INTRODUCTION: Product design is a key tobacco industry strategy to recruit consumers and hinder cessation. An important technology in tobacco product design is the use of additives including those that create characterising flavours, that is, flavours that are perceived by taste and or smell by the user. To curb youth use, many countries are implementing policies to limit the use of flavours and other additives in tobacco products. This paper reviews the global landscape of tobacco additives policies including those focusing only on flavouring additives, updating a previous review. METHODS: We gathered additives policy data from available online sources of tobacco control policies including parties' reports to the WHO Framework Convention on Tobacco Control (where WHO refers to World Health Organization), the website Tobacco Control Laws, government websites, searches with the Google search engine and consultations with the WHO to ensure comprehensiveness of the assessment. The policies were classified according to their objective and characteristics: Labelling regulations, sales bans and tobacco product design regulations. We classified and organised the regulations according to their stated intent. RESULTS: We found that 62 countries had tobacco products' additives-related regulations at the regional, national or subnational levels. Most of these regulations focused on flavouring additives especially bans on the selling, importing and manufacturing tobacco products with characterising flavours. CONCLUSIONS: The data demonstrate various global policies addressing additives and/or flavours and a range of definitions of additives and flavours. Establishing common definitions and standards could facilitate cooperation between countries and regions.

BACKGROUND: Low- and middle-income countries have increasingly banned e-cigarettes, as in Mexico. In these countries, little is known about where adults obtain e-cigarettes or who uses e-cigarettes with nicotine. METHODS: Data were analyzed from eight online surveys of Mexican adults who both smoked and used e-cigarettes (November 2018-March 2021; n = 2,060). For the e-cigarette they used most often, participants reported how they acquired it (social sources=reference; online purchase; vape shop purchase; other retail purchase) and if it contained nicotine (no=reference group; yes; don't know). Multinomial models regressed each of these outcomes on smoking- and e-cigarette-related factors, as well as sociodemographics. RESULTS: Almost half the sample (45.9 %) reported obtaining their e-cigarettes from social sources, with online purchase being the second most common source (28.7 %). Being male, having recently attempted to quit smoking, and more frequent e-cigarette use were positively associated with purchasing e-cigarettes (vs social sources) across all venues. Most reported that their e-cigarettes contained nicotine (58.2 %), a third reported using e-cigarettes without nicotine (35.9 %), and some did not know (5.8 %). More frequent smoking and e-cigarette use, using closed e-cigarette devices and purchasing e-cigarettes online were positively associated with using e-cigarettes with nicotine. CONCLUSIONS: Despite Mexico's e-cigarette ban, adults who smoke access e-cigarettes through multiple sources, including online and vape shop purchases. Most participants reported using e-cigarettes with nicotine, though many did not or did not know.

Introduction: Social media use and vaping nicotine are highly prevalent in the daily lives of young adults, especially among Mexican-American college students. The excessive and compulsive use of social media platforms, coupled with the urge to stay continuously connected, can lead to problematic social media use. To date, no studies have explored the impact of problematic social media use on the daily patterns of vaping among this vulnerable population. Methods: In Spring 2023, we employed Ecological Momentary Assessment over a period of 14 days to collect real-time daily data on participants' social media use and vaping behaviors via a mobile phone-based application. Participants were 51 Mexican-American college students aged 18-25 years, 72.5% female, who were current vapers. We used generalized linear regression models to examine differences in vaping behaviors among participants with and without problematic social media use. All regression models adjusted for age, sex, and SES. Results: Participants with problematic social media use vaped on an average of 5.9 days compared to 5.7 days reported by those without problematic social media use (p < .05). Problematic social media use is associated with more frequent daily vaping [b = 0.03; 95% CI: 0.02-0.05], increased number of days vaping [b = 0.14; 95% CI: 0.07-0.20], and vaping higher nicotine concentrations [b = 0.08; 95% CI: 0.03-0.14]. Conclusion: Results suggest that problematic social media use significantly increases the risk of daily vaping among Mexican-American college students. Findings highlight the need to strengthen digital resilience and social media literacy to help college students navigate and mitigate the risks of social media.

OBJECTIVE: To provide cutting-edge information on the impact and risks of using Electronic Nicotine Delivery Systems (ENDS) by children and adolescents, based on the latest evidence published in the literature. DATA SOURCE: A comprehensive search was carried out on PubMed, using the expressions ''electronic cigarettes'' OR ''electronic nicotine delivery systems" OR "vaping" AND ''adolescent'' AND "risks" AND ''acute lung injury'. All retrieved articles had their titles and abstracts read to identify and fully read the papers reporting the most recent evidence on each subject. SUMMARY OF FINDINGS: The use of ENDS has alarmingly increased in Brazil and around the world. The possibility of customizing use, the choice of flavors and nicotine content, and the general notion that these devices are harmless when compared to conventional cigarettes are some of the factors responsible for this increase. Numerous scientific studies have proven that electronic cigarettes have serious consequences for the respiratory system, such as EVALI (E-cigarette or Vaping-Associated Lung Injury) and difficult-to-control asthma, as well as harmful effects on the neurological, cardiovascular, gastrointestinal, and immunological systems. High concentrations of nicotine make many young people addicted to this substance. In Brazil, commercialization, import, and advertising are prohibited. The viable interventions to address the use of these devices in children and adolescents are prevention and behavioral counseling. CONCLUSION: There is clear scientific evidence that these devices pose a risk to the physical and mental health of children and adolescents.

