RESUMO

Objectives: To estimate the incidence of men seeking specialized care and receiving treatment for hydro or spermatocele complaints. Also, to determine the risk of complications of treatment. Materials and methods: The total number of men living in Sweden each year from 2005 to 2014 was used to calculate incidence and age distribution of adult (≥18 years) men seeking specialized healthcare with either hydro or spermatocele. This was done by using nationwide registries, mandatory by law. They contain information on primary or discharge diagnosis, procedure codes and antibiotic prescriptions. Also, complication rates comparing aspiration (with or without sclerotherapy) and conventional surgery were analysed. Results: The incidence of men with either hydro or spermatocele diagnosis in specialized healthcare was ∼100/100,000 men. The treatment incidence was 17/100,000 men. Orchiectomy was used as primary treatment in 2.4% of cases. The risk of experiencing a complication was clinically and statistically significantly increased with conventional surgery as compared with aspiration, 17.5% (1607/9174) vs 4.6% (181/3920), corresponding to relative risk of 3.79 (95% CI = 3.27-4.40). Hematoma and infections were the most common complications. Conclusion: Hydro and spermatoceles are common, affecting elderly men. Aspiration seems advantageous with respect to complications and can be recommended due to the benign course of the disease. The indication for conventional surgery might be questioned such as the use of orchiectomy as primary treatment.

AIMS: To detect systematically visible urogenital malformations (VUGM) in adolescents and describe their epidemiological and clinical aspects. PATIENTS AND METHODS: It was a cross sectional, descriptive and analytical study, conducted from February to August 2012. Upon 2724 adolescents from 10 to 19 years old, of the public secondary schools of Cotonou. Among the 26,594 registered pupils, 2724 were included and examined after a randomized sampling of 30 clusters. RESULTS: The mean age of the pupils was of 15 ± 2.30 years (11-19 years), with a peak at 18 years. The prevalence of the VUGM was slightly higher (9.57%) in individuals those had parents with low socioeconomic level. The prevalence of the VUGM was high in Yoruba and related (P=0.02). It was stronger (9.84%) among subjects in puberty period's than in pre-puberty (6.69%) (P=0.03). From the 253 having VUGM, 78 (30.83%) had 146 functional signs. It was indexed 266 affections and of malformatives associations in 21 pupils (8.30%). Varicocele was the most frequent (5.47%), followed by inguinal hernia (0.99%), hydrocele (0.88%), anomalies of testis migration (0.59%), cyst of spermatic cord (0.51%), insulated testicular hypotrophy (0.48%), micro-penile (0.33%), hypospadias (0.22%), penile curvature (0.22%) and epididymis cyst (0.07%). CONCLUSION: Among patients who had VUGM, the majority had at least a varicocele, which occupies so far the first place within these malformations at the adolescents in Cotonou.

OBJECTIVE: To sonographically investigate whether mountain bikers have a higher prevalence of scrotal abnormalities compared with on-road cyclists. DESIGN: We studied 85 male mountain bikers (mean age: 25 years; range 17-45 years) and 50 male on-road cyclists (mean age: 23 years, range 15-46 years) with regard to scrotal findings on ultrasound (US). SETTING: Medical University Innsbruck, Austria. PARTICIPANTS: Only males who gave a history of extensive mountain biking or on-road bicycling (2 hours or more per day on 6 days a week with a covered distance of more than 5,000 km/year) were entered in our study. INTERVENTIONS: In addition to clinical evaluation, a standard ultrasonographic examination of the scrotum was performed using a linear array transducer operating at a frequency of 8.0 MHz (Acuson Sequoia 512). MAIN OUTCOME MEASUREMENTS: The sonographic findings obtained in mountain bikers were compared with those obtained in on-road cyclists. RESULTS: Eighty of 85 mountain bikers (94%) and 24 of 50 on-road cyclists (48%) presented with abnormal findings on scrotal US. Abnormal US findings in mountain bikers included scrotoliths in 69 bikers (81%), spermatoceles in 39 bikers (46%), and epididymal calcifications in 34 bikers (40%). US findings in on-road cyclists were scrotoliths in 8 cyclists (16%), spermatoceles in 13 cyclists (26%), and epididymal calcifications in 6 cyclists (12%). The overall number of scrotal abnormalities was significantly greater in mountain bikers than in on-road cyclists (P < 0.001). CONCLUSIONS: Mountain bikers compared with on-road cyclists have shown to be at a higher risk for scrotal disorders on US examination. Not only protective measures but also the awareness of the bikers are required to reduce the potential risk. Further studies should be undertaken to determine the clinical significance of the sonographic changes.
