ABSTRACT A 7-week-old male delivered by cesarean section presented with a positive serology for dengue along with preretinal and retinal hemorrhages, vitreous opacities and cotton wool spots. The patient and his mother had positive serologies for Non Structural Protein 1 (NS1) by ELISA. Retinal and vitreous findings improved over a sixteen-week period. Spectral domain optical coherence tomography (OCT) showed preserved macular architecture. In this case report, we suggest that retinal and vitreous changes may be the ocular presenting features of vertically transmitted dengue in newborns, and that those findings may resolve with no major structural sequelae.


RESUMO Neonato de 7 semanas, do sexo masculino, nascido de parto cesárea, apresentou sorologia positiva para dengue com hemorragias retinianas e pré-retinianas, opacidades vítreas e manchas algodonosas. O paciente e sua mãe haviam apresentado sorologias positivas para Non Structural Protein 1 através de ELISA. Achados na retina e no vítreo melhoraram em um período de dezesseis semanas. O exame de tomografia de coerência óptica de domínio espectral demonstrou arquitetura macular preservada. Neste relato de caso, sugerimos que alterações na retina e no vítreo podem ser os achados oculares aparentes em neonatos com infecção vertical por dengue, e que estes podem se resolver sem maiores sequelas estruturais.

Objectives: To evaluate the cascade of care for the elimination of mother-to-child-transmission of human immunodeficiency virus (HIV) in Suriname and identify sociodemographic and clinical factors preventing transmission to exposed infants. Methods: A mixed-methods study design was used. Antenatal care data from the 2018 cross-sectional multi-indicator cluster survey on 1 026 women aged 15-49 years who had had a live birth in the previous 2 years were used. Furthermore, national data on a cohort of 279 mothers with HIV and their 317 infants born from 2016 to 2018 were evaluated. Additionally, 13 cases of mother-to-child-transmission of HIV were reviewed. Results: In 89.3% of cases, no mother-to-child HIV transmission occurred. Early cascade steps show that 28.4% of women had unmet family planning needs, 15% had no antenatal visits, 8% delivered outside a health facility, and 71.5% received an HIV test during antenatal care. Of the pregnant women with HIV, 84.2% received antiretroviral therapy, while 95.5% of their infants received HIV prophylactic treatment. Receiving antiretroviral therapy for the mother (odds ratio (OR) 45.4, 95% confidence interval (CI) 9.6-215.3) and the child (OR 145.7, 95% CI 14.4-1477.4) significantly increased the odds of a negative HIV test result in infants. Conversely, living in the interior decreased the odds (OR 0.2, 95% CI 0.4-0.7) compared with urban living. Conclusions: HIV medication for mothers with HIV and their infants remains key in the prevention of mother-to-child-transmission of HIV. Early prenatal care with follow-up should be strengthened in Suriname.

A sífilis persiste como um problema de saúde pública, sobretudo pelos entraves existentes no enfrentamento da sífilis gestacional e congênita. Considerando que a ocorrência dessas infecções se relaciona a fatores maternos e programáticos, este estudo buscou analisar as características epidemiológicas do binômio mãe-filho exposto à sífilis e sua distribuição espacial no Paraná entre 2012 e 2020. Trata-se de estudo descritivo e ecológico, com dados dos sistemas nacionais de informação do Brasil. Foram considerados os casos de gestantes e de crianças registrados entre 2012 e 2020 no estado do Paraná. Foram apresentadas as frequências absolutas e relativas para a caracterização, calculando-se a variação percentual entre o primeiro e o último triênio. Ainda, foi empregado o índice de Moran para a geoespacialização segundo regiões de saúde. Houve predomínio em mulheres de 20 a 39 anos (71,24%), brancas (67,22%) e com até oito anos de estudo (80,76%), com diagnóstico nas fases primária e latente (76,42%). Os casos concentraram-se em crianças do sexo masculino (48,72%), diagnosticadas na fase recente (96,42%), e naquelas cujas mães aderiram ao pré-natal (88,88%), mas os parceiros, por sua vez, não foram tratados (69,46%). Houve aumento do diagnóstico materno durante o pré-natal (16,61%) e redução dos óbitos infantis por sífilis (31,25%). Observou-se concentração das notificações nas regiões Metropolitana e de Pato Branco. Em suma, as gestantes apresentaram idade reprodutiva e baixa escolaridade e foram tratadas durante o pré-natal, contudo, sem a inclusão dos parceiros. Ademais, evidenciou-se comportamento espacial aleatório nas regiões de saúde, com disparidade entre a sífilis gestacional e a congênita.


Syphilis remains as serious public health issue due to existing obstacles in combating gestational and congenital syphilis. Since the onset of these infections is related to maternal and programmatic factors, this study analyzed the epidemiological profile of the mother-child binomial exposed to syphilis and its spatial distribution in Paraná from 2012 to 2020. This is a descriptive, ecological study with data from Brazilian national information systems. Cases of pregnant women and children recorded between 2012 and 2020 in the state of Paraná were considered. Absolute and relative frequencies were estimated for characterization, calculating the percentage variation between the first and last three years. The Moran index was also used for geospacialization according to health regions. Most women were aged 20 to 39 years (71.24%), white (67.22%), and had up to 8 years of schooling (80.76%), with diagnosis in the primary and latent stages (76.42%). Most cases concerned male children (48.72%), diagnosed in the recent phase (96.42%), and from mothers who adhered to prenatal care (88.88%), but the partners were untreated (69.46%). Maternal diagnosis increased during prenatal care (16.61%) and infant deaths by syphilis decreased (31.25%). Most cases were notified in the Metropolitan and Pato Branco regions. In short, the pregnant women were of reproductive age, had low education, and were treated during prenatal care, but their partners were not included. The health regions showed random spatial behavior, with disparity between gestational and congenital syphilis.