INTRODUCTION: Reasons for using and stopping the use of e-cigarettes and their associations with transitions in nicotine product use are relatively unknown in countries with e-cigarette bans, such as Mexico. METHODS: Data comes from an open cohort of people who smoke in Mexico, surveyed every 4 months from November 2018 to November 2021. Those who smoked and used e-cigarettes at time t (n=904 individuals, 1653 observations) were categorised at 4-month follow-up (t+1): (1) continued 'dual use', (2) exclusive smoking, (3) exclusive use of e-cigarettes or neither product. For people who formerly used e-cigarettes at time t (n=332 individuals, 372 observations), follow-up categories were: (1) continued exclusive smoking; (2) re-initiated e-cigarette use. Multinomial and logistic models regressed follow-up status (ref=status at time t) on reasons for using or stopping e-cigarette use, respectively, at time t, adjusting for covariates. RESULTS: The most prevalent reasons for current e-cigarette use were 'they were less harmful to others' (40.5%) and 'enjoyable' (39.0%). Those who reported using e-cigarettes because they were less harmful to others (Adjusted Relative Risk Ratio (ARRR)=0.67), more enjoyable (ARRR=0.52), could help them to quit smoking (ARRR=0.65), or to control weight (ARRR=0.46) were less likely to return to exclusively smoking. Among people who formerly used e-cigarettes, lack of satisfaction was the primary reason for stopping e-cigarette use (32%) and those who reported this were less likely to start using e-cigarettes again at follow-up (Adjusted Odds Ratio (AOR)=0.58). CONCLUSIONS: Specific reasons for using and stopping e-cigarettes predict changes in smoking and e-cigarette use, and targeting these beliefs could promote desired behaviour changes.

OBJECTIVES: Electronic nicotine delivery systems (e-cigarette, pod, and vape) are currently among the tobacco consumption of adolescents and young adults. The aim is to show oral mucosa and saliva alterations related to vape. MATERIAL AND METHODS: A vape-user patient, presenting a white plaque in the posterior region of the hard palate, underwent clinical examination, sialometry, pH evaluation, and excisional biopsy of the white lesion. Molecular changes in saliva and vape liquid were analyzed by vibrational spectroscopy. RESULTS: The histopathological analyses showed hyperparakeratosis without dysplasia. Formaldehyde, ketones, and aromatic hydrocarbon species were identified in e-cig liquid by the FTIR. CONCLUSIONS: The use of vape may be related to the development of hyperkeratotic lesions in the oral mucosa as well as significantly modify the patient's salivary patterns as the vape liquid presents carcinogenic and cytotoxic components in its composition.

Abuse-related drug usage is a public health issue. Drosophila melanogaster has been used as an animal model to study the biological effects of these psychoactive substances in preclinical studies. Our objective in this review is to evaluate the adverse effects produced by cocaine, nicotine, and marijuana during the development of D. melanogaster. We searched experimental studies in which D. melanogaster was exposed to these three psychoactive drugs in seven online databases up to January 2023. Two reviewers independently extracted the data. Fifty-one studies met eligibility criteria and were included in the data extraction: nicotine (n = 26), cocaine (n = 20), and marijuana (n = 5). Fifteen studies were eligible for meta-analysis. Low doses (â¼0.6 mM) of nicotine increased locomotor activity in fruit flies, while high doses (≥3 mM) led to a decrease. Similarly, exposure to cocaine increased locomotor activity, resulting in decreased climbing response in D. melanogaster. Studies with exposure to marijuana did not present a profile for our meta-analysis. However, this drug has been less associated with locomotor changes, but alterations in body weight and fat content and changes in cardiac function. Our analyses have shown that fruit flies exposed to drugs of abuse during different developmental stages, such as larvae and adults, exhibit molecular, morphological, behavioral, and survival changes that are dependent on the dosage. These phenotypes resemble the adverse effects of psychoactive substances in clinical medicine.

Globally, there are around 1.3 billion cigarette consumers, indicating it to be the second highest risk factor for early death and morbidity. Meanwhile, psychological therapy offers tools based on its different models and techniques, which can contribute to smoking cessation. In this context, this study gathers scientific evidence to identify psychological therapies that can be used to reduce cigarette consumption. A systematic review of controlled clinical studies was conducted, implementing the PRISMA methodology. Search queries were performed with terms extracted from MESH (Medical Subject Headings) and DECS (Descriptors in Health Sciences). Subsequently, the search was queried in the scientific databases of Medline/PubMed, Cochrane, Scopus, Science Direct, ProQuest, and PsycNet, with subsequent verification of methodological quality using the Joanna Briggs Institute checklists. The selected documents revealed that cognitive behavioral therapy prevails due to its use and effectiveness in seven publications (25%). The cognitive approach with mindfulness therapy is found in 4 publications (14%), the transtheoretical model with motivational therapy in 4 publications (14%), brief psychological therapy in 3 publications (10%), and the remaining 10 documents (37%) correspond with others. Intervention studies refer to cognitive behavioral therapy as the most used in reducing cigarette consumption; in terms of the duration of abstinence, scientific evidence shows beneficial effects with short-term reduction.

One of the biggest problems in the treatment of idiopathic Parkinson's disease is the lack of new drugs that slow its progression. L-Dopa remains the star drug in the treatment of this disease, although it induces severe side effects. The failure of clinical studies with new drugs depends on the use of preclinical models based on neurotoxins that do not represent what happens in the disease since they induce rapid and expansive neurodegeneration. We have recently proposed a single-neuron degeneration model for idiopathic Parkinson's disease that requires years to accumulate enough lost neurons for the onset of motor symptoms. This single-neuron degeneration model is based on the excessive formation of aminochrome during neuromelanin synthesis that surpass the neuroprotective action of the enzymes DT-diaphorase and glutathione transferase M2-2, which prevent the neurotoxic effects of aminochrome. Although the neurotoxic effects of aminochrome do not have an expansive effect, a stereotaxic injection of this endogenous neurotoxin cannot be used to generate a preclinical model in an animal. Therefore, the aim of this review is to evaluate the strategies for pharmacologically increasing the expression of DT diaphorase and GSTM2-2 and molecules that induce the expression of vesicular monoamine transporter 2, such as pramipexole.

