RESUMO

The author proposes ways of rethinking the concepts of the unconscious and time in the analytic setting, including the very existence of the unconscious. Freud (1915) stated that the success psychoanalytic thinking has in making inferences about the patient's unconscious makes the existence of the unconscious "incontrovertible." The author submits that this success does not establish the existence of the unconscious; rather, the inferences we think we make about the unconscious are inferences about consciousness itself - the totality of our experiences of thinking, feeling, sensing, observing, and communicating with ourselves. The author then offers thoughts about a second analytic concept, the experience of time in the analytic setting. He conceives of there being two inseparable sorts of experiences of analytic time that stand in a dynamic relationship with one another: diachronic time (clock time) and synchronic time (dream time). In diachronic time, time is sequential; one thing leads to another. In synchronic time, all time is contained in the present. In analysis, childhood trauma is experienced for the first time (in synchronic time) in the co-created subjectivity of patient and analyst.

Recent efforts in growing two-dimensional (2D) multilayers have enabled the synthesis of single crystalline 2D multilayers in a wafer scale through the seamless stitching of multiple epitaxial 2D islands. Unlike previously observed wedding-cake or inverted-wedding-cake structures, these multilayer islands have the same size and shape in each layer with aligned edges. In this study, we investigated the underlying growth mechanisms of synchronic 2D multilayers growth and have showed that a heterogenous layer on a crystalline substrate is critical for maintaining the synchronic growth of 2D multilayers. During growth, the heterogenous layer passivates the edges of multilayer 2D island and thus prevents the coalescence of these active edges, while the high interfacial energy between the heterogenous surface layer and the substrate stabilizes the synchronic structure. Based on this model, we have successfully explained the previously observed synchronic growth of graphene and hexagonal boron nitride multilayers (Nat Nanotech 2020, 15: 861; Nature 2022, 606: 88). The deep understanding on the mechanism paves a way towards the synthesis of wafer-scale single-crystal 2D multilayers with a uniform thickness.

BACKGROUND: Genetic evaluation is essential in assessing colorectal cancer (CRC) and colorectal liver metastasis (CRLM). The aim of this study was to determine the pragmatic value of KRAS on oncological outcomes after CRLM according to the ESMO recommendations and to query whether it is necessary to request KRAS testing in each situation. METHODS: A retrospective cohort of 126 patients who underwent surgery for hepatic resection for CRLM between 2009 and 2020 were reviewed. The patients were divided into three categories: wild-type KRAS, mutated KRAS and impractical KRAS according to their oncological variables. The impractical (not tested) KRAS group included patients with metachronous tumours and negative lymph nodes harvested. Disease-free survival (DFS), overall survival (OS) and hepatic recurrence-free survival (HRFS) were calculated by the Kaplan-Meier method, and a multivariable analysis was conducted using the Cox proportional hazards regression model. RESULTS: Of the 108 patients identified, 35 cases had KRAS wild-type, 50 cases had a KRAS mutation and the remaining 23 were classified as impractical KRAS. Significantly longer medians for OS, HRFS and DFS were found in the impractical KRAS group. In the multivariable analyses, the KRAS mutational gene was the only variable that was maintained through OS, HRFS and DFS. For HRFS (HR: 13.63; 95% confidence interval (CI): 1.35-100.62; p = 0.010 for KRAS), for DFS (HR: 10.06; 95% CI: 2.40-42.17; p = 0.002 for KRAS) and for OS (HR: 4.55%; 95% CI: 1.37-15.10; p = 0.013). CONCLUSION: Our study considers the possibility of unnecessary KRAS testing in patients with metachronous tumours and negative lymph nodes harvested. Combining the genetic mutational profile (i.e., KRAS in specific cases) with tumour characteristics helps patient selection and achieves the best prognosis after CRLM resection.

