RESUMO
Presentación del caso. Se trata de un masculino de 49 años, con historia de un trauma abdominal cerrado con un objeto contuso sin alteraciones hemodinámicas. La ecografía focalizada de traumatismos resultó positiva a líquido libre en la cavidad abdominal en tres ventanas. Intervención terapéutica. Fue sometido a múltiples laparotomías exploratorias con lavados de cavidad abdominal posterior a presentar un cuadro de abdomen séptico Björk 4, quien después de 38 días de estancia intrahospitalaria presentó una fístula enteroatmosférica que causaba un déficit nutricional. En una intervención quirúrgica se realizó el cierre de la fístula enteroatmosférica, con el hallazgo de tejido óseo trabecular en cavidad abdominal, correspondiente a osificación heterotópica intraabdominal de formación reciente. Evolución clínica. Luego de la anastomosis del intestino delgado presentó signos de obstrucción intestinal, a causa de esto, se realizó una nueva laparotomía exploratoria, en la que se encontró tejido óseo adherido al mesenterio del intestino delgado que generó una obstrucción completa, se extirpó el material óseo y se logra restablecer el flujo intestinal liberando la obstrucción en el intestino delgado
Case presentation. A 49-year-old male, with a history of blunt abdominal trauma with a blunt object without hemodynamic alterations. The focused trauma ultrasound was positive for free fluid in the abdominal cavity in three windows. Treatment.The patient underwent multiple exploratory laparotomies with peritoneal lavage after presenting a septic abdomen (Björk 4), who after 38 days of intrahospital stay presented an enteroatmospheric fistula that caused a large nutritional deficit. In a surgical intervention, the enteroatmospheric fistula was closed, with the finding of trabecular bone tissue in the abdominal cavity, corresponding to intra-abdominal heterotopic ossification of recent formation. Outcome. After the small intestine anastomosis, he presented signs of intestinal obstruction, because of this, a new exploratory laparotomy was performed, in which bone tissue was found adhered to the mesentery of the small intestine that generated a complete obstruction. The bone material was removed and the intestinal flow was reestablished, freeing the obstruction in the small intestine