La sífilis persiste como un problema de salud pública, principalmente por los obstáculos en hacer frente la sífilis gestacional y congénita. Teniendo en cuenta que la ocurrencia de estas infecciones está relacionada con los factores maternos y programáticos, este estudio tuvo como objetivo analizar las características epidemiológicas del binomio madre-hijo expuestos a la sífilis y la distribución espacial en Paraná (Brasil) entre 2012 y 2020. Se trata de un estudio descriptivo y ecológico, con datos de los sistemas de información nacionales de Brasil. Se consideraron los casos de mujeres embarazadas y niños registrados entre 2012 y 2020 en Paraná. Se presentaron las frecuencias absolutas y relativas, calculando la variación porcentual entre el primer y el último trienio. Asimismo, se utilizó el índice de Moran, según regiones de salud. Predominaron las mujeres de entre 20 y 39 años de edad (71,24%), blancas (67,22%), con hasta ocho años de estudio (80,76%), con diagnóstico en estadio primario y latente (76,42%). Los casos se concentraron en hijos varones (48,72%), diagnosticados en fase reciente (96,42%), y en aquellos cuyas madres tuvieron acceso a los cuidados prenatales (88,88%), pero su pareja no recibió el tratamiento (69,46%). Hubo un aumento en el diagnóstico materno durante la atención prenatal (16,61%) y una reducción en las muertes de niños por sífilis (31,25%). Se observó una concentración de casos en las regiones Metropolitana y de Pato Branco. En resumen, las mujeres embarazadas se encontraban en edad reproductiva, tenían bajo nivel de estudios y eran atendidas durante el prenatal, sin incluir su pareja. Además, se evidenció un comportamiento espacial aleatorio en las regiones de salud, con disparidad entre sífilis gestacional y congénita.

BACKGROUND: Syphilis is among the most common sexually transmitted infections worldwide. When it occurs during pregnancy, it can seriously affect the fetus and newborn`s health. The scarcity of studies on maternal and congenital syphilis in Indigenous Peoples remains an obstacle to its control in these populations. This study aimed to explore the breadth of the literature, map updated evidence, and identify knowledge gaps on maternal and congenital syphilis in Indigenous Peoples worldwide. METHODS: We conducted a Scoping review following Preferred Reporting Items for Systematic Reviews and Meta-Analyses - Extension for Scoping Reviews. In March 2021, we collected data through a priority search on PubMed, Web of Science, Embase, and SciELO. RESULTS: The strategy yielded 24 studies for analysis. Data in the articles were collected from 1989 to 2020, half from 2015 onwards. Studies were in Oceania and the Americas, mainly in South America (66.7%), particularly in Brazil (50.0%). The topics assessed were Data quality related to maternal and congenital syphilis (20.8%); Diagnosis, provision, access, and use of health services (62.5%); Disease frequency and health inequities (54.2%); Determinants of maternal syphilis and congenital syphilis (20.8%); and Outcomes of maternal and congenital syphilis in the fetus (20.8%). The results show that the available literature on maternal and congenital syphilis is sparse and concentrated in some geographic areas; the frequency of these diseases in Indigenous Peoples varies but is generally higher than in the non-indigenous counterparts; the quality of surveillance data and health information systems is poor; multiple healthcare barriers exist; and the diversity of terms to identify Indigenous Peoples is a challenge to mapping scientific outputs on Indigenous Peoples' health. CONCLUSIONS: Maternal and congenital syphilis in Indigenous Peoples is a double-neglected condition and research in this area should be given the priority and encouragement it deserves globally. Reliable data and improving access to health care are needed to reduce the burden of syphilis and correctly inform policies and health services response to mitigate ethnic-racial inequalities in maternal and congenital syphilis.

RESUMO

Escherichia coli is an important pathogen causing neonatal infectious diseases, such as neonatal meningitis and sepsis, and poses a great threat to the health of neonates. Currently, no effective screening and prevention measures are available. This article briefly reviews the rates of Escherichia coli colonization in pregnant women and newborns, the risk factors of colonization and transmission from mother to infant and the influences of Escherichia coli on pregnancy outcome, especially the harm to newborns. In clinical practice, more attention should be paid to high-risk pregnant women and the possible adverse pregnancy outcomes. For newborns born to mothers with Escherichia coli colonization, the possibility of Escherichia coli infection should be considered first if early meningitis and septicemia occur, which would be of great significance for the selection of antibiotics.