Resumen Se presenta aplicación y evaluación del Protocolo Unificado Transdiagnóstico para los trastornos emocionales grupal, online sincrónico a estudiantes universitarios que solicitaron atención al Servicio de Atención Psicológica de la Universidad de Cádiz. Se implementó el Protocolo Unificado de ocho módulos desarrollado en 12 sesiones semanales con el objetivo de examinar su eficacia en un formato grupal online. Los objetivos específicos fueron los siguientes: Incrementar la calidad de vida, el optimismo y el afecto positivo y disminuir la sintomatología depresiva y ansiosa, el afecto negativo, las conductas rumiativas, el malestar psicológico y la evitación experiencial. La investigación tuvo un enfoque cuantitativo, exploratorio, descriptivo e interactivo, con un diseño cuasi-experimental, pre-post test para muestras pareadas. Los instrumentos de evaluación fueron los siguientes: OASIS, ODSIS, QLI-Sp, COP, PANAS, RRS, CORE-OM, BEAQ, ficha sociodemográfica ad hoc y una Escala de satisfacción/opinión. Participaron nueve estudiantes con una edad promedio de 21.11. Se hallaron diferencias estadísticamente significativas entre las mediciones pre y pos test en afecto negativo (t = 3.250; p < .012) y en la escala Riesgo del CORE OM (t = 2.401; p < .043). La aplicación del PU en formato grupal online resultó eficaz en el contexto no clínico aplicado: los participantes evidenciaron un progreso en el tratamiento y mejoras en las variables en estudio.


Abstract: This study presents the results of the application of the Unified Transdiagnostic Protocol Treatment of Emotional Disorders in a group format in a non-clinical context, with university students who have asked for therapy at the University Psychological Services. The standardized eight modules-UP was implemented and it was administered in twelve group sessions. Synchronous online weekly and 90-minute long sessions were carried out. UP is an evidence-based cognitive behavioral intervention program to address a wide range of emotional disorders. The general target was to examine the efficacy of UP for the Transdiagnostic Treatment of Emotional Disorders in an online group format. The specific targets were: (a) to increase quality of life, optimism and positive affect; and (b) to reduce depressive and anxious symptoms, negative affect, ruminant behaviors, psychological distress and experiential avoidance. This study was carried out with a quantitative, exploratory, descriptive and interactive approach. A quasi-experimental design, pre- and posttest for paired samples was used. Instruments: OASIS, ODSIS, QLI-Sp, COP, PANAS, RRS, CORE-OM, BEAQ, ad hoc socio-demographic file and Opinion Scale. A total of 9 students voluntarily participated in the study (Mean age = 21.11 years). After the intervention, statistically significant diffe- rences were found between the pre- and posttest measurements in negative affect (t = 3.250; p < .012) and in the CORE OM´ Risk Scale (t = 2.401; p < .043). The UP application in a group format with students that asked for therapy has proven to be effective in a non-clinical context, participants have shown progress in the treatment as well as in some psychological areas.

Introducción: La metástasis hepática del cáncer de colon es una entidad frecuente. Objetivo: Identificar los factores asociados a la supervivencia en pacientes intervenidos de cáncer de colon con metástasis hepática sincrónica. Métodos: Se realizó un estudio observacional, descriptivo y transversal a 34 pacientes operados con este diagnóstico, en el servicio de Cirugía General del Hospital Provincial Docente "Saturnino Lora" de Santiago de Cuba, desde 2010 hasta 2019. Fue estimada la supervivencia global y por diferentes periodos de tiempo en la cohorte, según el método de Kaplan-Meier. La comparación de las curvas de supervivencia para las covariables seleccionadas se realizó mediante la prueba de igualdad de distribuciones (Log-Rank de Mantel y Cox). Resultados: La supervivencia global a los seis meses y al año del diagnóstico fue de 66,5 por ciento y 66,2 por ciento, respectivamente, con mediana de dos años (IC 95 por ciento: 0,97-3,02). La supervivencia fue superior si el paciente presentó una metástasis versus dos o más, así como la de ambos lóbulos marca la menor probabilidad de supervivencia. Los tumores bien y moderadamente diferenciados mostraron mayor probabilidad de supervivencia al año que los pocos diferenciados, sin diferencias significativas. Conclusiones: La cirugía permite mejorar la sobrevida global y libre de enfermedad, aunque el uso de las distintas opciones terapéuticas para el cáncer colónico con metástasis hepática sincrónica continúa controvertido. La supervivencia de estos enfermos está condicionada por la estadificación, diferenciación histológica del tumor, localización y número de metástasis, entre otros factores(AU)