Resumen Objetivo: Comparar la efectividad de los chicles de nicotina frente al uso de cigarrillos electrónicos y mostrar si resultan más efectivos en la reducción del hábito tabáquico. Metodología: se incluyeron artículos en español, inglés y portugués disponibles en SciELO, Google Académico, EBSCO, Cochrane Library y LILACS, a través de ecuaciones construidas con MeSH y DeCS y operadores booleanos AND y OR (("Cigarrillos electrónicos" OR "e-Cigs" OR "Cigarros electrónicos") OR ("Chicle" OR "Goma de mascar de nicotina") AND ("Cesación de tabaquismo")), se clasificaron por nivel de evidencia y grado de recomendación. Las consideraciones éticas se adoptaron conforme a la Resolución 8430 de 1993. Resultados: la búsqueda inicial arrojó 1.468 artículos, que se redujeron a 28 textos que cumplieron los criterios de inclusión. Se construyeron cinco temáticas en las que se incluyen comparación de métodos, formas de evaluación, rangos etarios, índices de adicción, patologías asociadas e índice de mortalidad, componentes y efectos fisiológicos del cigarrillo electrónico y convencional. Conclusiones: se evidencia que las cantidades de nicotina de los cigarrillos electrónicos producen efectos adversos irreversibles para la salud y pueden generar mayor dependencia.
Abstract Objective: Compare the effectiveness of nicotine gum versus the use of electronic cigarettes and show whether they are more effective in reducing smoking. Methodology: Articles in Spanish, English and Portuguese, available from SciELO, Google Academic, EBSCO, Cochrane Library and LILACS, were included through equations built with MeSH and DeCS and Boolean operators AND Dec's OR (("Electronic cigarettes" OR "E-Cigs" OR "Electronic Cigars") OR ("Chewing gum" OR "Nicotine chewing gum") AND ("Smoking cessation")). The articles were critically read, and classified by level of evidence and degree of recommendation. Ethical considerations were adopted in accordance with Resolution 8430 of 1993. Results: The initial search yielded 1,468 articles, which were reduced to 28 units that contributed to the stated objective. Five themes were built, which include a comparison of methods, evaluation forms, age ranges, addiction rates, associated pathologies and mortality rate, components and physiological effects of the conventional and electronic cigarette. Conclusions: It is evident that the amounts of nicotine of e-cigarettes produce irreversible adverse health effects and can lead to increased dependency.
Resumo Objetivo: Compare a eficácia da goma de mascar de nicotina versus o uso de cigarros eletrônicos e mostre se eles são mais eficazes na redução do tabagismo. Metodologia: Os artigos em espanhol, inglês e português, disponíveis na SciELO, no Google Acadêmico, na EBSCO, na Cochrane Library e na LILACS, foram incluídos por meio de equações construídas com os operadores MeSH e DeCS e booleanos AND e OR (("cigarros eletrônicos" OU "E-Cigs" ou "charutos eletrônicos") OR ("goma de mascar" ou "goma de mascar de nicotina") AND ("cessação do tabagism")). Os artigos foram lidos criticamente e classificados por nível de evidência e grau de recomendação. Considerações éticas foram adotadas de acordo com a Resolução 8430 de 1993. Resultados: A pesquisa inicial produziu 1.468 artigos, reduzidos a 28 unidades que contribuíram para o objetivo declarado. Foram construídos cinco temas que incluem a comparação de métodos, formas de avaliação, faixas etárias, taxas de dependência, patologias e taxa de mortalidade associadas, componentes e efeitos fisiológicos dos cigarros convencionais e eletrônicos. Conclusões: É evidente que as quantidades de nicotina dos Os cigarros eletrônicos produzem efeitos adversos irreversíveis à saúde e podem levar ao aumento da dependência.
RESUMO
Parkinson's disease (PD) is a progressive neurodegenerative disease characterized by resting tremor, bradykinesia, rigidity, and postural instability that also includes non-motor symptoms such as mood dysregulation. Dopamine (DA) is the primary neurotransmitter involved in this disease, but cholinergic imbalance has also been implicated. Current intervention in PD is focused on replenishing central DA, which provides remarkable temporary symptomatic relief but does not address neuronal loss and the progression of the disease. It has been well established that neuronal nicotinic cholinergic receptors (nAChRs) can regulate DA release and that nicotine itself may have neuroprotective effects. Recent studies identified nAChRs in nonneuronal cell types, including glial cells, where they may regulate inflammatory responses. Given the crucial role of neuroinflammation in dopaminergic degeneration and the involvement of microglia and astrocytes in this response, glial nAChRs may provide a novel therapeutic target in the prevention and/or treatment of PD. In this review, following a brief discussion of PD, we focus on the role of glial cells and, specifically, their nAChRs in PD pathology and/or treatment.