Evaluar la secuencia de la atención para la eliminación de la transmisión maternoinfantil del virus de la inmunodeficiencia humana (VIH) en Suriname y determinar los factores sociodemográficos y clínicos que previenen la transmisión a lactantes expuestos al virus. Métodos. En este estudio se empleó un diseño de métodos mixtos. Se utilizaron los datos de atención prenatal procedentes de la encuesta transversal de indicadores múltiples por conglomerados del 2018, realizada en 1 026 mujeres de entre 15 y 49 años que habían dado a luz a un nacido vivo en los dos años anteriores. También se evaluaron los datos nacionales correspondientes a una cohorte de 279 madres con infección por el VIH y sus 317 bebés nacidos entre el 2016 y el 2018. Además, se analizaron en detalle 13 casos de transmisión maternoinfantil del VIH. Resultados. En el 89,3% de los casos no hubo transmisión maternoinfantil del VIH. En las etapas iniciales de la secuencia de la atención se observó que el 28,4% de las mujeres no tenían cubiertas sus necesidades de planificación familiar; además, el 15% no dispusieron de consultas de atención prenatal, el 8% dieron a luz fuera de un centro de salud y en el 71,5% se llevó a cabo una prueba de detección del VIH en el marco de la atención prenatal. De las mujeres embarazadas con infección por el VIH, el 84,2% recibió un tratamiento antirretroviral, mientras que el 95,5% de los bebés recibieron un tratamiento profiláctico contra el VIH. La administración de tratamiento antirretroviral a la madre (cociente de posibilidades [OR] = 45,4; intervalo de confianza [IC] del 95%: 9,6-215,3) y al bebé (OR = 145,7; IC del 95%: 14,4-1477,4) hizo que aumentaran significativamente las posibilidades de obtener un resultado negativo en la prueba de detección del VIH en los lactantes. Por el contrario, residir en el interior del país hizo que disminuyeran dichas posibilidades (OR = 0,2; IC del 95%: 0,4-0,7), en comparación con residir en un entorno urbano. Conclusiones. Para las madres con infección por el VIH y para sus bebés, los medicamentos contra el VIH siguen siendo esenciales para prevenir la transmisión maternoinfantil del VIH. En Suriname debe reforzarse la atención prenatal temprana, incluido el seguimiento.

Avaliar a cascata de atendimento para a eliminação da transmissão materno-infantil do vírus da imunodeficiência humana (HIV) no Suriname e identificar fatores sociodemográficos e clínicos que impedem a transmissão a bebês expostos. Métodos. Foi utilizado um delineamento de estudo com métodos mistos. Foram usados dados de atendimento pré-natal de uma pesquisa de indicadores múltiplos por conglomerados de corte transversal realizada em 2018, que incluiu 1 026 mulheres com idades entre 15 e 49 anos que haviam tido um nascido vivo nos dois anos anteriores. Além disso, foram avaliados os dados nacionais de uma coorte de 279 mães com HIV e seus 317 bebês nascidos vivos de 2016 a 2018, além de 13 casos de transmissão materno-infantil de HIV. Resultados. Em 89,3% dos casos, não houve transmissão materno-infantil do HIV. As etapas iniciais da cascata demonstram que 28,4% das mulheres tiveram necessidades não atendidas de planejamento familiar, 15% não fizeram consultas pré-natais, 8% tiveram o parto fora de uma unidade de saúde e 71,5% receberam um teste de HIV durante o atendimento pré-natal. Das gestantes com HIV, 84,2% receberam terapia antirretroviral, e 95,5% de seus bebês receberam tratamento profilático para o HIV. O tratamento antirretroviral da mãe (razão de chances [RC]: 45,4; intervalo de confiança [IC] de 95%: 9,6-215,3) e da criança (RC: 145,7; IC 95%: 14,4-1477,4) aumentou significativamente a probabilidade de um resultado negativo no teste de HIV dos bebês. Por outro lado, morar no interior diminuiu a probabilidade (RC: 0,2; IC 95%: 0,4-0,7) em comparação com o ambiente urbano. Conclusões. A medicação de mães e bebês contra o HIV continua sendo fundamental para a prevenção da transmissão materno-infantil do HIV. Deve-se reforçar o atendimento pré-natal precoce com acompanhamento no Suriname.

ABSTRACT Objective: To describe the sociodemographic and gestational profile of HIV-positive women in Curitiba-PR, years 2018-2020. Method: Observational, cross-sectional research, with data obtained from the Information System of Diseases of Notification of Pregnant Women. Data were analyzed for consistency exploration, description and analysis. Results: The sample consisted mostly of women aged 13-30 years, white and with incomplete elementary school. Prenatal care was performed by 93.8% of pregnant women, 66.1% of whom knew their serological status before prenatal care and 45% received notification in the first gestational trimester. Access to antiretroviral medication occurred for 82.4% of pregnant women and for 74.6% the pregnancy outcome was alive newborns. The statistical variables associated with prenatal care were pregnancy evolution, ART prophylaxis, type of delivery and ART at delivery (p<0.00). Conclusion: The pregnant women in the sample presented desired gestational indicators. The collected data allowed describing the sample's profile and evaluating the performance of the health policy for pregnant women.


RESUMEN Objetivo: Describir el perfil sociodemográfico y gestacional de mujeres VIH positivas en Curitiba-PR, años 2018-2020. Método: Investigación observacional, de corte transversal, condatos obtenidos del Sistema de Información de Enfermedades de Notificación de la mujer embarazada. Los datos fueron analizados para exploración, descripción y análisis de consistencia. Resultados: La muestra estuvo compuesta en su mayoría por mujeres de 13 a 30 años, blancas y con instrucción básica incompleta. El control prenatal fue realizado por el 93,8% de las gestantes, siendo que el 66,1% conocía su estado serológico antes Del control prenatal y el 45% recibió notificación en el 1er trimestre. El acceso a la medicación antirretroviral ocurrió para el 82,4% de las mujeres embarazadas y para el 74,6% el resultado del embarazo fue nacido vivo. Las variables asociadas estadísticamente al control prenatal fueron evolución del embarazo, profilaxis antirretroviral, tipo de parto y antirretroviral al parto (p<0,001). Conclusión: Las gestantes de la muestra presentaron indicadores gestacionales deseados. Los datos recolectados permitieron describir el perfil de la muestra y evaluar El desempeño de la política de salud de las mujeres embarazadas.