Introduction: Liver metastasis from colon cancer is a frequent entity. Objective: To identify the factors associated with survival in patients operated on for colon cancer with synchronic liver metastasis. Methods: An observational, descriptive and cross-sectional study was carried out, from 2010 to 2019, with 34 patients with this diagnosis operated on in the general surgery service of Saturnino Lora Provincial Teaching Hospital of Santiago de Cuba. Overall survival was estimated, as well as by different time periods in the cohort, using the Kaplan-Meier method. The comparison of the survival curves for the selected covariates was carried out using the test of equality of distributions (log-rank or Mantel-Cox test). Results: Overall survival six months and one year after diagnosis was 66.5 percent and 66.2 percent, respectively, with a median of two years (95 percent CI: 0.97-3.02). Survival was higher if the patient had one metastasis versus two or more, while metastasis in both lobules represents the lowest probability of survival. Well and moderately differentiated tumors showed higher probability of survival at one year than the little differentiated ones, without significant differences. Conclusions: Surgery improves overall and disease-free survival, although the use of different therapeutic options for colon cancer with synchronic liver metastases remains controversial. The survival of these patients is conditioned by staging, histological differentiation of the tumor, location and number of metastases, among other factors(AU)

"I just couldn't control myself" are the infamous last words of a person that did something that they knew they should not have done. Consistent self-control is difficult to achieve, but it is also instrumental in achieving ambitious goals. Traditionally, the key to self-control has been assumed to reside in the brain. Recently, an alternative has come to light through the emergence of situated theories of self-control, which emphasize the causal role of specific situated factors in producing successful self-control. Some clinical interventions for motivational or impulse control disorders also incorporate certain situated factors in therapeutic practices. Despite remaining a minority, situated views and practices based on these theories have planted the seeds of a paradigm shift in the self-control literature, moving away from the idea that self-control is an ability limited to the borders of the brain. The goal of this paper is to further motivate this paradigm shift by arguing that certain situated factors show strong promise as genuine causes of successful self-control, but this potential role is too often neglected by theorists and empirical researchers. I will present empirical evidence which suggests that three specific situated factors - clenched muscles, calming or anxiety-inducing environmental cues, and social trust - exhibit a specialized effect of increasing the likelihood of successful self-control. Adopting this situated view of the ability to regulate oneself works to reinforce and emphasize the emerging trend to design therapies based on situated cognition, makes self-control more accessible and less overwhelming for laypeople and those who struggle with impulse control disorders, and opens a new avenue of empirical investigation.

Introducción: El cáncer de colon se erige como la neoplasia del tubo digestivo más frecuente en la presente centuria. Objetivo: Identificar algunos factores clínicos, epidemiológicos y diagnósticos en pacientes operados de cáncer de colon con metástasis hepática sincrónica. Métodos: Se realizó un estudio observacional, descriptivo y transversal de una muestra de 31 pacientes operados con diagnóstico definitivo de cáncer de colon con metástasis hepática sincrónica, en el servicio de Cirugía General del Hospital Provincial Docente "Saturnino Lora" de Santiago de Cuba durante el periodo comprendido entre 2010 y 2019. Resultados: La edad promedio fue de 63,2 años. Existió predominio de la enfermedad inflamatoria intestinal como antecedente patológico personal, y del alcoholismo como factor de riesgo. La sintomatología predominante fue dolor abdominal y cambios del hábito intestinal, así como el tumor palpable al examen físico del abdomen. La ecografía abdominal y el colon por enema fueron los procederes de diagnóstico más utilizados. Prevalecieron los tumores en el colon izquierdo a nivel del descendente. Todos los tumores malignos fueron adenocarcinomas a predominio de los moderadamente diferenciados. Conclusiones: Las edades avanzadas de la vida, así como la presencia de tabaquismo y alcoholismo son factores epidemiológicos característicos de la población de enfermos aquejados de cáncer de colon con metástasis hepática. Los elementos clínicos identificados constituyeron los habitualmente descritos en la literatura médica, aunque los estudios imaginológicos utilizados preoperatoriamente resultaron limitados para el diagnóstico del cáncer de colon con metástasis hepática sincrónica, precisándose el hallazgo de las lesiones metastásicas durante la intervención quirúrgica(AU)