Introdução: O presente estudo teve como objetivo investigar e identificar a prevalência e fatores de risco associados ao tabagismo e outras formas de consumo de tabaco entre acadêmicos da saúde. Métodos: A pesquisa contou com 407 acadêmicos dos cursos de Biomedicina, Educação Física, Enfermagem, Farmácia, Fisioterapia e Nutrição de uma Instituição de Ensino Superior. A coleta de dados foi realizada no mês de maio de 2020, por um formulário eletrônico, disponibilizado em e-mail institucional, grupos de estudos e redes sociais. O procedimento ocorreu após aprovação pelo Comitê de Ética, cujo parecer 3.966.951. Resultados: A maioria dos participantes (79,6%) eram do sexo feminino, solteiros (75,2%) com idade média de 25,32 anos. O consumo de tabaco foi confirmado por 10,8% participantes. Quanto ao consumo de produtos derivados de tabaco 24,1% dos entrevistados já consumiram produto de tabaco, 8,6% assumiram tabagismo ocasional, e 4,2% tabagismo ativo. Conclusões: A associação entre tabagismo e as variáveis sociodemográficas demonstrou que alunos do curso de Farmácia (OR: 5,25 [IC:1,34-20,22] p=0,017), homens (OR: 1,71 [IC: 1,01 2,91]), estudantes turno matutino (OR: 1,96 [IC:1,02-3,78] p=0,04) e que residem com tabagistas (OR:4,44 [IC: 2,24-8,80]) apresentaram maiores chances de serem tabagista. A associação em relação ao consumo de derivados do tabaco, os homens apresentaram maiores prevalências de consumo de derivados de tabaco (OR: 1,71 [IC: 1,01 2,91] p=0,045) bem como, alunos do curso de Farmácia (OR: 6,40 [IC:2,31-17,7] P<0,01), que estudam no turno Noturno (OR:1,85 [IC:1,16-8,82] p=0,009) e entre os que residem com tabagistas (OR: 4,49 [IC:2,24-8,80] p<0,001).
Introduction: The present study aimed to investigate and identify the prevalence and risk factors associated with smoking and other forms of tobacco consumption among health academics. Methods: The survey involved 407 students from Biomedicine, Physical Education, Nursing, Pharmacy, Physiotherapy and Nutrition courses at a Higher Education Institution. Data collection was carried out in May 2020, through an electronic form, available in institutional email, study groups and social networks. The entire procedure took place after approval by the Ethics Committee. Results: Most participants (79.6%) were female, single (75.2%) with a mean age of 25.32 years. Tobacco consumption was confirmed by 10.8% of the participants. As for the consumption of tobacco products, 24.1% of the interviewees had already consumed some tobacco product, 8.6% assumed occasional smoking, and 4.2% active smoking. Conclusions: The association between smoking and sociodemographic variables showed that Pharmacy students (OR: 5.25 [CI:1.34-20.22] p=0.017), men (OR: 1.71 [CI: 1.01 2.91]), morning shift students (OR: 1.96 [CI:1.02-3.78] p=0.04) and those who live with smokers (OR:4.44 [CI : 2.24-8.80]) were more likely to be smokers. The association in relation to the consumption of tobacco derivatives, men had a higher prevalence of consumption of tobacco derivatives (OR: 1.71 [CI: 1.01 2.91] p=0.045) as well as Pharmacy students (OR: 6.40 [CI:2.31-17.7] P<0.01), those who study the night shift (OR:1.85 [CI:1.16-8.82] p=0.009 ) and among those who live with smokers (OR: 4.49 [CI:2.24-8.80] p<0.001).

Psychostimulants regulate behavioral responses in zebrafish via epigenetic mechanisms. We have previously shown that DNA methylation and histone deacetylase (HDAC) inhibition abolish nicotine-induced conditioned place preference (CPP) but little is known about the role of histone methylation in addictive-like behaviors. To assess the influence of histone methylation on nicotine-CPP, zebrafish were treated with a histone (H3) lysine-9 (K9) dimethyltransferase G9a/GLP inhibitor, BIX-01294 (BIX), which was administered before conditioning sessions. We observed a dual effect of the inhibitor BIX: at high doses inhibited while at low doses potentiated nicotine reward. Transcriptional expression of α6 and α7 subunits of the nicotinic acetylcholine receptor and of G9a, DNA methyl transferase-3, and HDAC-1 was upregulated in zebrafish with positive scores for nicotine-CPP. Changes in relative levels of these mRNA molecules reflected the effects of BIX on nicotine reward. BIX treatment per sé did not affect transcriptional levels of epigenetic enzymes that regulate trimethylation or demethylation of H3. BIX reduced H3K9me2 protein levels in a dose-dependent manner in key structures of the reward pathway. Thus, our findings indicated that different doses of BIX differentially affect nicotine CPP via strong or weak inhibition of G9a/GLP activity. Additionally, we found that the lysine demethylase inhibitor daminozide abolished nicotine-CPP and drug seeking. Our data demonstrate that H3 methylation catalyzed by G9a/GLP is involved in nicotine-CPP induction. Dimethylation of K9 at H3 is an important epigenetic modification that should be considered as a potential therapeutic target to treat nicotine reward and perhaps other drug addictions.

Smoking is highly prevalent in people living with HIV/AIDS (PLHA), leading to detrimental effects in different tissues. We examined the effects of nicotine replacement therapy (NRT) on smoking cessation and vascular health. From December 2019 to October 2021, we prospectively enrolled PLHA who were actively smoking. The primary outcome was endothelial function measured by brachial artery flow-mediated dilatation (FMD). We evaluated the percent change in FMD compared to the baseline measure (Δ%FMD) to detect improvements among participants who quit smoking. To confirm the results, we used linear regression models to account for classical cardiovascular (CV) confounders. We included 117 participants with median age of 45.5 years (IQR= 36.4-54.8); 22 (20.4%) had hypertension, 9 (8.3%) had diabetes, almost half were smoking 20+ cigarettes/day (41.7%). After 12 weeks 30.76% participants quit smoking. Comparison of Δ%FMD change from baseline to week 12 showed that among participants adherent to therapy, there has been an increase in Δ%FMD when compared to those who relapsed (1.17% [0.29-2.98] vs -0.19% [-1.95-0.91], p<0.001). After adjustment for CV factors, multiple linear regression showed that Δ%FMD in participants who quit smoking presented a 2.54 mean increase in comparison to those who continued smoking (p=0.007). In conclusion, this study provides evidence that a strategy of NRT and counseling is modestly effective for smoking cessation in PLHA and improves vascular health in a short period of time. This reinforces the importance of the widespread anti-tobacco programs in HIV clinics and the expected impact lowering the incidence of future cardiovascular events.

