RESUMO

We investigated the efficacy of non-metal clasp dentures (NMCDs) with regard to the oral health-related quality of life (OHRQoL) and compare the findings with those for conventional metal clasp-retained dentures (MCDs). This single-centre, randomised controlled, two-phase, open label, cross-over trial included 28 partially dentate individuals. The patients were randomised to receive MCDs followed by NMCDs, or the opposite sequence (n = 14 in each group); each denture was worn for 3 months. OHRQoL was evaluated using the Oral Health Impact Profile-Japanese version (OHIP-J) at entry (T-entry; before treatment with the first denture) and at 3 months after treatment with each denture (T3). An examiner evaluated denture stability, oral appearance and surface roughness before denture delivery (T0) and at T3 and denture hygiene at T3. A total of 24 patients completed the trial. There were no complications related to the dentures, abutment teeth or denture-bearing mucosa during the follow-up periods for both dentures. The mean OHIP summary score was lower for NMCDs than for MCDs, and the difference (9 points) was greater than the minimal important difference (6 points), indicating the difference was clinically relevant. The effect size was medium (0·70). Statistical analyses with linear mixed models found a significant effect of the denture type on the OHIP summary score and scores for the Oro-facial appearance, Oro-facial pain and Psychological impact domains (NMCD < MCD; P < 0·05). The results of our study suggest that NMCDs allow for better OHRQoL compared with MCDs.

Ingested foreign bodies can prove a significant and potentially fatal threat. A case is reviewed where a patient swallowed his upper Kennedy Class I removable partial denture, highlighting the potential problems of this prosthesis design and the management of this scenario.

RESUMO

OBJECTIVE: This paper presents a case report about an unnoticed swallowing of a partial denture impacted in the esophagus in a 74-year old man without serious complications. BACKGROUND DATA: Dental prostheses (removable/fixed dental prostheses or fragments of dentures) appear to constitute a significant proportion of swallowed of impacted foreign bodies in the orolaryngopharynx. CONCLUSION: This case emphazises that is advisable to contraindicate this type of unilateral removable partial denture to replace teeth as temporary or definitive dental prosthesis.

RESUMO

Paper is devoted to the actual problem in stomatology - removable dentures influence upon the intensity of prosthetic bed relief changes. Study of prosthetic bed relief in patients who for 2 years had used plate removable and clasp dental prostheses was done. The most pronounced prosthetic bed relief changes were seen under plate type of prostheses with prolonged defects the least - in patients with plate type of prostheses with short defects. In cases of end defects under clasp dental prostheses changes of prosthetic bed relief exceeded relief changes under plate type of prostheses during all terms of use. Based upon the received data algorithm of patient conducting during different terms of use by plate removable and clasp dental prostheses.

In cases where there are no suitable natural undercuts on the teeth which can be used to provide clasp retention for a partial denture artificial undercuts can be created by the application of a composite filling material. An in-vitro investigation was undertaken to assess the surface integrity of three different composite restorative materials following repeated cycles of movement of cobalt-chromium I-bar clasps over artificial undercuts created using the composite materials. A laser reflection confocal microscope was used to quantify surface changes in the composite specimens. Simulation of the equivalent of 5 years of clinical usage showed that a composite resin with the smallest particle size exhibited the least surface wear.

RESUMO

A female patient, aged 62 years, unexpectedly swallowed a removable partial denture (RPD) during the supper. The doctor failed to take out the RPD from her stomach under the gastroscope during the emergency treatment at the first day. 2 days later, the PRD blocked the intestinal tract and its metal clasps penetrated the wall of the intestine near the ileocecal junction, it soon leaded to enterobrosis and diffuse peritonitis. At last, abdominal operation was conducted to take the RPD out from the intestine and enterobrosis was patched up successfully. Diffuse peritonitis was also cured by intravenous infusion of antibiotics for about 1 week.

RESUMO

PATIENT: A 50-year-old female who wore a cast-clasp-supported removable partial denture (RPD) complained of speech disturbance, because the clasps disturbed her tongue movement during speech. She wished to receive dental implant treatment, but it was difficult to solve the esthetic problem by the planned implant-supported fixed partial denture. Therefore, the RPD (overdenture) supported by magnetic attachments was inserted to recover the speech disturbance. DISCUSSION: The treatment outcome of this patient was evaluated with a self-administered questionnaire. The results suggested that the RPD with magnetic attachments improved the patient's pronunciation, esthetics and quality of life. CONCLUSION: It was suggested that the magnetic attachment overdenture effectively solved the speech disturbance.

