RESUMO

BACKGROUND: Sleep disturbance and burnout are prevalent among primary and secondary school teachers. Nevertheless, little is known about the relationship between sleep disturbance and burnout, and the mechanisms connecting this link. Our study aimed to explore the relationship between sleep disturbance and burnout among urban teachers, as well as to investigate this influencing mechanism further with resilience as a moderator. METHODS: 14,218 primary and secondary school teachers provided valid data. We assessed demographic information, sleep disturbance, burnout, and resilience. Multivariable logistic regression, Spearman correlation, and moderation analyses were used to evaluate the relationship between sleep disturbance and burnout and the moderating role of resilience. RESULTS: Of the participants, the prevalence of sleep disturbance and sleep deprivation among teachers was 20.0% and 31.7%, respectively. Additionally, 58.4% of teachers reported moderate or severe burnout, and 15.3% had both sleep disturbance and burnout. Sleep disturbance was significant and positively related to burnout. Resilience was found to moderate the association between sleep disturbance and burnout. CONCLUSIONS: These findings suggested strong associations between sleep disturbance and burnout. Interventions in improving resilience may protect teachers with sleep disturbance from burnout.

RESUMO

Many parents have days where they encounter emotional exhaustion, emotional distance from their children, and feeling fed up with being a parent. Some parents experience these characteristics to a severe extent-a clinical phenomenon termed parental burnout. Parental burnout arises when parents chronically endure severe stress without sufficient resources to cope, which may lead to detrimental consequences not only for the parent but also for their partner (e.g., marital conflict) and children (i.e., neglect and violence). However, uncertainty remains regarding how these features interact and trigger one another over time (potentially becoming increasingly severe), nor how the daily variations of the family context influence these features. Therefore, in this study, we recruited 50 parents (with main analyses focusing on 43 mothers with a co-parent, and sensitivity analyses with the full sample) from the general population to rate the core features of parental burnout and the family context daily over 56 days. We used multilevel vector autoregressive models to generate network models. Results suggest that exhaustion contributes to parental burnout: It self-predicts and is closely associated with feeling fed up and finding children difficult to manage. Distance, by contrast, is mainly negatively connected to sharing positive moments with children. Contextual variables also interact with parental burnout features, illustrating the relevance of examining parenting within the family system context. If future research confirms a central role of exhaustion in parental burnout development, prevention efforts can focus on decreasing parental exhaustion. RESUMO

INTRODUCTION: Medical work requires complete dedication to work, long hours of absence from home and family, as well as personal sacrifice on various issues. This results in impaired quality of life, and the phenomenon of depression and suicide is more common among physicians than in the general population. The impairment in quality of life and function was defined for years as burnout, and this figure of speech reflected the physician's inability to cope with existing pressures. In recent years, the concept of 'moral injury' has been emerging to explain a significant part of the burnout phenomenon. The term describes the poor mental state of the physician who is forced to make immoral choices as part of the organizational demands. This perception fundamentally changes the concept that the poor function at work and in personal life is only the weakness of the doctor who lacks coping resources and resilience, therefore needing personal support such as yoga and mindfulness exercise. It emphasizes the organization responsibility for change, which will minimize moral dilemmas, thereby the moral injury. This will allow the doctor to live professional, as well as personal life, in an optimal way.

RESUMO

OBJECTIVE: This study aimed to investigate the association of burnout and vital exhaustion with measures of glycemic control and type 2 diabetes (T2D) in a systematic review and meta-analysis. METHODS: PubMed, Embase, and PsycINFO were searched from inception to April 2, 2020. Data extraction and quality assessment were performed using the Effective Public Health Practice Project tool. When possible, results were meta-analyzed using random-effects models and rated using the Grading of Recommendations, Assessment, Development and Evaluation. RESULTS: A total of 5317 titles/abstracts were screened, 140 articles were read full text, of which 29 studies were included. Eighteen studies were cross-sectional, three prospective and eight were case-control studies. Burnout and vital exhaustion were significantly associated with T2D, with a pooled odds ratio of 1.8 (95% confidence interval [CI] = 1.4 to 2.4, I2 = 79%; 9 studies). Glycated hemoglobin A1c levels were not significantly higher in people with burnout and vital exhaustion, compared to those without, with a pooled standardized mean difference of 0.35 (95% CI = -0.62 to 1.33, I2 = 98%; 7 studies). In addition, no differences in glucose levels were observed (standardized mean difference = 0.02, 95% CI = -0.26 to 0.30, I2 = 90%; 9 studies). Sensitivity analyses showed no decrease in heterogeneity when excluding studies with low quality (I2glucose = 89%) or studies with a study n < 40 population (I2T2D = 77%). The level of Grading of Recommendations, Assessment, Development and Evaluation evidence was moderate to low quality because of 18 studies having a cross-sectional design. CONCLUSIONS: Burnout and vital exhaustion might be associated with a higher risk of T2D, but not with glycemic control. Methodological shortcomings and high heterogeneity of the studies included complicate the interpretation of our results.

