RESUMO

Ten patients with perianal necrotizing fasciitis were admitted to our department from June to December 2016. There were 8 men and 2 women among the patients, aged 42 to 69 years. Early and complete debridement surgery and comprehensive supportive treatment during perioperative period were carried out to quickly stabilize the patient's overall condition, and wounds were sutured directly or repaired with autologous scalps and or adjacent local random flaps. After debridement, wound areas ranged from 10 cm×8 cm to 54 cm×21 cm, and area of the flap was about 8 cm×5 cm. The donor site of flap was sutured directly. After the operation, all skin grafts and the flap survived, and wounds of all patients healed. During follow-up of six months to one year, there was no recurrence of perianal necrotizing fasciitis, and functions of the involved lower extremities didn't be influenced.

Objective: To describe the prevalence of preterm birth (PB), low birth weight (LBW), and small for gestational age (SGA) among HIV-infected pregnant women and to identify associated risk factors in Hunan province. Methods: This study appeared a retrospective one on HIV-infected pregnant women retrieved from Information System of Prevention of Mother-to-child Transmission of HIV management in Hunan province, between January 2011 and December 2017. Information regarding demographic characteristics, pregnancy, antiretroviral therapy (ART), husbands/partners' relevant situation and pregnancy outcomes, among these HIV-infected pregnant women were collected and analyzed. The incidence rates on PB, LBW and SGA were calculated. Multivariate logistic regression was used to analyze the associated risk factors. Results: A total of 780 HIV-infected pregnant women were enrolled. The prevalence rates on PB, LBW and SGA in HIV- infected pregnant women appeared as 7.9% (62/780), 9.9% (77/780) and 21.3% (166/780), respectively. Results from the multivariate logistic regression analysis showed that factors as pregnancy related diseases as moderate/severe anemia, hypertensive, initial time of ART <14 gestational weeks (compared to those women without ART during pregnancy) and husbands/partners' age >35 years old (compared to husbands/partners' age 26-30 years old) etc., were associated with an increased risk of PB with adjusted OR as 4.59 (95%CI: 1.51-13.95), 4.90 (95%CI: 1.56-15.46), 2.40 (95%CI: 1.26- 4.56) and 2.29 (95%CI: 1.21-4.36). For LBW, pregnancy moderate/severe anemia, pregnancy HBV infection and initial time of ART <14 gestational weeks were associated with an increased risk of LBW, with adjusted OR as 3.28 (95%CI: 1.13-9.54), 4.37 (95%CI: 1.42-13.44) and 2.68 (95%CI: 1.51-4.76), respectively. For SGA, pregnancy HBV infection and initial time of ART <14 gestational weeks were risk factors for SGA, with adjusted OR as 4.41 (95%CI: 1.43-13.63) and 2.67 (95%CI: 1.51-4.73), respectively. Conclusion: Preterm birth, LBW and SGA were common adverse pregnancy outcomes for HIV-infected pregnant women and were associated with factors as pregnancy complications, ART and husbands/partners' age.

Tuning of molecular conductance in a liquid environment is a hot topic in molecular electronics. In this article, we explore a new concept where the Fermi level positions of the metallic ends are varied simply by modifying the electroactive salt concentration in solution. We rely on the electrochemical scanning tunneling microscope break junction method that allows the construction in solution of copper atomic contacts that can be then bridged by single molecules. The experimental conductance evolution is first confronted with an analytical formulation that allows the deduction of the molecule's LUMO position and electronic coupling factors. These parameters are in close agreement with those obtained by independent DFT calculations.

The aim of this study was to explore the relationship between adiponectin (ADPN) and hemorrhagic shock (HS) and the recovery after HS. This is significant for further understanding of the pathophysiological processes of HS and the development of better treatments. In total, 72 male C57BL/6 mice were assigned randomly to three groups: control, HS, and recovery (N = 24). The HS mouse model was constructed by hemorrhage of the carotid artery and recovery was achieved by tail vein injection of Ringer's solution. The level of ADPN in the peripheral blood of mice before and after recovery was detected by enzyme-linked immunosorbent assay. Compared to control, HS mice showed significantly decreased ADPN levels with the extension of HS time while the level of ADPN in recovery mice increased significantly and remained high. The variation of ADPN levels was closely associated with the occurrence of HS in mice and their recovery, suggesting that ADPN might act as a biomarker of inflammation and have potential for the treatment of HS.