Método: Pesquisa observacional, transversal, com dados do Sistema de Informação de Agravos de notificação das gestantes. Os dados foram analisados para exploração de consistência, descrição e análise. Resultados: Amostra perfilou-se majoritariamente por mulheres brancas na faixa etária de 13-30 anos. Pré-natal foi realizado por 93,8% das gestantes, sendo que 66,1% sabiam sua condição sorológica antes do pré-natal e 45% receberam a notificação no 1º trimestre. O acesso à medicação antirretroviral ocorreu para 82,4% das gestantes e para 74,6% o desfecho da gestação foi bebê nascido vivo. As variáveis estatisticamente associadas ao pré-natal foram evolução da gravidez, profilaxia com antirretroviral, tipo de parto e antirretroviral no parto (p<0,001). Conclusão: As gestantes da amostra apresentaram indicadores gestacionais desejados. Os dados coletados permitiram descrever o perfil da amostra e avaliar o desempenho da política de saúde para gestantes.

Background: Since 1997 an effective prevention and treatment of infections with human immunodeficiency virus (HIV) have been available in the form of antiretroviral combination therapy. Objective: What has been achieved during this time by transmission prophylaxis and treatment of HIV infections in children in Germany? Material and methods: Presentation of the development of transmission prophylaxis and the epidemiological surveys of the Robert Koch Institute, presentation of drug development and guidelines for antiretroviral therapy in children and adolescents and presentation of surveys of the German pediatric and adolescent HIV cohort (GEPIC). Results: The introduction of HIV testing of pregnant women has made regular antiretroviral treatment of HIV positive pregnant women and the introduction of transmission-reducing measures possible. This has reduced the risk of vertical transmission from about 30% to < 1%. Currently, it is being investigated whether children exposed to HIV can be breastfed without risk. Antiretroviral therapy in children has decisively improved the survival rates and the physical and cognitive development of HIV positive children. New active substances have a more favorable side effect profile. In Germany, the World Health Organization (WHO) target of achieving complete suppression of the viral load in 90% of the children has been achieved. A delayed diagnosis in children and adolescents is still associated with severe infections. An HIV infection remains a disease with a stigma and is therefore usually kept secret from the children and their environment. Conclusion: With the transmission prophylaxis and therapy that are now available, it is possible to prevent transmission to a large extent and with a timely diagnosis to enable HIV positive children to lead a largely healthy life. The goal of children being able to know and name their disease without fear has still not been achieved.

Objective: To analyze the temporal trend and spatial distribution of HIV mother-to-children transmission in Santa Catarina, 2007-2017. Methods: Mixed ecological study with data from the Notifiable Diseases Information System. Linear regression was performed for time series analysis and the mean rates in the period and annual mean percentage changes in the rates of HIV-infected pregnant women were calculated; of children exposed to HIV during pregnancy; and seroconversion of children exposed to HIV/AIDS during pregnancy, in addition to data geoprocessing. Results: There were 5,554 pregnant women living with HIV, with a rate of 5.6 pregnant infected. The mean seroconversion rate was 13.5/100,000 live births (95%CI 6.8;20.1) and showed a decreasing trend (APC=-99.4%; 95%CI-99.9;-93.1). The seroconversion rate was more expressive in small towns. Conclusion: The rate of HIV-infected pregnant women was stable in the period, whereas the number of children infected with HIV by vertical transmission decreased.


Objetivo: Estimar la tendencia temporal y distribución de casos de VIH por transmisión vertical en Santa Catarina, 2007-2017. Métodos: Estudio ecológico mixto con datos del Sistema de Información de Enfermedades de Declaración Obligatoria. Se utilizó regresión lineal para análisis de series de tiempo y se calcularon las tasas medias en período y los cambios porcentuales medios anuales en las tasas de mujeres embarazadas infectadas por el VIH; de niños expuestos al VIH; y seroconversión de niños expuestos al VIH/SIDA durante el embarazo, además geoprocesamiento de datos. Resultados: Había 5.554 gestantes infectadas por el VIH, con tasa de 5,6 gestantes/1.000 nacidos vivos. La tasa media de seroconversión fue 13,5/100.000 nacidos vivos (IC95% 6,8;20,1) y mostró tendencia decreciente (APC=-99,4%; IC95%-99,9;-93,1). La tasa de seroconversión fue más expresiva en los municipios pequeños. Conclusión: Hubo una estable tasa de mujeres embarazadas infectadas por el VIH, mientras que el número de niños infectados por el VIH disminuyó.

Resultados: Foram registradas 5.554 gestantes infectadas com HIV, com taxa de 5,6 gestantes/1.000 nascidos vivos. A taxa média de soroconversão foi de 13,5/100.000 nascidos vivos (IC95% 6,8;20,1) e apresentou tendência decrescente (APC=-99,4%; IC95% -99,9;-93,1). A taxa de soroconversão foi mais elevada em municípios de pequeno porte. Conclusão: A taxa de gestantes infectadas pelo HIV foi estável e houve diminuição de crianças infectadas pelo HIV por via vertical.