Thus, this systematic review aimed to search the current literature for HO treatment and prevention. This study was conducted following PRISMA recommendations (Preferred Reporting Items for Systematic Reviews and Meta-Analysis) and searches were conducted in three databases (PubMed, Embase, and Web of Science). A total of 193 articles were found in an initial search. After screening following the established criteria, eight articles were included in this review; of these, two reported prevention and the others, treatments. Based on data analysis, the use of non-steroidal anti-inflammatory drugs in the acute post-traumatic period proved to be the best method of prevention. In cases of mature HO or accompanied by ankylosis, surgical resection proved to be the most effective treatment despite the high rate of postoperative infections. Foram encontrados 193 artigos inicialmente, e, após triagem considerando os critérios estabelecidos, oito foram eleitos para o trabalho final. Dois trabalhos abordaram a prevenção; e os demais, tratamentos. Pela análise dos dados, o uso de anti-inflamatórios não esteroidais (AINEs) no período agudo pós-traumático se mostrou como o melhor método de prevenção. RESUMO
Abstract The objective of this study was to conduct a systematic review and meta-analysis of relevant randomized control trials (RCTs) to determine the role of ibuprofen, as well as the optimum dose and duration of therapy, in preventing the incidence of heterotopic ossification (HO) after primary total hip arthroplasty (THA). A literature search was performed using the PubMed/MEDLINE and Cochrane Library databases for RCTs that compared the use of ibuprofen versus placebo as prophylaxis for HO in patients after THA. The main outcomes for this study were overall occurrence of HO, occurrence according to the Brooker classification, and gastrointestinal complications. A total of 27 potential articles were identified from the database. Eventually, four trials with 1,153 patients were included in the final analysis. When compared with placebo, the use of ibuprofen is associated with a reduction in the incidence of HO at the 3- and 12-month follow-up appointments, as well as the incidence of Brooker II and III HO (p < 0.05). However, there was no significant difference between the ibuprofen and placebo groups in terms of treatment discontinuation due to gastrointestinal complications or the incidence of Brooker I and IV HO (p > 0.05). The existing data indicates that ibuprofen is safe and efficacious in reducing the total incidence of HO along with Brooker II and III HO at follow-up. However, due to the small number of studies, the conclusions are limited; therefore, more high-quality clinical trials are required to develop guidelines for optimal dose and duration of therapy.
Resumo O objetivo deste estudo foi realizar uma revisão sistemática e metanálise de estudos clínicos randomizados (ECRs) relevantes para determinar o papel do ibuprofeno, sua dose ideal, e a duração do tratamento na prevenção de ossificação heterotópica (OH) após a artroplastia total primária do quadril (ATQ). Uma pesquisa bibliográfica foi feita nos bancos de dados PubMed/MEDLINE e Cochrane Library para a obtenção de ECRs quecomparassem ouso de ibuprofeno edeplacebo como profilaxiaparaOHem pacientes submetidos à ATQ. Os principais desfechos deste estudo foram ocorrência geral de OH, classificação de Brooker da OH, e complicações gastrintestinais. No total, 27 artigos foram identificados nos bancos de dados e 4 estudos, com 1.153 pacientes, foram incluídos na análise final. Em comparação ao placebo, o uso de ibuprofeno reduziu a incidência de OH aos 3 e 12 meses de acompanhamento e a incidência de OH Brooker II e III (p < 0,05). No entanto, não houve diferença significativa entre os grupos que receberam ibuprofeno e placebo em termos de interrupção do tratamento devido a complicações gastrintestinais ou da incidência de OH Brooker I e IV (p > 0,05). Os dados existentes indicam que o ibuprofeno é seguro e eficaz na redução da incidência total de OH e de OH Brooker II e III durante o acompanhamento. Heterotopic ossification has been described in papillary thyroid carcinoma in association with high incidence of extrathyroidal invasion, multifocality, lymph node metastasis, and older age. Nevertheless, it has not been described as a specific subtype of papillary thyroid carcinoma, because of its rarity. We described the case of an 80-year-old female patient, with familial history of papillary thyroid carcinoma. In the annual screening examination, she was diagnosed with thyroid nodules. The patient was submitted to a thyroidectomy because the fine needle aspiration cytology was positive for malignancy according to the Bethesda classification. The surgical specimen analyses showed a multifocal papillary carcinoma with one major lesion in the left lobe measuring 0.9 cm, and two small lesions (0.4 cm and 0.2 cm) in the right lobe. Only the biggest lesion in the right lobe had the osteoid matrix with rare osteoclasts and fat metaplasia with progenitor cells. There was perineural invasion, but vascular invasion was not identified. The margins were free and there was no extrathyroidal extension. In the left lobe there was an oncocytic nodule and a lipomatous follicular nodule. In recent years there has been a significant increase in the diagnosis of thyroid cancer, mainly because of the finding of microcarcinomas as a result of many requests for cervical image exams. Future studies may define (i) whether papillary thyroid carcinoma with heterotopic ossification is a true histological variant; (ii) the causes of that alteration; and (iii) eventual follow-up implications.

Forestier Disease, or Diffuse Idiopathic Skeletal Hyperostosis (DISH), causes a considerable ossification of the anterior longitudinal ligament of the spine. When it involves cervical segments, it can be accompanied by dysphagia, dysphonia and/or dyspnea. This entity usually has a benign course. Surgical treatment is required for progressive cases. We report a 72-year-old male with a history of chronic cervical pain and slight neck stiffness. On the imaging studies, there was an exuberant ossification of the anterior longitudinal ligament, at the cervical spine level, between C2 and C7. He was managed conservatively without clinical deterioration in a six-year follow-up.