N-Doped carbon materials are promising candidates as alternative catalysts to noble metals in promoting the oxygen reduction reaction (ORR) in fuel cells. However, methods to further reduce the ORR overpotential and improve related kinetics remain to be developed. This study reports that N-doped graphene frameworks (NGFs) synthesized from the rapid pyrolysis of solid glycine particles in the presence of sodium carbonate, display an extremely large specific surface area (1760 m(2) g(-1)) and a graphitic-N-dominant C-N configuration. The NGFs can efficiently catalyze the electrochemical reduction of molecular oxygen into water following a 4e pathway, with a low overpotential (0.98 V of onset potential vs. RHE), very high kinetic limiting current density (16.06 mA cm(-2)), and turnover frequency (121 s(-1)), much better than the commercial Pt/C catalyst.

Chronic rhinosinusitis(CRS) is one of upper respiratory inflammatory diseases, with a high incidence. It affects the patient's work efficiency and quality of life seriously. However, the pathogenesis of disease is not definite. In recent years, some reports have proved miRNA play an important role in the development and occurrence of CRS. This paper reviewed the role of miRNA in pathogenesis,diagnosis and treatment of chronic rhinosinusitis.

SETTING: The Fujian District in China has a high migrant worker population. Although tuberculosis (TB) among migrants is a serious threat to public health in Fujian, little is known about the molecular characteristics of TB isolates in this population. OBJECTIVE: To investigate the genetic profile of TB among the migrant population in Fujian. RESULTS: Our study enrolled 243 pulmonary TB patients registered in Fujian. Our data demonstrated that the Beijing genotype was the most common genotype in Fujian, and that the proportion of migrants with the Beijing genotype was significantly higher than that of permanent residents. Furthermore, the population structure of Mycobacterium tuberculosis strains in Fujian was diverse, with no difference in the distribution of mycobacterial interspersed repetitive units-variable number of tandem repeat (MIRU-VNTR) subgroups between the migrant and permanent populations. In addition, the discriminatory power of MIRU-VNTR in this study was higher than that found in other regions of China, possibly due to the high percentage of migrants in Fujian. CONCLUSION: The Beijing genotype was the predominant genotype in Fujian. TB strains isolated from this migrant population revealed a genetic profile similar to that of the permanent population. Improvement in public medical and insurance programmes for migrants might be crucial in the effective control of TB in Fujian.

