RESUMO

The necessity of considering the problem with which this paper is concerned arises from the current scientific discussion of the actions of physicians working in municipal healthcare facilities and sanatoria directed toward rehabilitation of patients after surgical treatment of periapical tissues and oral cavity phlegmonas. The study elucidated the mechanism of action of mineral waters (alkaline, sodium hydrocarbonate or boron-containing water with elevated fluorine and chlorine content) extracted from natural deposits of the Belgorod Region and Krasnodar Krai (Territory). It was shown that drinking these waters contributed to the correction of the disturbed balance in the ratio of sodium and potassium ions excreted with urine and to the positive dynamics of serum phosphorus level in the patients included in this study.

Urinary trypsin inhibitory capacity is mainly due to the excretion of a glycoprotein which is immunologically related to the inter alpha-trypsin inhibitor and may be a proteolytic degradation product of that substance. It was tested in 133 subjects divided into 7 groups: 24 healthy controls (group A), 21 patients with bacterial infection (group B), 37 with bacterial infection under antibiotic therapy (group C), 25 with connective tissue disease (group D), 8 with infected connective tissue disease (group E), 14 with cancer (group F) and 4 with infected cancer (group G). Urinary trypsin inhibitory capacity level was very low in controls (3.32 +/- 0.8 U/g urinary creatinine), but it was dramatically increased when infection was present (149.67 +/- 23.6 U/g urinary creatinine). This test appeared to be more effective than serum C-protein measurement simultaneous carried out in the same patients. Urinary trypsin inhibitory capacity is not related to the degree of proteinuria in the urine sample, but it is increased in patients with chronic renal failure excluded from this study. Thus, its measurement is a sensitive, easy and useful test for detecting and monitoring infections. The return to its physiological value is a very good argument in favour of therapeutic effectiveness.

A new chromatographic procedure is proposed for measuring 3 methyl-L-histidine (3 MH) in human urine. The sample was purified on a cation-exchange resin (AGR 50W-X4) and analysed by ion exchange high performance liquid chromatography on a PARTISIL 10 SCX Whatman column in UV light at 210 nm within 16 min. This procedure gave similar outputs of 3 MH to those described in human normal urine (mean +/- SEM = 213 +/- 15 mumol X 24 h-1, n = 19). It was used to measure the urinary outputs of 3 MH of five patients admitted to an intensive surgical care unit, for 48, 28, 25, 15 and 10 days, respectively. The urinary outputs of 3 MH were normal or lower than normal. The 10(3) urinary 3 MH/creatinine molar ratios were also calculated; this new 3 MH analysis could help the reanimator to prescribe an adequate nutritional assessment.

364 patients were examined for changes of cutaneous and subcutaneous adipose tissue, as they appear in idiopathic panniculosis. There was not found a fundamental difference between the generalized and by far more frequently appearing partial changes of the skin, the consecutive panniculitis. The transition between the two forms is flowing. In contrast to the simple placebo test 25 patients, among them 12 cases with idiopathic panniculosis, could for the most part be treated successfully by highly dosed intravenous applications of hyaluronic acid. There was not established a connection between the decrease of the cutaneous changes and the reduction of the secretion of the mycopolysaccharides in the urine.