The term heterotopic ossification refers to bone formation in normally non-ossifying tissue. It represents a benign, localized, self-limiting and well-circumscribed lesion, and the phenomenon is rather unusual in the immediate vicinity of bones. Likewise, it is very rare in soft tissues such as the gastrointestinal tract, where it is also known as heterotopic mesenteric ossification (HMO). Intra-abdominal heterotopic ossification (IHO) is also known as intra-abdominal myositis ossificans, mesenteritis ossificans, heterotopic mesenteric ossification, and heterotopic ossification of the intestinal mesentery. It is extremely rare and only approximately 30 cases have been reported in the literature since it was first described in 1983. This paper presents the case of a male 14 year-old patient diagnosed with mesenteric ossification who was treated by the pediatric surgeons. Additionally, the authors present a review of the medical literature regarding this condition.
El término osificación heterotópica se refiere a la neoformación de tejido óseo en sitios donde normalmente el tejido no se osifica. Es una condición benigna, localizada, bien definida y autolimitada; ocurre con mayor frecuencia en la vecindad inmediata de los huesos. Es muy raro que se presente en los tejidos blandos del tracto gastrointestinal, donde es conocida como osificación heterotópica del mesenterio (OHM). La osificación heterotópica intraabdominal (OHI) es además conocida como miositis osificante, mesenteritis osificante, osificación heterotópica del mesenterio y osificación heterotópica del mesenterio intestinal. Es una condición extremadamente rara, con solo 30 casos aproximadamente reportados en la literatura desde su primera descripción en 1983. Este artículo presenta el caso de un niño de 14 años con diagnóstico de mesenteritis osificante que fue tratado por un grupo de cirujanos pediátricos. Además, se presenta una revisión de la literatura médica sobre esta extraña condición.

A ossificação heterotópica intra-abdominal (OHI) é ademais conhecida como miosite ossificante, mesenterites ossificante, ossificação heterotópica do mesentério e ossificação heterotópica do mesentério intestinal. FUNDAMENTO: la osificación heterotópica es la formación de hueso ectópico y lamelar con médula ósea. La fisiopatología no se conoce en su totalidad. El tratamiento se basa en: antinflamatorios no esteroideos, radioterapia, bifosfonatos, cirugía y terapia física. OBJETIVO: presentar un caso de osificación heterotópica neurogénica de la cadera derecha como causa de limitación de la movilidad articular y ciática troncular. CASO CLÍNICO: paciente masculino de 28 años de edad con antecedentes de trauma craneoencefálico grave. Permaneció por más de dos meses encamado en estado crítico. Un año después comenzó con aumento de volumen de la región glútea derecha, dolor ligero en la pierna, que le dificultaba la posición de sentado, asociado a debilidad con predominio distal (pie péndulo), e hipoestesia. A la exploración física se constató masa tumoral en región glútea derecha de consistencia dura, pétrea, fija a planos profundos, no dolorosa con signo de Tinel positivo en la emergencia del nervio ciático. En la radiografía AP de la cadera derecha se pudo apreciar imagen ósea tumoral. Se comenzó tratamiento conservador sin mejoría, por lo que se realizó cirugía para resecar la masa ósea, obtener el diagnóstico histológico y mejorar la sintomatología. CONCLUSIONES: la osificación heterotópica es una complicación en pacientes con trauma craneoencefálico grave, el cuadro clínico no es específico y el diagnóstico es orientado por la imaginología y confirmado por la histopatología. El tratamiento quirúrgico ofrece los mejores resultados. La recurrencia se puede minimizar al combinar antiinflamatorios no esteroideos, radioterapia y rehabilitación.
BACKGROUND: heterotopic ossification is the formation of ectopic lamellar bone containing bone marrow. The physiopathology is not known in its entirety. The treatment is based on non-steroid anti-inflammatory drugs, radiotherapy, biphosphonates, surgery, and physical therapy. OBJECTIVE: to present a case of neurogenic heterotopic ossification of the right hip caused by limited articular mobility and truncal sciatica. CLINICAL CASE: a twenty-eight-year-old male patient with antecedents of severe traumatic brain injury. The patient stayed in bed for two months in a critical condition. A year later, his right gluteal region started to grow and he presented a slight pain in the leg that made him difficult to sit associated to debility with distal predominance (pendular feet) and hypoesthesia. In the physical examination, a tumor mass of hard consistence and fix to deep planes, painless with positive signs of Tinel in the emergence of the sciatic nerve, was found in the right gluteal region. In the radiography of the right hip, an osseous tumoral image was seen. A conservative treatment was started without any improvement, that's why a surgery was conducted to remove the osseous mass, obtain the histological diagnosis and improve the symptomatology. CONCLUSIONS: neurogenic heterotopic ossification is a complication that appears in patients with severe craniocerebral trauma. The clinical manifestations are not specific and the diagnosis is determined by means of imaging and confirmed through histopathology. The surgical treatment offers the best results. The recurrence can be minimized when combining non-steroid anti-inflammatory drugs, radiotherapy and rehabilitation.

