RESUMO

Global climate changes undermine the effectiveness of 'set and forget' phytosanitary regulations. Uncertainties in future greenhouse gas emission profiles render it impossible to accurately forecast future climate, thus limiting the ability to make long-term biosecurity policy decisions. Agile adaptive biosecurity frameworks are necessary to address these climatic uncertainties and to effectively manage current and emerging threats. This paper provides opinions on these issues and presents a case study focusing on the threats posed by Aleurocanthus woglumi (citrus blackfly) to Europe. It delves into the biology of the species, its preferred hosts, and how climate change could affect its spread. Utilizing a bioclimatic niche model, the paper estimates the potential distribution of A. woglumi in Europe under recent historical and medium-term future conditions, revealing a potential expansion of its range into higher elevations and more northern regions by the year 2050. The main aim is to leverage the results to showcase the system's sensitivity to likely emission scenarios, essentially stress-testing for potential emerging threats to biosecurity policies and phytosanitary regulations. The results underscore the significance of considering global change factors in pest risk assessment and phytosanitary regulations for effective risk mitigation. Consequently, adaptive biosecurity measures are essential, encompassing horizon scanning, enhanced targeted surveillance, periodic updates of risk assessments, and adjustments to regulations. For instance, biosecurity risk management could involve establishing a set of trigger conditions to prompt updates of risk assessments, such as identifying a zone where the confirmed establishment of a pest signifies a significant change in the pest risk profile. For jurisdictions containing areas modeled as being climatically suitable under historical climates or future climate scenarios, we caution against importing untreated host materials from regions that are likely to become suitable habitats for A. woglumi in the future. Moreover, it is important to consider both present and future climate change scenarios when making decisions to effectively address the threats posed by invasive species. In the case of highly impactful invasives, investing in preemptive biological control measures may prove to be a prudent choice.

The box tree moth (Cydalima perspectalis Walker, 1859; Lepidoptera: Crambidae) is an invasive species naturally distributed in Asia. The caterpillars in all developmental stages cause damage through defoliation of plants, and ultimately the death of the plant itself may occur. It is possible to recognize this species by its silk barriers and threads, and in the case of an intense attack, the entire plant will be covered with them. In Europe, this species' presence was first recorded in 2007 in Germany and the Netherlands, and it is now widely distributed. In Croatia, its existence was first recorded in 2012, in Istria, while substantial damages were recorded in 2013. This work aimed to determine the morphological variability of C. perspectalis from Croatia and assess its invasive character, the possibility of flight, and the risk of further spread. The methods of geometric morphometrics were used as the analysis of wing shape. A total of 269 moths from different locations in Croatia were collected, the upper wings of males and females were analyzed using 14 landmarks. Significant differences in wing shapes between terrestrial and coastal populations were found, as well as subtle wing shape sexual dimorphism. The implications of this variability in species invasiveness and capacity of spread are discussed in this paper. We also extrapolate the usefulness of our results and suggest strategies for predicting and managing invasive species.

Climate change and invasive species are major environmental issues facing the world today. They represent the major threats for various types of ecosystems worldwide, mainly managed ecosystems such as agriculture. This study aims to examine the link between climate change and the biological invasion of insect pest species. Increased international trade systems and human mobility have led to increasing introduction rates of invasive insects while climate change could decrease barriers for their establishment and distribution. To mitigate environmental and economic damage it is important to understand the biotic and abiotic factors affecting the process of invasion (transport, introduction, establishment, and dispersal) in terms of climate change. We highlight the major biotic factors affecting the biological invasion process: diet breadth, phenological plasticity, and lifecycle strategies. Finally, we present alien insect pest invasion management that includes prevention, eradication, and assessment of the biological invasion in the form of modelling prediction tools.

The Asian ladybird (Harmonia axyridis Pallas), native to Asia, is one of the 100 most invasive species in the world and has spread worldwide. This study aimed to characterize color forms of H. axyridis in Croatia and to analyze the variability of wing shape between populations and indicated forms. Geometric morphometric methods were used to analyze a total of 129 left and right wings in males and 126 left and right wings in females of H. axyridis collected from four different sites in Croatia. The results show a significant difference in wing shapes between the studied forms. Each form had its own specific morphotype that likely originated under the influence of genetic changes in the species. This study demonstrates that the use of geometric morphometric analysis is effective in studying the variability in H. axyridis populations. As this study is the first of its kind, for further clarity, it is necessary to conduct additional studies on a larger number of sites and an equal number of individuals of all forms.

