Background: The role of copper in cancer treatment is multifaceted, with copper homeostasis-related genes associated with both breast cancer prognosis and chemotherapy resistance. Interestingly, both elimination and overload of copper have been reported to have therapeutic potential in cancer treatment. Despite these findings, the exact relationship between copper homeostasis and cancer development remains unclear, and further investigation is needed to clarify this complexity. Methods: The pan-cancer gene expression and immune infiltration analysis were performed using the Cancer Genome Atlas Program (TCGA) dataset. The R software packages were employed to analyze the expression and mutation status of breast cancer samples. After constructing a prognosis model to separate breast cancer samples by LASSO-Cox regression, we examined the immune statement, survival status, drug sensitivity and metabolic characteristics of the high- and low-copper related genes scoring groups. We also studied the expression of the constructed genes using the human protein atlas database and analyzed their related pathways. Finally, copper staining was performed with the clinical sample to investigate the distribution of copper in breast cancer tissue and paracancerous tissue. Results: Pan-cancer analysis showed that copper-related genes are associated with breast cancer, and the immune infiltration profile of breast cancer samples is significantly different from that of other cancers. The essential copper-related genes of LASSO-Cox regression were ATP7B (ATPase Copper Transporting Beta) and DLAT (Dihydrolipoamide S-Acetyltransferase), whose associated genes were enriched in the cell cycle pathway. The low-copper related genes scoring group presented higher levels of immune activation, better probabilities of survival, enrichment in pathways related to pyruvate metabolism and apoptosis, and higher sensitivity to chemotherapy drugs. Immunohistochemistry staining showed high protein expression of ATP7B and DLAT in breast cancer samples. The copper staining showed copper distribution in breast cancer tissue. Conclusion: This study displayed the potential impacts of copper-related genes on the overall survival, immune infiltration, drug sensitivity and metabolic profile of breast cancer, which could predict patients' survival and tumor statement. These findings may serve to support future research efforts aiming at improving the management of breast cancer.

La enfermedad de Wilson es una condición genética autosómica recesiva poco frecuente. Se ha identificado el gen ATP7B como el que codifica la proteína transportadora de cobre y su deficiencia lleva al acúmulo del metal en el cerebro, hígado y otros órganos vitales. Su diagnóstico clínico precoz es esencial para mejorar la calidad de vida del paciente. A continuación, se presenta el caso de un paciente de 20 años, masculino, con un cuadro clínico de 2 años de evolución de desinhibición, impulsividad, anartria y apraxia de la marcha, movimientos distónicos faciales y en 4 extremidades. Al examen físico se evidenció el anillo de Kayser Flescher a nivel ocular. En Resonancia Magnética Encefálica hiperintensidad en ganglios de la base y mesencéfalo en T2. Ceruloplasmina en suero 4.08 mg/dL. Cobre sérico 26.03ug/dL y cobre en orina de 24 horas 224.30ug/ 24h. Se confirma el diagnóstico de Enfermedad de Wilson, tratándose con D- Penicilamina, evidenciándose una evolución adecuada, con mejoría notable del cuadro neurológico. El tratamiento precoz permite una evolución favorable temprana del paciente, disminuyendo las secuelas neurológicas secundarias a la enfermedad; de ahí la importancia del reporte del presente caso.(AU)


BackgroundWilson's disease is a rare autosomal recessive genetic condition. The ATP7B gene has been identified as the one that encodes the copper transport protein and its deficiency leads to the accumulation of metal in the brain, liver and other vital organs. Your early clinical diagnosis is essential to improve the quality of life of the patient. Following we present the clinical case of a 20-year-old male patient who since 2 years ago, presented disinhibition, impulsivity, anartria and gait apraxia, facial dystonic movements and in extremities. To the physical exam, Kayser Flescher ring was present. In Brain Magnetic Resonance hyperintensity in Basal Ganglia and Midbrain. Serum Ceruloplasmin 4.08. Serum Copper 26.03. Urinary Cupper 224.30. The diagnosis of Wilson's disease is confirmed, treating with D-Penicillamine, evidencing an adequate evolution, with notable improvement of the neurological symptoms. Early treatment allows a favorable early evolution of the patient, reducing the neurological sequelae secondary to the disease; so that the importance of the report of this case.(AU)

Objective: To investigate the relationship of heterogeneity of esophageal squamous cell carcinoma (ESCC) and chemotherapy sensitivity. Methods: Five different region specimens isolated from primary tumor(R1~R5)and 1 specimen(R6)isolated from adjacent non-neoplastic tissue from 10 ESCC patients who underwent surgical treatment were cultured in vitro. The inhibitory effect of cisplatin on proliferation of ESCC cells from different regions was determined by methyl thiazolyl tetrazolium (MTT). The cell cycle and apoptosis induced by cisplatin was determined by flow cytometry (FCM) analysis. The mRNA levels of ATP7A and ATP7B were determined by quantitive RT-PCR (qRT-PCR). Results: The result showed that different regions of each specimen exhibited different chemotherapy sensitivity to cisplatin, and the cell survival rates of region R6 of each specimen were higher than other regions from the same specimen. The cell survival rate of region R3 from the tenth specimen was (81.42±8.84)%, which is significantly higher than (11.90±2.75)% of region R5 (P<0.01). FCM analysis showed that significant differences of early apoptosis and later apoptosis were observed in six specimens induced by cisplatin (P<0.05), and significant differences of cell cycle and G(1) period were observed in seven specimens (P<0.05). The qRT-PCR results showed that the mRNA level of ATP7A in region R1, R2, R3, R4 and R5 was (100.00±3.42)%, (118.10±2.21)%, (75.40±4.15)%, (95.40±3.32)% and (41.70±2.57)%, respectively, with significant differences (P<0.05). The mRNA level of ATP7A in region R6 was (175.20±5.32)%, significantly higher than those of regions from R1 to R5 (P<0.05). The mRNA level of ATP7B in region R1, R2, R3, R4 and R5 was (100.00±4.89)%, (73.60±2.65)%, (175.60±6.12)%, (46.10±4.62)% and (363.70±8.67)%, respectively, with significant differences (P<0.05). The mRNA level of ATP7B in region R6 was (1 165.40±7.25)%, significantly higher than those of regions from R1 to R5 (P<0.05). Conclusion: The intratumor heterogeneity of ESCC results in the heterogeneity of resistance to cisplatin, which affects the chemotherapeutic effect.