BACKGROUND: Important prevention efforts have led to a reduction in mother-to-child transmission of HIV (MTCT) globally. However, new cases of paediatric HIV infections still occur. Early diagnosis of new HIV infections is essential to start an appropriate antiretroviral treatment to avoid childhood morbidity and mortality related to infection. The aim of this study was to describe the new cases of MTCT in Latin-American referral hospitals. METHODS: A retrospective, multicentre and descriptive study of the new cases of MTCT diagnosed during 2018 in 13 referral hospitals from 8 Latin-American countries (Costa Rica, Ecuador, El Salvador, Guatemala, Honduras, Mexico, Nicaragua, and Panama) belonging to PLANTAIDS (Paediatric Network for Prevention, Early Detection and Treatment of HIV in Children), was conducted. PLANTAIDS is included in CYTED (Ibero-American Programme of Science and Technology for Development). RESULTS: Eighty-one children (40.7% males) were included, median age at diagnosis of 2.33 years (IQR:0.7-4.7). Less than 3% of women knew their HIV diagnosis before pregnancy. More than 80% of them were diagnosed after delivery, 8.7% during pregnancy, and 2.9% at delivery. Only one patient underwent antiretroviral therapy (ART) prior to pregnancy. At diagnosis, 50.0% of the children presented with an advanced stage of disease (stage C following the current CDC classification for HIV infection), and 34.4% had less than 15% CD4+ cells/mm3. The time elapsed between delivery and the maternal diagnosis was correlated with the age of children at diagnosis, ρ = 0.760, p < 0.001. Younger age at diagnosis (p = 0.03), a smaller number of previous hospitalizations (p < 0.01), and better immunovirological status (p < 0.01) were found in children whose mothers knew their HIV status at delivery, compared to mothers who were not aware of it. CONCLUSIONS: Although MTCT in Latin America has declined in recent years, our series shows there are still cases that indicate some failures in prevention, being a critical point to improve an earlier diagnosis of pregnant women. Half of the children were diagnosed in an advanced stage of disease and the delay in maternal diagnosis entailed a worse clinical and immunological child' prognosis.

BACKGROUND: Hepatitis B virus birth dose (HepB-BD) vaccination is recommended to reduce mother to infant transmission. We evaluated the HepB-BD status of women who gave birth between 2011 and 2016 (N = 3,583) using the 2015-2016 Myanmar Demographic and Health Survey. METHODS: Frequency distributions of HepB-BD vaccination across maternal and health system factors, concentration indices, and logistic regression models were used to estimate coverage, inequity, and factors associated with vaccination. RESULTS: The majority of participants were younger than 30 years of age, lived in rural areas, and were multiparous. Almost all received antenatal care (ANC), but only 43% received recommended ANC services, and 60% gave birth at home. The overall HepB-BD coverage rate was 26%. Vaccination coverage was higher in urban areas and was inequitably concentrated among children of more educated and wealthier women. HepB-BD coverage was also positively associated with receipt of ANC at non-governmental facilities, and delivery at a facility, skilled provider at birth and Cesarean delivery. After adjusting for sociodemographic and health system factors, receipt of the HepB-BD was positively associated with weekly media exposure, receipt of recommended ANC, and Cesarean delivery, and inversely associated with home delivery. CONCLUSIONS: Both socioeconomic and health systems factors influenced suboptimal and inequitable vaccination coverage. Improved access to quality ANC and delivery services may increase HepB-BD coverage although targeted approaches to reach home births are likely required to achieve national goals.

OBJECTIVES: To identify the prevalence of viral congenital infections in newborns classified as premature, low-birthweight, small for gestational age or intrauterine growth restriction. METHODS: The definition considered for selecting papers were: P as newborns younger than 28 days; V as low-birthweight, prematurity and intrauterine growth restriction; O as frequency of congenital infections with Cytomegalovirus, Parvovirus B19, Herpes Simplex, and Zika virus. The research was performed using EMBASE, LILACS, SCOPUS and MEDLINE databases, with no limitations on date and language. RESULTS: Eight studies were included. Manuscripts including Herpes Simplex, Zika virus or Parvovirus B19 did not fulfill the defined criteria. A wide variation in the frequency of CMV congenital infection (0-4.8%) was found, which might be attributed to regional and methodological differences between investigations. CONCLUSIONS: Newborn characteristics associated with CMV congenital infections may direct investigations towards these patients with a higher probability of infection. However, as data are controversial, studies concerning screening of infection are important to define recommendations of diagnosis.

Introducción. La malaria gestacional, definida como la presencia de Plasmodium spp. en sangre periférica materna o el hallazgo del parásito en la placenta, es considerada un importante problema de salud pública en las regiones tropicales y subtropicales. Objetivo. Determinar la frecuencia de casos de malaria gestacional diagnosticados en Ecuador entre 2015 y 2018. Materiales y métodos. Se hizo un estudio descriptivo, retrospectivo y transversal. Resultados. Se determinaron 46 casos de malaria gestacional en el período evaluado, 25 por Plasmodium falciparum y 21 por Plasmodium vivax, siendo el 2018 el año con más casos. En cuanto a las variables de edad y trimestre de gestación, prevalecieron en el grupo de 20 a 29 años (46 %) y en el segundo trimestre (37 %). Solo se observó una diferencia significativa entre los casos por año y la especie parasitaria. Conclusión. La malaria gestacional en Ecuador ha aumentado en los últimos cinco años, por lo que es importante informar a las mujeres en estado de gravidez sobre las medidas preventivas para evitar el contagio con el parásito, dadas las graves consecuencias que conlleva para ellas y sus hijos.


Introduction: Gestational malaria, defined as the presence of Plasmodium spp. in maternal peripheral blood or in the placenta, is considered an important public health problem in tropical and subtropical regions. Objective: To determine the frequency of gestational malaria cases diagnosed in Ecuador between 2015 and 2018. Materials and methods: We conducted a descriptive, retrospective, and cross-sectional study. Results: There were 46 cases of gestational malaria between 2015 and 2018: 25 caused by Plasmodium falciparum and 21 by Plasmodium vivax. The year with the most cases in this period was 2018. The age group most affected was 20 to 29 years old with 21 cases (46%). Prevalence was found to be highest in the second trimester of pregnancy with 17 cases (37%). A significant difference was only observed between cases per year and parasitic species. Conclusion: The prevalence of gestational malaria in Ecuador increased in the last five years. Therefore, it is important to inform pregnant women about preventive measures to avoid infection given its serious consequences both for the mother and her unborn child.

