AIM: The aim of this case report is describe an unprecedented case with histological and immunohistochemical diagnosis of splenic heterotopy in the colon using material obtained by endoscopic ultrasound-guided biopsy. BACKGROUND: Splenic heterotopia is a benign condition characterized by the implantation of splenic tissue in areas distant from its usual anatomical site, such as the peritoneum, omentum, mesentery, liver, pancreas, and subcutaneous tissue and, more rarely, in locations such as the colon and brain. It is generally associated with a history of splenic trauma or splenectomy and typically does not cause specific symptoms. CASE PRESENTATION: A 35-year-old white male patient who was healthy, with no history of trauma or splenectomy, but had a family history of colorectal neoplasia underwent colonoscopy for screening. The examination revealed a large bulge in the proximal descending colon, covered by normal-appearing mucosa. Endoscopic ultrasound-guided puncture was performed with a 22 gauge fine needle biopsy, and the histopathological and immunohistochemical analysis results were consistent with a heterotopic spleen. CONCLUSIONS: This is the first report of a primary intramural colic splenosis case with histological and immunohistochemical diagnosis of splenic heterotopia in the colon, using material obtained by endoscopic ultrasound and ultrasound-guided biopsy.

OBJECTIVE: This study aimed to systematically evaluate the efficacy and safety of Endoscopic Ultrasonography (EUS) for the treatment of pancreatic cancer. METHODS: The PubMed, Embase, Web of Science, and Google Scholar databases were searched from the inception of the databases to June 2022. RevMan 5.3.0 software was utilized for data analysis. In total, 13 self-descriptive studies, which enrolled 382 patients, were finally included. RESULTS: It was revealed that EUS for the treatment of pancreatic cancer exhibited a lower incidence of adverse reactions (Relative Risk Ration [RR = 0.23], 95 % Confidence interval [95 % CI 0.23-0.23]), a higher success rate (RR = 0.90, 95 % CI 0.90-0.90), and a low failure rate (RR = 0.06, 95 % CI 0.06-0.06). Moreover, EUS-guided Celiac Plexus Neurolysis (EUS-CPN) not only significantly relieved pancreatic cancer patients' pain (RR = 0.83, 95 % CI 0.83-0.83), but also significantly eliminated pain in some patients (RR = 0.09, 95 % CI 0.09-0.09). The effects of EUS on pancreatic cancer treatment were satisfactory, and few adverse reactions were found. CONCLUSION: Owing to the restricted sample size in this meta-analysis, primarily consisting of descriptive studies, it was imperative to conduct more rigorously designed, multi-center, long-term follow-up, larger sample, and Randomized Controlled Trials (RCTs) to validate the findings.

Background and study aims There are rare data on the usefulness of endosonography-guided tissue acquisition (EUS-TA) in patients with pancreatic cystic lesions (PCLs). This study aimed to determine the accuracy of EUS-TA with ProCore 20G (PC20) for differentiating between mucinous neoplasia (MN) and non-MNs (n-MN) and identifying malignant PCLs, as well as its adverse events (AEs) in patients with PCLs without a classificatory diagnosis by imaging exams. Patients and methods In this observational, retrospective, single-center study, all patients with PCL who underwent EUS-TA due to diagnostic doubts in imaging studies were consecutively recruited from June 2017 to December 2021. The outcomes were to determine the diagnostic accuracy of EUS-TA with PC20 for differentiating between MN and n-MN, identifying malignant PCLs, and the AEs. Results Herein, 145 patients underwent EUS-TA, with 83 women (57.2%) and a mean age of 62.2 years. The mean size was 2.3 cm, with 81 patients (77.9%) having a PCL < 3.0 cm. The final diagnosis was made by EUS-TA (n = 81), surgery (n = 58), and follow-up (n = 6). The sensitivity, specificity, positive and negative predictive values, and accuracy for differentiating between MNs and n-MNs and identifying malignant PCLs were 92.6%, 98.4%, 98.7%, 91.3%, and 95.2% (kappa=0.9), and 92%, 99.2%, 95.8%, 98.3%, and 97.9% (kappa = 0.93), respectively. The AE rate was 2.7%, with no deaths in this cohort. Conclusions EUS-TA with PC20 has high accuracy and technical success with a low AE rate for PCL diagnosis.