Climate change and global warming are of great concern to agriculture worldwide and are among the most discussed issues in today's society. Climate parameters such as increased temperatures, rising atmospheric CO2 levels, and changing precipitation patterns have significant impacts on agricultural production and on agricultural insect pests. Changes in climate can affect insect pests in several ways. They can result in an expansion of their geographic distribution, increased survival during overwintering, increased number of generations, altered synchrony between plants and pests, altered interspecific interaction, increased risk of invasion by migratory pests, increased incidence of insect-transmitted plant diseases, and reduced effectiveness of biological control, especially natural enemies. As a result, there is a serious risk of crop economic losses, as well as a challenge to human food security. As a major driver of pest population dynamics, climate change will require adaptive management strategies to deal with the changing status of pests. Several priorities can be identified for future research on the effects of climatic changes on agricultural insect pests. These include modified integrated pest management tactics, monitoring climate and pest populations, and the use of modelling prediction tools.

Human noroviruses (HuNoV) are the dominant cause of viral gastroenteritis in all age groups worldwide. In this study, we investigated the effects of pomegranate peel extract (PPE) on the reduction of HuNoV in different food models, on surfaces of fresh produce (green onion and cherry tomato), in low-fat milk, and simulated gastrointestinal fluids. The antiviral efficacy of PPE against HuNoV was evaluated by quantifying the number of residual virus genomes using a quantitative reverse transcription PCR (qRT-PCR) assay. Pomegranate peel, considered as a waste product of industrial processing, is known for beneficial health effects and broad antimicrobial activity due to the high content of phenolic compounds and tannins. PPE showed significant antiviral properties against HuNoV both in phosphate-buffered saline (PBS) and simulated gastric fluid. The reduction of HuNoV by pomegranate juice was lower than with PPE, which could be attributed to the lower content of antimicrobial compounds. A pretreatment of cherry tomato and green onion surfaces with PPE significantly reduced the amount of HuNoV particles that adhered to those surfaces during subsequent virus suspension treatment. A detrimental effect of PPE on HuNoV structure was confirmed by transmission electron microscopy. Our results indicate that PPE is a natural antiviral agent effective against food-borne noroviruses.

The granary weevil, Sitophilus granarius Linnaeus 1875, is a primary pest of stored grains worldwide. Feeding damage and progeny production of S. granarius was estimated to identify the levels of resistance of the insect on different durum wheat cultivars. Insect attack on four different durum wheat cultivars was investigated over a period of 20 weeks. Durum wheats were artificially infected with 20 individuals of S. granarius. Every two weeks the sample weight, hectoliter weight, moisture and the number of live weevils, including their number of progenies, were recorded. Overall findings revealed different levels of resistance of different durum wheat cultivars to S. granarius infestation. The Primadur cultivar had the highest resistance, followed by the Marco Aurelio and Cesare cultivars followed finally by the Tito Flavio cultivar which was highly susceptible to S. granarius. For all cultivars, apart from Primadur, S. granarius metabolism increased humidity and temperature, leading to grain degradation and resulting in the potential complete loss of market value if under field conditions. Evidently, durum wheat characteristics affect the life cycle of S. granarius, primarily their progeny, and thus the damage they undertake to the wheat itself. These findings are important because they enable the strategic selection of wheat cultivars that can be stored for a longer time period, while more sensitive wheat cultivars can be selected for shorter storage time and thus faster delivery to market.

Introduction: Prescribing trends in maintenance therapy of patients with primary psychotic disorders (PSD) may vary worldwide. Present study aimed to investigate prescription patterns in a sample of outpatients with PSD from Serbia.Methods: In a sample of 73 PSD outpatients we analysed the rate of antipsychotic polypharmacy and psychotropic polypharmacy, concomitant continual benzodiazepine use, and associations between therapy, psychotic symptoms and quality of life.Results: Maintenance therapy (median daily dose 321 mg of chlorpromazine equivalents) predominantly consisted of monotherapy with second generation antipsychotics (45.2%), followed by antipsychotic polypharmacy based on first and second generation combination (25.0%). The median number of psychotropic drugs was 3. Benzodiazepines were continually prescribed to more than 60% of patients (mean daily dose 2.9 ± 2.0 mg lorazepam equivalents). Patients with benzodiazepine use had significantly more psychotropic medications and more antipsychotic polypharmacy, poorer quality of life and more severe psychopathology in comparison to another group.Conclusion: The present study demonstrated new information regarding the prescription patterns of psychotropic drugs in outpatients with PSD in Serbia, amplified with clinically relevant information. This study also revealed distinct prescription patterns concerning antipsychotic/benzodiazepine polypharmacy. Overall, such findings are likely to contribute to improving clinical practice and care for patients with PSD in general.KeypointsPresent exploratory research aimed to elucidate trends of antipsychotics polypharmacy and concomitant use of psychotropic medications including benzodiazepines in the maintenance treatment of outpatients with schizophrenia and other psychotic disorders, amplified with clinically relevant information (symptoms and quality of life).'Antipsychotic (AP) polypharmacy' was defined as concurrent use of more than one AP for at least 1 month; 'Psychotropic polypharmacy' was defined as the combination of AP and a different class of psychotropic drugs medication for at least one month.The median number of prescribed psychotropic drugs was 3 (mean 3.1 ± 1.1) and the average AP daily dose was moderate (median 321 mg of chlorpromazine equivalents). However, the rates of AP polypharmacy (45.2%) and benzodiazepine prescription on a continual basis (>60%) found in our sample could be considered relatively high.Outpatients with higher AP daily dose and higher BPRS symptom score were receiving more benzodiazepines.For improvement of the local, as well as general clinical practice and care for patients with psychotic disorders, and for education in psychiatry, such analyses need to be done on a regular basis and on larger samples.