O cigarro eletrônico, também conhecido como e-cig, tem sido amplamente utilizado nos últimos anos, principalmente pela população mais jovem. Substâncias citotóxicas e carcinogênicas podem estar presentes em sua composição, no entanto, pouco se sabe sobre os riscos do seu uso. O objetivo deste trabalho foi avaliar o perfil proteico da saliva de usuários de cigarro eletrônico por meio de estudo proteômico e as possíveis alterações salivares desses indivíduos. Para tanto, os participantes foram divididos em dois grupos: Grupo Cigarro Eletrônico (GCE), composto por 25 vaporizadores regulares e exclusivos de e-cig, e Grupo Controle (GCO), constituído por 25 indivíduos não fumantes e não vaporizadores de e-cig, pareados por sexo e idade ao GCE. Todos os participantes foram submetidos a exame clínico e coleta de saliva não estimulada para verificação da sialometria e avaliação do proteoma salivar. As amostras foram aliquotadas e submetidas às seguintes análises: viscosidade salivar, pH, capacidade tampão e mensuração da cotinina salivar realizada por meio do teste imunoenzimático (ELISA) utilizando Cotinine Enzyme Immunoassay Kit (Salimetrics). O perfil etílico dos pacientes foi avaliado por meio do teste AUDIT (Alcohol Use Disorders Identification Test) e por perguntas referentes ao consumo de álcool. Já o perfil de consumo do e-cig foi traçado por meio de perguntas referentes ao uso dos dispositivos e teste de concentração de monóxido de carbono no ar expirado. A análise proteômica foi realizada utilizando a metodologia shotgun e cromatografia líquida acoplada à espectrometria de massas (LC-MS). Os resultados obtidos, uma vez que não possuíam distribuição normal, foram submetidos ao teste de Mann Whitney. Foi observada maior concentração de CO exalado, maior concentração salivar de cotinina, maior pontuação no AUDIT, menor viscosidade salivar e resultado desfavorável na oximetria. Em relação ao proteoma salivar, 48 proteínas foram específicas do GCE, sendo que 3 das proteínas desse conjunto estão diferencialmente expressas nesse grupo. Conclui-se que os usuários de cigarro eletrônico apresentam maiores índices de consumo de bebida alcoólica e uma menor viscosidade salivar, alterações importantes na capacidade respiratória demonstrada pelo aumento da concentração de CO no ar expirado e diminuição da oximetria e altos níveis de cotinina presentes na saliva. Além disso, a maior abundância de proteínas, como Glutationa S-transferase P, Cadeia J de imunoglobulina e Acetína desidrogenase, sugerem uma resposta do organismo ao aumento do estresse oxidativo, presença de componentes inflamatórios e alterações microbiológicas salivares que podem favorecer o desenvolvimento de condições patológicas futuras.(AU)
The electronic cigarette, also known as e-cig, has been widely used in recent years, especially by younger populations. Cytotoxic and carcinogenic substances may be present in its composition; however, little is known about the risks of its use. The aim of this study was to evaluate the protein profile of saliva from electronic cigarette users through proteomic analysis and the possible salivary alterations in these individuals. For this purpose, participants were divided into two groups: the Electronic Cigarette Group (ECG), composed of 25 regular and exclusive e-cig users, and the Control Group (CG), consisting of 25 non-smokers and non-e-cig users, matched by sex and age to the ECG. All participants underwent clinical examination and unstimulated saliva collection for salivary flow rate measurement and evaluation of salivary proteome. The samples were aliquoted and subjected to the following analyses: salivary viscosity, pH, buffer capacity, and measurement of salivary cotinine performed using the enzyme-linked immunosorbent assay (ELISA) Cotinine Enzyme Immunoassay Kit (Salimetrics). The participants' alcohol consumption profile was assessed using the Alcohol Use Disorders Identification Test (AUDIT) and questions related to alcohol consumption. The e-cig consumption profile was traced through questions related to device usage and testing for carbon monoxide concentration in expired air. Proteomic analysis was performed using shotgun methodology and liquid chromatography coupled with mass spectrometry (LC-MS). Since the obtained results did not follow a normal distribution, they were subjected to the Mann-Whitney test. A higher concentration of exhaled CO was observed, along with a higher salivary concentration of cotinine, higher scores on the AUDIT, lower salivary viscosity, and an unfavorable result on oximetry. Regarding the salivary proteome, 48 proteins were specific to GCE, with 3 of these proteins being differentially expressed in this group. It is concluded that electronic cigarette users have higher rates of alcohol consumption and lower salivary viscosity, important alterations in respiratory capacity demonstrated by increased CO concentration in expired air and decreased oximetry, and high levels cotinine present in saliva. Furthermore, the higher abundance of proteins such as Glutathione S-transferase P, Immunoglobulin J chain, and Acetyl-CoA dehydrogenase, suggest an organism response to increased oxidative stress, presence of inflammatory components, and salivary microbiological alterations that may favor the development of future pathological conditions (AU)