Pancreatic carcinoma is an aggressive tumour with increasing incidence in both sexes worldwide. Early detection is, therefore, essential for patient management. A recent advancement involves the utilization of larger, thicker gauge needles, which enable the collection of core-type biopsies (FNB). Here, we investigated the role of fine needle aspiration and cytopathology in the diagnostic workflow of pancreatic lesions. A search query was designed to search for articles in the PubMed database comparing FNA and FNB for biopsy of pancreatic lesions, and detailed data were extracted from selected studies. Statistical analyses were performed using the R package meta version 6.2. Twenty-one studies made the final cut for data extraction. Overall, median age was 64.3 years (±6.1; 47.6-71.5), male: female proportion 53.9 (±11.3; 27.6-67.4), lesion size 3.1 cm (±0.5; 1.9-4.2 cm) and percentage of malignant cases 78.3% (±26.8; 2.1-100). FNA and FNB diagnostic yield was 85.8% (±10.3; 70.0-100.0) and 89.2% (±7.7; 70.0-98.6), respectively. Average accuracy was 89.5% (±11.7; 63.0-100.0) for FNA and 90.8% (±7.1; 77.0-100.0) for FNB. Adverse effects rate was 1.0% (±1.3; 0-4.3) for FNA and 2.2% (±4.4; 0-16.1) for FNB. None of the selected variables had a significant statistical difference between both methods. FNA and FNB perform similarly for diagnostic material acquisition in pancreatic lesions. The best outcome comes from the association of both techniques, emphasizing the value of combining cytological and histological morphology for the most accurate analysis.

Abstract Pakistan is an agricultural country and fisheries play a very important role in the economic development of the country. Different diseases are prevalent in Pakistani fish but information related to the causative agents is not well-known. Keeping in view the significance of bacterial pathogens as the causative agents of multiple fish diseases, the present study was conducted for identification, characterization and analysis of virulence genes of Aeromonas spp. isolated from diseased fishes. A total of fifty fish samples having multiple clinical indications were collected from different fish farms of district Kasur, Punjab Pakistan. For isolation of Aeromonas spp. samples were enriched and inoculated on Aeromonas isolation medium. Isolates were identified and characterized by different biochemical tests, Analytical Profile Index (API) 20E kit and Polymerase Chain Reaction (PCR) assays. All isolates were screened for three putative virulence genes including aerolysin (aer), haemolysin (hyl) and heat labile cytotonic enterotoxin (alt). Seven isolates of Aeromonas (A.) hydrophila were retrieved and identified based on API 20E. These isolates were further confirmed as A. hydrophila on the basis of PCR assays. Three isolates were detected positive for the presence of virulence genes (alt and hyl). Whereas aerolysin (aer) gene was not present in any of A. hydrophila isolates. The present study confirmed A. hydrophila as the causative agent of epizootic ulcerative syndrome and motile Aeromonas septicemia in fish farms of district Kasur, Punjab Pakistan. Moreover, detection of two virulence genes (alt and hyl) in A. hydrophila isolates is a threat for fish consumers of study area.

Mycobacterium fortuitum is associated with skin and soft-tissue infections, yet isolated liver involvement is rare. A 67-year-old asymptomatic man was referred for endoscopic ultrasound (EUS) to evaluate a gastric lesion and an incidental liver mass. EUS revealed a heterogeneous liver mass that was sampled. Pathology revealed necrotic granulomatous inflammation and positive acid-fast bacilli stain with M. fortuitum deoxyribonucleic acid. Levofloxacin plus trimethoprim and sulfamethoxazole for 3 months were used for complete resolution of liver lesion. Isolated nontuberculous liver involvement is uncommon. We report the first case of a liver mass caused by M. fortuitum diagnosed by EUS-fine needle aspiration.

