OBJECTIVE: To explore the association between habitual tea consumption and 10-year cardiovascular disease risk among middle-aged and elderly Chinese. METHODS: China Nutrition and Health Surveillance 2015 applied a stratified, multistage, and random sampling method. The current study selected middle-aged and elderly participants aged 45 and older, and they were investigated by basic information survey, dietary survey based on the food frequency questionnaire, physical measurements including height, weight, and blood pressure, and a laboratory examination with fasting blood samples. Habitual tea consumption was defined by asking about the number of cups of tea usually consumed per day in the past 12 months, and the risk of cardiovascular disease incidence was calculated using the Framingham risk score over a 10-year period. The association between tea consumption and 10-year risk of cardiovascular disease in this population was analyzed using multiple logistic regression, and further analyzed using gender as a stratification factor. RESULTS: A total of 42 704 Chinese middle-aged and elderly were included in this study, with an average age of(59.4±9.4) years, 20 104 males and 22 600 females, 17 194 in urban areas and 25 510 in rural areas. For tea drinking, there were 12 519(29.32%) tea drinkers in the included sample, of which 4153(9.73%) consumed 1-2 cups/d, 3336(7.81%) 3-4 cups/d, and 5030(11.78%) ≥5 cups/d. For 10-year cardiovascular risk, 28 267 participants(66.19%) were estimated to be low risk, while 14 437(33.81%) were in the high risk. After adjusting for age, sex, body mass index, place of residence, education, income, marital status, smoking, excessive alcohol consumption, physical activity, sedentary behavior, sleep, participation in medical examination within one year, DASH dietary score, energy intake and chronic disease status, the result showed a reduced risk of cardiovascular disease over a 10-year period among those who consumed 3-4 cups/d compared with those who did not consume tea(OR=0.820, 95%CI 0.719-0.934, P_(trend)=0.03). When stratified by gender, both gender showed participants who consumed 3-4 cups/d had a lower risk for cardiovascular disease than those who did not consume tea(males: OR=0.849, 95%CI 0.722-0.997; females: OR=0.697, 95%CI 0.527-0.922). And the result was more pronounced among females. CONCLUSION: Moderate habitual tea consumption could reduce the risk of cardiovascular disease among middle-aged and elderly Chinese people, and its protective effect is more pronounced among females.

Many countries use national nutrition surveys (NNSs) to assess their population's health and nutrition needs. However, NNS aims, approaches, tools, and measurements vary among countries. To date, there has been no review evaluating the NNSs and their practices worldwide to help conduct future NSSs. Therefore, this narrative review was conducted to 1) explore and tabulate current NNSs in five continents (Asia, Europe, Africa, North America, and Australia) and 2) help lay the foundation for establishing clear guidelines for future NNSs. The NNSs were identified using two approaches. First, an electronic database search was conducted with key terms in PubMed database. Second, a general web-based search on the survey webpages of governmental organizations was conducted using the same key terms to identify eligible surveys. The review included general adult population (≥ 18 years) with a cross-sectional design, excluding NNSs related to household-only surveys, specific age groups, or insufficient sample sizes. A total of 41 NNSs were identified in 37 countries across four continents: Asia (n = 15), Europe (n = 21), North America (n = 3), and Australia (n = 2). Broad differences between the surveys were identified, including survey purposes and designs, definitions of geographic areas and target groups, and dietary assessments. Currently, there are 26 ongoing NNSs, while 15 have ended. Among the ongoing NNSs, the cycles of the surveys were either at regular intervals (n = 8) or irregular intervals (n = 8). Of the 41 surveys, 24-h dietary recalls were used in 27 surveys, while only 6 surveys used diet diaries and 8 surveys relied on FFQs. Some surveys (n = 17) utilized multiple tools to assess dietary intake. Most of the surveys that assessed biochemical status (n = 12) focused on blood glucose, haemoglobin A1c (HbA1c), and lipid status, whereas some surveys (n = 6) tested for vitamin and mineral status in blood and/or urine samples. The review identified key characteristics, time frames, sampling methods, and dietary and physical assessment methods obtained from different surveys worldwide. The information organized in this review will be important for researchers, policymakers, and public health programme developers in developing and improving NNS.

BACKGROUND: The literature contains scarce data on inequalities in growth trajectories among children born to mothers of diverse ethnoracial background in the first 5 years of life. OBJECTIVE: We aimed to investigate child growth according to maternal ethnoracial group using a nationwide Brazilian database. METHODS: A population-based retrospective cohort study employed linked data from the CIDACS Birth Cohort and the Brazilian Food and Nutrition Surveillance System (SISVAN). Children born at term, aged 5 years or younger who presented two or more measurements of length/height (cm) and weight (kg) were followed up between 2008 and 2017. Prevalence of stunting, underweight, wasting, and thinness were estimated. Nonlinear mixed effect models were used to estimate childhood growth trajectories, among different maternal ethnoracial groups (White, Asian descent, Black, Pardo, and Indigenous), using the raw measures of weight (kg) and height (cm) and the length/height-for-age (L/HAZ) and weight-for-age z-scores (WAZ). The analyses were also adjusted for mother's age, educational level, and marital status. RESULTS: A total of 4,090,271 children were included in the study. Children of Indigenous mothers exhibited higher rates of stunting (26.74%) and underweight (5.90%). Wasting and thinness were more prevalent among children of Pardo, Asian, Black, and Indigenous mothers than those of White mothers. Regarding children's weight (kg) and length/height (cm), those of Indigenous, Pardo, Black, and Asian descent mothers were on average shorter and weighted less than White ones. Regarding WAZ and L/HAZ growth trajectories, a sharp decline in average z-scores was evidenced in the first weeks of life, followed by a period of recovery. Over time, z-scores for most of the subgroups analyzed trended below zero. Children of mother in greater social vulnerability showed less favorable growth. CONCLUSION: We observed racial disparities in nutritional status and childhood growth trajectories, with children of Indigenous mothers presenting less favorable outcomes compared to their White counterparts. The strengthening of policies aimed at protecting Indigenous children should be urgently undertaken to address systematic ethnoracial health inequalities.

