BACKGROUND: Regenerative endodontic procedures (REPs) are innovative treatments aimed at restoring damaged dental structures. However, the effect of orthodontic movement on REP-treated teeth is not well understood and may have significant long-term consequences. This study aimed to evaluate the impact of orthodontic movement on a mature permanent tooth associated with dens invaginatus that has undergone a regenerative endodontic procedure (REP). CASE PRESENTATION: This report describes the case of a 13-year-old healthy male who presented with pulp necrosis and a chronic apical abscess (tooth number 2.2). Following REP according to the American Association of Endodontists' guidelines, the patient began non-extraction orthodontic treatment with fixed appliances after a 9-month healing period, which lasted 17 months in the upper arch. Subsequent follow-ups at 24, 36 and 48 months post-REP revealed an asymptomatic state with minimal cervical discoloration and diminished cold sensitivity. Radiographic analyses revealed periapical healing, mild apical remodeling on tooth 2.2, and moderate apical remodeling on other maxillary incisors. The treated tooth displayed a positive response to both REP and orthodontic treatment, yet further research is required to determine the long-term effects of orthodontics on REP-treated teeth. CONCLUSION: Orthodontic movement following REPs in mature permanent teeth is feasible and do not seem to prone teeth to orthodontic tooth resorption. Our experience indicates that a 9-month healing period allows successful orthodontic outcomes following REPs. Nonetheless, the predictability of outcomes and the ideal healing period before orthodontic movement is initiated remain to be established.

This is a case report on the management of a vital lateral incisor with a radiolucent cervical area associated with the presence of necrotic pulp due to a dens invaginatus (DI) type III. A nine-year-old boy presented to a university pediatric dental clinic with dental trauma. The examination showed poor oral hygiene, an uncomplicated fracture in the permanent maxillary left central incisor, and a fistula near the permanent maxillary right lateral incisor. A radiograph showed that the right lateral incisor had incomplete root development and a type III DI. Although sensitive to thermal pulp testing, tracing of the fistula indicated that the radiolucent area was associated with the DI, extending laterally to the periodontal ligament. Endodontic treatment was performed only in the invagination, keeping the root canal of the lateral incisor vital. After two years of follow-up, complete root formation and injury repair associated with invagination were observed.

La sinusitis odontogénica afecta la mucosa de los senos maxilares y puede ser causada por la evolución de una lesión en el diente con formación de pus periapical que avanza en el interior de la cavidad sinusal. Se presenta un caso de un paciente con dens in dente en el diente incisivo lateral superior derecho, que sufrió el proceso de caries evolucionando en la región periapical, contaminando la mucosa sinusal de este lado, lo que causó el dolor, abultamiento y el desplazamiento del diente. La cirugía se realizó a través de acceso de Caldwell-Luc para el curetaje de la lesión, lo que indica la presencia en el diagnóstico histopatológico la presencia de quiste abscedado y sinusitis. Después de 18 meses de control, se observó la regeneración ósea y el reposicionamiento alveolar espontáneo de los dientes implicados. Los autores advierten que cíngulos profundos y dens in dente son las enfermedades de los dientes que merecen atención para el diagnóstico y la prevención de los procesos de caries con el fin de evitar complicaciones mayores.


Odontogenic sinusitis affects mucous membrane of the maxillary sinus and can be caused by the evolution of a tooth injury with formation of periapical abscess that advances into the sinus cavity. We present a patient with dens in dente in the upper right lateral incisor tooth, which suffered carious process and periapical abscess, contaminating the sinus mucosa on this side, which caused pain, bulging and tooth displacement. Surgery was performed via a Caldwell-Luc access for curettage of the lesion, indicating the presence in the histopathological diagnosis of radicular cyst abscess and sinusitis. After 18 months of control, alveolar bone regeneration and spontaneous repositioning of teeth involved was observed. The authors warn that cingulum deep in dens in dente are diseases of the tooth that deserve attention for the diagnosis and prevention of carious processes in order to avoid further complications.

Mesiodens is a midline supernumerary tooth commonly seen in the maxillary arch and the talon cusp is a rare dental developmental anomaly seen on the lingual surface of anterior teeth. This paper presents a rare clinical case of development of talon cusp in a mesiodens with multiple lobes, which interfered with both occlusion and appearance of an 11-year-old patient. During clinical interview, the patient reported difficulty on mastication. Clinical and radiographic examination revealed that a supernumerary tooth with completely formed root was causing an occlusal interference. The supernumerary tooth was diagnosed as multi-lobed mesiodens associated with a palatal talon cusp. The treatment plan consisted in the extraction of the supernumerary tooth followed by orthodontic treatment for diastema closure and tooth alignment.

A combined periodontal and orthodontic treatment demands a detailed evaluation in both specialties, particularly when the periodontium is reduced. This is especially true for adult patients, but young patients can also suffer from advanced periodontitis. This article describes combined periodontal and orthodontic therapy in a young patient with severe localized and aggressive periodontitis, tooth crown abnormalities, and missing maxillary second premolars. Periodontal treatment was carried out. Once attachment gain and bone stability were confirmed, orthodontic therapy commenced. It lasted 32 months, during which segmented mechanics and only light forces were used. The result of this intervention was satisfactory, and long-term stability (9 years) with periodontal maintenance was achieved.