OBJECTIVES: To report the incidence of symptomatic heterotopic ossification (HO) in a defined civilian amputee population, describe its characteristics, and compare these findings to published data in military amputees. DESIGN: Retrospective chart analysis from July 1998 to July 2009. SETTING: Ambulatory amputee clinic within a large university medical center. PARTICIPANTS: Adults with lower limb amputation (N=158). INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURE: Patients with symptomatic HO confirmed by radiographs. RESULTS: A total of 261 patients were evaluated; 158 met inclusion criteria, with 59% having traumatic etiology, 18% vascular etiology, 22% infection, and 1% tumor. Symptomatic HO was diagnosed in 36 (22.8%) patients, and 94% patients had mild HO on radiographic scoring. Rate of HO in amputations related to trauma was not increased compared with those of other etiologies. Surgical resection of the ectopic bone was required in 4 (11%) patients. CONCLUSIONS: HO is seen commonly after civilian lower limb amputation regardless of etiology. The prevalence was less than that observed in previous reports from military populations. This is the first report estimating the prevalence of HO in adult civilian amputees.

OBJECTIVE: To evaluate the stage of maturation and the metabolism of neurogenic heterotopic ossification by using SPECT/CT. METHODS: A total of 12 medical records of patients with spinal cord injury, all of them classified according to the ASIA protocol (disability scale from the American Spinal Injury Association) in complete lesion (A) and partial lesions (B, C and D) and registered at the Laboratory of Biomechanics and Rehabilitation of the Locomotor System, were submitted to SPECT/CT evaluation. RESULTS: Sixteen hips with heterotopic ossification observed in X-ray were studied and only two (12.5%) had high osteoblastic activity. Five hips showed medium activity, three (18.75%) low activity and six (37.5%) did not present any activity detected by SPECT/CT. CONCLUSION: SPECT/CT helps to determinate which patients have a greater risk of relapse after surgical resection, proving to be a useful imaging study in preoperative evaluation that can be used to determinate the postoperative prognosis of these patients. The purpose for the present study was to determine the frequency of sella turcica bridge and clinoid enlargement on lateral cephalometric plain film radiography from peruvians patients who attended our radiological center and identify any possible genre or age group associations. The sella turcica from 417 lateral cephalometric plain film radiography was evaluated in order to determine the presence of sella turcica bridge (fusion between the anterior and posterior clinoid process) or clinoid enlargement related to the Sella (S) cephalometric point. The sella turcica bridge and clinoid enlargement frequency was 4.31percent (n=18), and 16.3 percent (n=68). Cramer´s V and ETA statistics found, by separately, non significant association (p > 0.05) between sella turcica bridge and clinoid enlargement with respect to sex, and age groups (under 18 and over 18). The sella turcica bridge and clinoid enlargement are not so infrequent findings in the population studied, our results are similar to those reported in the literature. The existence of a sella turcica bridge or a clinoid enlargement must be considered as a structural anomaly that may influence the prognosis of surgical procedures of the sella turcica.
El propósito del estudio fue determinar la frecuencia de puente selar y alargamiento clinoideo en radiografías laterales cefalométricas de pacientes peruanos que asistieron a nuestro centro radiográfico entre enero a mayo del 2011 e identificar posibles asociaciones de sexo ó edad. El área de la silla turca de 417 radiografías cefalométricas laterales fue evaluada para determinar la presencia de puente selar (fusión entre los procesos clinoides) o alargamiento de los procesos clinoides anterior o posterior en relación con el punto cefalométrico Sella (S). La frecuencia de puente selar fue de 4.31por ciento (n=18), la frecuencia de alargamiento clinoideo fue de 16.3 por ciento (n=68). Los análisis estadísticos V de Kramer y ETA no encontraron asociación significativa entre el puente selar y alargamiento clinoideo con respecto al sexo y a la edad (p>0.05). El puente selar y el alargamiento clinoideo no son hallazgos radiográficos tan infrecuentes en la población estudiada; nuestros resultados son similares a los encontrados en la literatura. La presencia del puente selar y alargamiento clinoideo debe tenerse en cuenta como una anomalía estructural que puede influir en el pronóstico de los procedimientos quirúrgicos en la zona de la silla turca.

