RESUMO

In left bundle branch block (LBBB), the ventricles are activated in a sequential manner with alterations in left ventricular mechanics, perfusion, and workload resulting in cardiac remodeling. Underlying molecular, cellular, and interstitial changes manifest clinically as changes in size, mass, geometry, and function of the heart. Cardiac remodeling is associated with progressive ventricular dysfunction, arrhythmias, and impaired prognosis. Clinical and diagnostic notions about LBBB have evolved from a simple electrocardiographic alteration to a critically important finding affecting diagnostic and clinical management of many patients. Advances in cardiac magnetic resonance imaging have significantly improved the assessment of patients with LBBB and provided additional insights into pathophysiological mechanisms of left ventricular remodeling. In this review, we will discuss the epidemiology, etiologies, and electrovectorcardiographic features of LBBB and propose a classification of the conduction disturbance.

BACKGROUND: As the incidence of left bundle branch blocks (LBBBs) and paced-rhythms electrocardiograms (ECGs) increase in the aging global population, the need for rapid and accurate diagnosis of ST-elevation myocardial infarction (STEMI) or STEMI equivalents in patients with these rhythms becomes more imperative. The Sgarbossa and Smith-modified Sgarbossa criteria have been documented to enhance the diagnosis of STEMI in the setting of LBBBs. However, there is a growing body of literature that suggests that these criteria can also be applied for the diagnosis of STEMI in patients with paced rhythms. CASE REPORT: We present the case of an 84-year old man who was on admission for cellulitis when he developed acute respiratory distress. An ECG revealed findings that were consistent with positive Smith-modified Sgarbossa criteria, upon which the diagnosis of STEMI was made. WHY SHOULD AN EMERGENCY PHYSICIAN BE AWARE OF THIS?: Although the sensitivity and specificity of these criteria in paced rhythms is not well documented, if a patient meets these criteria in an appropriate clinical setting, cardiac catheterization laboratory activation by an emergency physician could be appropriate.

AIM: LBBB in heart failure patients has prognostic significance. Subtypes of LBBB (concordant and discordant) have not been considered when considering management. The aim of this study is to explore the clinical difference between the two subtypes. METHODS: 216 patients with LBBB were included and categorized into concordant (LBBBC) and discordant (LBBBD) groups. RESULTS: Of the 216 patients (age 69.13±11.7; 56% male 44% female), 133 (61.5%) were LBBBD and 83 (38.5%) were LBBBC. LBBBD patients presented with lower LVEF (mean 36% vs 51%; P<0.001), wider QRS (mean 160 ms vs 151 ms; P<0.001), larger LA (mean 45 cm(2) vs 40 cm(2); P<0.001), moderate to severe mitral and tricuspid regurgitation (17% vs 3%; P<0.05, 10% vs 1%; P<0.05 respectively), CKD (41% vs 18%; P<0.001), COPD (4.6% vs 0%; P<0.01), CAD (67% vs 36%; P<0.001), and CABG (39% vs 16%; P<0.001). CONCLUSION: LBBBD is significantly associated with worse cardiac function and clinical characteristics.

BACKGROUND: The Selvester QRS score consists of a set of electrocardiographic criteria designed to identify, quantify and localize scar in the left ventricle using the morphology of the QRS complex. These criteria were updated in 2009 to expand their use to patients with underlying conduction abnormalities, but these versions have thus far only been validated in small and carefully selected populations. AIM: To determine the specificity for each of the criteria of the left bundle branch block (LBBB) modified Selvester QRS Score (LB-SS) in a population with strict LBBB and no myocardial scar as verified by cardiovascular magnetic resonance imaging with late gadolinium enhancement (CMR-LGE). METHODS: We identified ninety-nine patients with LBBB without scar on CMR-LGE, who underwent a clinically indicated CMR scan at three different centers. The ECG recording date was any time prior to or <30days after the CMR scan. The LB-SS was applied and specificity for detection of scar in each of the 46 separate criteria was determined. RESULTS: The specificity ranged between 41% and 100% for the 46 criteria of LB-SS and 27/46 (59%) met ≥95% specificity. The mean±SD specificity was 90%±14%. CONCLUSION: Several of the criteria in the LB-SS lack adequate specificity. Elimination or modification of these nonspecific QRS morphology criteria may improve the specificity of the overall LB-SS.

