CONTEXT: A programmatic approach to assessment entails gathering and aggregating 'rich information' on candidates to inform progress decisions. However, there is little guidance on how such an approach might be implemented in practice. OBJECTIVE: We describe an approach to aggregating rich information across assessment formats to inform committee decision-making in a specialist medical college. METHODS: Each item (n = 272) for every examination was blueprinted to 15 curriculum modules and 7 proficiencies. We developed a six-point holistic rating scale with detailed rubrics outlining expected performance standards for every item. Examiners used this rating scale in making judgements for each item, generating rich performance data for each candidate. RESULTS: A colour-coded 'mosaic' of patterns of performance across modules and proficiencies was generated along with frequency distributions of ratings. These data allowed examiners to easily visualise candidate performance and to use these data to inform deliberations on borderline candidates. Committee decision-making was facilitated by maintaining the richness of assessment information throughout the process. Moreover, the data facilitated detailed and useful feedback to candidates. CONCLUSIONS: Our study demonstrates that incorporating aspects of programmatic thinking into high-stakes examinations by using a novel approach to aggregating information is a useful first step in reforming an assessment program.

RESUMO

Objective To determine the impact of new methods in teaching and examination on students' learning of psychiatry by increasing the proportion of objective questions in the final exam papers through examination reform, so as to help students adapt to the Practitioner Examination. Methods A total of 483 students from grade 2011&2012 were selected as research subjects. Through the comparative study of the test scores under different teaching methods before and after the examination reform, the SPSS 22.0 statistical software was used to analyze the structure, reliability, difficulty and level of differentiation of the examine papers via t tests. Results Examination papers after the reform showed an increase in difficulty in all question types; some question types also had higher levels of differentiation. Students receiving PBL-based education had higher scores compared to those under traditional teaching methods. The difference was statistically significant. Conclusion PBL teaching mode is more conducive to the mastering of knowledge points and helps students improve grades in objective questions. Medical schools should increase the use of PBL teaching reform in psychiatry courses and conduct examination reform, so as to improve the teaching effect and the passing rate of Practitioners Examination.

RESUMO

Examination reform was explored in Introduction to clinical medicine, and a new evaluation system was constructed which included the combination of formative assessment and sum-mative assessment as well as content system, operation system and monitoring system. The feedback mechanism of formative assessment was put into a full use to improve the teaching effect in this new evaluation system. At the same time, the method of summative assessment was substituted from the traditional closed-book exam to literature translation and presentation carried out by groups. The com-bination of summative assessment and formative assessment could evaluate the teaching efficiency of introduction to clinical medicine more comprehensively and objectively. In addition, it played an ac-tive role in improving students' comprehensive ability and their learning enthusiasm.