The codling moth, Cydia pomonella L., is a serious insect pest in pome fruit production worldwide with a preference for apple. The pest is known for having developed resistance to several chemical groups of insecticides, making its control difficult. The control and management of the codling moth is often hindered by a lack of understanding about its biology and ecology, including aspects of its population genetics. This review summarizes the information about the origin and biology of the codling moth, describes the mechanisms of resistance in this pest, and provides an overview of current research of resistant pest populations and genetic research both in Europe and globally. The main focus of this review is on non-pesticide control measures and anti-resistance strategies which help to reduce the number of chemical pesticides used and their residues on food and the local environment. Regular monitoring for insecticide resistance is essential for proactive management to mitigate potential insecticide resistance. Here we describe techniques for the detection of resistant variants and possibilities for monitoring resistance populations. Also, we present our present work on developing new methods to maintain effective control using appropriate integrated resistance management (IRM) strategies for this economically important perennial pest.

Insect infestations within stored product facilities are a major concern to livestock and human food industries. Insect infestations in storage systems can result in economic losses of up to 20%. Furthermore, the presence of insects and their waste and remains in grain and stored foods may pose a health risk to humans and livestock. At present, pests in commercial storage are managed by a combination of different methods ranging from cleaning and cooling to treatment of the stored material with contact insecticides or fumigation. The availability of pesticides for the treatment of grain and other stored products is decreasing owing, in some cases, to environmental and safety concerns among consumers and society, thus emphasizing the need for alternative eco-friendly pest control methods. One of the potential methods is the use of ozone. Although the mechanism of action of ozone on insects is not completely known, the insect's respiratory system is a likely the target of this gas. The main goal of this investigation was to determine the efficacy of ozone in the suppression of adult wheat weevils Sitophilus granarius. In the experiments conducted, different durations of ozone exposure were tested. In addition to ozone toxicity, the walking response and velocity of wheat weevils were investigated. The results showed the harmful effects of ozone on these insects. In addition to mortality, ozone also had negative effects on insect speed and mobility. The efficiency of the ozone treatment increased with increasing ozone exposure of insects. The ability of ozone to reduce the walking activity and velocity of treated insects is a positive feature in pest control in storage systems, thereby reducing the possibility of insects escaping from treated objects. The results of this investigation suggest that ozone has the potential to become a realistic choice for suppressing harmful insects in storage systems for humans and livestock, either alone or as a complement to other control methods.

The codling moth (CM) (Cydia pomonella L.) is the most important apple pest in Croatia and Europe. Owing to its economic importance, it is a highly controlled species and the intense selection pressure the species is under has likely caused it to change its phenotype in response. Intensive application of chemical-based insecticide treatments for the control of CM has led to resistance development. In this study, the forewing morphologies of 294 CM (11 populations) were investigated using geometric morphometric procedures based on the venation patterns of 18 landmarks. Finite element method (FEM) was also used to further investigate the dispersal capabilities of moths by modelling wing deformation versus wind speed. Three treatments were investigated and comprised populations from integrated and ecological (susceptible) orchards and laboratory-reared non-resistant populations. Forewing shape differences were found among the three treatment populations investigated. Across all three population treatments, the movement of landmarks 1, 7, 8, 9, and 12 drove the wing shape differences found. A reliable pattern of differences in forewing shape as related to control practice type was observed. FEM revealed that as wind speed (m/s-1) increased, so too did wing deformation (mm) for CM from each of the three treatments modelled. CM from the ecological orchards displayed the least deformation followed by integrated then laboratory-reared CM, which had the highest wing deformation at the highest wind speeds. This study presents an affordable and accessible technique that reliably demonstrates wing shape differences, and thus its use as a population biomarker to detect resistance should be further investigated.