Background and Objectives: EUS-guided choledochoduodenostomy (EUS-CDS) is commonly employed to address malignant biliary obstruction (MBO) after a failed ERCP. In this context, both self-expandable metallic stents (SEMSs) and double-pigtail stents (DPSs) are suitable devices. However, few data comparing the outcomes of SEMS and DPS exist. Therefore, we aimed to compare the efficacy and safety of SEMS and DPS at performing EUS-CDS. Methods: We conducted a multicenter retrospective cohort study between March 2014 and March 2019. Patients diagnosed with MBO were considered eligible after at least one failed ERCP attempt. Clinical success was defined as a drop of direct bilirubin levels ≥ 50% at 7 and 30 postprocedural days. Adverse events (AEs) were categorized as early (≤7 days) or late (>7 days). The severity of AEs was graded as mild, moderate, or severe. Results: Forty patients were included, 24 in the SEMS group and 16 in the DPS group. Demographic data were similar between the groups. Technical success rates and clinical success rates at 7 and 30 days were similar between the groups. Similarly, we found no statistical difference in the incidence of early or late AEs. However, there were two severe AEs (intracavitary migration) in the DPS group and none in the SEMS cohort. Finally, there was no difference in median survival (DPS 117 days vs. SEMS 217 days; P = 0.99). Conclusion: EUS-guided CDS is an excellent alternative to achieve biliary drainage after a failed ERCP for MBO. There is no significant difference regarding the effectiveness and safety of SEMS and DPS in this context.

RESUMO Este artigo tem como objetivo apresentar o manejo clínico para externalização dos papéis internos ou partes, através da experiência psicodramática, tendo por base as técnicas de Moreno e de Rojas-Bermúdez, a exemplo da construção de imagens tridimensionais e bidimensionais por meio de pessoas, objetos, bonecos, tecidos e o psicodrama interno desenvolvido por outros autores. A gênese das partes, em geral, está relacionada à dissociação decorrente de traumas ou ao introjeto de figuras parentais significativas, principalmente quando o apego é inseguro. Traumas complexos, relacionados a bullying, abuso sexual, violência doméstica de várias naturezas ao longo do desenvolvimento da criança vai trazer à tona essas partes internas que precisam ser bem compreendidas porque afetam vários papéis sociais. Papéis internos, partes, eu parcial, divisão interna, personagens, estados de ego, criança ferida, subpersonalidades são denominações de um mesmo fenômeno, que se manifesta pela multiplicidade de eus. Esse fenômeno é também constatado no campo da neurose e em normóticos.
RESUMEN Este artículo tiene como objetivo presentar el manejo clínico para la externalización de roles o partes internas, a través de la experiencia psicodramática, basada en las técnicas de Moreno y Rojas-Bermúdez, como la construcción de imágenes tridimensionales y bidimensionales a través de personas, objetos, marionetas, tejidos y el psicodrama interno desarrollado por otros autores. La génesis de las partes, en general, está relacionada con la disociación resultante del trauma o la introyección de figuras parentales significativas, especialmente cuando el apego es inseguro. Los traumas complejos relacionados con el bullying, el abuso sexual, la violencia doméstica de diversa índole a lo largo del desarrollo del niño sacarán a la luz estas partes internas, que deben entenderse bien porque afectan a varios roles sociales. Roles internos, partes, yo parcial, división interna, personajes, los estados del ego, los niños heridos, las subpersonalidades son denominaciones del mismo fenómeno, que se manifiesta por la multiplicidad de Yoes. Este fenómeno también se observa en el campo de la neurosis y en la normótica.
ABSTRACT This article aims to present the clinical management for externalization of internal roles or parts, through psychodramatic experience, based on the techniques of Moreno and Rojas-Bermúdez, such as the construction of three-dimensional and two-dimensional images through people, objects, puppets, tissues and the internal psychodrama developed by other authors. The genesis of the parts, in general, is related to dissociation resulting from trauma or the introjection of significant parental figures, especially when the attachment is insecure. Complex traumas related to bullying, sexual abuse, domestic violence of various natures throughout the child's development will bring to light these internal parts, which need to be well understood because they affect various social roles. Internal roles, parts, partial self, internal division, characters, ego states, wounded children, subpersonalities are denominations of the same phenomenon, which is manifested by the multiplicity of selves. This phenomenon is also observed in the field of neurosis and in normotics.

