RESUMO

In this paper, a log-linear multidimensional Rasch model is proposed for capture-recapture analysis of registration data. In the model, heterogeneity of capture probabilities is taken into account, and registrations are viewed as dichotomously scored indicators of one or more latent variables that can account for correlations among registrations. It is shown how the probability of a generic capture profile is expressed under the log-linear multidimensional Rasch model and how the parameters of the traditional log-linear model are derived from those of the log-linear multidimensional Rasch model. Finally, an application of the model to neural tube defects data is presented.

A sample of 90 persons who had been hospitalized for severe burns were interviewed 1-4 years after the incident. Current DSM-IV post-traumatic stress disorder (PTSD) was assessed with the Composite International Diagnostic Interview. Perceived attributed responsibility and related positive and negative emotional states were examined using a semi-structured interview. Findings showed that PTSD was established in 8% of the participants and partial PTSD in 13%. In a homogeneity analysis (HOMALS), PTSD was associated with the attribution of responsibility for the incident to impersonal relationships and with a negative emotional state. The absence of (partial) PTSD was associated with the attribution of responsibility to close relationships, internal and circumstance-related attribution of responsibility and neutral or forgiving feelings. In logit analyses, both emotional state as well as attributed responsibility are significantly related to (partial) PTSD. However, the model including emotional state showed to have the best fit. Although further research is needed, these results may indicate that professionals working in burn care should consider the emotional state in relation to perceived attribution of responsibility when considering PTSD. Promoting forgiveness may be a beneficial strategy in dealing with post-traumatic stress reactions.

The sub-scales of the SF-36 in the Dutch National Study are investigated with respect to unidimensionality and reliability. It is argued that these properties deserve separate treatment. For unidimensionality we use a non-parametric model from item response theory, called the Mokken scaling model, and compute the corresponding scalability coefficients. We estimate reliability under the Mokken model, assuming that the items are double homogeneous, and compare it to Cronbach's alpha. The scalability of the sub-scale general health perceptions is medium (H = 0.46), and for the other sub-scales it is strong (H > or = 0.6). The reliability in terms of alpha indicates that all sub-scales can be used in basic research (alpha > 0.70), but that only physical functioning can be used for clinical applications of quality of life (alpha > 0.90). The relative merits of our approach are discussed.

Following criticism of Kohlberg's theory of moral judgment, an empirical re-examination of hierarchical stage structure was desirable. Utilizing Piaget's concept of reflective abstraction as a basis, the hierarchical stage structure was investigated using a new method. Study participants (553 Dutch university students and 196 Russian high school students) sorted statements in terms of moral sophistication. These statements were typical for the different stages of moral development as defined in Colby and Kohlberg. The rank ordering performed by participants confirmed the hypotheses. First, despite large individual variation, the ordering of the statements that gave the best fit revealed that each consecutive Kohlbergian stage was perceived to be more morally sophisticated. Second, the lower the stage as represented by the items, the higher the agreement among the participants in their ranking; and the higher the stage as represented by the items, the lower the agreement among the participants in the rankings. Moreover, the pivotal point depended on the developmental characteristics of the sample, which demonstrated a developmental effect: The ordering of statements representative of moral stages below one's own current stage was straightforward, whereas the ordering of statements above one's own stage was difficult. It was concluded that the Piagetian idea of reflective abstraction can be used successfully to operationalize and measure the hierarchical nature of moral development.

RESUMO

The capture-recapture method is useful for the estimation of the incidence and prevalence of a disease if there exist multiple lists that can be linked. Some assumptions have to be fulfilled for the estimate to be adequate. For example, for every list every individual should have the same probability to be on the list, and the lists should be independent. In case of two lists one has to assume that these are independent in order to get an estimate. When three or more lists are available this strong assumption may be mitigated.

This study explores communication patterns between nurses and elderly patients in two different care settings. In a sample of 181 video-taped nursing encounters, involving 47 nurses and 109 patients, a study was made of nurse-patient communication. The video recordings were observed using an adapted version of Roter's Interaction Analysis System, which yields frequencies of 23 types of verbal behaviours. These data were analyzed using correspondence analysis, to reduce them to a smaller number of verbal categories, in which two socio-emotional categories and three categories with task-related communication, could be distinguished. For each encounter five summary statistics corresponding to these categories were calculated. Using analysis of variance, it was shown that the amount of socio-emotional interaction in both settings appeared to be higher than was reported in previous studies into nurse-patient communication. Compared with the home for the elderly, communication was more task-related in home care.