INTRODUCTION: Even though the risk of COVID-19 in pregnancy may be increased, large-scale studies are needed to better understand the impact of the infection in this population. The aim of this study is to describe obstetric complications and the rate of vertical transmission in pregnant women with SARS-CoV-2 infection. MATERIAL AND METHODS: Detected cases of SARS-CoV-2 infection in pregnancy were registered in Portuguese hospitals by obstetricians. Epidemiological, pregnancy and childbirth data were collected. RESULTS: There were 630 positive cases in 23 Portuguese maternity hospitals, most at term (87.9%) and asymptomatic (62.9%). The most frequent maternal comorbidity was obesity. The rates of preterm birth and small-to-gestational-age were 12.1% and 9.9%, respectively. In the third trimester, 2.9% of pregnant women required respiratory support. There were eight cases (1.5%) of fetal death, including two cases of vertical transmission. There were five cases of postpartum respiratory degradation, but no maternal deaths were recorded. The caesarean section rate was higher in the first than in the second wave (68.5% vs 31.5%). RT-PCR SARS-CoV-2 positivity among newborns was 1.3%. CONCLUSION: SARS-Cov-2 infection in pregnancy may carry increased risks for both pregnant women and the fetuses. Individualized surveillance and the prophylaxis of this population with vaccination. is recommended in these cases.


Introdução: Apesar do risco da COVID-19 na gravidez poder ser acrescido, são necessários estudos em larga escala para o melhor conhecimento do impacto desta infeção nesta população. O objetivo deste estudo é descrever as complicações obstétricas e a taxa de transmissão vertical em grávidas com infeção a SARS-CoV-2. Material e Métodos: Os casos conhecidos de infeção por SARS-CoV-2 na gravidez foram registados nos hospitais portugueses por obstetras. Foram recolhidos dados epidemiológicos, da gravidez e do parto. Resultados: Registaram-se 630 casos positivos em 23 maternidades portuguesas, a maioria no termo (87,9%) e assintomática (62,9%). A comorbilidade materna mais frequente foi a obesidade. A taxa de parto pré-termo e de leves para a idade gestacional foi de 12,1% e 9,9%, respectivamente. No terceiro trimestre, 2,9% das grávidas necessitaram de suporte respiratório. Verificou-se uma taxa de 1,5% de morte fetal, incluindo dois casos de transmissão vertical. Houve cinco casos de degradação respiratória no pós-parto, mas sem mortes maternas registadas. A taxa de cesarianas foi mais elevada na primeira do que na segunda vaga (68,5% vs 31,5%). A positividade do RT-PCR SARS-CoV-2 entre os recém-nascidos foi de 1,3%. Conclusão: A infeção pelo SARS-Cov-2 na gravidez pode acarretar riscos aumentados para as grávidas e fetos. Recomenda-se uma vigilância individualizada nestes casos e a profilaxia desta população com a vacinação.

Group B Streptococcus (GBS) is a major cause of severe perinatal infection, stillbirth, premature delivery, and neonatal infectious diseases. Ascending infection after vaginal colonization is the main route of prenatal GBS transmission. The pathogenic mechanism of GBS from asymptomatic colonization to invasive infection mainly relates to virulence factors, the regulation of two-component systems and immune escape. This paper reviews progress in pathogenic mechanism of perinatal GBS infection in recent years, aiming to provide a theoretical basis for the development of vaccines and new treatment approaches against GBS.

Objective:To summarize the clinical features, viral load changes, and outcomes of severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) Omicron variant infection in mother-infant dyads during lactation period.Methods:A total of 24 pairs of lactating mothers and infants under one year old who were infected with SARS-CoV-2 and hospitalized in Lingang Branch of Shanghai Sixth People's Hospital from April 8 to May 30, 2022, were selected as the lactation group in this retrospective study. Another 24 non-lactating mothers, with children of one to three years old, who matched with those mothers in the lactation group in clinical classification and admission date were selected as the control group. Vaccination status, clinical symptoms, daily cycle threshold (Ct) of open reading frame 1ab ( ORF1ab) gene and nucleocapsid protein ( N) gene, and the duration of positive nucleic acid test were compared between the groups and were analyzed using two independent samples t test, one-way analysis of variance, LSD test, and Chi-square test. Results:Among the 24 infants in the lactation group with an age of (6.5±2.1) months, 23 cases were mild type, one was common, and none had been vaccinated against SARS-CoV-2. The maternal age of the lactation and the control group did not differ statistically [(28.7±6.4) vs (28.2±5.2) years, t=0.30, P=0.768]. Mothers with mild type accounted for 88% (21/24) and those with common for 12% (3/24) in both groups of mothers. Three mothers received one dose of vaccine and two received two in the lactation group, while three received one dose and three received two in the control group [21%(5/24) vs 25%(6/24), χ 2=0.12, P=0.731]. The most common symptoms of lactating infants were fever (100%, 24/24) , followed by diarrhea (58%, 14/24) , cough (50%, 12/24), and wheeze (29%, 7/24), those of the lactating mothers were fever (75%, 18/24) , cough (75%, 18/24) , and sore throat (63%, 15/24) , while those of non-lactating mothers were cough (88%, 21/24) , sore throat (71%, 17/24), and fever (58%, 14/24). The duration of positive nucleic acid test was the shortest in the lactating infants [(9.2±2.1) d (5-14 d)], followed by mothers in the control group [(11.2±2.4) d (6-16 d)] and mothers in the lactation group [(14.0±4.2) d (8-26 d)] (LSD test, all P<0.05). Each day from day 2 to 9 after diagnosis, Ct values of nucleic acid of infants in the lactation group were all higher than those of mothers in both the lactation and control groups (LSD test, all P<0.05). On day 10, Ct value of nucleic acid infants was higher than that in mothers in the lactation group ( ORF1ab gene: 37.91±4.34 vs 32.79±5.47; N gene: 37.95±4.58 vs 32.66±5.77), which was lower than those in mothers in the control group ( ORF1ab gene: 32.79±5.47 vs 35.90±4.17; N gene: 32.66±5.77 vs 36.08±4.16) (LSD test, all P<0.05). On day 11, the nucleic acid Ct values of mothers in the lactation group were all lower than those in the control group ( ORF1ab gene: 35.03±3.74 vs 37.84±3.26, t=-2.78, P=0.008; N gene: 35.30±3.75 vs 38.11±2.90, t=-2.90, P=0.006). On day 12, Ct value of ORF1ab gene and N gene in mothers in the lactation group were similar to those in mothers in the control group (both P>0.05). Conclusions:The SARS-CoV-2 vaccination rate of mothers and infants were low during lactation. Lactating infants infected with SARS-CoV-2 Omicron variant have low virus load and may have a quick recovery, while for the lactating mothers, the virus load is high and the recovery is slow.