RESUMO Este artigo tem como objetivo compreender os benefícios da utilização da técnica do desdobramento do eu em demandas psicológicas relacionadas às masculinidades em um processo de psicodrama bipessoal. Por meio de uma pesquisa-ação com um homem adulto, foi possível identificar conservas culturais vinculadas ao papel de gênero. Observou-se uma suposta obrigatoriedade de vigor físico e psicológico, de não demonstração de sentimentos, vulnerabilidades e sensibilidade, bem como uma exigência constantede trabalho e produtividade. Ao se fazer os eus internos do protagonista contracenarem por meio da técnica do desdobramento do eu, possibilitou-se o encontro com caminhos efetivos de busca da saúde biopsicossocial, por vezes integrando e/ou acionando outros eus para atuarem diante dos desafios cotidianos.
ABSTRACT This article aims to understand the benefits of using the self unfolding technique in psychological demands related to masculinities in a bipersonal psychodrama process. Through action research with an adult man, it was possible to identify cultural conserves linked to gender role. There is an assumption that masculinity requires physical and psychological vigor, not showing feelings, vulnerabilities and sensitivity, as well as a constant demand for work and productivity. By making the protagonist's internal selves act together through the self unfolding technique, it was possible to encounter effective paths in search of biopsychosocial health, sometimes integrating and/or triggering other selves to act in the face of daily challenges.
RESUMEN Este artículo tiene como objetivo comprender los beneficios de utilizar la técnica de desdoblamiento del yo en demandas psicológicas relacionadas con las masculinidades en un proceso de Psicodrama Bipersonal. A través de una investigación con un hombre adulto, fue posible identificar las acciones culturales vinculadas al rol de género. Se observa una supuesta exigencia de vigor físico y psicológico, de no manifestar sentimientos, vulnerabilidades y sensibilidad, así como una constante demanda de trabajo y productividad. Al hacer que mis yo internos actuaran como protagonistas a través de la técnica de desdoblamiento del yo, fue posible encontrar caminos efectivos para la búsqueda de la salud biopsicosocial, integrando en ocasiones y / o desencadenando otros yo para actuar ante los desafíos cotidianos.

Resumen La endosonografía endoscópica es una alternativa que sirve como guía para la realización de derivaciones biliodigestivas en los casos en los que la colangiopancreatografía retrógrada endoscópica (CPRE) ha fallado. Se han descrito técnicas como la coledocoduodenostomía o coledocoantrostomía guiadas por ultrasonografía endoscópica (USE). Se describe el caso de una paciente de 72 años con adenocarcinoma de páncreas, compromiso portal y de paredes duodenales, en quien se usó la CPRE para intentar una derivación paliativa y fue fallida, por lo cual se realizó la colocación de un stent metálico guiado por endosonografía endoscópica, con adecuada respuesta clínica al tratamiento. En conclusión, el procedimiento es seguro y la endosonografía es una vía alterna efectiva en los casos de CPRE fallidas para lograr derivaciones biliares en casos de obstrucción de la vía biliar de origen maligno.
Abstract Endoscopic endosonography is an alternative to guide biliodigestive shunting in cases where endoscopic retrograde cholangiopancreatography (ERCP) has failed. Techniques such as choledoduodenostomy or choledochoantrostomy guided by endoscopic ultrasonography have been described. This is the case of a 72-year-old patient with pancreatic adenocarcinoma, portal vein and duodenal wall involvement, in whom ERCP to try a palliative shunt failed. Therefore, a metal stent was placed using endoscopic endosonography, with adequate clinical response to the procedure and treatment. It is concluded that the procedure is safe and that endosonography is an effective alternative in cases of ERCP failure to achieve biliary bypass in cases of malignant biliary obstruction.

