconversion have been obtained from the MP2/6-31G(d) calculations. The results are discussed and compared with those obtained for some similar molecules.

RESUMO

OBJECTIVE: To investigate a good method for repairing the long bone defect of tibia combined with soft tissue defect. METHODS: From 1988-1998, sixteen patients with long bone defect of tibia were admitted. There were 12 males, 4 females and aged from 16 to 45 years. The length of tibia defect ranged from 7 cm to 12 cm, the area of soft tissue defect ranged from 5 cm x 3 cm to 12 cm x 6 cm. Free fibula grafting was adopted in repairing. During operation, the two ends of fibular artery were anastomosised with the anterior tibial artery of the recipient, and the composited fibular flap were transplanted. RESULTS: All grafted fibula unioned and the flap survived completely. Followed up for 6 to 111 months, 14 patients acquired the normal function while the other 2 patients received arthrodesis of the tibial-talus joint. In all the 16 patients, the unstable ankle joint could not be observed. CONCLUSION: The modified method is characterized by the clear anatomy, the less blood loss and the reduced operation time. Meanwhile, the blood supply of the grafted fibula can be monitored.

RESUMO

The speciality of limb replantation in children included thread and thin for blood vessels, growth development for muscles and epiphyses, morable and incooperation in clinical. The function exercises after limb replantation in children must be connected with the clinical speciality of vascular microsurgery, the anthor divided it into four stages; vascular crises stage, limb aliving stage, clinical healing of bone stage, and function recovery stage. The different treatment performed in four stages, and excellent clinical results were achieved.

RESUMO

The consensus and complex human papillomavirus-polymerase chain reaction (HPV-PCR) technique was used to detect HPV-DNA in normal cervical tissue of 148 women. The investigation showed that HPV-DNA positive rate was 31.76% (47/148) including HPV type 6,11, 6.76% (10/148), HPV type 16, 18, 3.38% (5/148) and other types 21.62% (32/148). HPV infection was more common in young women from 22 to 34 years of age. The transmission route of female genital HPV infection and the importance of subclinical infection were also discussed.