Introduction: Colon cancer is the most frequent digestive-tract neoplasm in the present century. Objective: To identify some clinical, epidemiological and diagnostic factors in patients operated on for colon cancer and synchronic hepatic metastasis. Methods: An observational, descriptive and cross-sectional study was carried out in a sample of 31 patients operated on with a definitive diagnosis of colon cancer and synchronic hepatic metastasis, in the general surgery service of Saturnino Provincial Teaching Hospital in Santiago de Cuba, during the period between 2010 and 2019. Results: The average age was 63.2 years. There was a predominance of inflammatory intestinal disease as a personal pathological antecedent, as well as alcoholism as a risk factor. The predominant symptoms were abdominal pain and changes in intestinal habits, as well as a tumor palpable on physical abdominal examination. Abdominal ultrasound and lower barium enema were the most used diagnostic procedures. Tumors prevailed at the level of the left descending colon. All malignant tumors were adenocarcinomas, predominantly moderately differentiated ones. Conclusions: Advanced ages of life, as well as smoking and alcoholism are characteristic epidemiological factors among the population of patients suffering from colon cancer with hepatic metastases. The clinical elements identified were those usually described in the medical literature, although the imaging studies used preoperatively were limited for the diagnosis of colon cancer with synchronic hepatic metastasis, a fact that required finding metastatic lesions during surgery(AU)

OBJECTIVE: To describe the experience of physicians and patients in the Telepsychiatry programme at the University of Antioquia's Faculty of Medicine in the first 12 months after its implementation in eight towns across Antioquia. METHODOLOGY: A descriptive study involving the evaluation of 111 patients during the programme's first year. An instrument was designed to evaluate patients' satisfaction and the Technology Acceptance Model (TAM) was used to evaluate the health professionals' satisfaction. RESULTS: 111 patients were seen on 340 occasions. 70 out of the 111 patients (63.1%) were seen by Telepsychiatry at least twice in the first year of implementation. A sample of 38 patients (34%) was used to evaluate their experience, of which 94.7% said their problem had been solved and 100% were highly satisfied. Nine health professionals took part in the programme, who agreed that the technology was useful and easy to use. They also stated that they wanted to continue using it. CONCLUSION: Health systems across the globe have failed to provide an adequate response to the mental health burden. Therefore, strategies such as telepsychiatry are considered an ideal treatment modality to give patients living in remote locations the specialised attention that they need.

Objetivo: Describir la experiencia que han tenido los médicos y pacientes del programa de Telepsiquiatría de la Facultad de Medicina de la Universidad de Antioquia en los primeros 12 meses de su implementación en 8 municipios del departamento. Métodos: Estudio descriptivo, que incluyó a 111 pacientes atendidos durante el primer año. Se diseñó un instrumento para evaluar la satisfacción de los pacientes y se empleó el instrumento de aceptación de tecnología para evaluar la de los profesionales de la salud. Resultados: Se realizaron 340 atenciones durante ese periodo a 111 pacientes; 70 (63,1%) de ellos recibieron al menos 2 atenciones por telepsiquiatría en el primer año. Se evaluó la experiencia en una muestra de 38 pacientes (34%), quienes manifestaron la resolución del problema (94,7%) y una satisfacción alta (100%). En el programa participaron 9 profesionales de la salud, que estuvieron de acuerdo en que la tecnología es útil y fácil de usar y tienen la intención de seguir usándola. Conclusiones: Los sistemas de salud de todo el mundo no han dado una respuesta adecuada a la carga de trastornos mentales; por esto, estrategias como la telepsiquiatría se consideran una modalidad de atención ideal para personas que viven en lugares remotos y tienen dificultad de acceso a los servicios de salud especializados, con adecuada aceptación.

