RESUMO

A case of papillary cystic tumor (PCT) of the pancreas in a 40-yr-old woman is reported. This rare neoplasm was discovered fortuitously by a CT-scan examination for a palpable abdominal uterine leiomyoma. Percutaneous fine-needle aspiration (FNA) of the pancreatic mass was performed under CT-scan guidance. Cytologic examination of the material gave the diagnosis of PCT, which was confirmed by histologic examination of the resected tumor. Immunohistochemical staining showed a high degree of positivity to wide-spectrum anticytokeratin and anticytokeratin 20, and a weak positivity to anti-NSE antibody. In addition, the tumor cells were highly reactive for progesterone antibody, while they were negative for estrogen. These findings suggest a ductal origin for PCT, and also suggest that sex hormones may play a role in its growth, but not in its genesis. We emphasize the value of FNA cytologic features in the diagnosis of these rare tumors.

RESUMO

With the help of Integral-2MT cytoanalyzer, the average area of nuclei and the percentage of epitheliocytes with small, medium and large nuclei at the site of dysplasia of serous cysts and serous ovarian carcinomas have been evaluated. An increase in the degree of dysplasia of tumorous epithelium is associated with an increase in the average area of nuclei and the number of cells with medium nuclei. A decrease in the degree of differentiation of serous ovarian carcinomas is accompanied by increasing percentage of cells with large nuclei. Preoperative chemotherapy does not affect this regularity.

RESUMO

The authors reported a case of a young woman. From her omentum a clinically and radiologically benign tumour was removed that had been observed for years and caused minor symptoms. Histologically the tumour proved to be a so called papillary and cystic tumour that originated from ectopic pancreatic tissue. The histological diagnosis was justified by immunohistochemical and electronmicroscopical examinations. Reports of this type of tumour evolving in an ectopic pancreas appeared only twice in the world literature to the knowledge of the authors.

RESUMO

Morphofunctional state of nucleolar organizers in epithelial cells of benign, and malignant serous ovarian tumours has been studied. A rise in the activity of nucleolar organizers depending upon an increase of the level of malignancy has been revealed.

Fourteen cases of solid-cystic tumor of the pancreas have been investigated at the ultrastructural level. A heterogeneous cell population has been found, and some distinctive features have been identified: oncocytoid cells; clear cells reminiscent of ductular and centro-acinar cells; cells with exocrine and endocrine features; cells with bundles of microfilaments and dense bodies, reminiscent of myoepithelium. This varied differentiation strongly suggests the tumor histogenesis to be from multipotent cell capable of recapitulating the normal ontogenesis during its neoplastic proliferation. Furthermore, the heterogeneity in ultrastructural appearance is a characteristic and reliable feature of solid-cystic tumor of the pancreas.

Papillary cystadenoma of the epididymis is a rare neoplasm that is sometimes associated with von Hippel-Lindau's syndrome. Electron microscopic study of the present case revealed that neoplastic cells contained abundant glycogen granules and large lipid droplets, but a few organelles. On the apical surface there were numerous microvilli and a few single cilia, but no ciliated cells. Subepithelial basal lamina was noted, but it was occasionally disrupted. Furthermore, microvilli sprang from the circumference of the small tumor-cell nest and became associated with matrix components (microvillus-matrix associations). On immunohistochemical study, neoplastic cells showed epithelial characteristics, but positive reactivity for S-100 protein. These findings resembled those of the epithelial cells of the efferent ductules of the epididymis. In the stroma, prominent vasculature was characteristic and fenestrated-type capillaries were found in the peripheral portion of the tumor. Papillary cystadenoma of the epididymis may originate from non-ciliated epithelial cells of the efferent ductules.

A case of papillary-cystic tumor (PCT) of the pancreas in a young woman is reported. Fine-needle aspiration (FNA) was done preoperatively under ultrasound guidance. The aspirate showed numerous delicate papillary fragments, dyscohesive and monomorphic tumor cells with folded nuclear membranes, and foamy macrophages. A diagnosis of PCT of the pancreas was made based on clinical, radiologic, and cytologic findings. The patient underwent distal pancreatectomy without complications. The histopathologic examination of the surgical tissue confirmed the diagnosis of PCT of the pancreas. The tumor cells were faintly positive with mucicarmine and periodic acid-schiff (PAS) stains. Immunocytochemistry using Ki67 monoclonal antibody showed a cycling index of 0.1 percent, supporting the clinical observation of low metastatic and recurrence rates of this rare tumor. DNA analysis of the tumor showed a DNA index of 1.09 (diploid) and an S-phase fraction of 5.38%. The tumor cells were positive for progesterone receptors (> 15 fmol/mg protein) but negative for estrogen receptors (< 15 fmol/mg protein). Abundant mitochondria, prominent endoplasmic reticulum and few junctional complexes were noted on electron microscopy. Emphasis is placed on accurate diagnosis based on preoperative FNA cytology in order to maximize cure rates while minimizing surgical risk and complications.