BACKGROUND: The Global Diet Quality Score (GDQS) was developed for monitoring nutrient adequacy and diet-related noncommunicable disease risk in diverse populations. A software application (GDQS app) was recently developed for the standardized collection of GDQS data. The application involves a simplified 24-h dietary recall (24HR) where foods are matched to GDQS-food groups using an onboard database, portion sizes are estimated at the food group level using cubic models, and the GDQS is computed. OBJECTIVES: The study aimed to estimate associations between GDQS scores collected using the GDQS app and nutrient adequacy and metabolic risks. METHODS: In this cross-sectional study of 600 Thai males and nonpregnant/nonlactating females (40-60 y), we collected 2 d of GDQS app and paper-based 24HR, food-frequency questionnaires (FFQs), anthropometry, body composition, blood pressure, and biomarkers. Associations between application scores and outcomes were estimated using multiple regression, and application performance was compared with that of metrics scored using 24HR and FFQ data: GDQS, Minimum Dietary Diversity-Women, Alternative Healthy Eating Index-2010, and Global Dietary Recommendations score. RESULTS: In covariate-adjusted models, application scores were significantly (P < 0.05) associated with higher energy-adjusted mean micronutrient adequacy computed using 24HR (range in estimated mean adequacy between score quintiles 1 and 5: 36.3%-44.5%) and FFQ (Q1-Q5: 40.6%-44.2%), and probability of protein adequacy from 24HR (Q1-Q5: 63%-72.5%). Application scores were inversely associated with BMI kg/m2 (Q1-Q5: 26.3-24.9), body fat percentage (Q1-Q5: 31.7%-29.1%), diastolic blood pressure (Q1-Q5: 84-81 mm Hg), and a locally-developed sodium intake score (Q1-Q5: 27.5-24.0 points out of 100); positively associated with high-density lipoprotein cholesterol (Q1-Q5: 49-53 mg/dL) and 24-h urinary potassium (Q1-Q5: 1385-1646 mg); and inversely associated with high midupper arm circumference (Q5/Q1 odds ratio: 0.52) and abdominal obesity (Q5/Q1 odds ratio: 0.51). Significant associations for the application outnumbered those for metrics computed using 24HR or FFQ. CONCLUSIONS: The GDQS app effectively assesses nutrient adequacy and metabolic risk in population surveys.

Introduction. Brazil's health system offers insights into addressing the double burden of malnutrition by proper population monitoring, coupled with local policies and national guidelines. Objective. To investigate the recent temporal trends in nutritional status indicators and its coverage of children aged two to five years from Campinas, a metropolis in the state of São Paulo, Brazil. Material and methods. The analysis of time series from 2018 to 2022 were conducted by accessing data from the Brazilian Food and Nutrition Surveillance System (SISVAN) due to regression analysis. Results. The number of children aged two to five in the SISVAN registry rose from 7,300 in 2018 to 11,171 in 2022, forming the study sample. In 2018, 700 were chronically undernourished, 306 underweight, and 977 overweight; by 2022, 530 showed stunting, 457 were underweight, and 1,084 overweight. Stunting prevalence declined from 9.6% (2018) to 4.7% (2022). Underweight dropped slightly from 4.2% to 4.1% over the years and overweight, consistently the highest indicator, ranged from 13.4% (2018) to 9.7% (2022). SISVAN's coverage varied between 16.6% (2018) and 26.1% (2022), the lowest at 12.3% in 2020. The trend for stunting decreased significantly (APV: -15.01; CI95% -22.64; -6.62). Adding SISVAN's coverage variable the model slightly reduced the declining stunting trend (APV: -12.12; CI95% -13.19; -11.04). Conclusions. Coordinated efforts to address nutritional challenges, from adequate population monitoring to the interaction between local policies and national guidelines, have shown positive health outcomes(AU)

El sistema de salud de Brasil ofrece conocimientos valiosos para abordar la doble carga de malnutrición mediante el seguimiento adecuado de la población y políticas locales y nacionales. Objetivo. Investigar las tendencias temporales recientes en indicadores nutricionales y cobertura en niños de dos a cinco años en Campinas, São Paulo, Brasil. Materiales y métodos. Se analizaron datos del SISVAN de 2018 a 2022 con regresión temporal. Resultados. Niños de dos a cinco años en el SISVAN pasaron de 7,300 en 2018 a 11,171 en 2022. En 2018, 700 estaban crónicamente desnutridos, 306 con bajo peso y 977 con sobrepeso; en 2022, 530 con retraso en el crecimiento, 457 con bajo peso y 1,084 con sobrepeso. La prevalencia del retraso en el crecimiento bajó de 9,6% a 4,7%. El bajo peso disminuyó levemente del 4,2% al 4,1% y el sobrepeso osciló del 13,4% al 9,7%. La cobertura de SISVAN varió de 16.6% a 26.1%, con un mínimo de 12.3% en 2020. La tendencia del retraso en el crecimiento disminuyó significativamente (APV: -15.01; CI95% -22.64; -6.62). La inclusión de la cobertura de SISVAN redujo levemente esta tendencia (APV: -12.12; CI95% -13.19; -11.04). Conclusiones. Coordinar esfuerzos para abordar los desafíos nutricionales, desde el seguimiento de la población hasta políticas locales y nacionales, ha tenido resultados positivos para la salud(AU)