Este estudo objetivou estimar a prevalência do uso do cigarro eletrônico e a associação com fatores preditores. Trata-se de um estudo transversal, analítico, com amostra probabilística de universitários matriculados em um Centro Universitário de Montes Claros, Minas Gerais, Brasil no segundo semestre de 2022. A variável dependente referiu-se ao uso do cigarro eletrônico. As variáveis independentes referiram a fatores sociodemográficas, laborais e comportamentais. A associação entre as variáveis investigadas e a prevalência do uso do CE foi verificada pela análise bivariada e a regressão de Poisson. Foram entrevistados 730 universitários, com a média de idade de anos 22,56 (±6,25). Destes, 21,8 % faziam o uso cigarro eletrônico e após a análise multivariada manteve-se associado ao desfecho não ter companheiro (RP= 3,31; IC95% 1,04-10,48), morar com amigos/sozinho (RP=1,53; IC95% 1,07-2,18), ter histórico de usuários de cigarro eletrônico na residência (RP= 1,76; IC95%: 1,17- 1,89), consumir bebida alcoólica (RP= 3,07; IC95%: 1,72-5,49) e não praticar atividade física (RP= 3,37; IC95% 2,35-4,83). Conhecer sobre o cigarro eletrônico foi fator protetor (RP= 0,31 IC95% 0,20-0,46). Registrou-se elevada prevalência do uso do cigarro eletrônico, e manteve associados a fatores sociodemográficos e comportamentais. Esses achados chamam a atenção para a necessidade de novas medidas regulatórias, a fim de reduzir o uso desse dispositivo.
This study aimed to estimate the prevalence of e-cigarette use and the association with predictors. This is a cross-sectional, analytical study with a probabilistic sample of university students enrolled in a University Center of Montes Claros, Minas Gerais, Brazil in the second half of 2022. The dependent variable referred to the use of electronic cigarettes. The independent variables referred to sociodemographic, labor and behavioral factors. The association between the variables investigated and the prevalence of EC use was verified by bivariate analysis and Poisson regression. A total of 730 university students were interviewed, with a mean age of 22.56 (±6.25). Of these, 21.8% used electronic cigarettes and after multivariate analysis, it remained associated with the outcome of not having a partner (PR= 3.31; CI95% 1.04-10.48), living with friends/alone (PR=1.53; CI95% 1.07-2.18), having a history of e-cigarette users in the residence (PR= 1.76; CI95%: 1.17- 1.89), alcohol consumption (PR= 3.07; CI95%: 1.72-5.49) and not practicing physical activity (PR= 3.37; IC95% 2.35-4.83). Knowing about electronic cigarettes was a protective factor (PR= 0.31 CI95% 0.20-0.46). There was a high prevalence of e-cigarette use, and it was associated with sociodemographic and behavioral factors. These findings draw attention to the need for new regulatory measures in order to reduce the use of this device.