Helwingia chinensis Batal is distributed in the western and southern regions of China. The aerial part of this plant has long been used to treat dysentery, hematochezia, and swelling. An outbreak of cankers and dieback was observed for the first time on H. chinensis in China during June of 2010. Disease symptoms included dieback of shoots and branches, lesions, and canker formation on the stems. In order to identify the causal agent(s) of this canker disease, samples composed of inner bark and woody tissues were collected from the junction of healthy and diseased tissues of declining trees from Kunming and Wenshan districts of China during July to October of 2010. Pieces of surface-sterilized tissue samples were plated on potato dextrose agar (PDA) and incubated at 26°C. Fungal colonies developed copious, white, aerial mycelium that became dark green with age. Pycnidia started to develop after 20 days. Macroconidia, which were 20 to 29 × 4 to 6 µm, were hyaline, aseptate, and fusiform. No fungus was isolated from water-inoculated tissues of control plants and healthy trees. Identity was confirmed by analysis of the rDNA internal transcribed spacer region (ITS1-5.8S-ITS2) with primers ITS1 and ITS4. BLAST searches showed 99% identity with Botryosphaeria dothidea isolates from GenBank (Accession Nos. HQ660454 and FJ790846). Representative sequences of B. dothidea from H. chinensis from China have been deposited in GenBank (Accession No. JQ766122). On the basis of morphological and molecular results, the fungus isolated from diseased H. chinensis was confirmed to be B. dothidea. Pathogenicity tests were conducted by stem inoculation of 1-year-old H. chinensis seedlings. Mycelial plugs (3 to 4 mm in diameter) of B. dothidea from actively growing colonies were applied to same-sized bark wounds on the middle point of the stems. Control seedlings were inoculated with sterile PDA plugs. Inoculated and control seedlings (three each) were kept in a greenhouse and watered as needed. After 4 weeks, all H. chinensis seedlings developed vascular tissue discoloration and leaf wilting; no such symptoms were manifested by seedlings in the control treatment. B. dothidea was reisolated from all B. dothidea-inoculated, symptomatic tissues, fulfilling Koch's postulates. In China, B. dothidea has previously been reported to cause canker and dieback disease of Eucalyptus grandis (2) and gummosis of peach (1); however, to our knowledge, this is the first report of B. dothidea causing canker and dieback on H. chinensis. The potential of embryonal day 14 (ED) fetal liver epithelial progenitor (FLEP) cells to repopulate the normal and damaged liver was studied throughout a 3-month period in syngeneic mice. In normal liver, FLEP cells proliferated and differentiated into hepatocytes after transplantation, and liver repopulation was moderate (5%-10%) after 3 months. In diethylnitrosamine-treated livers FLEP cells continued to proliferate at 3 months after transplantation; both the number and size of clusters derived from FLEP cells gradually increased throughout time. Transplanted cells proliferated and differentiated into both hepatocytes and bile ducts to produce extensive liver repopulation (30%-50%). This report showed that isolated fetal liver epithelial cells exhibit bipotential properties of stem cells, which can engraft, proliferate, and differentiate into hepatocytes and bile duct epithelial cells with high repopulation capacity in the injured liver.

The model of membranous glomerulonephritis (MGN) in rats was successfully established using self-made cationic bovine serum albumin (C-BSA) and treated with Huangdan Decoction (HDD) and Tripterygium Wilfordii Co. tablet (TW). Results indicated that the levels of urinary protein, blood urea nitrogen (BUN) and serum creatinine (Scr) in treated groups (groups A, B and C) were significantly decreased as compared with the control group (group D) (P < 0.01). By light and electron microscope and immunofluorescent technique, the damage to kidney in groups A, B and C was found much milder than that in group D with lesion in group A being slightest. These findings suggest that HDD and TW may alleviate the pathological lesions of MGN, prevent or retard its progression, and have remarkable therapeutic effects on MGN.

With DNA polymerase chain reaction-single strand conformation polymorphism assay followed by direct DNA sequencing, p53 gene mutation was examined in bladder transitional epithelial cell carcinoma, renal cell carcinoma and testicular seminoma. p53 gene mutation was found in 7 cases (35%) of bladder carcinoma and 4 cases (23.5%) of testicular seminoma. Inactivation of Rb gene and activation of ras and c-erbB-2 were also studied. The results suggest that development of urologic neoplasms is closely associated with p53 gene mutation and involves loss of expression of Rb and aberrant expression of ras and c-erbB-2.

Expression of p53 protein was examined in 67 cases of primary transitional cell carcinoma of the bladder and 6 normal controls using an immunohistochemical method on paraffin sections. Positive nuclear staining for p53 in malignant cells was found in 34 (51%) of the 67 cancer patients; no positive staining for p53 was detected in any of the normal controls or in the benign cells, including stromal and inflammatory cells, within the tumor tissue. There were 8 positive cases (33%) in 24 grade G1 tumors, 12 (48%) in 25 G2 tumors and 14 (78%) in 18 G3 tumors. p53 protein was detected positively in 14 (36%) of 39 superficial tumors (Tis-T1) and in 20 (71%) of 28 invasive tumors (T2-T4). Thus, positive staining for p53 was found more frequently in poorly differentiated tumors (chi-squared test: G3/G1 + G2 P < 0.01) and in invasive tumors (chi-squared test: T2-T4/Tis-T1 P < 0.01). Expression of p53 was also closely associated with recurrence of tumors. Alterations in p53 expression may be of prognostic value in cases of bladder transitional cell carcinoma.

