For many years it has been the work of Buschke and Löwenstein that has justified calling the exophytic, locally destructive tumour of the anogenital mucosal surface 'giant condyloma of Buschke and Löwenstein' or GCBL. In order to investigate the early history of this rare disease we examined the writings of the barber-surgeon Wilhelm Fabry (1560-1634) who had a serious interest in dermatological disorders and their treatment. We analysed Fabry's 600 Latin case reports and identified the case of a 'monstrous penile tumour'. We then translated this text into English and compared it point by point with later publications. This was followed by a cursory review of surgical treatises from the 16th to the 18th centuries. In 1614 Fabry described and depicted a tumour of the penis; the clinical characteristics (gradual formation of a warty lesion, considerable size, invasive growth, absence of metastases) indicated it was a giant condyloma. His mention of the urethral fistulization enables discrimination from 'common' condylomata acuminata, and the survival period of 10 years after amputation allows exclusion of a 'true' carcinoma. This report is singular among 17th-century case histories. The neoplasias described 300 years later are most probably biologically identical. Thus, Fabry's is the first clinical report; the histological classification, however, belongs to Buschke and Löwenstein. From now on the disease should be designated with the eponym giant condyloma of Fabry-Buschke-Löwenstein or GCFBL.

RESUMO

Human papillomavirus has been a cause of infection in humans for thousands of years. The history of papillomaviruses, knowledge of their causative role in benign and malignant disease, and their structural characteristics have led to the development of vaccines to prevent cervical and anogenital cancers. Many questions remain unanswered before HPV vaccines can be optimised; however, the concept of virtual eradication of cervical cancer is not impossible, and remains a realistic aspiration.

In the second part of their review the authors focus on palaeopathological studies, performed in mummified tissues, with reference to infectious diseases. The analysis of samples obtained from both natural and artificial mummies may provide, in some favourable events, a more complete knowledge of these findings in comparison to information obtained from only comprehensive examination of the skeleton. The acquired data enable us to understand not only the diseases which afflicted mankind, but also dietary and hygiene conditions of ancient populations. We report knowledge acquired regarding some palaeopathological conditions, including schistosomiasis, smallpox, cisticercosis, trichinosis, ascaridiasis, echinococcosis, filariasis, hepatitis E virus, condylomatosis, pulmonary tubercolosis, pediculosis, visceral leishmaniasis as well as Diphyllobotrium sp., Dicrocoelium dendriticum and Fasciola hepatica infestations. In addition some interesting findings concerning the relationship between dietary and food intake colonized by streptomyces are described. This review reports the discovery of human remains from different geographic areas: while most of these studies describe findings in two Mediterranean countries (Italy and Egypt), some refer to Britain and German-speaking countries (Austria and Germany) as well as the area in Africa known as Nubia, along the Nile. Both histological and biomolecular diagnosis are useful not only to identify a specific disease in a subject from the remote past, but also to achieve information concerning its frequency and evolution. Such knowledge may thus allow us to understand the intensity of cultural exchanges and links among different populations and the role of these relationships in transmitting and spreading infectious diseases in a certain geographic area.

In 1925 Abraham Buschke and Ludwig Loewenstein described a neoplasm of the penis (carcinomähnliche condylomata acuminata des penis), which, to them, bore resemblances to common condyloma acuminatum and squamous cell carcinoma, but it had histopathologic and clinical characteristics that differed from these 2 proliferations. Later Lauren V. Ackerman described a similar neoplasm of the oral mucosa that he termed "verrucous carcinoma." I have previously reviewed that neoplasm, now referred to as "oral verrucous carcinoma of Ackerman." Still later Aird and his colleagues described a unique neoplasm of the foot, "carcinoma cuniculatum." These 3 have been grouped generically as "verrucous carcinoma." Verrucous carcinomas occur at many sites including the female genitalia and the aerodigestive system. This report will synopsize the lives of Abraham Buschke and Ludwig Loewenstein, review their original articles, and mention the changing concepts over time concerning condylomata, particularly large condylomata. Some other reports of giant condyloma (verrucous carcinoma of the penis) will be discussed, emphasizing the histopathology of this neoplasm and its differentiation from "warty-like" carcinomas of the penis. Verrucous carcinoma of other sites and epithelioma (carcinoma) cuniculatum will not be discussed.

El papiloma virus humano (HPV) es una de las infecciones más comunes en las mujeres. La historia natural del HPV ha cambiado, así como su diagnóstico y tratamiento. El cuadro clínico es variable, con remisión y exacerbaciones. Con los avances de la biología molecular, su diagnóstico y tipificación han modificado su tratamiento. Existen diversos tratamientos que dependen de la experiencia del médico y de las características de la paciente, algunas veces su erradicación es imposible ya que nunca desaparece del aparato genital

Se recalca la importancia de efectuar controles médicos periódicos con biopsias para pesquisar la aparición de un epitelioma espinocelular, principalmente si el H.P.V. causal es de conocido potencial carcinogenético. Realizamos una breve revisión del tema
