OBJECTIVES: The aim of this systematic review is to compare conventional peri-implant flap surgery and reconstructive surgical techniques regarding evidence of remission from peri-implantitis. MATERIAL AND METHODS: Searches were made among randomized controlled trials evaluating clinical aspects and the changes in marginal bone level before and after surgical treatment of peri-implantitis, with and without bone substitute. RESULTS: Nine published articles and 442 patients were eligible for inclusion in the study. Reconstructive techniques exhibited a greater extent of defect fill than conventional surgical techniques alone. No significant differences could be found for clinical measures of peri-implant disease (bleeding on probing and reduction of probing depth) from baseline to the 12-month follow-up. CONCLUSIONS: With regards to the clinical measures of disease, our review shows that there are no differences between open flap debridement and regenerative surgery. From an esthetic standpoint, it may however be that regenerative measures may lead to improvement but further publications with this focus will be necessary to verify this.

OBJECTIVE: In skull base surgery, postoperative complications may be lethal and it is important to know how to deal with these. There are several reports on complications after skull base reconstruction, but it is difficult to understand which are important for plastic surgeons in charge of reconstruction. The objective of this study is to clarify the early postoperative survival-related complications after skull base reconstruction of which plastic surgeons should participate in treatment. METHODS: One hundred and seventy-seven patients who underwent skull base reconstruction at our department over the last 12 years were retrospectively surveyed. The cases of early complications in which plastic surgeons were actively involved in treatment were investigated and the preventive measures actually taken at our facility are examined in addition to treatment methods after development. RESULTS: Plastic surgical complications were wound infection in seven patients, impaired blood flow in transplanted tissue in two patients, cerebrospinal fluid leakage in five patients, and intracranial compression in three patients. Total number of postoperative complications were seventeen and the total complication rate was 9.6%. Complication rates tended to be higher in anterior-middle skull base cases than in anterior or middle alone cases and higher with free tissue transfer than with locoregional flap. Multivariate analysis revealed that operation time was a significantly higher risk factor (p=0.012) and preoperative chemotherapy was a significantly lower risk factor (p=0.033) for the development of complications. It was also found that the hospitalization length was significantly longer when complications occurred (p<0.0001). Wound infection was treated with removal of the cause, sufficient drainage, and irrigation. Regarding preventive measures, it is necessary to pay attention especially to intracranial contamination with epithelial components. Rapid surgical measures were necessary when blood flow of the transplanted tissue was impaired. For pedicle flaps, conservation of the feeding vessel is needed, and for free flaps, reliable vascular anastomosis is needed as preventive measures. Plastic surgeons play a role in reducing the risk of cerebrospinal fluid leakage through covering the repaired dura mater with tissue with favorable blood flow, especially in reoperation after development of leakage. Skull base reconstruction with soft tissue may cause intracranial compression, and sometimes, it is not resolved by conservative treatment and reoperation is selected. CONCLUSION: Plastic surgical postoperative complications were clarified, and treatment methods and preventive measures for them were examined. Careful consideration of blood flow, placement, and volume of the transplanted flap is important to prevent complications.

Paciente masculino de 47 años de edad, con antecedentes de hipertensión arterial. En mayo de 2009 sufrió un accidente en su hogar con gasolina inflamada, lo que le produjo lesiones en cara, cuello anterior, tronco y extremidades. Estas alcanzaron el 67,7 por ciento de la superficie corporal total, de ellas el 51,3 por ciento fueron quemaduras hipodérmicas y el 16,4 por ciento dérmicas AB, para un índice de gravedad de 59,5 y pronóstico de vida de crítico extremo. Se decide ingreso en régimen de terapia intensiva del Servicio de Caumatología del Hospital Militar Central Dr Luis Díaz Soto, donde se le realizaron varios tratamientos quirúrgicos. Tuvo una estadía hospitalaria de 98 días. Al alta quedaron como secuelas posquemaduras, múltiples cicatrices hipertróficas y queloideas en todo su cuerpo, sindactilia en mano izquierda y microstoma. Regresó a nuestro servicio a los 2 años y 6 meses para tratamiento reconstructivo de cicatriz queloidea que interesaba mentón y labio superior, así como del microstoma. Se realizó la reconstrucción de la zona afectada mediante la combinación de plastias locales y a distancia. Se obtuvo satisfactorios resultados estéticos y funcionales, lo que demuestra que el tratamiento integral del paciente quemado garantiza la sobrevida, el control de la sepsis, la mejoría estética, funcional y de la esfera psíquica(AU)


A 47 year-old male patient with a history of hypertension suffered an inflamed gasoline accident at home in May 2009. This accident produced lesions on his face, anterior neck, trunk and limps. These burns reached 67.7 percent of the total body surface area, out of which 51.3 percent were hypodermic burns, and 16.4 percent dermal AB, for a 59.5 severity index and prognosis of extremely critical. His admission is decided in intensive care regimen of the Burn Therapy Service at the Central Military Hospital where various surgical treatments were performed. He had a hospital stay of 98 days. At discharge sequelae were left as post burns, multiple keloids and hypertrophic scars all over his body, syndactyly microstoma in his left hand. The patient returned to our service at 2 years and 6 months for reconstructive treatment of keloid scar affecting his chin and upper lip, and the microstoma. Reconstruction of the affected area was performed by combining local and distance plasty. Satisfactory aesthetic and functional results were obtained, demonstrating that comprehensive treatment of burn patients guarantees survival, sepsis control, aesthetic, functional and psychic sphere improvement(AU)