BACKGROUND: The superiority of dabigatran has been well proven in the standard dosing regimen in prevention of stroke and systemic embolism in patients with non-valvular atrial fibrillation (NVAF) and extended venous thromboembolism (VTE) treatment. Dabigatran, an anticoagulant with a good safety profile, reduces intracranial bleeding in patients with atrial fibrillation and decreases major and clinically relevant non-major bleeding in acute VTE treatment. However, several important clinical issues are not fully covered by currently available directions with regard to dabigatran administration. The prominent one is reflected in the fact that dynamic impairment in renal function due to dehydratation may lead to haemorragic complications on the one hand, while on the other hand glomerular hyperfiltration may be a possible cause of dabigatran subdosing, hence reducing the drug's efficacy. Furthermore, limitations of the Cockcroft-Gault formula, considered a standard equation for assessing the renal function, may imply that other calculations are likely to obtain more accurate estimates of the kidney function in specific patient populations. Method and Conclusions: Although not routinely recommended, a possibility of monitoring dabigatran in special clinical settings adds to optimization of its dosage regimens, timely perioperative care and administration of urgently demanded thrombolytic therapy, therefore significantly improving this drug's safety profile. Despite the fact that dabigatran has fewer reported interactions with drugs, food constituents, and dietary supplements, certain interactions still remain, requiring considerable caution, notably in elderly, high bleeding risk patients, patients with decreased renal function and those on complex drug regimens. Additionally, upon approval of idarucizumab, an antidote to dabigatran solution, hitherto being a major safety concern, has been finally reached, which plays a vital role in life-threatening bleeding and emergency interventions and surgery.

Despite the application of new antiplatelet drugs (prasugrel and ticagrelor), dual antiplatelet therapy with clopidogrel and aspirin remains the standard for patients with acute coronary syndrome undergoing percutaneous coronary intervention, especially in countries of low socioeconomic status. Regardless of the proven benefits, numerous studies have shown that certain groups of patients who receive standard doses of clopidogrel and aspirin do not respond adequately, and many of them also exhibit adverse cardiovascular events. Studies have shown that the risk of stent thrombosis and ischemic complications is higher in patients with: acute coronary syndrome, diabetes mellitus, thrombocytosis, reduced systolic function of the left ventricle with ejection fraction less than 30%, presence of multiple stents, longer and thinner stents, and renal failure. In these patients it is particularly important to assess the response to clopidogrel and selecting adequate antiplatelet therapy; this provides an impetus for platelet function tests. The second especially significant group to target for laboratory evaluation includes patients with increased risk of bleeding, such as elderly patients, patients with low body weight, anemia, thrombocytopenia, renal failure, past or current ventricular or duodenal ulcer, coagulopathy, or liver disease. The third important application of platelet function tests entails the preparation and evaluation of the time for surgical interventions or invasive diagnostic procedures in patients on antiplatelet therapy. These tests can also be helpful for monitoring the effects of therapy of bleeding due to platelet dysfunction. For high-risk patients the careful selection of optimal antiplatelet drug(s) on the basis of estimated individual risk of thrombosis and bleeding, pharmacodynamic characteristics of each drug, and patient̀s comorbidity remains essential.

The renin-angiotensin-aldosterone system can be blocked at specific levels by using different classes of pharmacologic agents, including angiotensin-converting-enzyme inhibitors, angiotensin II receptor blockers and mineralocorticoid receptor antagonists. Broad use of the latter, such as spironolactone, has been limited by significant incidence of gynecomastia and other sex-related adverse effects. These problems can be overcome with use of eplerenone, a selective mineralocorticoid receptor antagonist. Eplerenone has been specifically developed to bind selectively to the mineralocorticoid receptors in order to minimize binding to the progesterone and androgen receptors. In the last decade, multiple scientific evidences have been accumulated showing the efficacy and safety of the drug in multiple clinical conditions, including heart failure and arterial hypertension. Eplerenone is generally well tolerated, with the most frequent adverse event being hyperkalemia, with sexual adverse events (i.e. gynecomastia) being more uncommon, due to the selectivity of eplerenone. This review focuses on the pharmacodynamic and pharmacokinetic properties of eplerenone, thus providing the scientific basis to fully understand drug-to-drug interactions, in particular, and its efficacy and tolerability, in general. Noteworthy, the activity of eplerenone in special conditions and different patient populations is summarized.