INTRODUCTION: Endoscopic retrograde cholangiopancreatography (ERCP) biliary drainage is considered the reference standard in patients with biliary obstruction, but it is not free of complications. EUS-guided biliary drainage (EUS-BD) is considered an alternative in patients with failed ERCP; however, data are scarce as to whether EUS-BD could be considered a first option. OBJECTIVE: The aim of our study was to compare the need for reintervention and cost between ERCP biliary drainage vs. EUS-BD. MATERIAL AND METHODS: We conducted a retrospective and comparative study of patients with distal malignant biliary obstruction with biliary drainage with ERCP + plastic stent (ERCP-PS) vs. ERCP + metal stent (ERCP-MS) vs. EUS-BD. RESULTS: 124 patients were included, divided into three groups: ERCP-PS, 60 (48.3%) patients; ERCP-MS, 40 (32.2%) patients; and EUS-BD, 24 (19.3%) patients. The need for reinterventions (67 vs. 37 vs. 4%, respectively), the number of procedures [3 (1-10) vs. 2 (1-7) vs. 1 (1-2)], and the costs (4550 ± 3130 vs. 5555 ± 3210 vs. 2375 ± 1020 USD) were lower in the EUS-BD group. No differences in terms of complications were detected. CONCLUSION: EUS-BD requires fewer reinterventions and has a lower cost compared to drainage by ERCP with metal or plastic stents.

RESUMEN Introducción: El drenaje guiado por ecoendoscopía es considerado como la mejor opción para el drenaje de las colecciones liquidas peripancreáticas. No hay reportes en nuestro medio de la experiencia en esta terapia endoscópica. Objetivos: detallar las características, eficacia y seguridad de los drenajes ecoendoscópicos de las colecciones liquidas peripancreáticas en el Hospital Nacional Edgardo Rebagliati Martins. Material y métodos: se realizó un estudio transversal retrospectivo de todos los pacientes con colecciones liquidas peripancreáticas sintomáticas sometidos a drenaje ecoguiado. Se analizaron variables demográficas, características ecoendoscópicas asociadas al procedimiento terapéutico y dispositivos empleados. La eficacia se evaluó con la tasa de éxito técnico y de éxito clínico. La seguridad se evaluó en base a las principales complicaciones inmediatas y posteriores. Se empleó estadística descriptiva básica. Resultados: Se realizaron 17 drenajes ecoguiadas de colecciones peripancreáticas (10 pseudoquistes y 7 necrosis pancreáticas encapsuladas o WON) en 17 pacientes: 9 mujeres (52,9%) y 8 hombres (47,1%). El rango de edad de los pacientes fue de 26-72 años, con una mediana de 59 años. El tamaño promedio de las colecciones liquidas fue 14,8 cm con un rango de 8-24 cm. El acceso transgástrico se empleó en 16 casos (94,1%) y el acceso transduodenal en 1 caso (5,9%). Se empleó prótesis plástica (6 casos), prótesis metálica autoexpandibles (SEMS) biliar (4 casos) y prótesis de aposición luminal (LAMS) en 7 casos. El éxito técnico fue del 100% (17 pacientes), mientras que el éxito clínico fue de 70,6% (12 pacientes). Las complicaciones ocurrieron en 41,2%: infección de colección (2 casos), perforación, sangrado en zona de drenaje, obstrucción de prótesis plástica, migración de prótesis al interior de colección y al lumen gástrico (1 caso cada uno). El manejo quirúrgico de éstas complicaciones fue necesaria en 3 casos, mientras que en los otros 4 casos solo requirió manejo médico - endoscópico. Conclusiones: los drenajes guiados por ecoendoscopía de las colecciones liquidas peripancreáticas tienen alta tasa de éxito técnico y clínico con reducido porcentaje de complicaciones.
ABSTRACT Introduction: Endoscopic ultrasound (EUS) guided drainage is considered the best option for peripancreatic fluid collections drainage. There are no previous reports in our country of this type of endoscopic therapy. Objectives: To detail the characteristics, efficacy and safety of EUS-guided drainage of peripancreatic fluid collections at Edgardo Rebagliati Martins National Hospital. Materials and methods: We carried out a retrospective cross-sectional study of all patients who underwent EUS-guided drainage of symptomatic peripancreatic fluid collections. Demographics, procedure-related endosonographic features and devices used were analyzed. We assessed the effectiveness with technical and clinical success rate. Clinical safety was assessed based on major immediate and delayed adverse events. Basic descriptive statistics was used. Results: A total of 17 patients (9 women, 52.9% and 8 men, 47.1%) with peripancreatic fluid collections (10 pseudocysts and 7 walled-off necrosis) underwent EUS-guided drainage. The median age of the patients was 59 years (range 26 - 72 years). The mean diameter of the fluid collections was 14.8 cm (range 8-24 cm). Transgastric route was used in 16 cases (94.1%) and transduodenal route in 1 case (5.9%). Plastic stent (6 cases), biliary self expanding metal stent (4 cases) and lumen apposing stent (7 cases) were used. The technical success rate was 100% (17 patients), while clinical success rate was 70.6% (12 patients). Adverse events occurred in 41.2%: infection (2 cases), perforation, bleeding, plastic stent occlusion, stent migration into the collection cavity and out of the collection cavity (1 case each one). Surgical management of adverse events was necessary in 3 cases, while the 4 other cases only required medical - endoscopic management. Conclusions: EUS-guided drainage of peripancreatic fluid collections have a high technical and clinical success rate with a low percentage of adverse events.

