RESUMO

The aim of the study was to determine the content of vaskuloendotelian growth factor and nitric oxide in children with chronic inflammatory diseases of the stomach and duodenum. The study involved 63 children with chronic inflammatory diseases of the gastroduodenal zone. Substrate study was serum. The data obtained were compared with a group of healthy children. The highest possible content vaskuloendotelian growth factor noted in the group of children with duodenal ulcer in the acute phase and in the group of chronic gastroduodenita associated with Helicobacter pylori. According to the results of the study established the role of nitric oxide and vaskuloendotelian growth factor in the pathogenesis of gastroduodenal diseases.

UNLABELLED: The aim was to investigate apoptotic dynamics by IL-2 and TNF-α levels in the blood serum of children with chronic gastroduodenites. MATERIALS AND METHODS: There were examined 90 children with chronic gastroduodenitis. Cytokine level in the serum was studied at admission and in dynamics. RESULTS: Chronic gastroduodenitis is accompanied by the increase of IL-2 and TNF-α concentration thus reflecting the activity of apoptotic processes. The decrease of the markers' (IL-2 and TNF-α) levels after 10-11 days, 3 and 9 months after treatment indicates the decrease of apoptotic activity. In hyperplastic and atrophic gastritis there has been registered the highest and the lowest concentration of the markers, respectively. H. pylori contributes to the TNF-α production mainly.

OBJECTIVE: To evaluate the therapeutic effect of Liujunzi decoction combined with Zuojin pills in treating the radioactive duodenitis and their mechanism, and compare with clinical routine acid suppressants combined with mucous membrane protective preparations to study the mechanism of their efficacy. METHOD: According to the study of Williams J P and characteristics of duodenitis, and by reference to the radiation enteritis modeling standard, we took the lead in establishing the mouse radioactive duodenal injury model. The model mice were randomly divided into the control group (n = 26), traditional Chinese medicine (TCM) group (n = 16) and the western medicine (oral administration with famotidine 0.5 mL + almagate suspension 0.3 mL per mouse, once a day) group (n = 16). After the standard administrating, such objective indexes as general condition, weight, changes in health score, pathology and expression of inflammatory factors were observed to evaluate the efficacy. RESULT: The radioactive duodenitis model of mice was successfully established with 12 Gy. Mice in the control group suffered from weight loss, anorexia, low fluid intake, loose stools, and occasionally mucous bloody stool, poor spirit, dim fur, lack of exercise and arch back. Mice in drug intervention groups were generally better than those in the pure irradiation group. The IL-6, IL-1beta, TNF-alpha mRNA expressions in spleen and mesenteric lymph node tissues in TCM and western medicine groups showed a declining trend compared with the control group. Their concentrations in peripheral blood serum also slightly changed. The TCM group revealed notable advantage in reducing inflammatory factors. The microscopic observation showed that a better mucosa repair in intervention groups than the pure irradiation group. The improved Chiu's scoring method showed a statistical significance in the difference between TCM and western medicine groups (P < 0.05). CONCLUSION: Liujunzi decoction combined with Zuojin pills could treat acute radiation enteritis, regulate organic immunity, and inhibit acute injury, promote local tissue repair, with the potential to resist such adverse effects as radiation intestinal fibrosis. The regulation of inflammatory factor release is one of efficacy generation mechanisms.

At present, chronic gastroduodenitis (CGD) occupies one ofthe leading places in the structure of diseases of the gastrointestinal tract in children. In the etiology of CGD, along with the leading pathogenic Helicobacterpylori (HP), the role of the fungal flora increased. The aim of the work was to evaluate the functional activity of neutrophils in children with the CGD, associated with HP and Candida albicans. Among 110 children in the age from 7 to 17 years with chronic gastroduodenitis, associated with Helicobacter pylory(HP), as well as the association of HP with Candida albicans and the markers of secondary immune insufficiency, a study of the phagocytic activity and immune phenotype of neutrophils by flow cytofluorimetry was conducted. Differentiated peculiarities of the phagocytic activity of neutrophils in association with bacterial pathogens (HP) and fungal flora were identified. The transformation of the immune phenotype was combined with a significant depression of the phagocytic and microbicidal functions, more pronounced with the association of HP and Candida albicans. Circulating mannano protein antigen of Candida albicans influenced on the surface of phenotype of neutrophils, increasing the expression of protopathic and HLADR-receptors, and decreasing the expression of adhesion receptors and cytolysis. Thus, in case of chronic gastroduodenitis in children, there was a considerable transformation of the phenotype of neutrophil with differentiated characteristics at the association with bacterial (HP) pathogens and fungal flora. The obtained data should be taken into account when carrying out medical activities, and the doctors should include in the composition of complex therapy of CGD, associated with Candida albicans, drugs, aimed at immunocorrection of the identified violations

In order to understand better the molecular mechanisms involved in the pathogenesis of anaemia of inflammation, we carried out a time-course study on the effects of turpentine-induced acute and chronic inflammation on duodenal proteins involved in Fe absorption in mice. Expression levels of these proteins and hepatic hepcidin and serum Fe levels were determined in inflamed mice. In acutely inflamed mice, significantly increased expression of ferritin was the earliest change observed, followed by decreased divalent metal transporter 1 expression in the duodenum and increased hepcidin expression in the liver. Ferroportin expression increased subsequently, despite high levels of hepcidin. Hypoferraemia, which developed at early time periods studied, was followed by increased serum Fe levels at later points. The present results thus show that acute inflammation induced several changes in the expression of proteins involved in duodenal Fe absorption, contributing to the development of hypoferraemia. Resolution of inflammation caused attenuation of many of these effects. Effects in chronically inflamed mice were less consistent. The present results also suggest that inflammation-induced increases in ferritin appeared to override the effects of hepcidin on the expression levels of ferroportin in enterocytes.