RESUMO

Parental burnout (PB)-a condition characterized by intense exhaustion related to parenting, emotional distancing from one's children, and a loss of parental fulfillment-has received increasing attention in recent years, even more since the worldwide COVID-19 crisis and the confinement of parents with their children. This crisis put the spotlight on parents' suffering, and the need to better understand parental burnout and how to best assess and treat it emerged as a priority. This brief article introduces the Thematic Issue of New Directions in Child and Adolescent Development focused on the measurement of parental burnout across various regions of the world. It briefly reviews the concept of parental burnout, its phenomenological experience, its etiology and consequences, and its measurement.

RESUMO

AIMS: This study aimed to corroborate the main hypotetized dimensions of diabetes burnout, and its distinction from diabetes distress and depressive symptoms among adults with type 1 diabetes (T1D). METHODS: A qualitative descriptive study was employed to collect data from 31 eligible participants using semi-structured individual phone interviews. Participants were recruited through purposive sampling among a larger sample of adults with T1D in a previous study. Data was analyzed using qualitative content analysis including immersion, reduction, and interpretation. RESULTS: Exhaustion and detachment were frequent manifestations of diabetes burnout that often accompanied by instances during which participants experienced a loss of control over diabetes. Whereas individuals' support systems could either help or hinder diabetes burnout. Participants saw burnout as separate but closely related to distress and depressive symptoms, with some suggesting correlations among burnout, distress, and depressive symptoms, while others expressed that distress caused their burnout and depressive symptoms. CONCLUSIONS: Diabetes burnout reflects a significant yet unexplored concept which might explain the complexities of suboptimal self-management and poor diabetes outcomes. Emerging evidence suggesting diabetes burnout as a distinct concept calls for more studies to mitigate diabetes burnout as an obstacle to optimal diabetes care.

RESUMO

Research confirms that the mental health burdens following community-wide disasters are extensive, with pervasive impacts noted in individuals and families. It is clear that child disaster outcomes are worst among children of highly distressed caregivers, or those caregivers who experience their own negative mental health outcomes from the disaster. The current study used path analysis to examine concurrent patterns of parents' (n = 420) experience from a national sample during the early months of the U.S. COVID-19 pandemic. The results of a multi-group path analysis, organized by parent gender, indicate good fit to the data [X2(10) = 159.04, p < .01]. Results indicate significant linkages between parents' caregiver burden, mental health, and perceptions of children's stress; these in turn are significantly linked to child-parent closeness and conflict, indicating possible spillover effects for depressed parents and compensatory effects for anxious parents. The impact of millions of families sheltering in place during the COVID-19 pandemic for an undefined period of time may lead to unprecedented impacts on individuals' mental health with unknown impacts on child-parent relationships. These impacts may be heightened for families whose caregivers experience increased mental health symptoms, as was the case for fathers in the current sample.

RESUMO

Many GI training programs have needed to adjust to the serious disruption to the training and education of fellows worldwide due to the COVID-19 pandemic. A silent problem that has arisen within programs is the issue of burnout among their trainees. Burnout is common among gastroenterologists, especially in fellows (Keswani et al. in Gastroenterology 147(1):11-14, 2014. https://doi.org/10.1053/j.gastro.2014.05.023 , Am J Gastroenterol 106(10):1734-1740, 2011. https://doi.org/10.1038/ajg.2011.148 ), with negative consequences to patient care and the safety of the trainees if not effectively dealt with. In this article, the author describes several additional factors potentially contributing to the intensifying burnout of the fellows in their home institution during this pandemic. Moreover, he describes specific practical interventions that the hospital and program have taken in order to address these factors.

RESUMO

PURPOSE: This cross-sectional study aims to evaluate the role of chronic diseases, and their interactions with age and gender, on the emotional exhaustion component of the burnout syndrome. METHODS: Data came from the Salveo Study conducted in 2009-2012. It contained a random sample of 2075 Canadian workers employed in 63 workplaces. Multilevel regression models were estimated. Main effects of chronic diseases were first evaluated, and then age-chronic diseases interactions were tested. Analyses were performed on the total sample and stratified by gender. All analyses were adjusted for work conditions decision latitude, physical and psychological demands, work hours, social support and rewards. RESULTS: Mental and behavioural disorders, diseases of the nervous system, the musculoskeletal system and connective tissue, and genitourinary system are associated with a higher level of burnout symptoms. Associations with mental and behavioural disorders, and diseases of the nervous system are stronger for men than women. Age increases the association of behavioural disorders, diseases of the nervous system, and genitourinary system and burnout. Age may moderate the association of nervous and genitourinary systems diseases with burnout in women. CONCLUSION: Workplaces must be more proactive to better recognise the role of chronic diseases on burnout and to implement preventive measures. The development of interventions towards specific risk groups is needed.