The present systematic review and meta-analysis aimed to evaluate the available evidence base on endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) combined with either endoscopic ultrasound-guided fine-needle aspiration (EUS-FNA) or endoscopic ultrasound using the EBUS scope-guided fine-needle aspiration (EUS-B-FNA) for diagnosing and staging mediastinal diseases. PubMed, Web of Science, and Embase were searched to identify suitable studies up to June 30, 2019. Two investigators independently reviewed articles and extracted relevant data. Data were pooled using random effect models to calculate diagnostic indices that included sensitivity and specificity. Summary receiver operating characteristic (SROC) curves were used to summarize the overall test performance. Data pooled from up to 16 eligible studies (including 10 studies of 963 patients about EBUS-TBNA with EUS-FNA and six studies of 815 patients with EUS-B-FNA) indicated that combining EBUS-TBNA with EUS-FNA was associated with slightly better diagnostic accuracy than combining it with EUS-B-FNA, in terms of sensitivity (0.87, 95%CI 0.83 to 0.90 vs. 0.84, 95%CI 0.80 to 0.88), specificity (1.00, 95%CI 0.99 to 1.00 vs. 0.96, 95%CI 0.93 to 0.97), diagnostic odds ratio (413.39, 95%CI 179.99 to 949.48 vs. 256.38, 95%CI 45.48 to 1445.32), and area under the SROC curve (0.99, 95%CI 0.97 to 1.00 vs. 0.97, 95%CI 0.92 to 1.00). The current evidence suggests that the combination of EBUS-TBNA with either EUS-FNA or EUS-B-FNA provides relatively high accuracy for diagnosing mediastinal diseases. The combination with EUS-FNA may be slightly better.

The EUS-guided biliary drainage (EUS-BD) has gained broad acceptance as the preferred approach after failed ERCP for malignant biliary obstruction. Despite the drainage route, namely, transhepatic or transduodenal, the technical and clinical success rates are high. Because of such good outcomes with tolerable adverse events (AEs) rate, the EUS-BD might soon even replace the ERCP for primary biliary decompression in patients at high risk of failed biliary cannulation. Among the EUS-BD techniques, the choledochoduodenostomy seems to carry the lower risk of AEs and should be considered the first-line EUS approach for biliary decompression.

BACKGROUND AND AIMS: There are no systematic reviews comparing the use of endoscopic retrograde cholangiopancreatography (ERCP)-based brush cytology and forceps biopsy and endoscopic ultrasound (EUS)-guided fine-needle aspiration (FNA) for the diagnosis of malignant biliary stricture; so in this revision, we will compare ERCP against EUS-FNA for tissue diagnosis of malignant biliary stricture. DESIGN: A systematic review was conducted of comparative studies (prospective or retrospective) analyzing EUS and ERCP for tissue diagnosis of malignant biliary stricture. MATERIALS AND METHODS: The databases Medline, EMBASE, Cochrane, LILACS, CINAHL, and Scopus were searched for studies dated previous to November 2014. We identified three prospective studies comparing EUS-FNA and ERCP for the diagnosis of malignant biliary stricture and five prospective studies comparing EUS-FNA with the same diagnosis of the other three studies. All patients were subjected to the same gold standard method. We calculated study variables (sensitivity, specificity, prevalence, positive and negative predictive values, and accuracy) and performed a meta-analysis using the Review Manager (RevMan) 5.3 software. RESULTS: A total of 294 patients were included in the analysis. The pretest probability for malignant biliary stricture was 76.66%. The mean sensitivities of ERCP and EUS-FNA for tissue diagnosis of malignant biliary stricture were 49% and 75%, respectively; the specificities were 96.33% and 100%, respectively. The posttest probabilities positive predictive value (98.33% and 100%, respectively) and negative predictive value (34% and 47%, respectively) were determined. The accuracies were 60.66% and 79%, respectively. CONCLUSION: We found that EUS-FNA was superior to ERCP with brush cytology and forceps biopsy for diagnosing malignant biliary strictures. However, a negative EUS-FNA or ERCP test may not exclude malignant biliary stricture because both have low negative posttest probabilities.