The goal of the investigation was to detect and assess the specific features of abnormal biochemical and immunological parameters in children living in the industrially developed areas in order to solve the tasks of early diagnosis and to enhance the efficiency of prevention of the chronic pattern of gastroduodenal diseases. It was found that children with increased contamination of biological media had more active inflammatory reactions with a trend towards the chronic pattern of an inflammatory process, altered antioxidant defense and a more common chronic inflammatory process in the biliary tract, and more significant cytolysis syndrome with gallbladder concentration dysfunction.

RESUMO

The paper considers problems in the assay of volatile fatty acids in blood by capillary gas chromatography using a flame ionization detector. The blood levels of different volatile fatty acids were analyzed in children with environment-induced chronic gastroduodenitis with secretory failure. The performed studies allowed the authors to recommend that the altered blood levels of volatile fatty acids as an indicator of the environment-modified influence of poor environmental factors on the course of the disease.

RESUMO

The usage of soy products for dietary treatment of children with erosive-ulcer lesions of alimentary tract brings positive clinical dynamics, normalization of colon function and protein metabolism, acceleration of erosive defects repair, immune status correction. The obtained results justify inclusion of soy products into duodenal pathology treatment--especially when accompanied by protein deficiency.

RESUMO

43 patients and 20 healthy children, aged 11-15, were examined. Increased contents of primary, secondary, intermediary and final products of lipid peroxidation (LPO) in serum lipids as well as an intensified antioxidant activity were registered in children with erosive gastroduodenitis. Ventramine, when used in the treatment of erosive gastroduodenitis in children, arrests rapidly the clinical signs of the disease and normalizes LPO.

A total of 97 patients were studied for the dynamics of hemostasis by combined methodological approaches. Of these, 15 subjects had chronic erosive gastroduodenitis, 22--duodenal ulcer in the phase of exacerbation, 60 patients presented with that concurrent with chronic pancreatitis. Time-related indices were determined for the stage of the illness concerned.

Parameters were studied characterising the system of blood aggregate state control (BASC) in 693 patients with chronic gastritis and duodenitis. Suggested in the paper are six variants of the hemocoagulation and fibrinolysis inadequacy, such as latent hypercoagulation (a decline in spontaneous fibrinolysis), manifest hypercoagulation (enhanced tolerability of plasma to heparin), isolated paracoagulation (positive fibrinogen B or ethanol test), latent hypocoagulation (clot retraction is reduced or levels of fibrinogen are raised), structural hypocoagulation (spontaneous fibrinolysis is on the increase), manifest hypocoagulation (lowered tolerance of plasma to heparin). An algorithm of the BASC system is submitted allowing the diagnosis to be rendered automatic and differentiated therapy treatments to be prescribed.

RESUMO

A method for measuring fucose not bound to proteins in the serum is developed. The level of this enzyme is shown to increase with age. It is increased in adult patients with myeloma, liver cirrhosis, and rheumatic fever and in children with exacerbations of chronic osteomyelitis and gastroduodenitis.

The effects of acid, infectious (Helicobacter pylori) and neuroendocrine factors on the course and outcome of chronic duodenitis were studied in 57 patients aged 15-35 years. Within 5-year follow-up ulcer emerged in 26% of duodenitis patients. Ulceration occurred as a result of high acid production in the basal and stimulated phases, contamination of pyloric-antral mucosa with Helicobacter pylori, unbalance of serum hormones T4, TTH, FSH, ACTH. In risk of ulcerogenesis there were psychological shifts with appearance of pathological reaction to the disease. Consideration of pathogenetic specificity of different duodenitis forms and basing on objective and subjective criteria allow identification of ulcer risk group and early start of optimal therapy.

The Plasma motilin level in 44 cases patients with Spleen-Yin deficiency (SYD) was observed and compared with Spleen-Qi Insufficiency (SQI), the normal control group. SYD patients were treated using self-made Shen Rou Yang Zhen Tang oral liquor. The results showed that plasma motilin level was higher significantly than that of the control group (P < 0.01), but was remarkably lower than that of SQI. The difference was significant among the three groups (P < 0.01). The abnormal incidence of positive plasma motilin was 65.91% and 75.00% respectively. It is conjectured that the plasma motilin level in SYD was related to excitability of vagus nerve, disturbance of gastro-intestinal internal environment, indigestion and malabsorption of GI tract.

Reactivity of organism was studied in a total of 48 patients with chronic duodenitis, actually in the zone of increased radiation. Hypo- and normotensive type reactions were found to prevail, each having its peculiar clinical picture. While studying the lipid peroxidation- and antioxidant defence systems, changes were identified in those parameters characterizing the above systems, their degree being higher in blood leukocytes. It is recommended that the data obtained be used for determining a correcting therapy.