The current study is to explore the association of the Chinese visceral adiposity index (CVAI) with hypertension, and to compare the predictive power of different adiposity indexes regarding hypertension among Chinese adults aged over 45 years. A total of 99,201 participants aged over 45 years from the China Nutrition and Health Surveillance 2015-2017 were included in this study. Multivariate adjusted logistic regression was used to calculate the odds ratio (OR) and 95% confidence interval (CI) of hypertension. Multivariate adjusted restricted cubic spline analyses were applied to explore the association of adiposity indexes with hypertension. Receiver operating characteristic (ROC) analyses were used to compare the predictive powers of different adiposity indexes of hypertension. All eight adiposity indexes included in this study were positively associated with hypertension. Compared with those in the lowest quartile of the CVAI, the participants in the highest quartile showed a significantly higher risk of hypertension (OR = 3.70, 95% CI = 3.54-3.86) after multiple adjustments. The ROC analyses suggested that the CVAI was the strongest predictor of hypertension compared to other adiposity indexes in both genders. The findings supported that the CVAI could serve as a reliable and cost-effective method for early identifying hypertension risk.

BACKGROUND: High intake of ultra-processed foods (UPFs) is associated with increased risk of chronic disease; thus, it is important to understand how UPFs influence diet quality early in life. OBJECTIVES: We describe complementary foods and beverages (CFBs) according to the Nova Classification System of Food Processing for infants and toddlers in the United States and estimate how Nova groups and subgroups contribute to energy and select nutrients and food groups. METHODS: We used day 1 24-h recall from infants and toddlers aged 6-23 mo from the cross-sectional, nationally representative 2013-18 National Health and Nutrition Examination Survey (n = 1140). We estimated contributions of Nova groups and subgroups to energy and select nutrients and food groups consumed as CFBs (excluding human milk and formula) using the population ratio with weighted survey commands in SAS. RESULTS: For infants and toddlers in the United States, 42 ± 0.9% (mean ± standard error of the mean) of energy intake from CFBs came from unprocessed/minimally processed foods (U/MPFs) and 45 ± 0.8% from UPFs. U/MPFs contributed most to nutrient intakes (except iron, zinc, and sodium); ≥20% of all selected nutrients was from UPFs. UPFs contributed most to iron (75 ± 1.0%) and zinc (48 ± 1.3%); breakfast cereals were the top source. Most fruit, vegetables, and dairy were from U/MPFs. More than 80% of total grains, whole grains, refined grains, and added sugars were UPFs. CONCLUSIONS: U/MPFs support healthy dietary intake of infants and toddlers in the United States, whereas UPFs contribute meaningfully to nutrients and food groups to be encouraged (iron, zinc, and whole grains), as well as some that should be limited (added sugars and sodium). More research is needed to better understand the utility and sensitivities of using Nova for providing dietary guidance for infants and toddlers in the United States.

BACKGROUND: Certain foods and food groups could positively or negatively impact serum uric acid (SUA) levels. However, evidence on the holistic dietary strategy to prevent and control hyperuricemia (HUA) development remains limited. OBJECTIVE: The aim of this research work was to explore the association of dietary approaches to stop hypertension (DASH) diet with SUA levels and odds of HUA among Chinese adults. METHODS: This research premise included 66,427 Chinese adults aged 18 and above who were part of the China Adult Chronic Disease and Nutrition Surveillance in 2015. Dietary consumptions were assessed via the household condiment weighing approach and a three-day, 24-hour dietary recall. Total fat, saturated fat, calcium, protein, potassium, cholesterol, magnesium, fiber, and sodium were all adopted to calculate the DASH score (score range, 0-9). The associations of DASH score with SUA levels and odds of HUA were evaluated using multiple linear and logistic regression models, respectively. RESULTS: We established that a higher DASH score was linked with a lower SUA levels (ß = - 0.11; 95% CI: - 0.12, - 0.1; p < 0.001) and odds of HUA (OR = 0.85; 95% CI: 0.83, 0.87; p < 0.001) after adjustment for age, sex, ethnicity, education status, marital status, health behaviours and health factors. The association of the DASH diet with odds of HUA was stronger among men (p-interaction = 0.009), non-Han Chinese (p-interaction< 0.001) as well as rural residents (p-interaction< 0.001). CONCLUSIONS: Our results illustrate that the DASH diet was remarkably negatively with SUA levels and odds of HUA in the Chinese adult population.

Resumo O objetivo deste estudo foi estimar a cobertura populacional e a taxa de incremento anual (TIA) média, em âmbito nacional, do registro de marcadores do consumo alimentar do Sistema de Vigilância Alimentar e Nutricional (Sisvan), entre 2015 e 2019, considerando a entrada dos dados via Estratégia e-SUS APS e Sisvan Web. Trata-se de estudo ecológico de séries temporais, com estratificação por macrorregiões e faixas etárias. A TIA da cobertura foi calculada utilizando regressão de Prais-Winsten e a correlação com IDH, PIB per capita e cobertura da APS foi avaliada por teste de Spearman. A cobertura populacional do registro de marcadores de consumo alimentar no país foi de 0,92% em 2019, com TIA significante de 45,63% desde 2015. As maiores coberturas foram observadas na região Nordeste (4,08%; TIA=45,76%, p<0,01) e em crianças entre 2 e 4 anos (3,03%; TIA=34,62%, p<0,01). Houve tendência crescente para a entrada dos dados via e-SUS APS, em detrimento do Sisvan Web. A TIA da cobertura dos registros via e-SUS APS exibiu correlação positiva com IDH e PIB per capita em alguns recortes etários. A cobertura populacional do registro de marcadores de consumo alimentar do Sisvan ainda se mostra incipiente no país e o e-SUS APS parece ser uma estratégia relevante para sua expansão.