Se trata de un estudio analítico transversal con una muestra probabilística de estudiantes universitarios matriculados en un Centro Universitario de Montes Claros, Minas Gerais, Brasil, en el segundo semestre de 2022. La variable dependiente se refería al uso de cigarrillos electrónicos. Las variables independientes se refirieron a factores sociodemográficos, laborales y conductuales. Objetivo: Determinar el riesgo de padecer enfermedades respiratorias por el uso del cigarrillo electrónico. Materiales y métodos: Revisión integrativa, basada en el análisis de artículos científicos completos en bases de datos como: PubMed, Scopus y SciELO utilizando el método PRISMA, en el periodo 2018-2023, en los idiomas inglés y español. Resultados: Se obtuvo 95 artículos científicos, de los cuales se excluyeron un total de 75 por no cumplir con los criterios de selección, quedando seleccionados 20 artículos científicos que responden a los objetivos planteados. Conclusiones: Este estudio identificó los riesgos de padecer enfermedades respiratorias por el uso del cigarrillo electrónico, siendo las más relevantes: enfermedad pulmonar obstructiva crónica, neumonía, lesión pulmonar asociada al cigarrillo electrónico o al vapeo, síndrome de distrés respiratorio, hipertensión pulmonar y asma, además de acompañarse de signos y síntomas como infección de las vías aéreas, el desarrollo de enfermedades cardiovasculares, gastrointestinales y el inicio de adicciones. El estudio identificó que el sexo masculino es el predominante en exponerse más al riesgo de enfermedades respiratorias y la edad oscila entre los 14 a 35 años, convirtiéndose en un problema de salud pública que cada año va en aumento.
Objective: To determine the risk of respiratory diseases due to the use of electronic cigarettes. Materials and Methods: Integrative review, based on the analysis of complete scientific articles in databases such as: PubMed, Scopus and SciELO, using the PRISMA method, in the period 2018-2023, in English and Spanish. Results: Of the 95 scientific articles that were obtained, 20 met the selection criteria and were selected to meet the stated objectives. The remaining 75 articles were excluded. Conclusions: This study identi fied the risks of suffering from respiratory diseases associated with the use of electronic cigarettes. The most significant risks include chronic obstructive pulmonary disease, pneumonia, lung injury associated with electronic cigarettes or vaping, respiratory distress syndrome, pulmonary hypertension, and asthma. Symptoms may also include signs and symptoms of respiratory infections, development of cardiovascular and gastrointestinal diseases, and the onset of addiction. The study found that males are more susceptible to respiratory diseases, with the 14-35 age group being particularly affected. This is becoming a growing public health concern.
Objetivo: Determinar o risco de doenças respiratórias devido ao uso de cigarros eletrônicos. Materiais e métodos: Revisão integrativa, baseada na análise de artigos científicos completos em bases de dados como: PubMed, Scopus e SciELO, utilizando o método PRISMA, no período de 2018 a 2023, em inglês e espanhol. Resultados: Foram obtidos 95 artigos científicos, dos quais um total de 75 foram excluídos por não atenderem aos critérios de seleção, restando 20 artigos científicos selecionados que respondem aos objetivos estabelecidos. Conclusões: Este estudo identificou os riscos de doenças respiratórias decorrentes do uso de cigarros eletrônicos, sendo as mais relevantes a doença pulmonar obstrutiva crônica, pneumonia, lesão pulmonar associada ao cigarro eletrônico ou vaping, síndrome do desconforto respiratório, hipertensão pulmonar e asma, além de serem acompanhadas de sinais e sintomas como infecção das vias aéreas, desenvolvimento de doença cardiovascular, doença gastrointestinal e início de dependência. O estudo identificou que o sexo masculino está predominantemente exposto ao risco de doenças respiratórias e a idade varia de 14 a 35 anos, tornando-se um problema de saúde pública que aumenta a cada ano.
RESUMO
ABSTRACT Objective. To document tobacco industry strategies to influence regulation of new and emerging tobacco and nicotine products (NETNPs) in Latin America and the Caribbean. Methods. We analyzed industry websites, advocacy reports, news media and government documents related to NETNPs, focusing on electronic cigarettes and heated tobacco products. We also conducted a survey of leading health advocates. We applied the policy dystopia model to analyze industry action and argument-based strategies on NETNP regulations. Results. Industry actors engaged in four instrumental strategies to influence NETNP regulation - coalition management, information management, direct involvement in and access to the policy process, and litigation. Their actions included: lobbying key policy-makers, academics and vaping associations; providing grants to media groups to disseminate favorable NETNP information; participating in public consultations; presenting at public hearings; inserting industry-inspired language into draft NETNP legislation; and filing lawsuits to challenge NETNP bans. The industry disseminated its so-called harm reduction argument through large/influential countries (e.g., Argentina, Brazil, and Mexico). Industry discursive strategies claimed NETNPs were less harmful, provided safer alternatives, and should be regulated as so-called harm reduction products or have fewer restrictions on their sale and use than those currently in place. Conclusion. Our analysis provides a better understanding of industry strategies to undermine tobacco and nicotine control. To help counter industry efforts, health advocates should proactively strengthen government capacities and alert policy-makers to industry attempts to create new regulatory categories (so-called reduced-risk products), provide misleading information of government authorizations of NETNPs, and co-opt so-called harm-reduction messages that serve the industry's agenda.
RESUMEN Objetivo. Documentar las estrategias de la industria tabacalera para influir en la regulación de los productos de tabaco y nicotina novedosos y emergentes (PTNNE) en América Latina y el Caribe. Métodos. Se analizaron los sitios web de la industria, los informes de defensa de la salud, los medios de información y los documentos gubernamentales relacionados con los PTNNE, prestando especial atención a los cigarrillos electrónicos y los productos de tabaco calentado. También se realizó una encuesta a líderes de la promoción de la salud. Aplicamos el modelo de distopía política para analizar las estrategias de acción y argumentación de la industria en relación con la regulación de los PTNNE. Resultados. Las partes interesadas de la industria recurrieron a cuatro estrategias instrumentales para influir en la regulación de los PTNNE: gestión de coaliciones, gestión de la información, participación directa y acceso al proceso de formulación de políticas, y litigios. Sus acciones incluyeron: trabajar con los principales responsables de la formulación de políticas, académicos y asociaciones de vapeo; conceder subvenciones a grupos de medios de comunicación para que difundan información favorable a los PTNNE; participar en consultas públicas; realizar presentaciones en audiencias públicas; introducir un lenguaje inspirado por la industria en la legislación sobre los PTNNE; y presentar demandas judiciales para dificultar las prohibiciones de los PTNNE. La industria difundió su argumentación, denominada de reducción de daños, en países grandes e influyentes como, por ejemplo, Argentina, Brasil y México. Las estrategias discursivas de la industria afirmaban que los PTNNE eran menos nocivos, proporcionaban alternativas más seguras y debían regularse del mismo modo que los denominados productos de reducción de daños o tener menos restricciones que las vigentes en la actualidad para su venta y consumo. Conclusión. Este análisis permite comprender mejor las estrategias de la industria para socavar el control del tabaco y la nicotina. Para contribuir a contrarrestar los esfuerzos de la industria, los defensores de la salud deberían fortalecer activamente las capacidades gubernamentales y alertar a los responsables políticos de los intentos de la industria de crear nuevas categorías reglamentarias (los denominados productos de riesgo reducido), proporcionar información engañosa sobre las autorizaciones gubernamentales de los PTNNE y apropiarse de los denominados mensajes de reducción de daños que responden a la agenda de la industria.