The data that episodes and sequels of venous thromboembolism (VTE) are recorded in a significant percentage of patients receiving standard anticoagulants as VTE prophylaxis (unfractionated, low-molecular-weight heparin and vitamin K inhibitors) as well as the fact that these drugs have significant limitations and that they may cause serious side-effects in some patients indicate the need for the introduction of new anticoagulant drugs. Fondaparinux, a selective inhibitor of Factor Xa, administered following major orthopedic surgeries having a high risk for the development of VTE, is more efficient than enoxaparin sodium used in European and North-American approved doses. The increased incidence of major bleeding (excluding fatal) due to fondaparinux could be perhaps lowered by dosage reduction in patients with a mildly decreased creatinine clearance. Dabigatran, a peroral direct thrombin inhibitor, administered for VIE prophylaxis in elective hip and knee surgery, showed in to date studies the efficacv comparable (if dabiqatran is given in both dosage regimes of 150 mg and 220 mg daily) or superior (if dabigatran is given at a dose of 220 mg daily) to enoxaparin administered in European-approved doses, while North American-approved doses of enoxaparin were superior than dabigatran in VTE reduction. No significant differences in bleeding rates were determined in any of the study groups. We consider that the introduction of new anticoagulants, including fondaparinux and dabigatran, will contribute to the establishment of a better safety profile and efficacy, and will also enable adequate therapy individualization for each patient depending on his/hers clinical characteristics. The introduction of novel peroral anticoagulants will, inter alia, significantly contribute to improvement in the quality of life, release the patient from numerous limitations in nutrition, interreaction, frequent laboratory monitoring, and also significantly improve therapeutic predictability.

Acute heart failure (AHF) is one of the most important cardiovascular syndromes associated with high cardiovascular morbidity, and is the major cause of admission in emergency departments worldwide. The clinical complexity of AHF has significantly increased, mostly due to the comorbidities: diabetes, arterial hypertension, dyslipidemia, obesity, peripheral vascular disease, renal insufficiency and anemia. Numerous clinical trials have demonstrated a frequent association of AHF and diabetes. Since AHF is a very heterogeneous condition, it is important to identify clinical and laboratory parameters useful for risk stratification of these populations. Hyperglycemia may be one of the most convenient, since it is widely measured, easily interpreted, and inexpensive. Acute coronary syndrome (ACS), arrhythmias and poor compliance to chronic medications are considered to be the most frequent precipitating factors of AHF in diabetics. Several studies identified diabetes as the most prominent independent predictor of morbidity and mortality in both acute and chronic heart failure (HF) patients. The following parameters were identified as the independent predictors of in-hospital mortality in patients with AHF and diabetes: older age, systolic blood pressure <100 mmHg, ACS, non-compliance, history of hypertension, left ventricular ejection fraction (LVEF) <50%, serum creatinine >1.5 mg/dL, marked elevation of natriuretic peptides, hyponatremia, treatment at admission without ACE inhibitors/ARBs/ß-blockers, and no percutaneous coronary intervention (PCI) as a treatment modality. The most frequent cause of AHF is ACS, both with ST segment elevation (STEMI) or without (NSTEMI). Hyperglycemia is very common in these patients and although frequently unrecognized and untreated, has a large in-hospital and mortality significance.

RESUMO

Adequate thromboprophylaxis primarily requires timely detection of reversible and irreversible risk factors of venous thromboembolism (VTE) and their categorization. It is important to note that the highest percentage ofVTE episodes occur in non-surgical (medical) patients and that VTE develops in a large number of surgical patients upon hospital discharge; this emphasizes the need for adequate VTE prevention in inflammatory diseases, acute medical illness and other medical diseases as well as for prolonging and optimizing the anticoagulant regimen after surgical intervention in the primary VTE prophylaxis. As almost completely unrecognized and neglected major risk factors of VTE in clinical practice, we particularly point out the chronic obstructive pulmonary disease (COPD) and heart failure, especially in NYHA functional class III and IV patients with significantly reduced left heart ventricle. It is necessary to raise clinicians' awareness of a potential danger from wrongly and one-sidedly interpreted dyspnea and coughing signs in patients with COPD as typical symptoms of basic respiratory disease as well as from ascribing the signs of disease aggravation in heart failure patients exclusively to cardial status worsening, neglecting the possibility of having unrecognized and untreated pulmonary embolism at issue. Contemporary way of life enhances the development of new VTE risk factors such as traveler's thrombosis, in particular during long-haul flights as well as in individuals sitting at a computer for prolonged periods (e-thrombosis). Determining and recognizing VTE risk factors, especially those formerly neglected nonsurgical ones and simultaneous presence of multiple risk factors within a given period is required for defining an adequate anticoagulant regimen in primary VTE prophylaxis for surgical and non-surgical (medical) patients.