Objective: Synchronic colorectal peritoneal carcinomatosis (SCRPC) was recognized as a predictor of poor prognosis. The aim of this study was to investigate the role of neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR) and systemic immune-inflammation index (SII) on the survival outcome, which might help determine the treatment management of SCRPC patients. Methods: A total of 103 SCRPC patients following cytoreduction surgery (CRS) and systematic chemotherapy (CT) between 1997 and 2013 in the First Affiliated Hospital of Sun Yat-sen University were retrospectively analyzed. The comparison of the clinicopathological variables and systematic inflammatory biomarkers, including NLR, PLR and SII, was performed by Chi-test and Cox regression analysis. According to the results of multivariate analysis, a prognostic nomogram was generated, and its prediction ability was measured by the concordance index (C-index). The survival curves were generated using the Kaplan-Meier method and survival comparison between groups was conducted via the log-rank test. Results: Univariate analysis revealed that elevated NLR, PLR and SII were significantly correlate with worse survival outcome. Only low SII value was recognized as an independent favorable prognostic factor for overall survival (HR=1.772, 95% CI=1.015-3.095, P=0.044), except for NLR and PLR. The nomogram could perform well in the prediction of overall survival in SCRPC patients (c-index 0.782). Moreover, SII had strong prognostic discriminatory ability to predict survival outcome for the patients receiving completeness of cytoreduction score (CCR) 0/1 or CCR2/3, rather than NLR and PLR. Conclusions: SII was a better inflammation factor to predict the outcomes of SCRPC patients receiving CRS and systematic CT. Low SII value was the most favorable factor benefiting from different level of CRS and it was useful for determining the appropriate treatment strategy for SCRPC patients.

The present work proposes the use of total synchronous fluorescence spectroscopy (TSFS) as a discrimination methodology for fluorescent compounds in edible oils, which are preserved after the transesterification processes in the biodiesel production. In the same way, a similar study is presented to identify fluorophores that do not change in expired vegetal oils, to associate physicochemical parameters to fluorescent measures, as contribution to a fingerprint for increasing the chemical knowledge of these products. The fluorescent fingerprints were obtained by Tucker3 decomposition of a three-way array of the total synchronous fluorescence matrices. This chemometric method presents the ability for modeling non-bilinear data, as Total Synchronous Fluorescence Spectra data, and consists in the decomposition of the three way data arrays (samples×Δλ×λ excitation), into four new data matrices: A (scores), B (profile in Δλ mode), C (profile in spectra mode) and G (relationships between A, B and C). In this study, 50 samples of oil from soybean, corn and sunflower seeds before and after its expiration time, as well as 50 biodiesel samples obtained by transesterification of the same oils were measured by TSFS. This study represents an immediate application of chemical fingerprint for the discrimination of non-expired and expired edible oils and biodiesel. This method does not require the use of reagents or laborious procedures for the chemical characterization of samples.

RESUMO

The transition from chemistry to biology is an extremely complex issue because of the huge phenomenological differences between the two domains and because this transition has many different aspects and dimensions. In this paper, I will try to analyze how chemical systems have developed a cohesive, self-maintaining and functionally differentiated system that recruits its organization to stay far from equilibrium. This organization cannot exist but in an individualized form, and yet, it unfolds both a diachronic-historical and a synchronic collective dimension. I will argue that, far from being a problem, these different dimensions of the phenomenon of life, appear as a consequence of the nature of this individualized organization.

Heterochrony is an enabling concept in evolution theory that metaphorically captures the mechanism of biologic change due to mechanisms of growth and development. The spatio-temporal patterns of morphogenesis are determined by cell-to-cell signaling mediated by specific soluble growth factors and their cognate receptors on nearby cells of different germline origins. Subsequently, down-stream production of second messengers generates patterns of form and function. Environmental upheavals such as Romer's hypothesized drying up of bodies of water globally caused the vertebrate water-land transition. That transition caused physiologic stress, modifying cell-cell signaling to generate terrestrial adaptations of the skeleton, lung, skin, kidney and brain. These tissue-specific remodeling events occurred as a result of the duplication of the Parathyroid Hormone-related Protein Receptor (PTHrPR) gene, expressed in mesodermal fibroblasts in close proximity to ubiquitously expressed endodermal PTHrP, amplifying this signaling pathway. Examples of how and why PTHrPR amplification affected the ontogeny, phylogeny, physiology and pathophysiology of the lung are used to substantiate and further our understanding through insights to the heterochronic mechanisms of evolution, such as the fish swim bladder evolving into the vertebrate lung, interrelated by such functional homologies as surfactant and mechanotransduction. Instead of the conventional description of this phenomenon, lung evolution can now be understood as adaptive changes in the cellular-molecular signaling mechanisms underlying its ontogeny and phylogeny.