Expression of the ras and the c-erbB-2 oncogene products was investigated in 56 cases of human bladder transitional cell carcinoma and 6 samples of human normal bladder tissue using an immunohistochemical method. Thirty of the 56 cases of bladder tumor were found to be immunohistologically positive with the monoclonal anti-ras p21 antibody, while 19 of 56 cases were positive with the polyclonal anti-c-erbB-2 oncoprotein antibody. All 6 controls were negative with both antibodies. The ras p21 positive staining was found more frequently in the well or moderately differentiated, superficial and non-recurrent tumors than in the poorly differentiated (p < 0.01), muscle invasive (p < 0.05) and recurrent tumors (p < 0.01), while the c-erbB-2 gene product was more commonly detected in high-grade (p < 0.01), invasive (p < 0.01) and recurrent tumors (p < 0.05). Thus, the expression of either ras or c-erbB-2 was closely associated with the histological grade, clinical stage and recurrence of bladder transitional cell carcinomas.

The DNA-Malignancy-Grade(DNA-MG) of paraffin-embedded tumor specimens from 38 patients with bladder tumor was determined by image cytometry. A good correlation was found of DNA-MG with clinical staging and histopathological grading. The DNA-MG of T1 and G1 bladder tumor was less than 3.00, while that of T3-4 and G3 bladder tumor was greater than 3.00. The 5-year survival of patients with DNA-MG < 3.00 and > 3.00 was 100% and 36.9%, respectively. The data indicate that DNA-MG plays an important role in the assessment of the prognosis and the planning of treatment.

Effects of impulse noise exposure (25 impulses, peak level 165 dB SPL) on endocochlear potentials (EP) and CAP threshold were investigated in guinea pigs. Eight hours after exposure, EP recorded from the second turn of the cochlea was very low (19.0 +/- 1.7 mV) and the negative EP resulted from prolonged anoxia was markedly diminished (-EPmax = -6.4 +/- 1.3 mV). The return rate of EP after reventilation was also reduced. The positive EP returned to normal values 7 days after exposure but the negative EP and the CAP threshold did not. The results suggest that the stria vascularis was also damaged together with the organ of Corti, the degree of damage seemed to be heavier in the latter if only complete recovery of positive EP is taken into consideration. The stria vascularis was also not completely restored when examining the return rate of EP after reventilation. Mechanisms underlying the changes in EP and hearing sensitivity after noise exposure are discussed.

Flavoxate is a smooth muscle relaxant widely used to treat urgency and urge incontinence. It has been used in an unblinded, uncontrolled clinical trial in 14 urology departments in universities and major hospitals in the People's Republic of China involving 361 patients with urgency/incontinence of various types. Patients were given 200 mg three times daily, orally, for 2 weeks, although 33 patients received a daily dosage of 1200 mg. Frequency, urgency, dysuria, nocturia and incontinence were assessed and scored clinically prior to and after treatment. Three departments also included urodynamic investigations, e.g. monitoring of the end-residual volume. Results from 336 evaluable patients indicate that 228 (67%) were completely cured of urgency/incontinence symptoms, 66 (20%) were improved and 42 (13%) patients were unchanged. Flavoxate was also effective in 77.4% of patients refractory to previous anti-cholinergic treatment. Treatment did not increase the end-residual volume and adverse events occurred only in four (1.3%) patients, two (0.6%) of which discontinued the therapy. The 1200 mg dose produced a complete cure in 82% of patients and improvement in the remaining 18%, with no side-effects. In conclusion, flavoxate is an effective and well tolerated treatment for urgency/incontinence of various causes.