RESUMO Objetivo. Documentar as estratégias usadas pela indústria do tabaco para influenciar a regulamentação de produtos novos e emergentes de tabaco e nicotina (NETNPs, sigla em inglês) na América Latina e no Caribe. Métodos. Foram analisados sites do setor, relatórios de ativistas, notícias em meios de comunicação e documentos governamentais relacionados aos NETNPs, com foco em cigarros eletrônicos e produtos de tabaco aquecido. Também foi realizada uma pesquisa com ativistas importantes na área da saúde. O modelo de distopia política foi utilizado para analisar as ações e as estratégias baseadas em argumentos do setor para a regulamentação de NETNPs. Resultados. Os agentes do setor utilizaram quatro estratégias instrumentais para influenciar a regulamentação de NETNPs: gestão de coalizões; gestão de informações; envolvimento direto e acesso ao processo de formulação de políticas; e ações legais. As ações dos agentes incluíram: trabalhar com os principais formuladores de políticas, acadêmicos e associações de cigarros eletrônicos; conceder subsídios a grupos de comunicação para disseminar informações favoráveis aos NETNPs; participar de consultas públicas; fazer apresentações em audiências públicas; inserir linguagem gerada pela indústria na legislação de NETNPs; e entrar com ações judiciais para contestar proibições de NETNPs. O setor disseminou seu argumento de "redução de danos" em países grandes e influentes (por exemplo, Argentina, Brasil e México). As estratégias discursivas do setor afirmavam que os NETNPs eram menos prejudiciais, ofereciam alternativas mais seguras e deveriam ser regulamentados como "produtos de redução de danos" ou ter menos restrições à venda e ao uso do que as atualmente em vigor. Conclusão. Nossa análise oferece uma melhor compreensão das estratégias usadas pelo setor para enfraquecer o controle do tabaco e da nicotina. Para ajudar a combater os esforços do setor, os ativistas em saúde devem fortalecer de forma proativa a capacidade dos governos e alertar os formuladores de políticas sobre as tentativas da indústria de criar novas categorias regulatórias (os chamados "produtos de risco reduzido"), fornecer informações enganosas sobre as autorizações governamentais de comercialização dos NETNPs e cooptar mensagens de "redução de danos" para atender aos interesses do setor.
RESUMO
INTRODUÇÃO: O tabagismo é considerado uma doença crônica de dependência da nicotina e um fator de risco para doenças graves como câncer e doenças cardiovasculares, além de ser uma das principais causas de mortes evitáveis no mundo. Os programas de controle do uso do tabaco atingiram um bom resultado nas últimas décadas, com a redução do número de fumantes. Contudo, verifica-se que o tratamento medicamentoso e comportamental não apresenta 100% de eficácia, com recaídas e efeitos colaterais para o fumante. Novos desafios foram impostos após a pandemia da COVID-19, principalmente no setor da saúde, no qual a preocupação com os impactos na saúde dos trabalhadores e das trabalhadoras ficou em evidência. Nesse cenário, foi verificada a importância de cuidar dos que mais se expuseram aos efeitos nocivos desse período, voltando o olhar não somente para transtornos a nível psíquico dessa parcela populacional, mas também para a promoção da qualidade de vida do trabalhador, principalmente do trabalhador que apresenta um comportamento de risco em saúde, como por exemplo, o trabalhador tabagista. A aromaterapia, uma Prática Integrativa e Complementar em Saúde apoiada em evidências de efetividade clínica nas mais diversas condições clínicas, foi proposta como intervenção neste grupo. OBJETIVO: Avaliar a efetividade da aromaterapia na dependência de nicotina em trabalhadores tabagistas. MÉTODO: Estudo piloto, clínico, randomizado, duplo-cego que utilizou dois óleos essenciais, piper nigrum e angelica archangelica, por meio de inalação, em dois grupos de trabalhadores da saúde, para avaliar o efeito desses óleos nos níveis de dependência do tabagismo. Os desfechos avaliados foram o nível de dependência de nicotina (desfecho primário), mensurado por meio do Teste de Fagerstrom, e a medida da carga tabágica (desfecho secundário) do participante. A pesquisa foi desenvolvida em um período de 30 dias nas dependências de um hospital universitário da cidade de Belo Horizonte/MG com uma população de 29 trabalhadores tabagistas. A análise dos dados foi realizada por meio do programa SPSS versão 21.0 e as mudanças nos níveis de dependência da nicotina e de carga tabágica no estudo foram avaliadas utilizando o teste de Wilcoxon emparelhado. As associações entre as variáveis categorizadas foram feitas por meio do teste qui-quadrado de Pearson. A efetividade dos óleos essenciais entre os grupos foi realizada pelo Teste de Mann-Whitney. Para todos os testes, foi fixado o nível de significância de 5%. RESULTADOS: Os grupos aleatorizados foram semelhantes entre si na maioria das variáveis analisadas (p>0,05). Houve diminuição tanto da carga tabágica quanto no teste de dependência à nicotina após a intervenção com os óleos essenciais em ambos os grupos (p<0,05), com o grupo Angélica iniciando a pesquisa com um grau moderado de dependência no Teste de Fagerstrom e carga tabágica moderada, e ao final demonstrando uma baixa dependência e carga tabágica baixa. Da mesma maneira, o grupo Pimenta-preta exibiu uma baixa dependência de nicotina e carga tabágica baixa no teste inicial e após a intervenção, uma baixíssima dependência. Dois participantes do sexo masculino, cada um de um grupo avaliado, alcançou a abstinência. Não houve diferença significativa (p>0,05) no tamanho do efeito em relação ao tipo de óleo essencial utilizado. CONCLUSÃO: Os óleos essenciais utilizados na pesquisa foram efetivos na redução da dependência da nicotina e na diminuição da carga tabágica dos sujeitos avaliados. Não houve diferença significativa entre o efeito dos OE do grupo controle em relação ao grupo experimental.
INTRODUCTION: Smoking is considered a chronic nicotine addiction disease and a risk factor for serious illnesses such as cancer and cardiovascular disease, as well as being one of the main causes of preventable deaths worldwide. Tobacco control programs have achieved good results in recent decades, with a reduction in the number of smokers. However, drug and behavioral treatment is not 100% effective, with relapses and side effects for smokers. New challenges have been imposed following the COVID-19 pandemic, especially in the health sector, where concerns about the impact on workers' health have been highlighted. In this scenario, the importance of caring for those who have been most exposed to the harmful effects of this period has been verified, looking not only at the psychological disorders of this population, but also at promoting the quality of life of the worker, especially the worker who presents a health risk behavior, such as the smoking worker. Aromatherapy, an Integrative and Complementary Health Practice supported by evidence of clinical effectiveness in a wide range of clinical conditions, was proposed as an intervention for this group. OBJECTIVE: To evaluate the effectiveness of aromatherapy on nicotine dependence in workers who smoke. METHOD: A pilot, clinical, randomized, double-blind study using two essential oils, piper nigrum and angelica archangelica, by inhalation, in two groups of health workers, to evaluate the effect of these oils on levels of smoking dependence. The outcomes assessed were the level of nicotine dependence (primary outcome), measured using the Fagerstrom Test, and the measure of the participant's smoking load (secondary outcome). The study was carried out over a period of 30 days on the premises of a university hospital in the city of Belo Horizonte/MG with a population of 29 workers who were smokers. Data analysis was carried out using the SPSS program version 21.0 and changes in levels of nicotine dependence and smoking load in the study were assessed using the paired Wilcoxon test. Associations between the categorized variables were made using Pearson's chi-square test. The effectiveness of the essential oils between the groups was assessed using the Mann-Whitney test. A significance level of 5% was set for all tests. RESULTS: The randomized groups were similar in most of the variables analyzed (p>0.05). There was a decrease in both smoking load and nicotine dependence after the intervention with essential oils in both groups (p<0.05), with the Angelica group starting the study with a moderate degree of dependence in the Fagerstrom test and moderate smoking load, and at the end showing low dependence and low smoking load. Similarly, the Black Pepper group exhibited a low nicotine dependence and low smoking load in the initial test and after the intervention, a very low dependence. Two male participants, each from an evaluated group, achieved abstinence. There was no significant difference (p>0.05) in effect size in relation to the type of essential oil used. CONCLUSION: The essential oils used in the study were effective in reducing nicotine dependence and the smoking burden of the subjects evaluated. There was no significant difference between the effect of EOs in the control group and the experimental group.