INTRODUCTION: Mutations in KRAS and NRAS genes are negative predictors of anti-EGFR therapies response in metastatic colorectal cancer. There are few reports on RAS testing in synchronous primary colorectal cancer (SP-CRC) and a lack of recommendations on which tissue should be tested for the mutation in this disease. This study analyzed the RAS status of both lesions in SP-CRC patients and in their metastasis. MATERIALS AND METHODS: DNA was obtained from formalin-fixed-paraffin-embedded tissue, and mutations were analyzed by pyrosequencing. RESULTS: RAS status was heterogeneous in 6 (75%) of 8 SP-CRC patients between primary lesions. Five showed heterogeneity regarding RAS mutational status, and from these, four presented with metastasis: 3 cases (75%) had WT metastatic tissue, and 1 case (25%) had mutated metastatic tissue. One patient showed divergence regarding RAS mutation type. DISCUSSION: RAS mutations vary significantly between SP-CRC lesions, and the status of the metastasis is unpredictable. Testing for RAS mutations in only 1 of the primary lesions can misguide clinical decisions and hind the predictive potential of anti-EGFR treatment. A more appropriate approach in metastatic SP-CRC is to test the metastatic tissue or both primary lesions for providing more accurate mutation scenery and support more assertive clinical decisions.

Introduction: Bilateral synchronous breast cancer (BSBC) has a variable incidence and conflicting data on prognosis. Our goal is to evaluate the characteristics and survival of patients with BSBC treated at Cancer Center of the Pontificia Universidad Católica de Chile. Patients and Methods: Descriptive study. We identified patients treated between January 1999 and May 2013. We evaluated and compared characteristics from BSBC patients with a local cohort of non-synchronous breast cancer. Results: The incidence of BSBC was 1.8 percent (n = 28). Three patients were excluded because of missing data. Therefore we analyzed 25 patients (50 tumors). Median age: 50 years (33-84). The dominant tumor's diagnosis was clinical in 47.6 percent of cases, and imaging in 90.5 percent of the contra-lateral non-dominant (p = 0.04). Average tumor size of invasive tumors was 2.5 cm for dominant tumor versus 1.7 cm for the non-dominant (p = 0.02). 81.4 percent of invasive tumors were estrogen receptor (ER) positive. Histological and subtype concordance between both tumors was 88 percent and 72 percent respectively. 54 percent of patients with BSBC were managed with total mastectomy versus 28.4 percent in the control group no BSBC (p = 0.0001). The estimated overall survival at 10 years was 76.5 percent in patients with no BSBC versus 62 percent in those with BSBC (p = 0.08). Conclusions: The BSBC is rare. Frequently they are ER positive tumors, mostly diagnosed through images and managed with less conservative surgery.


Introducción: La incidencia y pronóstico del Cáncer de mama (CM) bilateral sincrónico (CMBS), no son bien conocidos. Evaluamos las características y sobrevida de pacientes con CMBS tratadas en el Centro de Cáncer de la Pontificia Universidad Católica de Chile. Pacientes y Método: Estudio descriptivo. Identificamos pacientes con CMBS tratadas entre enero de 1999 y mayo de 2013. Evaluamos sus características y las comparamos con una cohorte local de CM no sincrónico. Resultados: La incidencia de CMBS fue de 1,8 por ciento (n = 28). Se excluyeron 3 pacientes por falta de datos. Analizamos por tanto, 25 pacientes (50 tumores). Mediana de edad: 50 años (33-84). El diagnóstico del tumor dominante fue clínico en el 47,6 por ciento de los casos, y por imágenes en 90,5 por ciento de los contra-laterales no dominantes (p = 0,04). Tamaño tumoral promedio de los tumores invasores fue de 2,5 cm en tumores dominantes vs 1,7 cm en los no dominantes (p = 0,02). Un 81,4 por ciento de los tumores invasores fueron receptor de estrógenos (RE) positivos. La concordancia histológica y de subtipo histológico entre ambos tumores fue de 88 por ciento y 72 por ciento respectivamente. Un 54 por ciento de las pacientes con CMBS fueron manejadas con mastectomía total versus un 28,4 por ciento en el grupo control de CM no BS (p = 0,0001). La sobrevida global estimada a 10 años fue de un 76,5 por ciento en pacientes con CM no BS versus 62 por ciento en aquellos con CMBS (p = 0,08). Conclusiones: El CMBS es poco frecuente. Usualmente son tumores RE positivos que reciben con menos frecuencia manejo conservador de la mama.