Abstract The aim of the present study was to estimate the population coverage of recording food intake markers in Brazil's Food and Nutrition Surveillance System (Sisvan) and mean annual percent change (APC) in coverage according to the system used for data entry (e-SUS APS and Sisvan Web). We conducted an ecological time series study of the period 2015-2019. The data were stratified into region and age group. APC in coverage was calculated using Prais-Winsten regression and the correlation between APC and HDI, GDP per capita and primary healthcare coverage was assessed using Spearman's correlation coefficient. Population coverage of recording food intake markers at national level was 0.92% in 2019. Mean APC in coverage throughout the period was 45.63%. The region and age group with the highest coverage rate were the Northeast (4.08%; APC=45.76%, p<0.01) and children aged 2-4 years (3.03%; APC=34.62%, p<0.01), respectively. There was an upward trend in data entry using e-SUS APS, to the detriment of Sisvan Web. There was a positive correlation between APC in coverage using e-SUS APS and HDI and GDP per capita in some age groups. Population coverage of recording Sisvan food intake markers remains low across the country. The e-SUS APS has the potential to be an important strategy for expanding food and nutrition surveillance.

Comprehensive metrics that provide a measure of dietary patterns at global and national levels are needed to inform and assess the effectiveness of policy actions that promote sustainable healthy diets. In 2019, the Food and Agriculture Organization of the United Nations and the World Health Organization reported 16 guiding principles of sustainable healthy diets, but it is still unknown how these principles are considered in dietary metrics. This scoping review aimed to explore how principles of sustainable healthy diets are considered in dietary metrics used worldwide. Forty-eight food-based, investigator-defined dietary pattern metrics assessing diet quality in free-living, healthy populations at the individual or household level were assessed against the 16 guiding principles of sustainable healthy diets, which was used as a theoretical framework. A strong adherence of the metrics to health-related guiding principles was found. Metrics had a weak adherence to principles related to environmental and sociocultural aspects of diets, except for the principle related to diets being culturally appropriate. No existing dietary metric captures all principles of sustainable healthy diets. Notably, the significance food processing, environmental, and sociocultural aspects of diets are generally understated. This likely reflects the lack of focus on these aspects in current dietary guidelines, which highlights the importance of including these emerging topics in future dietary recommendations. The absence of quantitative metrics that comprehensively measure sustainable healthy diets limits the body of evidence that would otherwise inform national and international guideline developments. Our findings can help grow the quantity and quality of the body of evidence available to inform policy activities to realize 2030 Sustainable Development Goals of multiple United Nations. Objective: to analyze the temporal trend of Food and Nutrition Surveillance System (Sistema de Vigilância Alimentar e Nutricional - SISVAN) coverage and the nutritional status of older adults, and its correlation with indicators of social inequality in Brazil between 2008-2019. Methods: this was an ecological study using records from SISVAN, related to the population aged 60 years and older; the temporal trend of coverage and the correlation between indicators of social inequality and increment rate of nutritional status were analyzed; slope index of inequality and concentration index were used to measure absolute and relative inequalities. Results: 11,587,933 records were identified; national coverage increased from 0.1% (2008) to 2.9% (2019), with a statistically significant upward trend; a moderate inverse correlation with an annual increment rate of overweight between human development index and gross domestic product per capita, was found. Conclusion: there was an increasing trend in SISVAN coverage; the increase in overweight was associated with social inequality.


Objetivo: analizar la tendencia temporal de cobertura del Sistema de Vigilancia Alimentaria y Nutricional (SISVAN), y el estado nutricional de adultos mayores, correlacionándolos con indicadores de desigualdad social, en el período 2008-2019. Métodos: estudio ecológico mediante registros del SISVAN sobre la población ≥60 años. Se realizaron análisis de correlación entre indicadores de desigualdad social y la tasa de incremento del estado nutricional y análisis de desigualdades absolutas y relativas para obtener el índice de desigualdad angular y el índice de concentración. Resultados: se identificaron 11.587.933 registros. La cobertura nacional evolucionó del 0,1% en 2008 al 2,9% en 2019, con una tendencia ascendente estadísticamente significativa. Se encontró una correlación inversa moderada con la tasa de incremento anual de sobrepeso para IDH y PIB per cápita. Conclusión: hubo una tendencia de crecimiento en la cobertura del SISVAN. El aumento del sobrepeso se asoció con la desigualdad social.


Objetivo: analisar a tendência temporal da cobertura do Sistema de Vigilância Alimentar e Nutricional (SISVAN) e do estado nutricional de idosos, e sua correlação com indicadores de desigualdade social no Brasil, no período 2008-2019. Métodos: estudo ecológico, sobre registros do SISVAN relativos à população na idade de 60 anos ou mais; analisaram-se a tendência temporal da cobertura e a correlação entre indicadores de desigualdade social e taxa de incremento do estado nutricional; os índices angular e de concentração foram utilizados para medir desigualdades absolutas e relativas. Resultados: foram identificados 11.587.933 registros de idosos; a cobertura nacional evoluiu de 0,1% (2008) para 2,9% (2019), com tendência de aumento estatisticamente significativa; foi encontrada correlação inversa moderada com taxa de incremento anual de sobrepeso, para índice de desenvolvimento humano e produto interno bruto per capita. Conclusão: houve tendência de crescimento da cobertura do SISVAN; o aumento de sobrepeso esteve associado à desigualdade social.

