INTRODUCTION AND OBJECTIVES: Although much has been published on the effect of the pandemic on trauma emergencies, it has not been analysed whether the incidence of infection or different organisation of available resources according to the Spanish region were influential factors. We present a multicenter study of three tertiary hospitals in three Spanish cities with different characteristics to study how these factors influenced paediatric trauma emergencies during the first wave of the pandemic. MATERIAL AND METHODS: We present a retrospective cohort study that compares and analyzes paediatric trauma emergencies in three different hospitals during the period of the first wave of the COVID-19 pandemic and we compare them with the same period of the previous year, analyzing the number of emergencies, the severity of the pathology, the average age of the patients, the days of admission and the waiting time of the cases that required surgical treatment. RESULTS: A total of 6474 paediatric trauma emergency episodes have been analysed. There was a drastic reduction in activity in the three hospitals, but in different amounts: 83.5% in the hospital located in Madrid, 75% in the hospital located in Valencia and 65.9% in the one located in Palma, Majorca. The emergencies attended were more serious in 2020 compared to 2019. The average age of patients treated during the pandemic was lower compared to the previous year. There were no differences in the days of admission, but there were differences in the delay in surgery for surgical fractures. CONCLUSIONS: The impact of the first wave of the COVID-19 pandemic and the decreed confinement had a different impact on paediatric trauma emergencies in the different hospitals according to the incidence of infection and the measures adopted in the different regions. In all cases there was a significant reduction in activity, a greater proportion of medium and severe pathologies were attended to, the average age of the patients attended was lower and a delay was observed in surgeries performed in the emergency room, probably due to the need to request a PCR test to detect a COVID-19 infection.

La fisura labiopalatina unilateral completa afecta la región del tercio medio facial y los individuos que nacen con esta condición presentan estructuras alteradas en el lado donde se desarrolló la malformación. El objetivo del presente reporte de caso es presentar una alternativa de ortopedia prequirúrgica para pacientes con fisura labiopalatina unilateral completa en una infante de sexo femenino de 1 mes y 5 días de edad con el diagnóstico de fisura labiopalatina unilateral izquierda completa. Se colocó el retractor nasal modificación Hinostroza, que permite obtener simetría de la estructura nasal afectada con respecto al lado no afectado; por sus buenos resultados, bajo costo y fácil manejo debe ser considerada una alternativa de tratamiento en neonatos nacidos con fisura labiopalatina unilateral completa. Complete unilateral palatine fissure affects the region of the middle facial third and individuals born with this condition have altered structures on the side where the malformation developed. The objective of the present case report was to describe a pre-surgical orthopedics alternative in a female infant of 1 month 5 days of age with complete unilateral palatine fissure diagnosed with complete unilateral left palatine fissure. The Hinostroza nasal retractor modification was placed, allowing symmetry to the affected nasal structure with respect to the unaffected side. This procedure provides good results, low cost and easy handling and should be considered as an alternative treatment in neonates born with complete unilateral palatine fissure. (AU)

A pesar de que las fracturas de pelvis son relativamente frecuentes en los adultos y se tiene una amplia disponibilidad de datos en la literatura, el ortopedista general debe tener conocimiento de la entidad en los niños para que cuando esta se presente elmanejo sea inmediato y eficaz. Las fracturas de la pelvis en los niños no son frecuentes; reportes actuales en la literatura hablan de 2,4 a 7,5% de todos los ingresos por traumatismos cerrados; otros reportes indican que se presentan en uno de cada 100.000 niños al año, con un 25% de mortalidad. Este tipo de fracturas en niños tienen una epidemiología, una incidencia y unmanejo distintos que en los adultos. Las principales diferencias están basadas en condiciones anatómicas que merecen un manejo distinto, ya que la pelvis absorbe mayor energía antes de fracturarse, lo que indica que cuando estamos frente a un niño con fractura de pelvis, se trata de un trauma mayor, multisistémico, que requiere apoyo multidisciplinario. En la literatura se encuentra gran cantidad de información para la población adulta a diferencia de la escasa para la población pediátrica...(AU)