Introducción: en Cuba, el cáncer colorrectal constituye la tercera causa de mortalidad después del cáncer de pulmón y próstata. Se realizó este trabajo con el objetivo de caracterizar endoscópica e histológicamente a los pacientes con cáncer colorrectal diagnosticados en el hospital estudiado. Métodos: se realizó un estudio descriptivo con 76 informes de videocolonoscopia realizadas en el Hospital Universitario Clínico Quirúrgico Comandante Faustino Pérez Hernández, desde junio de 2006 a junio de 2010, lo que constituyó el universo de trabajo. Se excluyeron los informes donde el proceder no pudo realizarse completo o no presentaban el resultado histológico de la lesión diagnosticada. Se analizaron las variables: grupo etario, sexo, localización de la lesión, aspecto endoscópico, variedad histológica y presencia de lesiones sincrónicas, estas fueron representadas en tablas y gráficos por frecuencias absolutas y relativas. Resultados: el grupo etario más frecuente fue de 50 a 64 años, con predominio del sexo masculino (56,6 %). La localización más frecuente fue en el colon rectosigmoide (34,2 %). El aspecto endoscópico más encontrado fue el ulcerado (63,2 %). La variedad histológica más frecuente fue el adenocarcinoma moderadamente diferenciado (52,6 %), predominando la presencia de adenomas sincrónicos (43,4 %). Conclusiones: los pacientes mayores de 50 años siguen siendo los más propensos a padecer de cáncer colorrectal. El aspecto endoscópico ulcerado, localizado en el rectosigmoide, histológicamente diagnosticado como adenocarcinoma moderadamente diferenciado, responden a un diagnóstico tardío.


Introduction: in Cuba, colorectal cancer is the third cause of mortality after lung and prostate cancer. We developed our work with the objective of arriving to an endoscopic and histological characterization of the patients with colorectal cancer diagnosed in the mentioned hospital. Method: we carried out a descriptive study with 76 reports of videocolonoscopies made in the Teaching Clinico-surgical Hospital Comandante Faustino Pérez Hernández, from June 2006 to June 2010, and that was the universe of our work. We excluded the reviews where the procedure could not be completely finished or that were lacking of the diagnosed lesion histological result. We analyzed the following variables: age group, gender, lesion localization, endoscopic aspect, histological variety and presence of synchronic lesions. They were represented in tables and charts showing the absolute and relative frequencies. Results: the most frequent age group was the one comprehending people aged 50-64 years old, with predomination of the male gender (56,6 %). The most frequent localization was the rectosigmoid colon (34,2 %). The most common endoscopic aspect we found was the ulcerated one (63,2 %). The most frequent histological variety was the moderately differenced adenocarcinoma (52,6 %), predominating the synchronic adenomas (43,4 %). Conclusions: patients elder 50 years old are still the ones who tend more of suffering from colorectal cancer. The ulcerated endoscopic aspect, found in the rectosigmoid colon, histologically diagnosed as moderately differentiated carcinoma, is the result of a late diagnosis.

Se presenta el caso de una mujer de 60 años que ingresó al Hospital General de México con diagnóstico de cirrosis hepática criptogénica y tumor hepático de estirpe no determinada, quien falleció por hemorragia digestiva alta secundaria a várices esofágicas. En la autopsia se encontró un hepatocarcinoma y un mixoma auricular izquierdo con elementos glandulares. Se hace una revisión de la literatura, buscando la asociación de estas dos neoplasias entre sí, y con otros tumores.


The case of a 60-year-old woman with history of cryptogenic hepatic cirrhosis and a probable hepatic tumor, who died of upper gastrointestinal bleeding secondary to esophageal varices, is presented. An autopsy study was carried out, which showed a hepatocellular carcinoma and a left atrial cardiac myxoma with glandular elements. A review of the literature, searching for the association between these two tumors, in addition to other tumors, is presented.