Introdução: Osteoma cutâneo miliar múltiplo é doença rara, caracterizada por tecido ósseo ectópico na derme e/ou hipoderme. Usualmente ocorre na face, em pessoas entre 17 e 79 anos. A etiologia é ainda desconhecida. Relata-se caso de paciente do sexo feminino, apresentando placa papulosa de coloração castanho-escura, endurecida, acometendo regiões malares e mentoniana, mal delimitada, com superfície irregular, formada por lesões papulonodulares, hipercrômicas, algumas branco-amareladas, há dois anos, referindo acne, desde os 16 anos de idade, não tratada, tendo o diagnóstico sido confirmado histopatologicamente e apresentado resultado terapêutico satisfatório com uso de tretinoína creme 0,1%.
Summary: Multiple cutaneous miliary osteoma is a rare condition characterized by ectopic bone tissue in the dermis and/or hypodermis. It usually occurs on the face, in individuals of 17 to 79 years old. Its etiology is still unknown. The present article reports the case of a female patient affected by a hardened, poorly delimited, papular plaque of dark brown hue and irregular surface, affecting the malar and mentonian regions. The patient sought medical care two years before the publication of this study, describing untreated acne since the age of 16. The papular plaque was composed of hyperchromic papular nodular lesions some with white-yellowish hue. The diagnosis was confirmed histopathologically. Satisfactory therapeutic results were achieved with the use of 0.1% tretinoin cream.

OBJECTIVE: The spinal cord-injured patients begin to present a new configuration of forces on the joints. The hip joint is one of the most affected, because these patients generally use a wheelchair as a means of locomotion. Osteoarticular changes, such as heterotopic ossification, can be found in these patients, as evidenced by radiographic studies. This study aims to identify radiographic changes in hips of spinal cord-injured patients. METHODS: 15 patients (30 hips) were evaluated and followed up at the Laboratory of Biomechanical Rehabilitation of the Musculoskeletal System of HC-Unicamp, through the analysis of radiographs of the pelvis in anterior-posterior and Lowenstein lateral positions. RESULTS: Of the total hips, only seven (23%) had no evidence of articular damage. The prevalence of heterotopic ossification found (16.6%) was similar to the literature. CONCLUSION: The radiographic assessment of these patient's hips is justified by the prevalence of joint changes found. Level of Evidence II, Development of diagnostic criteria in consecutive patients (with universally applied reference "gold" standard).

Myositis ossificans progressiva is a rare autosomal dominant disease with less than 1,000 case reports. Such patients present edema, caused by inflammatory processes that progressively calcify, and with loss of mobility in the region affected. The objective of this study was to describe a case of myositis ossificans progressiva, present its clinical manifestations and discuss the treatments available (oral ascorbic acid and intravenous bisphosphonate).

OBJETIVO: Os pacientes lesado-medulares passam a se submeter a uma nova conformação de forças sobre as articulações. O quadril é uma das mais afetadas, por utilizarem a cadeira de rodas como meio de locomoção. Alterações osteoarticulares, como ossificação heterotópica, podem ser encontradas nesses pacientes, sendo evidenciadas por estudos radiográficos. Este estudo visa identificar a incidência das alterações radiográficas em quadris de paciente lesados medulares. MÉTODOS: Foram avaliados 15 pacientes (30 quadris) acompanhados no Laboratório de Reabilitação Biomecânica do Aparelho Locomotor do HC-Unicamp, analisando-se radiografias da bacia em posições antero-posterior e lowenstein. RESULTADOS: Dos quadris avaliados, apenas sete (23 por cento) não possuíam alguma evidência de dano à superfície articular. A prevalência de ossificação heterotópica encontrada (16,6 por cento) aproximou-se a da literatura. CONCLUSÃO: Devido à prevalência de alterações articulares encontradas, justifica-se o acompanhamento radiográfico dos quadris destes pacientes. Nível de Evidência II. Desenvolvimento de critérios diagnósticos em pacientes consecutivo. (com padrão de referência "ouro" aplicado).

