INTRODUCTION: Unilateral gamma knife thalamotomy (GKT) is a treatment option for pharmacoresistant tremor of various aetiologies. There have been to date no randomised controlled trials performed to assess its safety and efficacy. Our aim was to summarise a two-year multimodal observation of patients with tremor caused by Parkinson's Disease (PD) or essential tremor (ET). MATERIAL AND METHODS: 23 patients with PD (n = 12) or ET (n = 11) were included. They underwent assessments before, V0 (n = 23), and 12 months, V12 (n = 23), and 24 months, V24 (n = 15), after unilateral GKT. Patients were assessed with psychological tests and acoustic voice analysis. Tremor assessment was performed with a digitising table using the Fahn-Tolosa-Marin rating scale (FTMRS). The Unified Parkinson's Disease rating scale part III (UPDRS-III) was also used in the PD group. Gait and balance was assessed using clinical tests, stabilometric platform, and treadmill. RESULTS: No side effects were observed in a two-year follow-up. There was no notable deterioration observed in the patients' psychological evaluation, speech, or assessment of gait and balance. The scores were significantly lower (p = 0.01) in parts A and B of FTMRS one year after GKT. In post hoc analysis, the scores did not differ significantly between V0 and V24. In FTMRS part C (activities of daily living), no significant change was observed. There was no significant difference in total UPDRS part III score or in score of UPDRS part III domains 3 and 4 ('tremor at rest' and 'action and postural tremor of hands') between measurements. CONCLUSIONS: UGKT may be a safe treatment modality if performed in an experienced centre. Tremor reduction may diminish over time, and UGKT did not lead to cognitive, gait or speech deterioration in a long-term observation.

The aim of this paper is to demonstrate progress in textronic UHF RFID transponder (RFIDtex tag) technology. The fundamental idea behind the RFIDtex tag design involves galvanic separation between circuits of the sewn antenna and the chip, which are electromagnetically coupled through a system of inductive loops. To advance the development of this concept, it is crucial to detect factors affecting the performance of the transponders. To achieve this goal, a mathematical model of the textronic UHF RFID transponder was developed. It involves relationships that describe the impedance of each element, the mutual inductance of the loops, and the chip voltage, and it enables the exploration of the influence of these variables on general parameters such as impedance matching and read range. Various analytical and numerical approaches were considered to obtain the value of the mutual inductance of the loops. The dimensions and geometry of the antenna, as well as the matching circuit in the microelectronic module, were taken into account. Based on the mathematical model, it was determined that mutual inductance strongly affects the chip voltage for frequencies higher than 800 MHz. The calculations from the mathematical model were compared with numerical simulations. Experimental studies were also conducted to investigate how the transponder performance is affected by either the distance between the centers of the loops or the conductivity of the threads used to embroider the antenna. The measurement results allowed us to conclude that even small imperfections in the manufacturing of the transponder, which slightly increase the vertical or horizontal distance between the centers of the loops, cause a dramatic decrease in the mutual inductance and coupling coefficient, significantly impacting the transponder's performance.

BACKGROUND: In melanoma treatment, an approach following positive sentinel lymph node biopsy (SLNB) has been recently deescalated from completion lymph node dissection (CLND) to active surveillance based on phase III trials data. In this study, we aim to evaluate treatment strategies in SLNB-positive melanoma patients in real-world practice. METHODS: Five-hundred-fifty-seven melanoma SLNB-positive patients from seven comprehensive cancer centers treated between 2017 and 2021 were included. Kaplan-Meier methods and the Cox Proportional-Hazards Model were used for analysis. RESULTS: The median follow-up was 25 months. Between 2017 and 2021, the percentage of patients undergoing CLND decreased (88-41%), while the use of adjuvant treatment increased (11-51%). The 3-year OS and RFS rates were 77.9% and 59.6%, respectively. Adjuvant therapy prolonged RFS (HR:0.69, p = 0.036)), but CLND did not (HR:1.22, p = 0.272). There were no statistically significant differences in OS for either adjuvant systemic treatment or CLND. Lower progression risk was also found, and time-dependent hazard ratios estimation in patients treated with systemic adjuvant therapy was confirmed (HR:0.20, p = 0.002 for BRAF inhibitors and HR:0.50, p = 0.015 for anti-PD-1 inhibitors). CONCLUSIONS: Treatment of SLNB-positive melanoma patients is constantly evolving, and the role of surgery is currently rather limited. Whether CLND has been performed or not, in a group of SLNB-positive patients, adjuvant systemic treatment should be offered to all eligible patients.

Obesity and metabolic syndrome (MetS) are growing problems among children and adolescents. There are no reports of changes in the activity of butyrylcholinesterase (BChE) in children and adolescents with metabolic syndrome especially after supplementation with extract from Aronia melanocarpa. Materials studied included plasma and erythrocytes isolated from peripheral blood of patients with MetS and healthy subjects. We have estimated the following parameters: acetylcholinesterase (AChE) and butyrylcholinesterase (BChE) activity, lipid peroxidation and lipids levels in plasma, and erythrocytes membrane. In patients with MetS, a significant increase in AChE and BChE activity, higher LDL-cholesterol and triacylglycerol levels, and lower HDL-cholesterol level were observed. Supplementation with A. melanocarpa extract resulted in mild but statistically significant reduction of total cholesterol, LDL-cholesterol, and triacylglycerol levels and caused an increase in HDL-cholesterol level and a decrease in lipid peroxidation in plasma patients with MetS. Additionally, a decrease in lipid peroxidation and cholesterol level and a decrease in AChE activity in the erythrocyte membranes after supplementation with A. melanocarpa were noted. Summarizing, an increase in AChE and BChE activity and disruption of lipid metabolism in patients with MetS were observed. After supplementation of MetS patients with A. melanocarpa extract, a decrease in AChE activity and oxidative stress was noted.

RESUMO

The Total Antioxidant Capacity of several human fluids was compared and the following sequence of TAC values was found: urine > saliva > blood plasma > milk approximately amniotic fluid >> sweat. Lower TAC values were found for the saliva of smokers than for that of non-smokers. Drinking of a cup of instant coffee increased the hydrogen peroxide content of urine but did not decrease the TAC of urine.
