CONTEXT: Strength assessment is essential to prescribe exercise in sports and rehabilitation. Low-cost valid equipment may allow continuous monitoring of training. OBJECTIVE: To examine the validity of a very low-cost hanging scale by comparing differences in the measures of peak force to a laboratory grade load cell during shoulder abduction, flexion, extension, and internal and external rotations. DESIGN: Analytical study. PARTICIPANTS: Thirty-two healthy subjects (18 women, age 26 [10] y, height 172 [8] cm, mass 69 [13] kg, body mass index 23 [4] kg/m2). MAIN OUTCOME MEASURES: The dependent variable was the maximal peak force (in kilogram-force). The independent variable was the instrument (laboratory grade load cell and hanging scale). RESULTS: No differences were observed while comparing the results. The intraclass correlation coefficients1,1 ranged from .96 to .99, showing excellent results. The Cronbach alpha test also returned >.99 for all comparisons. The SEM ranged from 0.02 to 0.04 kgf, with an averaged SD from 0.24 to 0.38 kgf. The correlation was classified as high for all tested movements (r > .99; P < .001), with excellent adjusted coefficients of determination (.96 < r2 < .99). Bland-Altman results showed high levels of agreement with bias ranging from 0.27 to 0.48. CONCLUSIONS: Hanging scale provides valid measures of isometric strength with similar output measures as laboratory grade load cell.

Muscle imbalance and deficit are key parameters for guiding rehabilitation and sports sessions and avoiding injuries. However, the high cost and non-portable nature of most instruments employed for muscle strength assessment frequently hamper an affordable evaluation in field conditions. This study evaluated the validity and reliability of a low-cost digital dynamometer to assess the maximal isometric strength of upper limb muscles. Fourteen physically active volunteers performed the main movements of the upper limb at maximal isometric strength: flexion-extension, internal-external rotation and abduction-adduction of the shoulder; and flexion-extension of both elbow and wrist. Validity was examined by comparing the strength values from the low-cost digital dynamometer and those measured by an isokinetic dynamometer, assumed as the gold standard. The correlation between both devices was perfect (r > 0.913), while Bland-Altman plots showed absolute agreement between both devices, the maximum range of the values of bias was -0.99-1.00 N in wrist extension. Inter-tester and intra-tester reliability were excellent for all movements (ICC ≥ 0.855). The low-cost digital dynamometer showed strong validity and excellent reliability in assessing maximal isometric strength during the main movements of the upper limb. Professionals may use it for an affordable isometric muscle strength assessment in field situations.

The evaluation of hip abductor strength is useful in assessing of the outcome of hip surgery. Hand-held dynamometers are available, but they are less reliable in assessing hip abductor strength than some other muscle groups. We describe a new device designed to measure hip abductor strength, which is practical in a clinical setting. A system of constraints, pads and reference points was devised to make force measurements as little examiner-dependent as possible. Reproducibility was assessed in a controlled setting. The abductor strength of ten healthy young subjects (average age 28 years) was tested twice on each side by two independent examiners. Tests were performed in a supine position, eliminating the influence of gravity and examiner intervention. The results indicated high reproducibility, the maximal measurement uncertainty being within 1 N. Intra-class correlation coefficients ranged from 0.85-0.98 for intra-rater reproducibility, and 0.81-0.96 for inter-rater reporoducibility. The coefficient of variation was lower than 10%. The device described may be suitable for routine clinical assessment of patients after hip surgery.