BACKGROUND: Estimation of the infarct size from body-surface ECGs in post-myocardial infarction patients has become possible using the Selvester scoring method. Automation of this scoring has been proposed in order to speed-up the measurement of the score and improving the inter-observer variability in computing a score that requires strong expertise in electrocardiography. In this work, we evaluated the quality of the QuAReSS software for delivering correct Selvester scoring in a set of standard 12-lead ECGs. METHOD: Standard 12-lead ECGs were recorded in 105 post-MI patients prescribed implantation of an implantable cardiodefibrillator (ICD). Amongst the 105 patients with standard clinical left bundle branch block (LBBB) patterns, 67 had a LBBB pattern meeting the strict criteria. The QuAReSS software was applied to these 67 tracings by two independent groups of cardiologists (from a clinical group and an ECG core laboratory) to measure the Selvester score semi-automatically. Using various level of agreement metrics, we compared the scores between groups and when automatically measured by the software. RESULTS: The average of the absolute difference in Selvester scores measured by the two independent groups was 1.4±1.5 score points, whereas the difference between automatic method and the two manual adjudications were 1.2±1.2 and 1.3±1.2 points. Eighty-two percent score agreement was observed between the two independent measurements when the difference of score was within two point ranges, while 90% and 84% score agreements were reached using the automatic method compared to the two manual adjudications. CONCLUSION: The study confirms that the QuAReSS software provides valid measurements of the Selvester score in patients with strict LBBB with minimal correction from cardiologists.

INTRODUCTION: About one-third of patients undergoing cardiac resynchronization therapy because of left bundle branch block (LBBB) and heart failure do not improve. Strauss et al. have developed strict criteria to more accurately define complete LBBB in this patient group. The aim of this study was to investigate the prevalence of the manual application of the Strauss criteria for LBBB (QRS ≥ 140 ms in men, ≥ 130 ms in women, along with mid-QRS notching/slurring) in consecutive patients who have been diagnosed with LBBB by the automated Glasgow criteria (QRS ≥ 120 ms). METHOD: In 158 consecutive patients (78 females) diagnosed with LBBB according to the automated Glasgow criteria, the manual Strauss criteria were applied. RESULTS & CONCLUSION: A majority of patients (87%) diagnosed with LBBB using the Glasgow criteria were positive for the Strauss criteria. In 70% (13/20) of the cases of disagreement the reason for disagreement was short QRS duration.

BACKGROUND: Baseline QRS duration (QRSd) ≥150 ms is a recognized predictor of clinical improvement by cardiac resynchronization therapy (CRT), particularly for those with left bundle branch (LBBB). Patients with QRSd <150 ms are considered less likely to respond. OBJECTIVE: The purpose of this study was to test our theory that left ventricular dyssynchrony, although usually associated with wider QRSd, also exhibits lower QRS frequency characteristics and that low-frequency content predicts CRT response in LBBB patients. METHODS: We retrospectively examined the QRS frequency content of 170 heart failure patients with LBBB and QRSd ≥120 ms using Fourier transformation. Ninety-four responders to CRT (defined as reduction in left ventricular end-systolic volume by ≥15% from baseline) were compared to 76 nonresponders (<15% reduction). Analysis of 3 standard ECG leads (I, aVF, and V3) representing the 3 dimensions of depolarization was performed, and V3 provided the best predictive value. RESULTS: The QRSd of responders (160.3 ± 17.8 ms) and nonresponders (161.8 ± 21.1 ms, P = .604) were similar. We found that the percentage of total QRS frequency power below 10 Hz that exceeded 52% was most predictive of CRT response compared to other cutoff values. However, the percentage of patients with total QRS power >52% below 10 Hz was especially predictive of response in those with QRSd <150 ms. In these patients, this power threshold was highly predictive of CRT response (positive predictive value 85.7%, negative predictive value 71.4%). CONCLUSION: In this group of CRT recipients with LBBB, retrospective analysis of QRS frequency content below 10 Hz had greater predictive value for CRT response than baseline QRSd, particularly in those with QRSd <150 ms.

IMPORTANCE: The benefits of cardiac resynchronization therapy (CRT) in clinical trials were greater among patients with left bundle-branch block (LBBB) or longer QRS duration. OBJECTIVE: To measure associations between QRS duration and morphology and outcomes among patients receiving a CRT defibrillator (CRT-D) in clinical practice. DESIGN, SETTING, AND PARTICIPANTS: Retrospective cohort study of Medicare beneficiaries in the National Cardiovascular Data Registry's ICD Registry between 2006 and 2009 who underwent CRT-D implantation. Patients were stratified according to whether they were admitted for CRT-D implantation or for another reason, then categorized as having either LBBB or no LBBB and QRS duration of either 150 ms or greater or 120 to 149 ms. MAIN OUTCOMES AND MEASURES: All-cause mortality; all-cause, cardiovascular, and heart failure readmission; and complications. Patients underwent follow-up for up to 3 years, with follow-up through December 2011. RESULTS: Among 24 169 patients admitted for CRT-D implantation, 1-year and 3-year mortality rates were 9.2% and 25.9%, respectively. All-cause readmission rates were 10.2% at 30 days and 43.3% at 1 year. Both the unadjusted rate and adjusted risk of 3-year mortality were lowest among patients with LBBB and QRS duration of 150 ms or greater (20.9%), compared with LBBB and QRS duration of 120 to 149 ms (26.5%; adjusted hazard ratio [HR], 1.30 [99% CI, 1.18-1.42]), no LBBB and QRS duration of 150 ms or greater (30.7%; HR, 1.34 [99% CI, 1.20-1.49]), and no LBBB and QRS duration of 120 to 149 ms (32.3%; HR, 1.52 [99% CI, 1.38-1.67]). The unadjusted rate and adjusted risk of 1-year all-cause readmission were also lowest among patients with LBBB and QRS duration of 150 ms or greater (38.6%), compared with LBBB and QRS duration of 120 to 149 ms (44.8%; adjusted HR, 1.18 [99% CI, 1.10-1.26]), no LBBB and QRS duration of 150 ms or greater (45.7%; HR, 1.16 [99% CI, 1.08-1.26]), and no LBBB and QRS duration of 120 to 149 ms (49.6%; HR, 1.31 [99% CI, 1.23-1.40]). There were no observed associations with complications. CONCLUSIONS AND RELEVANCE: Among fee-for-service Medicare beneficiaries undergoing CRT-D implantation in clinical practice, LBBB and QRS duration of 150 ms or greater, compared with LBBB and QRS duration less than 150 ms or no LBBB regardless of QRS duration, was associated with lower risk of all-cause mortality and of all-cause, cardiovascular, and heart failure readmissions.