Mesiodens é um dente supranumerário mais comumente visto no arco maxilar na linha média, e a cúspide em garra (talon cusp) é uma anomalia dental de desenvolvimento rara que ocorre na face lingual de dentes anteriores. Este artigo apresenta um caso clínico raro de desenvolvimento de uma cúspide em garra em um mesiodens multilobulado que causava interferência tanto na oclusão quanto na aparência de um paciente de 11 anos de idade. Durante a entrevista clínica, o paciente relatou dificuldade na mastigação. Exame clínico e radiográfico revelou que um dente supranumerário com raiz completamente formada estava causando interferência oclusal. O dente supranumerário foi diagnosticado como sendo um mesiodens multilobulado associado a uma cúspide em garra palatina. O plano tratamento consistiu na extração do dente supranumerário seguida de tratamento ortodôntico para fechamento de diastema e alinhamento dos dentes.

AIM: To report the endodontic treatment of an immature maxillary central incisor with dens invaginatus. SUMMARY: Dens invaginatus is a rare malformation of teeth, probably resulting from an infolding of the dental papilla during tooth development. The present case describes the complex endodontic treatment of a type III dens invaginatus in an immature maxillary central incisor with a necrotic pulp and abscess formation. The initial treatment goal was to achieve apexification of the pseudocanal root and conservative root canal treatment in the main canal. Following 1-year of treatment with calcium hydroxide dressings, radiography revealed a healing response, but no sign of a hard tissue barrier at the apex. Periapical surgery with the placement of a zinc oxide cement (IRM) root-end filling was considered successful at the 4-year follow-up. KEY LEARNING POINTS: The complexity of the canal system and open apex in dens invaginatus present a challenge to endodontic treatment. Correct diagnosis and treatment planning are fundamental to treatment of dens invaginatus. Periapical surgery is indicated in cases of unsuccessful apexification in immature teeth with dens invaginatus and nonvital pulp.

Dens invaginatus is a rare malformation of the teeth resulting from the invagination of the tooth crown before biological mineralization occurs. In most cases, the thin or incomplete enamel lining of the invagination cannot prevent the entry of bacteria into the pulp, which leads to pulp necrosis with an eventual periapical inflammatory response. The treatment options include preventive sealing or filling of the invagination, root canal treatment, endodontic apical surgery and extraction. The root canal treatment of such teeth is often complicated because of their anatomical complexity. This case describes a successful non-surgical endodontic treatment of a maxillary lateral incisor with type 2 dens invaginatus with a large periradicular lesion. At follow-up examinations after 6 and 12-months, the tooth was asymptomatic and the healed lesion was evident radiographically

Dens invaginatus is a developmental variation resulting from an alteration in the normal growth pattern of the dental papilla. Synonyms of this disturbance include dens in dente, invaginated odontome, tooth inclusion, and dentoid in dente. Radiographically, it is observed as infolding of a radiopaque ribbon-like structure, with equal density as enamel, extending from the cingulum into the root canal and sometimes reaching the root apex, assigning the appearance of a small tooth within the coronal pulp cavity. This article presents 2 case reports. The first describes an 8-year-old girl with dens invaginatus in a mesiodens; the second report describes a 16-year-old boy presenting with 2 mesiodens, both associated with dens invaginatus.

AIM: To report the simultaneous endodontic and surgical treatment of a tooth associated with Oehlers type III dens invaginatus and a persistent periapical lesion, which comprised root-end resection, root-end filling and application of a calcium hydroxide barrier placed on the resected dentine surface. SUMMARY: Three root canals were identified in a tooth with a type III dens invaginatus, which presented with a necrotic pulp, wide foraminal opening and extensive periapical lesion, and with a previous history of acute abscess, intracanal exudate and fistula. After root canal preparation followed by intracanal application of calcium hydroxide pastes, the clinical-pathological status persisted. After periapical curettage and root-end resection, the root canals were filled, followed by root-end filling with Sealer 26 mixed with zinc oxide powder to a clay-like consistency. Calcium hydroxide paste was then applied over the exposed dentinal surface forming a covering over the root apex. At the 20-month follow-up examination the patient had no symptoms and no fistula; advanced periapical bone repair was obvious on the radiograph. KEY LEARNING POINTS: Because of the variable morphology and extent of invagination, type III dens invaginatus represents a challenge for conventional treatment, often leading to the need for a surgical approach. Sealer 26 thickened with zinc oxide powder provided satisfactory clinical properties for use as a root-end filling material. Application of a calcium hydroxide barrier over the resected root-end is a potential treatment option to encourage tissue repair.

RESUMO

An accurate understanding of the morphology of the root canal system is a prerequisite for successful root canal treatment. Invaginated teeth have a complex root canal configuration that cannot be instrumented effectively and should be treated by both endodontic therapy and surgery. A case of dens invaginatus Type 3 in a maxillary lateral incisor with a periapical lesion and its successful treatment by these combined methods is reported.

RESUMO

A patient with multiple anomalies of the maxillary teeth, including shovel-shaped incisors, talon cusp, bilateral dens invaginatus and bilateral peg-shaped supernumerary incisors is reported. The patient also exhibited Carabelli's cusp on both maxillary first molars. No developmental syndrome was identified. This very unusual combination of anomalies has not been reported previously.

CASE REPORT: This paper presents a rare case of bilateral talon cusp in permanent maxillary central incisors, one on the labial and another on the lingual surface of each tooth. The condition was associated with dens invaginatus in a maxillary permanent lateral incisor, but no developmental syndrome was identified. The left central incisor required no treatment because the aesthetic appearance was satisfactory and neither occlusal interferences nor caries were present. After careful investigation of the right lateral incisor, a sealant was applied. Root canal treatment was indicated for the right central incisor that had a wide open apex with thin, weak, divergent walls, and an apical radiolucency.

Endodontic treatment was performed in a maxillary lateral incisor presenting two different types of anomalies: dens invaginatus and transposition to the region of the canine. The two transposed teeth were subsequently restored with light-cured composite, bringing dental esthetics to normal in a single session.