RESUMO

OBJECTIVES: To determine the prevalence of imposter syndrome (IS) among Saudi adults and its association with burnout. METHODS: Using non-probability convenience sampling, we carried out a cross-sectional analytical study among adults in Northern Saudi Arabia. Data collection toll was a self-administered questionnaire containing young imposter scale (YIS), Maslach burnout inventory (MBI) and depression, anxiety, and stress scale 21 (DASS 21). These 3 scales are valid and reliable measurement tools for measuring IS, burnout, and psychological morbidity. RESULTS: In the study sample (n=384), imposterism was present among 222 adults (57.8%). Moderate and severe symptoms of depression were present in 217 (56.5%), symptoms of stress in 210 (54.7%), and symptoms of anxiety in 126 (32.8%). Burnout was measured in 3 domains of depersonalization, emotion exhaustion, and personal accomplishments. High degree of depersonalization was found in 221 (57.6%), higher emotional exhaustion in 246 (64.1%) while lower personal accomplishment was present in 126 (32.8%). We found statistical association between IS and gender (p less than 0.01), depression (p less than 0.001) stress (p less than 0.001) and all 3 domains of burnout (p less than 0.01). Imposter syndrome was not found associated with anxiety (p=0.97). Young imposter scale score was associated with all the 3 domains of burnout even after controlling for depression, anxiety, and stress. CONCLUSION: Imposter syndrome is frequent among Saudi adults and associated with all 3 burnout domains. Mental health domains like burnout, depression, stress, and anxiety were found associated with IS.

RESUMO

BACKGROUND: One of the most important psychosocial factors to which doctors are exposed is the burnout syndrome, whose origin is possibly the chronic stress experienced during the development of their professional practice. OBJECTIVE: To determine the prevalence of burnout syndrome in medical specialists and risk factors. MATERIAL AND METHODS: Observational, descriptive and cross-sectional study, census of medical specialists physicians in a regional hospital, with the participation of 196 of them, identification card and self-administered Maslach Burnout Inventory-Human Services Survey participated. Descriptive statistics and inferential analysis with SPSS 15.0 Epidat 3.1. RESULTS: 86.7% response. Burnout at 45.9%. Significant differences in those under 40 and under 15 with a stable partner. With the syndrome, a negative correlation was observed between emotional exhaustion and depersonalization and positive with a lack of personal accomplishment at work. CONCLUSIONS: Burnout is frequent (45.9%) in medical specialists, therefore, it is considered necessary to establish preventive or intervention measures to reduce the prevalence found.

Introduction: Health educators and general workers in the sector are among the people vulnerable to stress and burnout Objective: To discuss, based on the scientific literature, burnout levels and coping strategies in higher education professors. Methods: Systematic review of articles published between 2010 and 2018 in the databases EBSCO host, SCIELO, ERIC, SCIENCE DIRECT, SCOPUS, REDALYC, DOAJ, and LILACS. In the search strategy, the Preferred Reporting Items for Systematic Review and Meta-Analyzes (PRISMA) flowchart was used. Eighteen articles (in Spanish, Portuguese, and English) were reviewed after applying the inclusion and exclusion criteria. Conclusions: It is not clear if burnout exists in higher education professors. High levels of exhaustion and depersonalization are slightly associated with high work commitment. Job stress and high level of exhaustion are associated with less job satisfaction. Relationship between high level of exhaustion is associated with younger professors with less experience. Exhaustion is associated with academic factors such as teaching hours. Burnout is associated with research activities. Increased stress is associated with an employment condition as contracted. No significant relationship was found between burnout and feelings of self-efficacy or teaching performance, or between emotional fatigue and occupational satisfaction. RESUMO

Physician burnout is an important issue that can have serious implications for both physicians and patients. Many of the techniques used by larger medical groups attempt to compel improvements in patient satisfaction scores at the potential cost of increased physician burnout. Because burnout has been associated with poorer patient care and experience, medical groups large and small should work aggressively to reduce the causes of burnout as a way to also improve patient experience. In this contribution, the patient experience measure and the electronic medical record are reviewed in the context of the regulatory and bureaucratic pressures they place on physicians to examine how they may contribute to burnout and, therefore, worsen patient experience.

RESUMO

BACKGROUND: People with type 1 diabetes are at increased risk for diabetes burnout, resulting in suboptimal diabetes care and quality of life. While the existence of diabetes burnout is widely acknowledged, there is no evidence-based definition, means of measurement, or interventions to address it. OBJECTIVE: This study was aimed at increasing our understanding of the lived experiences of diabetes burnout among adults with type 1 diabetes. METHODS: A qualitative descriptive study was conducted with a sample of 18 adults with type 1 diabetes who reported a current or previous experience of diabetes burnout. Data were collected using in-depth interviews and analyzed using qualitative content analysis. RESULTS: Four main themes were identified: mental, emotional, and physical exhaustion from having diabetes; detachment from illness identity, diabetes self-care, and support systems; contributing factors to diabetes burnout; and strategies for preventing or overcoming diabetes burnout. CONCLUSION: Although exhaustion is an entry point for diabetes burnout, the findings suggest that detachment from illness identity, diabetes self-care, and support systems form a core component. Detachment may explain poor outcomes in individuals experiencing diabetes burnout.