ABSTRACT OBJECTIVE To characterize the internal structure of the Food and Nutrition Surveillance System (Sisvan) form of food intake markers for individuals over 2 years of age and to investigate measurement invariance between Brazilian macro-regions, life stages and over the years. METHODS A parallel analysis with factor estimation was carried out, complemented with exploratory factor analysis using all Sisvan records with valid responses in the country in 2015 (n = 298,253). Only the first record per individual was considered. Next, multigroup confirmatory factor analysis was used to investigate configural, metric and scalar invariance between the five macro-regions (Midwest, Northeast, North, Southeast, South) and life stages (children, adolescents, adults, elderly) in the same reference year. Invariance was evaluated longitudinally using valid individual records from 2015 to 2019 (n = 4,578,960). The adequacy of fit indices was observed at each step. RESULTS Acceptable fit indices and adequate factor loadings were found for a two-dimensional model, which grouped ultra-processed foods (factor 1) and unprocessed or minimally processed foods (factor 2). The two-dimensional structure, with the respective items in each factor underlying the set of markers, was equivalent across macro-regions, life stages and longitudinally, confirming the configural invariance. The weights of each item and its scale were homogeneous for all groups of interest, confirming metric and scalar invariances. CONCLUSIONS The internal structure of the Sisvan form of food intake markers adequately reflected its conceptual foundation, with stability of factors related to healthy and unhealthy eating in configuration, weights and scale in the investigated categories. These findings qualify food and nutritional surveillance actions, enhancing the use of Sisvan food intake markers in research, monitoring, individual guidance, and care production in the Brazilian Unified Health System.


RESUMO OBJETIVO Caracterizar a estrutura interna do formulário de marcadores do consumo alimentar do Sistema de Vigilância Alimentar e Nutricional (Sisvan) para indivíduos maiores de 2 anos de idade e analisar evidências de invariância de mensuração entre macrorregiões brasileiras, fases do curso da vida e ao longo do tempo. MÉTODOS Realizou-se análise paralela com estimação de fatores complementada com análise fatorial exploratória a partir de todos os registros de primeiro acompanhamento do Sisvan com respostas válidas no país em 2015 (n = 298.253). A seguir, empregou-se a análise fatorial confirmatória multigrupo para investigação de evidências de invariância configural, métrica e escalar entre as cinco macrorregiões (Centro-Oeste, Nordeste, Norte, Sudeste e Sul) e fases do curso da vida (crianças, adolescentes, adultos e idosos), no mesmo ano de referência. Avaliou-se a invariância longitudinalmente a partir de registros individuais válidos de 2015 a 2019 (n = 4.578.960). A adequação de índices de ajuste foi observada a cada etapa. RESULTADOS Verificaram-se índices de ajuste aceitáveis e valores de carga fatorial adequados para um modelo bidimensional, que agrupou alimentos ultraprocessados (fator 1) e alimentos in natura ou minimamente processados (fator 2). A estrutura bidimensional, com os respectivos itens em cada fator subjacente ao conjunto de marcadores, foi equivalente entre macrorregiões, fases do curso da vida e longitudinalmente, confirmando a invariância configural. Os pesos de cada item e sua escala foram homogêneos para todos os grupos de interesse, confirmando as invariâncias métrica e escalar. CONCLUSÕES A estrutura interna do formulário de marcadores do consumo alimentar refletiu adequadamente seu embasamento conceitual, com estabilidade dos fatores relacionados à alimentação saudável e não saudável em configuração, cargas e escala nos recortes investigados. Esses achados qualificam ações de vigilância alimentar e nutricional, potencializando o uso dos marcadores do consumo alimentar do Sisvan em pesquisas, monitoramento, orientação individual e produção de cuidado no Sistema Único de Saúde.

ABSTRACT OBJECTIVE To evaluate the quality of anthropometric data of children recorded in the Food and Nutrition Surveillance System (SISVAN) from 2008 to 2017. METHOD Descriptive study on the quality of anthropometric data of children under five years of age admitted in primary care services of the Unified Health System, from the individual databases of SISVAN. Data quality was annually assessed using the indicators: coverage, completeness, sex ratio, age distribution, weight and height digit preference, implausible z-score values, standard deviation, and normality of z-scores. RESULTS In total, 73,745,023 records and 29,852,480 children were identified. Coverage increased from 17.7% in 2008 to 45.4% in 2017. Completeness of birth date, weight, and height corresponded to almost 100% in all years. The sex ratio was balanced and approximately similar to the expected ratio, ranging from 0.8 to 1. The age distribution revealed higher percentages of registrations from the ages of two to four years until mid-2015. A preference for terminal digits "zero" and "five" was identified among weight and height records. The percentages of implausible z-scores exceeded 1% for all anthropometric indices, with values decreasing from 2014 onwards. A high dispersion of z-scores, including standard deviations between 1.2 and 1.6, was identified mainly in the indices including height and in the records of children under two years of age and residents in the North, Northeast, and Midwest regions. The distribution of z-scores was symmetric for all indices and platykurtic for height/age and weight/age. CONCLUSIONS The quality of SISVAN anthropometric data for children under five years of age has improved substantially between 2008 and 2017. Some indicators require attention, particularly for height measurements, whose quality was lower especially among groups more vulnerable to nutritional problems.