Although pelvic fractures are relatively common in adults, and have been widely reported in the literature, the general orthopaedic surgeon should have knowledge of these fractures in children, in order for them to be able to manage them immediately and effectively. Pelvic fractures are uncommon in children. Current reports in the literature estimate that they account for 2.4 to 7.5% of all admissions for closed injuries. Other reports indicate that they are presented in one per 100,000 children per year, with a mortality of 25%. These types of fractures in children have an epidemiology, incidence, and management different to that of adults. The main differences are based on anatomical conditions that require a particular management, since the pelvis absorbs more energy before fracturing. This means that when faced with a child with a pelvic fracture, it is treated as a major multisystemic trauma that requires multidisciplinary support. Estudio observacional, descriptivo longitudinal (nivel III). Objetivo: describir las características clínicas, demográficas y posibles factores de riesgo en pacientes con enfermedad de Legg-Calvé-Perthes (LCP) en tres hospitales de Bogotá DC, Colombia entre 2003 y 2013. Materiales y métodos: historias clínicas ortopédicas de pacientes con LCP en rangos de edad entre 4 y 14 años. Resultados: el total de la población fue de 83 pacientes con 89 caderas comprometidas; 88% fueron hombres. Los factores de riesgo encontrados fueron: en 24 casos (29%) trastorno de déficit de atención e hiperactividad (TDAH), 23 (28%) evidenciaron exposición pasiva a humo de cigarrillo de la madre durante el embarazo y 4.8% tenían discrasias sanguíneas. Conclusiones: teniendo en cuenta los resultados no se demostró que la enfermedad de LCP en pacientes pediátricos tuviera relación con madres añosas (>35 años) al momento del parto; 29% cursó con TDAH y 28% con exposición pasiva a humo de cigarrillo durante el embarazo...


Longitudinal, observational, descriptive (level III) study. Materials and Methods: review of orthopedic clinical records of patients with LCPD aged 4 to 14 years. Results: a total population of 83 patients with 89 compromised hips were included, 88% were males. Risk factors found were: attention deficit hyperactivity disorder (ADHD) in 24 cases (29%), passive exposure to cigarette smoke of the mother during pregnancy in 23 (28%) and coagulation disorders in 4.8%. Conclusions: the results did not demonstrate that LCPD in pediatric patients had relation to advanced maternal age (>35 years) at delivery; 29% were associated to ADHD and 28% with passive exposure to cigarette smoke of the mother during pregnancy...

RESUMO

En el Hospital Universitario San Vicente de Paúl (HUSVP) de Medellín, Colombia, se realizó un estudio observacional descriptivo y prospectivo tipo serie de casos, de fracturas diafisiarias del fémur en niños entre 6 y 12 años tratadas con clavos flexibles intramedulares de acero. Se trataron 70 pacientes en el período comprendido entre 1 de agosto de 2000 y 1 de enero de 2004. Se revisaron los pacientes y las radiografías en el servicio de consulta externa del Hospital Infantil del HUSVP con un seguimiento mínimo de 6 meses. La mayoría de los 70 pacientes eran del sexo masculino (74 por ciento), el mecanismo más frecuente de trauma fue el accidente de tránsito (66 por ciento), el promedio de edad fue de 8.3 años, el promedio de estancia hospitalaria fue de 7.8 días atribuible a causas administrativas y a presentar traumas asociados. A 59 pacientes se les retiraron los clavos en un tiempo promedio de 5.1 meses, sin complicaciones; los 11 restantes todavía tienen el material de osteosíntesis en el momento del análisis.Se encontraron 3 pacientes con discrepancia de longitud a expensas de la extremidad fracturada y 2 con deformidad angular; ninguno de ellos, hasta la fecha, ha requerido tratamiento médico o quirúrgico adicional.Los resultados de esta técnica, relativamente nueva en Medellín, Colombia, son alentadores; además, en este proyecto participaron 15 ortopedistas lo que muestra la facilidad de la técnica, con la que hemos obtenido resultados tan buenos como los reportados en la literatura mundial.