El ultrasonido endoscópico (EUS) ha revolucionado el diagnóstico y el manejo de muchas patologías de la vía digestiva, particularmente la patología pancreática, convirtiéndose en un examen prácticamente imprescindible en el abordaje diagnóstico y terapéutico de un paciente con un problema de páncreas. Es necesario dejar en claro que el método no es único y que para lograr una sensibilidad alta y cumplir el objetivo de realizar lo más adecuado, debe sumarse a otros métodos de acuerdo a cada caso, como la ecografía, la tomografía axial computarizada (TAC), la resonancia magnética (MRI) en sus diferentes modalidades, y las pruebas del laboratorio clínico microbiológico y patología. En este artículo se revisarán algunos casos de enfermedades evaluadas con este método, que muestran por qué el EUS, es una herramienta clave para el médico de urgencias y de consulta externa, el internista, el cirujano, el médico del servicio hospitalario y el personal de salud en general, al momento de definir, clasificar y orientar el manejo de determinadas patologías en el tubo digestivo. El EUS es una importante ayuda y no debe ser extraña al personal médico, debe tenerla presente junto a las demás pruebas diagnósticas en patología pancreática. Se señalarán los aspectos más relevantes en cada caso y las indicaciones del EUS.
Endoscopic ultrasound (EUS) has revolutionized the diagnosis and management of many diseases of the digestive tract, particularly the pancreatic ones, becoming a practically essential test in the diagnosis and therapeutic management of a patient with a pancreatic problem. It's necessary to establish the final diagnosis are necesary many tests to achive high sensitivity. It should join with other methods according to each case, such as ultrasound, computed tomography (CT), magnetic resonance imaging (MRI) in its various forms, and chemical, microbiological and pathology tests. In this article we reviewed some cases of pathologies evaluated by this diagnosis test, which demonstrate why the EUS, is a key for the emergency and outpatient physician, internist, surgeon, doctor of the hospital service and staff health in general, when defining, classifying and guide the management of certain diseases in the digestive tract. The EUS is an important tool and should not be foreign to the medical staff, who must consider it, with other diagnostic tests for pancreatic disease. This article point out the most important aspects in each case and indications of EUS.

Las lesiones o tumores subepiteliales (TSE) son raros. Se considera que en 1 de cada 300 endoscopias puede encontrarse un TSE, lo que corresponde al 0,36% de las endoscopias. De estos, solo el 10% se ubican en el duodeno, lo que proporciona una idea de lo infrecuente que es este hallazgo. Al realizar la endoscopia y detectar un TSE en el duodeno, inicialmente se describirá su tamaño, forma, color, movilidad, pulsación y finalmente la consistencia, la cual puede evaluarse con la pinza de biopsia cerrada. Esto permitirá detectar si es quístico, firme o si tiene el signo de la almohada, que es altamente sugestivo de lipoma. En caso contrario, está indicada la realización de una ecoendoscopia, especialmente si el tumor es mayor de 1 cm.
Subepithelial lesions (SE) are rare. They are found in only one out of every 300 endoscopies (0.36%). Of these, only 10% are located in the duodenum. When endoscopy does detect a subepithelial lesion in the duodenum it is important to make an initial description of its size, shape, color, mobility, and consistency to touch. Next, a closed biopsy sample must be taken with pincers and analyzed to detect whether it is a cyst and whether it has a pillow sign which is highly suggestive of lipoma. When the lesion is neither a cyst nor a lipoma, endoscopic ultrasound is indicated, especially if the lesion is larger than one centimeter.
