RESUMO

The aim of this study was to compare the impact of the first year of wearing of a fixed orthodontic appliance on the Oral Health-Related Quality of Life (OHRQoL) between boys and girls, by means of a condition-specific instrument. The study included 69 adolescents aged 10 to 18 years, who were undergoing orthodontic treatment with a fixed appliance. Of the 69 adolescents, 38 were girls (55.1%) and 31 were boys (44.9%). They answered the Brazilian version of the Impact of Fixed Appliance Measure (B-IFAM) questionnaire three months (T1) and one year (T2) after the fixed appliance was installed. This questionnaire contains 43 questions, distributed across nine domains. The higher the scores, the more negative the perception of the adolescent concerning the impact of the fixed appliance on his/her OHRQoL. Sociodemographic and clinical variables were also analyzed, and statistical analysis was performed. For the domains, the effect size (the magnitude of the difference between girls and boys) and the minimal clinically important difference were also calculated. The adjusted regression showed that there was a significantly greater increase in the overall B-IFAM score in girls than in boys, indicating a more negative perception of the OHRQoL over the study time [Coefficient=11.77 (3.47- 20.60), p=0.006]. From T1 to T2, there was a significantly greater increase in the scores (more negative perception of OHRQoL over time) in girls than in boys for the domains aesthetics (p=0.034) and physical impact (p=0.011). These differences were clinically significant. The effect size (the magnitude of the difference) was moderate. The impact of wearing a fixed appliance on the OHRQoL was more negative in girls than in boys during the first year of orthodontic treatment.


O objetivo deste estudo foi comparar o impacto do primeiro ano de uso do aparelho fixo na qualidade de vida relacionada a saúde bucal (QVRSB) entre meninas e meninos, através de um instrumento condição específica. Sessenta e nove adolescentes entre 10 e 18 anos, em tratamento ortodôntico com aparelho fixo foram incluídos. Adolescentes responderam ao questionário Impact of fixed appliance measure (B-IFAM) no terceiro mês de uso do aparelho fixo (T1) e um ano após a colagem do aparelho fixo (T2). Este questionário possui 43 perguntas, distribuídas em nove domínios. Quanto maior os escores, mais negativa a percepção do adolescente com relação ao impacto do aparelho fixo na QVRSB. Variáveis sociodemográficas e clínicas também foram avaliadas. Análise estatística foi realizada. Para os domínios, tamanho de efeito (a magnitude da diferença entre meninas e meninos) e diferença mínima clinicamente importante também foram calculadas. Dos 69 adolescentes, 38 eram meninas (55,1%) e 31 eram meninos (44,9%). Na regressão ajustada, meninas apresentaram um aumento significativamente maior do escore total do B-IFAM do que meninos, indicando uma percepção mais negativa da QVRSB ao longo do tempo de acompanhamento [Coeficiente=11,77 (3,47­20,60), p=0.006]. Meninas apresentavam um aumento significativamente maior dos escores de T1 para T2 (percepção mais negativa da QVRSB ao longo do tempo) em relação aos meninos para os domínios estética (p=0,034) e impacto físico (p=0,011). Estas diferenças também foram clinicamente significativas. O tamanho do efeito (a magnitude da diferença) foi moderado. Meninas demonstraram um impacto mais negativo do uso do aparelho fixo na QVRSB do que meninos nos 12 primeiros meses de tratamento ortodôntico.

RESUMO

This systematic review and meta-analysis aimed to evaluate the association between sense of coherence (SOC) and periodontal outcomes. Electronic searches were performed in 6 databases. Seventeen studies that evaluated the association between SOC and periodontal outcomes were included. The included studies demonstrated that individuals with a stronger SOC were more likely to present improved periodontal outcomes. The meta-analysis showed that individuals with a lower SOC were 3.31 times more likely to present bleeding on probing. Sons/daughters of mothers with a lower SOC were 3.22 times more likely to present gingival bleeding. Individuals with a stronger SOC have better periodontal health.

OBJECTIVE: To compare removable and fixed orthodontic devices in the correction of non-skeletal anterior crossbite in children and adolescents in the mixed dentition. MATERIALS AND METHODS: Electronic searches were conducted in the following databases: PubMed, Web of Science, Scopus, Medline Ovid, Lilacs, US Clinical Trials, and Proquest. A hand search of the reference lists of the included articles and a Google Scholar search were also conducted. References were evaluated by 2 review authors. Articles that met the eligibility criteria were included. Data extraction, methodological quality assessment (Cochrane tool), and strength of the evidence evaluation (GRADE) were also carried out. RESULTS: Seven articles were included. The results showed that removable and fixed devices were equally efficacious for overjet correction. Removable and fixed devices can also present inconveniences regarding pain and discomfort levels, the accomplishment of everyday activities (leisure and school), and the performance of functions, such as chewing and speech. However, treatment time and costs were significantly lower in orthodontic therapy with fixed appliances. Sequence generation, allocation concealment, and complete outcome data were not a concern. Blinding of participants or personnel was not reported in any article, and blinding of the assessor was a concern in 2 articles. Selective reporting was a concern in 2 articles. The certainty of the evidence for overjet correction was very low. CONCLUSION: Removable and fixed orthodontic devices are efficacious for overjet correction in non-skeletal anterior crossbite. However, treatment time and costs are lower for cases treated with fixed devices.

