Nasal septal abscess following septoplasty in a patient with type 2 diabetes mellitus.

A man in his 50s with type 2 diabetes mellitus (T2DM) presented with a nasal septal abscess 3 weeks following septoplasty. Diabetes mellitus has been reported in association with nasal septal abscess, thought to be due to a relative immunodeficient state. We present an unusual, delayed presentation of nasal septal abscess following septoplasty and performed a literature review. Nasal septal abscess is rare. It is associated with significant complications if not diagnosed and management expediently. The association between T2DM and nasal septal abscess following septoplasty emphasises the importance of good perioperative blood sugar control and postoperative nasal care and raises the question of empirical antibiotics in this group.

[Septic metastatic complications in facial furuncles and carbuncles]. / Septico-metastaticheskie oslozhneniia pri furunkulakh i karbunkulakh litsa.

According the information of Maxillo-Facial Surgery Clinic of Moscow Medical Academy the percentage of the patients with face furuncles and carbuncles from all the hospitalised is the following: 1994--4.5%, 1995--7.9%, 1996--11.4%, 1997--17.0%. That gives about 3.7 times during the last 2 years. The rate of dangerous for life septic metastatic complications in the course of such diseases considerably exceeds those at phlegmons. This figure has increased 2 to 2.5 times during the last 2 years. Two clinical cases which demonstrate possibility of bacterial thromboemboli spreading in face furuncles and carbuncles not only through small, but also through large blood circulation with formation of septic centres in organs are described. The treatment tactics for such patients requires a strictly individual approach. At slightest suspicion of the complication urgent hospitalisation of the patients for active conservative therapy is necessary. In case of purulent process extension surgery is indicated.

The frequency of acute purulent inflammatory diseases of maxillofacial area and their complications (thrombosis of the cavernous sinus, abscess of the brain, sepsis) were investigated. These complications are responsible for 0.56% of lethal outcomes. Early diagnosis and intensive treatment of such conditions are a must.

The authors analyze the results of diagnosis and treatment in 1062 patients with diffuse inflammatory diseases of the maxillofacial area and of complications of these diseases, such as mediastinitis, thrombosis of the cavernous sinus of the dura mater, meningoencephalitis. Improvement of the program of diagnosis and treatment helped improve the results: mortality from sepsis reduced from 50 to 26%. Approaches to prevention of progressive purulent infection are outlined.

Case study: carbuncle of the neck with extensive tunnelling.

A 40 year old female developed a carbuncle at the base of the neck on the right side near the hairline. Despite her attempts to treat the carbuncle at home "it continued to enlarge and fester" (became inflamed and suppurated). She was admitted to the hospital and, following surgical incision and drainage, ET nurse consultation was requested to establish a wound treatment regimen.

[The extracorporeal connection of a donor spleen in the treatment of suppurative diseases of the face and neck complicated by sepsis]. / Ekstrakorporal'noe podkliuchenie donorskoi selezenki v lechenii gnoinykh zabolevanii litsa i shei, oslozhnennykh sepsisom.

Analyzes the results of extracorporeal connection of donor spleen in 9 patients with purulent diseases of the face and neck, complicated with sepsis. Presents the indications for the use of this treatment modality and its technique. Describes the changes in the clinical picture and time course of the basic homeostasis parameters over the course of therapy. Reduction of mortality rates to make them as low as 12.5% demonstrates a high efficacy of the suggested treatment as part of multiple-modality complex used in the treatment of odontogenic and surgical sepsis.

[Metastatic lung abscesses]. / Metastaticheskie abstsessy legkikh.

The metastatic origin of abscesses was established in 10 of 142 patients (7%) with acute pulmonary abscesses. The disease was observed to be of severe course, being accompanied by pleural and other complications in all the cases. Thirteen distinctive signs of metastatic abscesses were revealed which determine the curative methods. The main problem in the treatment of such patients is struggle against sepsis, correction of immune processes in the organism, sanitation of the abscess cavity through the tracheobronchial tree and liquidation of pleural complications.