RESUMO OBJETIVOS Avaliar a qualidade dos dados antropométricos de crianças registradas no Sistema de Vigilância Alimentar e Nutricional (Sisvan) no período 2008-2017. MÉTODOS Estudo descritivo sobre a qualidade dos dados antropométricos de crianças menores de 5 anos atendidas nos serviços de atenção primária do Sistema Único de Saúde, a partir das bases de dados individuais do Sisvan. A qualidade dos dados foi avaliada anualmente por meio dos indicadores: cobertura, completude, razão entre sexos, distribuição da idade, preferência por dígitos de peso e estatura, valores de escore-z implausíveis, desvio-padrão e normalidade dos escores-z. RESULTADOS No total, 73.745.023 registros e 29.852.480 crianças foram identificados. A cobertura aumentou de 17,7% em 2008 para 45,4% em 2017. A completude da data de nascimento, peso e estatura correspondeu a quase 100% para todos os anos. A razão entre sexos foi equilibrada e aproximadamente similar a razão esperada, variando entre 0,8 e 1. A distribuição da idade revelou maiores percentuais de registros entre as idades de 2 a 4 anos até meados de 2015. Uma preferência pelos dígitos terminais "zero" e "cinco" foi identificada entre os registros de peso e estatura. As porcentagens de escores-z implausíveis excederam 1% para todos os índices antropométricos, com redução dos valores a partir de 2014. Uma alta dispersão dos escores-z, incluindo desvios-padrão entre 1,2 e 1,6, foi identificada principalmente nos índices incluindo estatura e nos registros de crianças menores de 2 anos e residentes das regiões Norte, Nordeste e Centro-Oeste. A distribuição dos escores-z foi simétrica para todos os índices e platicúrtica para estatura/idade e peso/idade. CONCLUSÕES A qualidade dos dados antropométricos do Sisvan para crianças menores de 5 anos melhorou substancialmente entre 2008 e 2017. Alguns indicadores requerem atenção, sobretudo para medidas de estatura, cuja qualidade foi principalmente inferior entre os grupos mais vulneráveis a agravos nutricionais.

ABSTRACT OBJECTIVE To investigate the performance of food consumption markers of the Food and Nutrition Surveillance System (Sisvan) in assessing the overall dietary quality. METHODS The study was carried out based on the reproduction of responses to markers in 24-hour recall data from 46,164 individuals aged ≥ 10 years, from the 2017-2018 Household Budget Survey (POF). Seven Sisvan markers were evaluated, and two scores were calculated for each participant, based on the sum of the number of healthy food markers (beans, fruits, and vegetables, ranging from 0 to 3) and unhealthy (hamburgers/sausages, sweetened beverages, instant noodles/salt snacks/crackers, stuffed cookies/sweets/candies, ranging from 0 to 4) consumed. Linear regression analyses were used to assess the association between scores and diet quality indicators (ultra-processed foods, dietary diversity, and levels of saturated and trans fat, added sugar, sodium, potassium, and fiber in the diet). RESULTS The score of healthy eating markers increased significantly with increasing dietary diversity and potassium and fiber contents in the diet, while the opposite trend was observed for the densities of added sugar, sodium, saturated and trans fat (p < 0.001). The score of unhealthy eating markers increased significantly with the increase in the consumption of ultra-processed foods and densities of added sugar, saturated and trans fat levels in the diet, while an inverse trend was observed for potassium and fiber (p < 0.001). The joint analysis of the combination of the two marker scores showed that individuals with better performance (3 in the healthy food score, and 0 in the unhealthy food score) have a lower number of inadequacies in nutrient consumption. CONCLUSION Sisvan food consumption markers, quickly and easily applied and already incorporated into the Brazilian public health system, have good potential to reflect the overall dietary quality.


RESUMO OBJETIVO Investigar o desempenho dos marcadores do consumo alimentar do Sistema de Vigilância Alimentar e Nutricional (Sisvan) na avaliação da qualidade global da alimentação. MÉTODOS O estudo foi realizado a partir da reprodução de respostas aos marcadores em dados de recordatórios de 24 horas, de 46.164 indivíduos com idade menor ou igual a 10 anos, da Pesquisa de Orçamentos Familiares 2017-2018. Foram avaliados sete marcadores do Sisvan e calculados dois escores para cada participante, a partir do somatório do número de marcadores de alimentação saudável (feijão, frutas, verduras/legumes, variando de 0 a 3) e não saudável (hambúrguer/embutidos, bebidas adoçadas, macarrão instantâneo/salgadinhos/biscoitos salgados, biscoito recheado/doces/guloseimas, variando de 0 a 4) consumidos. Análises de regressão linear foram usadas para avaliar a associação entre os escores e indicadores de qualidade da alimentação (participação de alimentos ultraprocessados, diversidade e teores de gordura saturada, trans, açúcar de adição, sódio, potássio e fibra da dieta). RESULTADOS o escore de marcadores de alimentação saudável aumentou de forma significativa com o aumento da diversidade e dos teores de potássio e fibra da dieta, enquanto tendência oposta foi observada para as densidades de açúcar de adição, sódio, gordura saturada e trans (p < 0,001). Observou-se que o escore de marcadores de alimentação não saudável aumentou de forma significativa com o aumento da participação de alimentos ultraprocessados e dos teores de açúcar de adição, gordura saturada e trans da dieta, enquanto tendência inversa é observada para potássio e fibra (p < 0,001). A análise conjunta da combinação dos dois escores de marcadores mostrou que indivíduos com melhor desempenho (3 no escore de alimentos saudáveis, e 0 no de alimentos não saudáveis) possuem menor número de inadequações no consumo de nutrientes. CONCLUSÃO Os marcadores do consumo alimentar do Sisvan, aplicados de forma rápida e prática e já incorporados no sistema público de saúde brasileiro, possuem bom potencial para refletir a qualidade global da alimentação.

The objective of this study was to analyze the temporal trend of malnutrition in children under five years of age assisted by the Bolsa Família Program (PBF) between 2008 and 2019, exploring regional inequalities and seeking to determine the impact of the economic and political crises worsened in 2014 and of government adherence to fiscal austerity policies on the trend. Analyses were conducted using aggregate data from infants (0-23 months) and preschoolers (24-59 months) extracted from the Food and Nutrition Surveillance System (SISVAN) assisted by the PBF (n=34,272,024). Trends were analyzed using generalized linear models with age-specific mixed effects (negative binomial distribution and log-link function). Regional inequalities were analyzed by grouping the federative units according to the social vulnerability index (SVI) and the influence of crises and austerity policies on the prevalence of malnutrition by means of the interaction between "year" and "crisis" (2008-2013 vs. 2014-2019). There was a reduction in the prevalence of child malnutrition until mid-2013, when trends became stationary for preschoolers and upward for infants. Higher risk of malnutrition was also observed in states with medium and high social vulnerability compared to those with low social vulnerability. The turning point in the trends supports the hypothesis that the political and economic crises, and government responses to these crises, have negatively impacted the nutritional status of children in poverty and extreme poverty in Brazil.


