RESUMO
We present the case of a 29-years-old male patient, affected by a voluminous post-traumatic mesenteric cyst, a rare abdominal disease; our patient represents a rarely affected age group. Treatment was based on interventional radiology with an US-guided drainage and sclerosis by ethyl alcohol of the lesion. The intervention performed on this patient represents the application of a standardized radiological technique to a new contest, mesenteric cysts, whose gold-standard treatment is represented in literature by surgery. In our case we obtained an optimal result, with complete regression of the treated cyst: it proved to be an effective, feasible, safe and minimally invasive procedure.

Hidatid cysts of the spleen are a rare occurrence, the spleen being the third most common organ for the development of Echinococcus Granulosus. Splenic hydatid cysts are commonly part of multi-organ hydatid disease. Diagnosis is often established when investigating a splenomegaly or by chance during an unrelated consult. It can also be diagnosed after rupture, be it following trauma (the most common occurrence)or spontaneous. Splenic hydatid cyst rupture requires immediate action and is a life-threatening condition. It results, most often, in splenectomy. We present the case of a patient with multi-organ hydatid disease that presented with a ruptured splenic cyst and developed anaphylaxis. The case was resolved by splenectomy and recovered well.

Los quistes mesentéricos son tumoraciones abdominales raras. Cerca de 60% de estos quistes ocurren antes de los 5 años de edad y pueden localizarse en cualquier punto del tracto gastrointestinal, aunque son mas frecuentemente encontrados en el mesenterio del intestino delgado. La presentación clínica depende de la localización y tamaño del quiste y muchos de los casos son asintomáticos y son diagnosticados incidentalmente. Los síntomas más frecuentes son dolor abdominal, distensión abdominal, masa abdominal, náuseas, vómitos, constipación, diarrea, pérdida de peso, fiebre y peritonitis. Las complicaciones incluyen torsión, infarto, formación de vólvulos, perforación, infección, anemia por hemorragia intraquística, obstrucción intestinal y uropatía obstructiva. Mesenteric cysts are rare abdominal tumors. About 60% of these cysts occurs before 5 years of age and can be located anywhere in the gastrointestinal tract, but are most often found in the small bowel mesentery. The clinical presentation depends on the location and size of the cyst and many cases are asymptomatic and are diagnosed incidentally. The most common symptoms are abdominal pain, bloating, abdominal mass, nausea, vomiting, constipation, diarrhea, weight loss, fever and peritonitis. Complications include torsion, infarction, volvulus formation, perforation, infection, anemia, intracystic hemorrhage, intestinal obstruction and obstructive uropathy. Mesenteric cyst is a rare condition. Presentation with non-typhoid spontaneous infection in an unusual area makes it even more a rare situation with mesenteric cyst. Its diagnosis is mainly based on the imaging modalities. However, there are difficulties in diagnosis when it is present in an uncommon area and rare known complications. Mesenteric cyst can present with few uncommon emergency conditions which pose difficulties in diagnosis as well as treatment options as mentioned in this case.

BACKGROUND: Mucinous cystic neoplasms arise in the ovary and various extra-ovarian sites. While their pathogenesis remains conjectural, their similarities suggest a common pathway of development. There have been rare reports involving the mesentery as a primary tumour site. CASE PRESENTATION: A cystic mass of uncertain origin was demonstrated radiologically in a 22 year old female with chronic abdominal pain. At laparotomy, the mass was fixed within the colonic mesentery. Histology demonstrated a benign mucinous cystadenoma. METHODS AND RESULTS: We review the literature on mucinous cystic neoplasms of the mesentery and report on the pathogenesis, biologic behavior, diagnosis and treatment of similar extra-ovarian tumors. We propose an updated classification of mesenteric cysts and cystic tumors. CONCLUSION: Mucinous cystic neoplasms of the mesentery present almost exclusively in women and must be considered in the differential diagnosis of mesenteric tumors. Only full histological examination of a mucinous cystic neoplasm can exclude a borderline or malignant component. An updated classification of mesenteric cysts and cystic tumors is proposed.

E descrito um caso mesenterico, em paciente do sexo masculino, cuja forma de apresentacao clinica se deu de forma aguda. O diagnostico foi realizado atraves de exame fisico, ultrassonografico e radiologico. O tratamento realizado foi a excisao completa do tumor. O paciente apos 12 anos encontra-se bem, sem sinais de recidiva.

The authors describe three cases where laparotomy was performed within short time intervals after puncture under ultrasonic control. In none of the patients substantial changes were detected in the abdominal cavity in conjunction with the punctures.

This study is based on a group of 34 primary tumours of the mesenterium which were treated in the first surgical Clinic between 1960 and 1981. In this series were 19 women and 15 men, and the age of the patients varied between 16 and 68 years, being in the average of 44 years. Ten cystic tumours were recorded, and 24 solid tumours. Nine of the tumours were benign and the remaining 25 were malignant. The cystic tumours, for the most of the benign type, were almost exclusively cystic lymphangiomas. One serous cyst was also recorded, as well as one hematic cyst, and one dermoid cyst that had underwent malignization. The solid tumours were all malignant, of the sarcomatous type,, and their starting point was the mesenteral connective-vascular structures. The clinical picture of the mesenteral tumours is polymorphous and non-characteristic. In the frame of paraclinical examinations the radiologic investigation holds an important place and this is especially true for selective arteriography of the upper mesenteric artery, which provides useful informations concerning the topography of the tumour in relation to the mesenteral vascular trunk. Therapeutic possibilities are discussed, and the fact is stressed that, for improving the prognosis an early diagnosis is essential.