A miosite ossificante progressiva é uma doença rara, com menos de 1.000 casos descritos, autossômica dominante. O paciente apresenta edemas, devidos a processos inflamatórios, que vão se calcificando, com perda da mobilidade da região afetada. O objetivo deste trabalho é descrever um caso de miosite ossificante progressiva, apresentando as manifestações clínicas e discutindo os tratamentos disponíveis (ácido ascórbico oral e bifosfonato endovenoso).

A fibrodisplasia ossificante progressiva (FOP) é uma doença genética rara, atinge um indivíduo a cada dois milhões de nascimentos e tem como principal consequência a ossificação heterotópica: formação de ossos extras em locais indevidos. Trata-se de uma doença autossômica dominante, geralmente resultado de uma mutação nova no gene do receptor ACVR1 no caminho da sinalização da proteína osteomorfogênica. Esta anormalidade não tem relação com sexo, etnia ou consanguinidade. O presente trabalho relata o caso de de A.C., 17 anos, cuja investigação iniciou-se aos quatro anos de idade, mas cujo diagnóstico de FOP foi firmado aos 15 anos, depois de passar por vários especialistas em diferentes centros. A paciente faz parte de uma família de três irmãos, cuja antecedência não revela casos semelhantes.
Progressive ossifying fibrodysplasia is a rare genetic disease that affects one individual in every two million births. Its main consequence is heterotopic ossification, i.e. formation of additional bone in abnormal locations. It is an autosomal dominant disease, usually caused by a new mutation in the ACVR1 receptor gene, which is in the signaling pathway for bone morphogenic protein. This abnormality is not related to gender, ethnicity or consanguinity. The present study reports the case of A.C., a 17-year-old girl. Her clinical investigation began at the age of four years, but she was only diagnosed with FOP at the age of 15 years, after being evaluated by several specialists in different centers. The patient has two siblings, but her family history did not reveal any similar cases.

OBJETIVO: apresentar o quadro clínico das pacientes com metaplasia óssea e avaliar os fatores de risco, as mudanças dos sinais e sintomas após a retirada do fragmento ósseo. MÉTODOS: foi realizado um estudo transversal com 16 pacientes diagnosticadas com fragmentos ósseos na cavidade uterina no período de julho de 2006 a janeiro de 2009. O critério de inclusão foi o achado de fragmento ósseo retirado da cavidade uterina. Todas as pacientes tiveram confirmação histológica de presença de tecido ósseo na cavidade endometrial. Obtivemos os dados de todas as pacientes antes e depois da retirada por meio de um questionário para avaliar o efeito da retirada sobre a sintomatologia das pacientes, além da pesquisa de possíveis fatores relacionados ao aparecimento da doença. RESULTADOS: metade das pacientes (8/16) tinha sintomas hemorrágicos e um terço (6/16) apresentava infertilidade. A retirada dos fragmentos foi efetiva na melhora das queixas, havendo desaparecimento dos sintomas em todos os casos de menorragia e dor pélvica. CONCLUSÃO: A retirada do fragmento ósseo pode restaurar a fertilidade em pacientes selecionadas e que tenham como causa a metaplasia óssea, além de ser bastante efetiva em proporcionar melhora nos casos que cursam com dor pélvica e menorragia.
PURPOSE: to describe the clinical signs and symptoms of patients with bone metaplasia and to assess the risk factors for changes in these symptoms after removal of the bone fragment. METHODS: a cross-sectional study was conducted on 16 patients with a diagnosis of bone fragments in the uterine cavity during the period comprising July 2006 to January 2009. The inclusion criterion was the detection of a bone fragment removed from the uterine cavity. The presence of bone tissue in the endometrial cavity was histologically confirmed in all patients. The data of all patients were obtained before and after removal by means of a questionnaire for the evaluation of the effect of removal on the symptoms and for the search of possible factors related to the onset of the disease. RESULTS: half the patients (8/16) had hemorrhagic symptoms and one third (6/16) were infertile. Removal of the fragments was quite effective in improving the complaints, with the disappearance of symptoms in all cases of hemorrhage and of pelvic pain. CONCLUSION: removal of bone fragments can restore the fertility of selected patients whose infertility is caused by bone metaplasia and is quite effective in leading to improvement in patients with pelvic pain and menorrhage.