O objetivo deste estudo foi analisar a tendência temporal da desnutrição em crianças menores de cinco anos de idade assistidas pelo Programa Bolsa Família (PBF) entre 2008 e 2019, explorando desigualdades regionais e buscando determinar o impacto das crises econômica e política agravadas em 2014 e da adesão governamental às políticas de austeridade fiscal na tendência. As análises foram realizadas utilizando dados agregados de lactentes (0-23 meses) e pré-escolares (24-59 meses), extraídos do Sistema de Vigilância Alimentar e Nutricional (SISVAN) assistidas pelo PBF (n=34.272.024). As tendências foram analisadas por meio de modelos lineares generalizados com efeitos mistos específicos para as faixas etárias (distribuição binomial negativa e função de ligação log). As desigualdades regionais foram analisadas a partir do agrupamento das unidades federativas segundo o índice de vulnerabilidade social (IVS) e a influência das crises e das políticas de austeridade na prevalência de desnutrição por meio da interação entre "ano" e "crise" (2008-2013 vs. 2014-2019). Houve redução na prevalência de desnutrição infantil até meados de 2013, quando as tendências passaram a ser estacionárias para pré-escolares e ascendentes para lactentes. Observou-se também maior risco de desnutrição nos estados com média e alta vulnerabilidade social, quando comparadas àqueles com baixa vulnerabilidade social. O ponto de inflexão nas tendências corroboram a hipótese de que as crises política e econômica, e as respostas governamentais a estas crises, provocaram impacto negativo sobre o estado nutricional de crianças em situação de pobreza e extrema pobreza no Brasil.

BACKGROUND: Tea consumption is widely reported to have beneficial effects on metabolic functions. The current study is to evaluate the association between habitual tea consumption and risk for metabolic syndrome and its components among Chinese adults aged 18~59 years. METHODS: 43,757 participants aged 18~59 years from China Nutrition and Health Surveillance 2015-2017 were included and divided into four groups based on the amount of daily tea consumption in the current study. Using multiple-adjustment logistic regression to explore the relationship between habitual tea consumption and metabolic syndrome-related health outcomes. RESULTS: Compared with those who did not consume tea habitually, participants who drank over 5 cups of tea per day showed a significantly lower risk of metabolic syndrome (OR = 0.836, 95% CI = 0.771-0.905), blood pressure elevated (OR = 0.906, 95% CI = 0.845-0.972), triglyceride elevated (OR = 0.797, 95% CI = 0.741-0.857), and fasting plasma glucose elevated (OR = 0.772, 95% CI = 0.715-0.833), but higher risk for central obesity (OR = 1.354, 95% CI = 1.236-1.484). Regardless of gender, higher tea consumption was related to lower risk of triglyceride and fasting blood glucose elevated but higher risk for central obesity. While for protective effect on metabolic syndrome, blood pressure elevated, and HDL-C reduction only showed in females. CONCLUSIONS: Results from current study support that habitual tea consumption would benefit metabolic syndrome and its related components, especially among females.

Introduction: between 2006 and 2020, obesity in Mexico increased across all age groups and displayed a homogenizing evolutionary trend throughout, prevalence overweigth and obesity (Ow+Ob) has 38.2% in school age children. Objectives: to analyze the changes of Ow+Ob in a cohort with four years of evolution of students from Mexico elementary schools and to evaluate its association with socio-demographic factors. Methods: information comes from a nutritional surveillance system of 52,545 elementary schools, with weight and height measurements from 2,008,474 children from six to eleven years old. A follow-on panel longitudinal analysis was performed from 2015 to 2019 in a dynamic cohort with three measurements. Ow+Ob prevalences were obtained; through a logistic regression with random effects, odds ratios (OR) were calculated, adjusting by sociodemographic characteristics (p < 0.05). Results: between 2015 and 2019, positive OR were observed for Ow+Ob development in 2017-2018 (OR = 1.02) and 2018-2019 (OR = 1.06). Students from the northern and southern regions of the country showed a greater probability of suffering Ow+Ob (OR = 1.58 and 1.64) when compared with the center. Attending to community or indigenous schools was a protective factor (OR = 0.54) whereas attending to a private school increased the risk (OR = 1.75). Adjusted Ow+Ob prevalences showed an accelerated increasing trend in males through all the periods. Conclusions: in Mexico, obesity in school children is a growing problem related to sociodemographic factors, therefore, urgent actions are needed for its restraining. (AU)


Introducción: México se sitúa dentro de los primeros lugares a nivel mundial en cuanto a sobrepeso y obesidad en escolares, con una prevalencia del 38,2 %. Objetivo: analizar los cambios de sobrepeso y obesidad (Sob+Ob) durante cuatro años en una cohorte de alumnos de escuelas primarias en México y evaluar su asociación con factores sociodemográficos. Métodos: la información proviene de un sistema de vigilancia nutricional en escolares. Se realizó un análisis longitudinal de panel con seguimiento a través de una corte dinámica de tres mediciones realizadas entre 2015 y 2019. Se obtuvieron las prevalencias de Sob+Ob a través de una regresión logística con efectos aleatorios y se calcularon razones de momios (OR), ajustando por características sociodemográficas. Resultados: entre 2015 y 2019 se observaron OR positivos para el desarrollo de Sob+Ob, 2017-2018 (OR = 1,02) y 2018-2019 (OR = 1,06). Los estudiantes de las regiones norte y sur del país mostraron una mayor probabilidad de sufrir Sob+Ob (OR = 1,58 y 1,64) en comparación con el centro. La asistencia a escuelas comunitarias o indígenas fue un factor protector (OR = 0,54), mientras que la asistencia a una escuela privada aumentó el riesgo (OR = 1,75). Las prevalencias ajustadas de Sob+Ob demostraron una tendencia creciente acelerada en los hombres a lo largo de todos los periodos. Conclusiones:en México, la obesidad en escolares es un problema creciente relacionado con factores sociodemográficos, por lo que se requieren acciones urgentes para su contención. (AU)