RESUMO

The design of removable partial dentures (RPDs) is an important factor for good prognostication. The purpose of this study was to clarify the effectiveness of denture designs and to clarify the component that had high rates of failure and complications. A total of 91 RPDs, worn by 65 patients for 2-10 years, were assessed. Removable partial dentures were classified into four groups: telescopic dentures (TDs), ordinary clasp dentures (ODs), modified clasp dentures (MDs) and combination dentures (CDs). The failure rates of abutment teeth were the highest and those of retainers were the second highest. The failure rates of connectors were generally low, but they increased suddenly after 6 years. Complication and failure rates of denture bases and artificial teeth were generally low. Complication and failure rates of TDs were high at abutment teeth and low level at retainers. Complication and failure rates of ODs were high at retainers.

Estudou-se a capacidade retentiva de grampos tipo "T" em prótese parcial removível (PPR) comparando-se armaçöes confeccionadas em liga de Co-Cr e Ti. Foram utilizados dez corpos de prova com cada tipo de liga que foram levados a um dispositivo desenvolvido para simular a inserçäo e remoçäo de armaçöes de PPR, como acontece clinicamente em pacientes, por períodos de 6 meses, 1, 2, 3, 4 e 5 anos. Depois de cada período de simulaçäo, estes corpos de prova eram levados a uma máquina de traçäo para se medir a quantidade de retençäo ainda presente em cada armaçäo, respectivamente em ambiente seco e umedecido com saliva artificial; depois disso, eram pesados em uma balança de precisäo para se observar prováveis perdas de peso, tanto das armaçöes quanto dos dentes naturais dos corpos de prova. A análise estatística dos resultados nos permitiu concluir que näo houve diferença estatisticamente significante de retençäo entre as armaçöes confeccionadas em Ti e Co-Cr, nos períodos de tempo simulados, tanto em ambiente seco quanto em umedecido com saliva artificial, e que foi estatisticamente significante a perda de peso tanto das armaçöes quanto dos dentes naturais dos corpos de prova da utilizaçäo do Ti e do Co-Cr nos referidos tempos

RESUMO

Three patients presented with their dentures stuck in their mouths due to a retainer clasp becoming embedded in either the anterior fauces or the soft palate. All three dentures were extricated successfully and the resulting puncture wounds were left to heal spontaneously with only antibiotics and analgesics. In all three cases, a sharp pointed molar clasp tip was found to be a common factor. The management and prevention of such cases are presented.

RESUMO

Five pairs of bilateral distal-extension removable partial dentures with different clasp designs were constructed on the same dried human mandible in order to compare their load distribution characteristics. The technique of holographic interferometry was used. Among the dentures tested, those with RPI and back action clasps having mesial rests provided the best results from the mechanical point of view. The most unfavourable behaviour was presented by the Akers clasps design. The RPA design and dentures having clasps with continuous MOD rest gave intermediate results.

The effect on the gingiva of a variety of relationships of a removable partial denture connector to the gingival margin was investigated in 10 subjects. Acrylic resin baseplates were designed incorporating a variety of relationships of the connector to the gingival margin. The appliances were worn for a 12 hour period daily, for 21 days. At baseline and at days 7, 21 and 49, plaque index, gingival index and probing pocket depth measurements were recorded, and subgingival plaque samples were examined using darkfield microscopy. No increase in plaque accumulation was detected in any area at any time interval. There was a significant increase in gingival index at day 21 in areas where the appliance covered the gingival margin. Small increases in probing pocket depth were recorded at day 21, there being no difference between any of the test areas. Baseline values were re-established by day 49. No significant differences were detected in the percentage of motile organisms or spirochaetes counted in the subgingival plaque samples collected from any area. It was concluded that deterioration in gingival health occurred rapidly following the insertion of a removable appliance, and that coverage of the gingival margin, irrespective of the degree of gingival relief, had a detrimental effect.