O SARS-CoV-2 é um vírus RNA transmitido pelo contato direto ou indireto por gotículas infectadas. No que se refere à COVID-19 e à gestação, referências apontam que nesse período as mulheres possuem maior susceptibilidade a complicações obstétricas e perinatais. O presente estudo objetiva compreender e compilar aspectos da infecção e os principais desfechos negativos maternos e fetais documentados na literatura atual, relacionados à infecção pelo novo coronavírus durante a gestação. Trata-se de uma revisão integrativa de literatura embasada pela análise de 2.441 artigos no total, dos quais 62 foram incluídos na pesquisa, sendo 38 deles da base de dados PubMed e 24 da BVS (Biblioteca Virtual em Saúde), nos idiomas inglês e português. Em conclusão, mulheres grávidas com diagnóstico da patologia podem precisar de assistência de alta complexidade. A associação à doença pode apresentar riscos ou complicações como coagulopatias, pré-eclâmpsia, prematuridade e outros desfechos negativos que serão abordados neste artigo.(AU)


SARS-CoV-2 is an RNA virus, transmitted by direct or indirect contact by infected droplets. Regarding to COVID-19 and pregnancy, references indicate that during this period, women are more susceptible to obstetric and perinatal complications. This study aims to understand and compile aspects of infection and the main negative maternal and fetal outcomes documented in the current literature, related to the infection by the new coronavirus during pregnancy. This is an integrative literature review based on the analysis of 2,441 articles in total, of which 62 were included in the survey, 38 from the PubMed database and 24 from BVS (Biblioteca Virtual em Saúde) in English and Portuguese languages. In conclusion, pregnant women diagnosed with the pathology may need highly complex assistance. The association with the disease may present risks for complications such as coagulopathies, pre-eclampsia, prematurity and other negative outcomes that will be addressed in this article.(AU)

ABSTRACT Objective: To test the effectiveness of the booklet, compared to the usual service care, in the increase of the knowledge of pregnant/puerperal women living with HIV, for the prevention of HIV-VT. Method: Pilot study of a randomized controlled clinical trial, initially with 104 pregnant women living with HIV, with a final sample of 45 women. It was held in three public maternity hospitals in Fortaleza-CE, from January/2017 to May/2018. The control group received regular care from the service and the intervention group had access to the booklet as an additive. The research was carried out in three phases: baseline; evaluation 2, in prenatal care; and evaluation 3, in the postpartum period. Results: There was no intergroup difference in the women's mean knowledge score (short-term p = 0.473; long-term p = 0.151). However, in the intragroup analysis, the booklet proved to be effective in improving the pregnant women's knowledge in the intervention group, in the short term (p = 0.002) and long term (p = 0.033). Conclusion: There was an improvement in knowledge within the intervention group over time, but there was no difference in women's knowledge in the intergroup analysis. Thus, based on this pilot, a broader study on the use of booklet is required to prove its effectiveness (ReBEC: UTN: U1111-1191-9954).


RESUMEN Objetivo: Testar la eficacia del cuardenillo, en comparación al servicio habitual, en el incremento del conocimiento de las mujeres embarazadas/puérperas que viven con el VIH, para la prevención del TV-VIH. Método: Piloto de un ensayo clínico controlado aleatorizado, inicialmente con 104 mujeres embarazadas que viven con el VIH, con una muestra final de 45 mujeres. Realizado en tres maternidades públicas de Fortaleza-CE, de enero/2017 a mayo/2018. El grupo control recibió atención regular del servicio y el grupo intervención tuvo acceso al cuadernillo como complemento. La investigación se llevó a cabo en tres etapas: línea de base; evaluación 2, en atención prenatal; y evaluación 3, en el puerperio. Resultados: No hubo diferencia entre grupos en la puntuación media de conocimiento de las mujeres (a corto plazo p = 0,473; a largo plazo p = 0,151). Sin embargo, en el análisis intragrupo, el cuardenillo se mostró eficaz en la mejora del conocimiento de las gestantes del grupo intervención, a corto plazo (p = 0,002) y a largo plazo (p = 0,033). Conclusión: Hubo una mejora en el conocimiento dentro del grupo de intervención con el tiempo, pero no hubo diferencia en el conocimiento de las mujeres en el análisis intergrupal. Por lo tanto, el cuadernillo necesita realizar un estudio más amplio, basado en este piloto, para probar su efectividad (ReBEC: UTN: U1111-1191-9954).