INTRODUCTION/BACKGROUND: This work aims to analyse the prevalence of excess weight in Brazil to demonstrate the nutritional transition that is occurring. The data mobilized in the research are from the Food and Nutritional Surveillance System (FNSS). MATERIALS AND METHODS: This study employed a quantitative approach from the FNSS online secondary data survey through reports of nutritional status in different phases of life (child, adolescent, adult, elderly and pregnant), in different macroregions of the country (South, Southeast, Midwest, Northeast and North) and with a 12-year historical series (2008 to 2019). RESULTS: In the adult life stage, there was a time trend of increasing excess weight in all regions of this historical series. The southern region of Brazil and the adult life stage had the highest national percentage of excess weight (69,1%) in 2019 and had the lowest percentage of eutrophy (29.3%) in the region in 2019. In the elderly life phase, in the South, Southeast and Midwest regions, excess weight was higher than the other outcomes in the time series, with the highest annual prevalence in the South region (58,6%) in 2019. In the adolescent life stage, there was a time trend of increasing excess weight in all regions, and excess weight had the highest prevalence in 2019 in the South (35,8%). The lowest prevalence rates of excess weight were in the following age groups: children aged 0 to < 2 years old and children aged 2 to < 5 years old. Additionally, it was in Group 5 to < 10 years old that the most critical prevalence of excess weight (35,07%) was found in 2018. However, malnutrition (low weight) persists, especially in the elderly and children. In the pregnancy life stage, there was a temporal trend of increase in excess weight in all regions, with higher percentages in 2019 in the South (53.5%) and Southeast (50.8%). CONCLUSION: Excess weight has shown increasing time trends in the adolescent, adult, elderly and pregnant life stages in all regions of Brazil, suggesting that public FNS policymakers should be more assertive in the planning and management of programs and actions to reduce the percentages of diseases.

Background: Practice-based experiences documenting development and implementation of nutrition and health surveillance systems are needed. Objectives: To describe processes, methods, and lessons learned from developing and implementing a population-based household nutrition and health surveillance system in Guatemala. Methods: The phases and methods for the design and implementation of the surveillance system are described. Efforts to institutionalize the system in government institutions are described, and illustrative examples describing different data uses, and lessons learned are provided. Results: After initial assessments of data needs and consultations with officials in government institutions and partners in the country, a population-based nutrition surveillance system prototype with complex sampling was designed and tested in 5 Guatemalan Highland departments in 2011. After dissemination of the prototype, government and partners expanded the content, and multitopic nutrition and health surveillance cycles were collected in 2013, 2015, 2016, 2017/18, and 2018/19 providing nationally representative data for households, women of reproductive age (15-49 y), and children aged 0-59 mo. For each cycle, data were to be collected from 100 clusters, 30 households in each, and 1 woman and 1 child per household. Content covered ∼25 health and nutrition topics, including coverage of all large-scale nutrition-specific interventions; the micronutrient content of fortifiable sugar, salt, and bread samples; anthropometry; and biomarkers to assess annually, or at least once, ∼25 indicators of micronutrient status and chronic disease. Data were collected by 3-5 highly trained field teams. The design was flexible and revised each cycle allowing potential changes to questionnaires, population groups, biomarkers, survey design, or other changes. Data were used to change national guidelines for vitamin A and B-12 interventions, among others, and evaluate interventions. Barriers included frequent changes of high-level government officials and heavy dependence on US funding. Conclusions: This system provides high-quality data, fills critical data gaps, and can serve as a useful model for others.

This study aimed to assess the association of dietary fiber intake with serum uric acid (SUA) levels and risk of hyperuricemia (HUA) among Chinese adults using the latest nationally representative data. A total of 66,427 Chinese adults aged 18 years and over from the China Adult Chronic Disease and Nutrition Surveillance in 2015 were included in this study. Dietary intakes were measured with a 3-day 24 h dietary recall and the household condiment weighing method. Mixed-effect linear and logistic regression models were used to evaluate the associations of dietary fiber intake with SUA levels and risk of HUA, respectively. Compared to the lowest intake group, the coefficient and 95% confidence in the highest intake group of total fiber were −0.06 (−0.08, −0.04) (p-trend < 0.001), −0.18 (−0.2, −0.16) (p-trend < 0.001) for cereal fiber, 0.03 (0.01, 0.04) (p-trend = 0.051) for legume fiber, 0 (−0.01, 0.02) (p-trend = 0.869) for vegetable fiber and 0.01 (−0.001, 0.04) (p-trend = 0.296) for fruit fiber. The odds ratio (OR) and 95% confidence interval (CI) of HUA for the highest vs. lowest intake group of total fiber were 0.88 (0.84, 0.91) (p-trend = 0.001), 0.67 (0.63, 0.71) (p-trend < 0.001) for cereal fiber, 1.05 (1, 1.09) (p-trend = 0.248) for legume fiber, 1.01 (0.97, 1.05) (p-trend = 0.982) for vegetable fiber and 1.06 (1, 1.12) (p-trend = 0.264) for fruit fiber. Our findings suggest that consumption of total fiber and cereal fiber were significantly inversely associated with SUA levels and HUA risk among the Chinese adult population. Developing and implementing effective public education programs are urgently needed to increase the intake of dietary fiber, especially cereal fiber among Chinese adults.