BACKGROUND: Right bundle branch block (RBBB) evident in the precordial ECG leads may be associated with evidence of left bundle branch involvement in the limb leads. Any of the components of the left bundle can be involved, and this complex interventricular conduction abnormality has previously been described in patients with underlying heart disease. OBJECTIVES: To analyze the electro-vectorcardiographic manifestations of RBBB with left middle septal fiber block (LMSFB), with or without left anterior fascicular block (LAFB) in premature atrial beats of patients without apparent structural heart disease. METHODS: Twelve patients (8 men/4 women; mean age: 32±8years) with premature atrial contractions with this conduction abnormality were included. Surface 12 simultaneous lead ECG recordings and the corresponding vectorcardiographic loops were analyzed. RESULTS: The QRS complexes with RBBB and also LMSFB persisted for between 150 and 190ms. There were no q waves in lead I. The maximum spatial vector (72-86ms) was directed posteriorly, superiorly, and leftward, and the terminal forces were oriented anteriorly, inferiorly and rightward. In 10 patients, small q waves were apparent in leads V1-V2 and the frontal QRS axis was -60° and -70°, with the 46ms vector located at -50°±5. All of these patients most probably had LAFB in addition to LMSFB. In two patients, the initial electrical forces were directed anteriorly, inferiorly, and leftward, and the 46ms vector axis in the frontal plane was 6° and 11°, respectively, indicating absence of LAFB. CONCLUSIONS: The combination of RBBB and LMSFB occurring in patients without apparent structural heart disease may be related to the simultaneous occurrence of block of conduction through these components of the Purkinje network. The anterior fascicle of the left bundle may also be involved.

In a patient with atrial tachycardia with a rate of 200 per minute, the A-V conduction ratio was at times 2:1, but often it was 4:3 or 3:2 with progressive P-R interval prolongation (Wenckebach mechanism, an expression of presumable A-V nodal block). In each episode of 4:3 conduction, the first QRS complex was narrow, and the 2 ensuing beats were wide due to aberrant conduction. Aberration did not occur with a constant configuration, but in consecutive episodes of 4:3 conduction ratio there was a regular alternation of left bundle branch block and right bundle branch block. The pattern was explained by concealed retrograde conduction into the anterogradely blocked bundle branch. This caused 2 distinct effects: (1) shifting "to the right" of the refractory period of the affected bundle branch, resulting in maintenance of aberration with the same configuration, if consecutive atrial impulses were conducted to the ventricles, and (2) shortening of the effective cycle of the affected bundle branch, resulting in aberration due to block of the controlateral bundle branch, whenever a pause occasioned by a nonconducted atrial impulse was followed by restoration of 1:1 conduction for 2 or more consecutive beats.

RESUMO

This paper presents a method of using feature searching algorithm based on maximal divergence value to get the optimized feature combinations at different dimensions from feature space. Feature space is obtained through wavelet transform on ECG beat. Then the feature vector is determined by analyzing the changes of divergence value of those optimized feature combinations along with the dimensions. BP artificial neural network is trained by the feature vector and four types of ECG beats(normal beat, left bundle branch block beat, right bundle branch block beat and paced beat) obtained from MIT-BIH database are classified with a success of 93.9%.

RESUMO

A 32-year-old woman with no structural heart disease was referred for evaluation due to daily occasional palpitations and documented left bundle branch block (LBBB) on surface electrocardiogram (EKG). Continuous 24-hour Holter monitoring revealed a broad range of repetitive intermittent LBBB grades with a rate-dependent pattern that was also associated with circadian variations.