OBJECTIVES: We sought to investigate the prevalence of HIV in pregnant women and the maternal, obstetrical, and neonatal outcomes over 13 years (2005-2017) at Sultan Qaboos University Hospital, a tertiary hospital in Muscat, Oman. METHODS: Our study design was a retrospective descriptive cross-sectional study of HIV-positive women in the department of obstetrics and gynecology. We had an unlimited sample size due to the rarity of the disease. We aimed to include all pregnant Omani women who tested positive for HIV in their antenatal screening from 1 January 2005 to 31 December 2017. The patient records were reviewed using the hospital information system service. RESULTS: There were a total of 13 688 women with 104 281 pregnancies over the study period. The prevalence of Omani pregnant women with HIV was 0.1% (1:1000) with 0.03% of seropositive pregnancies (3:10 000). Of these seropositive pregnancies, 78.6% were known HIV carriers, while 21.4% were newly diagnosed cases. The live birth rate was 90.3%, with 9.7% resulting in miscarriage. All the miscarriages were to known HIV-positive mothers who were managed in accordance with international guidelines. Of the live births, 10.7% were unknown HIV-positive mothers with a mean gestational age of 39.67 weeks and a mean birth weight of 3.2 kg. The rate of mother-to-child transmission (MTCT) of HIV in this group was 33.3%. In contrast, 89.3% of the live births to known HIV-positive mothers delivered neonates with a mean gestational age of 37.4 weeks and a mean birth weight of 2.6 kg with 0.0% MTCT. Modes of delivery, antepartum, intrapartum and postpartum complications as well as long-term neonatal outcomes were analyzed according to the status of the mother at first presentation. CONCLUSIONS: Strategies have been placed by programs in Oman to focus on the wellbeing of pregnant women and the protection of newborns against HIV infection. Strict implementations on preventing MTCT allowed preventing HIV in children possible. Antiretroviral therapy significantly reduces vertical transmission of HIV, in addition to abstinence of breastfeeding. More importantly, all HIV-positive pregnant women should follow the prevention of MTCT programs set out by the Ministry of Health.

Introduction: Congenital infections are related to a higher risk of morbidity and mortality in the neonatal period and can cause serious complications in the newborn. Among the sexually transmitted infections (IST) present in this group, syphilis and vertical exposure to the human immunodeficiency virus (HIV) are still a challenge in the 21st century. Objective: To evaluate the prevalence of congenital syphilis and exposure to HIV among congenital infections. Methods: Cross-sectional, analytical study with data collected from medical records of newborns admitted to the Conventional Neonatal Intermediate Care Unit, Kangaroo and Joint Accommodation, in the database of the service of the Federal Hospital from Bonsucesso, from January 2015 to December 2018. Results: During the study, 2,202 newborns were discharged from the hospital and 474 were positive for congenital infection (21.8%). In cases of congenital infection, congenital syphilis (398­84%) and maternal HIV infection (40­8,4%) were the most frequently encountered clinical situations. The relationship between prenatal care and the absence of congenital infection was evident. Among the newborns, 117 (5.3%) were premature, 352 (16.3%) were older than 37 weeks and 95 (23.7%) had low birth weight. Conclusion: Congenital infection was one of the main causes of neonatal morbidity; syphilis and HIV had the highest prevalence, being associated with reducible deaths when adequate care is provided for women during pregnancy. Perinatology services demand a great effort in order to manage these avoidable and undesirable situations.


Introdução: As infecções congênitas estão relacionadas a um risco mais elevado de morbimortalidade no período neonatal e geram sérias complicações no recém-nascido. Dentre as infecções sexualmente transmitidas (IST) presentes nesse grupo, a sífilis e a exposição vertical ao vírus da imunodeficiência humana (HIV) ainda são um desafio em pleno século XXI. Objetivo: Avaliar a prevalência da sífilis congênita e exposição ao HIV entre as infecções congênitas. Métodos: Estudo de corte transversal e analítico, com dados coletados nos prontuários de alta dos recém-nascidos que ficaram internados na Unidade de Cuidado Intermediário Neonatal Convencional, Canguru e Alojamento Conjunto, no banco de dados do Serviço do Hospital Federal de Bonsucesso, de janeiro de 2015 a dezembro de 2018. Resultados: No período do estudo, 2.202 recém-nascidos receberam alta hospitalar e 474 apresentaram positividade para infecção congênita (21,8%). Nos quadros de infecção congênita, a sífilis congênita (398­84,0% dos casos) e a infecção materna pelo HIV (40­8,4%) foram as situações clínicas mais frequentemente encontradas. Ficou evidente a relação entre a realização do pré-natal e a ausência de infecção congênita. Desses recém-nascidos, 117 (5,3%) foram prematuros, 352 (16,3%) tinham mais de 37 semanas e 95 (23,7%) tinham peso abaixo de 2.500 g. Conclusão: A infecção congênita foi uma das principais causas de morbidade neonatal; a sífilis e o HIV tiveram as maiores prevalências, estando associados às mortes redutíveis por adequada atenção à mulher na gestação. Os serviços de perinatologia demandam um grande esforço para atender essas situações evitáveis e indesejáveis.