BACKGROUND: Quality of life (QoL) measurements have been employed in medicine and dentistry to assess the effect of diseases and treatment on individuals' lives. AIM: To assess the effect of the initial 12 months of fixed orthodontic appliance therapy on the families of Brazilian adolescents. DESIGN: A cohort of 11- to 12-year-old adolescents and their parents/guardians participated. Treatment group (TG) consisted of 101 parents/guardians of adolescents who began fixed orthodontic appliance therapy. They completed four sets of the Family Impact Scale (FIS) at baseline, 1 month, 6 months and 12 months after the bonding of fixed appliances. Control group (CG) was composed of 175 parents/guardians of adolescents, who did not begin treatment, answering four sets of the FIS at the same intervals. The subscales of FIS were parental/family activity (PA), parental emotions (PE), family conflict (FC) and financial burden (FB). Statistical analysis was performed. RESULTS: The interaction of group (treatment) with time allowed one to observe significant changes in the PA (P < .001), PE (P < .001), FC (P < .001) and overall FIS score (P < .001), demonstrating an improvement in the subscale and overall QoL of families of adolescents. CONCLUSIONS: The effect of fixed orthodontic appliance therapy on the families of adolescents was highly beneficial.

Sense of coherence (SOC) is regarded as the individuals' capacity for managing the stressors over their lifespan. The aim of this systematic review and meta-analysis was to evaluate the association between SOC and dental caries. Electronic searchers were conducted in six databases: PubMed, Medline, Scopus, Web of Science, Lilacs and Proquest from their date of inception until November/2017. An update took place in August/2018. A hand search in the reference list of the included articles and gray literature search were also carried out. Retrieved titles/abstracts were screened by two review authors. Data of the included articles were extracted and quality assessment was also conducted. The first search retrieved 346 titles/abstracts. The update retrieved 33 records. Following the removal of 70 duplicates, 309 references were screened and 17 were included. The quality assessment ranged from low quality articles to high quality articles. High quality study showed that adult individuals with lower SOC were more likely to have dental caries. Meta-analysis showed that adolescents with low SOC were 5.41 times more likely to present dental caries than adolescents with high SOC (CI = 2.15-13.59). Mothers with low SOC were 5.55 times more likely to have children/adolescents with dental caries than mothers with high SOC (CI = 2.92-10.57). The subgroup analysis exploring continuous data showed that the SOC of mothers of children/adolescents without dental caries was significantly higher than the SOC of mothers of children/adolescents with dental caries (CI = 0.10-0.51). Higher levels of SOC seem to be associated with lower levels of dental caries.

OBJECTIVE: To evaluate the impact of the first six months of orthodontic treatment with fixed appliances on the oral health-related quality of life (OHRQoL) of adolescents, comparing boys and girls, using a specific condition questionnaire. DESIGN: Cross-sectional. SETTING: School of Dentistry of the Federal University of Minas Gerais, Belo Horizonte, Brazil. PARTICIPANTS: Adolescents aged 10-17 years. METHODS: Forty-six adolescents participated in this study. Adolescents' OHRQoL was evaluated using the Brazilian version of the questionnaire Impact of Fixed Appliances Measure. This questionnaire comprises 43 questions distributed across nine domains: aesthetics; functional limitation; dietary impact; oral hygiene impact; maintenance impact; physical impact; social impact; time constraints; and travel/cost/inconvenience implications. The answer options follow the Likert scale, in the range of 1-5. A higher score indicates a more negative impact on the OHRQoL. Adolescents' malocclusion was assessed using the Dental Aesthetic Index. Information on family income, adolescents' age and tooth extraction was also collected. A directed acyclic graph was used to identify potential confounders. Statistical analysis involved the chi-square test, the Student's t test and the analysis of covariance. The minimal clinically important difference was also calculated. RESULTS: Of the 46 participants, 25 were girls and 21 were boys. The female adolescents presented a more negative impact from the orthodontic treatment on the quality of life when compared to the male adolescents (P < 0.001), regardless of the influence of the confounding variables. The most negative repercussions were identified in the domains of oral hygiene (P = 0.002), physical impact (P < 0.001) and social impact (P < 0.001). CONCLUSION: The impact of the first six months of orthodontic treatment on OHRQoL is more negative in female individuals than in male individuals. The results of this study may be useful for the oral healthcare provider during the counselling of adolescents undergoing fixed appliance therapy.
