RESUMO

A Silky terrier weighing 4.7 kg was presented with an airway foreign body after having aspirated a fragment of an orotracheal tube that was identified on radiological examination. Due to the small size of the patient, flexible endoscopy could not be performed through the lumen of a tracheal tube. Following IV induction of general anesthesia, the airway was instrumented with a laryngeal mask airway that was attached via a three-way connector to an anesthesia breathing circuit. A flexible endoscope was passed through the free port of the connector. That arrangement allowed for the passage of an endoscope through the lumen of the laryngeal mask airway and into the trachea without interrupting the continuous supply of O2 and sevoflurane.

PRACTICAL RELEVANCE: Respiratory endoscopy is a useful diagnostic tool to evaluate the airways for the presence of mass lesions or foreign material while allowing for sample collection for cytologic and microbiologic assessment. While bronchial disease (eosinophilic or neutrophilic) is the most common lower respiratory disease identified in cats, infectious, anomalous and neoplastic conditions can clinically mimic inflammatory bronchial disease. Diagnostic imaging is unable to define the etiology for clinical signs of cough, tachypnea or respiratory difficulty, necessitating visual evaluation and collection of airway samples. Endoscopy allows intervention that can be life-saving and also confirmation of disease, which is important given that life-long medication is likely to be required for management of inflammatory airway disease. PATIENT GROUP: Cats with either airway or pulmonary disease benefit from laryngoscopy, tracheoscopy and bronchoscopy to determine an etiologic diagnosis. In the best situation, animals that require these procedures present early in the course of disease before clinical decompensation precludes anesthetic intervention. However, in some instances, these tests must be performed in unstable cats, which heightens the risk of the procedure. Cats that do not respond to empiric medical therapy can also benefit from bronchoscopic evaluation. CLINICAL CHALLENGES: Due to the small size of feline airways and the tendency for cats to develop laryngospasm, passage of endoscopic equipment can be difficult. Bronchoconstriction can lead to hemoglobin desaturation with oxygen and respiratory compromise. EVIDENCE BASE: This article reviews published studies and case reports pertaining to the diagnostic approach to feline respiratory disease, focusing specifically on endoscopic examination of the lower airways in cats. It also discusses appropriate case selection, equipment, endoscopic techniques and visual findings based primarily on the authors' experiences.

Interstitial lung diseases are a heterogeneous group of disorders with a variety of causes. In veterinary medicine, such lung diseases with a prominent fibrotic component of unknown etiology are often called idiopathic pulmonary fibrosis (IPF). In human medicine, this term is reserved for a distinct disease entity with specific histologic findings labeled as usual interstitial pneumonia (UIP). We identified 23 cats displaying histologic criteria of UIP The purpose of this retrospective study is to describe the presentation and response to therapy of these cats to better define this disease entity. All but 2 cats were middle aged to older (median 8.7 years), with no apparent sex or breed predisposition. Complaints included respiratory distress (n = 18) and cough (13). Duration of signs was less than 6 months in 17 cats. Physical-examination abnormalities included tachypnea, inspiratory or mixed inspiratory and expiratory effort, and adventitial lung sounds. No consistent hematologic or biochemical abnormalities, parasites, or positive serologic results for feline retroviruses, heartworms, or toxoplasmosis were present. Radiographic changes included dense patchy or diffuse interstitial, bronchiolar, and alveolar infiltrates. Analysis of bronchial lavage fluid revealed mild neutrophilic inflammation (n = 6) with no consistent pathogen growth. Clinical condition of 5 cats worsened after lavage. Coincident pulmonary neoplasia was identified in 6 cats. Response to therapy (corticosteroids, antibiotics, bronchodilators, and diuretics) was poor, and most cats died within days to months. Cats with histologic changes compatible with UIP had signs that mimicked many of the clinical findings of human IPF, and treatment response was similarly unrewarding.

RESUMO

Foreign bodies in the airways or intestines of dogs or cats are a common problem in companion animal practice. Improved endoscopic techniques, together with their greater availability, mean that it is often possible to remove the foreign body endoscopically, making surgery unnecessary. This article describes 53 cats and dogs in which a foreign body was detected and, where possible, removed. Results are compared with those in the literature. Practical guidelines for foreign body removal in specific cases are described, focusing on therapy and prognosis.

RESUMO

Bronchoalveolar lavage is a valuable diagnostic technique when appropriate patient selection is made, which includes diseases that involve the pulmonary parenchyma and patients that can safely undergo general anesthesia. Bronchoalveolar lavage cytology has the potential to provide a definitive diagnosis or additional diagnostic information with a less invasive procedure than lung aspiration or thoracotomy. Limitations of accuracy in cell counts and negative findings are considerations when interpreting bronchoalveolar lavage fluid results.

RESUMO

Bronchoscopy is a standard diagnostic and therapeutic procedure in respiratory medicine and has been performed on many animal species. Cetaceans suffer considerable morbidity and mortality from lower respiratory tract infections, and it is very difficult to sample lower respiratory tract secretions for microbiology and other analyses. We report our experience on performing fiberoptic bronchoscopy in three bottle-nosed dolphins (Tursiops truncatus) and one false killer whale (Pseudorca crassidens), which should help other clinicians in performing bronchoscopy in cetaceans.

RESUMO

Bronchoscopy is a useful diagnostic and therapeutic tool in veterinary medicine. The increased availability of fiberoptic technology and video-endoscopy has enhanced recognition of the benefits gained from visualization of the lower airways in animals with lower respiratory tract disease. Specimens retrieved from the lower airway during bronchoscopy have greater diagnostic capacity, and a better understanding of the pathophysiology of disease is provided through application of bronchoscopy to animals with respiratory tract disease.

Bronchoscopy is a useful tool in the evaluation and management of canine and feline respiratory diseases. Diagnostic indications include the evaluation of structural diseases (tracheobronchial collapse, stricture, intraluminal mass); inflammatory conditions (chronic bronchitis, pneumonia); and traumatic injuries. Several airway-sampling techniques are available with bronchoscopy; bronchoalveolar lavage has proved to be the most satisfactory specimen-collection technique. Therapeutic indications of bronchoscopy at this time in veterinary medicine are mainly limited to foreign body removal. As advances are made in veterinary bronchopulmonary medicine, other therapeutic applications of the bronchoscope may be realized.
