RESUMO
Meniscal tears in skeletally immature patients are usually associated with discoid menisci and knee ligament lesions. We retrospectively evaluated 20 patients, less than 16 years old, with isolated meniscal tears who underwent partial meniscectomies. A traumatic event caused the tear in 14 patients and in 6 patients there was no evident cause. 9 knees had a discoid meniscus. The average follow-up was longer than 3 years. 18 patients had excellent post operative Lysholm scores. We did not observe degenerative signs on radiographic follow up. 3 patients required another surgery: one had a meniscal re-tear and 2 had a symptomatic chondral lesion of the external femoral condyle. These 2 patients had a bad functional result. Partial meniscectomies in patients younger than 16 years old have excellent functional results without degenerative radiological signs at short to mid term follow-up. Bad results were associated with external tears and secondary chondral lesions.
Las roturas meniscales en niños son poco frecuentes, especialmente sin lesiones asociadas. Se estudian retrospectivamente 21 rodillas de 20 pacientes menores de 16 años sometidos a una meniscectomía parcial por una lesión meniscal aislada. De estas rodillas, 9 presentaron un menisco discoídeo roto. En 14 pacientes la rotura meniscal se produjo por un traumatismo y en 6 la sintomatología se inició espontáneamente. Al final de un seguimiento promedio de 37 meses, 18pacientes tuvieron un resultado funcional excelente. Dos pacientes evolucionaron con una lesión condral del cóndilo femoral externo y tuvieron un resultado insatisfactorio. Tres pacientes fueron reintervenidos. No se observaron signos radiográficos degenerativos. La meniscectomía parcial en pacientes menores de 16 años presenta un porcentaje alto de resultados funcionales excelentes y ausencia de signos radiográficos degenerativos a corto plazo. Los resultados insatisfactorios se asociaron a lesiones condrales secundarias luego de una meniscectomía externa.

Introducción. La estafilococcemia es una complicación seria del paciente en estado crítico. Objetivo. Reportar la incidencia de estafilococemia en una unidad de cuidados intensivos de enfermedades infecciosas (UCIEI). Pacientes y métodos. Revisamos los expedientes de 901 pacientes admitidos a un hospital de infectología. Incluimos a todos los pacientes con manifestación clínica de estafilococcemia. Resultados. Cuatro pacientes reunieron los criterios para el diagnóstico de estafilococcemia. Encontramos fiebre, dolor, rash, hipotensión, síndrome compartamental, ictericia y síndrome de disfunción orgánica múltiple. Se aisló Staphylococcus aureus en un caso y la mortalidad fue del 75 por ciento. Conclusión. La incidencia de la estafilococcemia es baja, pero la morbilidad y la mortalidad es alta

A 9 month old boy was admitted to the hospital with the diagnosis of meningoencephalitis 15 days after having a clinically diagnosed chickenpox. Lumbar puncture showed clear CSF with 0.23 g/l of proteins, 57 mg/dl of glucpse, 30 red cells/mm3 and 5 leukocytes/mm3. Blood count showed a packed red cell volume of 22 percent, a hemoglobin of 7 g/dl, 14800 leukocytes with 1 percent eosinophils, 5 percent band and 39 percent segmented neutrophils, 50 percent lymphocytes and 5 percent monocytes and a decreased platelet count. On the fourth hospitalization day, the patient had vomiting, irritability and stiff neck. A new lumbar puncture showed a clear CSF that differed from the former only on the glucose level that increased to 102 mg/dl. The patient died and the necropsy showed a congestive and enlarged brain and congestive meninges infiltrated with lymphocytes. There was lymphoid follicle hyperplasia in the small bowel and enlarged mesenteric lymph nodes. Samples of brain, brain stem, spinal cord and tools were sent for virological study. A coxsakie B-5 virus was isolated from the spinal cord sample

Surgery in 227 cases of urinary stress incontinence (USI) associated to other abdominal and/or vaginal pathologies, by means Burch colposuspension, has given a success rate of 90.3% recovery of USI 6.6% improved urinary incontinence grade, an 98.3% recovery rate of the concomitant genital prolapse, in a follow-up period mean of 42 moths. Postoperative complication of the surgical procedure are analized.
