During February and March 2019, an accumulation of clinically similar erythematous plantar nodules was observed at the University Children's Hospital and several primary care facilities in Uppsala, Sweden. At least 20 children sought medical advice, and all cases presented with a recurrent plantar hidradenitis after within a day after visiting Uppsala's largest waterpark and arena for swimming. The presented symptoms were identical with a condition called pseudomonas hot-foot syndrome described in the literature. An investigation led by the local public health authorities revealed heavy growth of Pseudomonas aeruginosa in water-filled toys in a children's play area and in samples taken from the floor of a pool where the surface was partly damaged. After closing the affected part of the pool and removal of the contaminated toys, no more people sought medical advice. Pseudomonas hot-foot syndrome is believed to be more frequent than diagnosed today, and increased awareness is essential to avoid unwarranted diagnostic tests and treatments, and to identify and eradicate the source of infection.

Neutrophilic eccrine hidradenitis (NEH) is a nonspecific clinicopathological reaction pattern, classified as a neutrophilic dermatosis, that usually develops in patients receiving chemotherapy for a hematologic malignancy. More rarely, it has been reported in association with infectious agents such as Serratia and Enterobacter species, Staphylococcus aureus, and human immunodeficiency virus. We describe 3 cases of infectious eccrine hidradenitis secondary to infection with Nocardia species, Mycobacterium chelonae, and S aureus. Histological findings revealed a dense infiltrate with perivascular and periductal neutrophils in the dermis. In the eccrine glands, there was vacuolar degeneration and necrosis of the epithelial cells. Our cases support the assertion that NEH is a characteristic cutaneous response to nonspecific stimuli. Clinical and histopathological findings of infectious and noninfectious NEH are generally indistinguishable and when NEH is suspected, the possibility of an infectious association must be investigated by skin tissue culture. In this article we also discuss differential diagnoses and review the literature.

Infectious eccrine hidradenitis (IEH), which usually manifests as singular or multiple erythematous papules or plaques, is a rare dermatosis involving an infectious agent and histologic findings identical to that of neutrophilic eccrine hidradenitis (NEH). We report a case of IEH in a 24-year-old woman who developed a pruritic, erythematous, papular rash after a sunburn. A culture of a pustule revealed methicillin-sensitive Staphylococcus aureus. Our patient had complete resolution of her rash within 2 weeks of starting amoxicillin and clavulanate. This case of IEH and NEH related to both intense sun exposure and infection supports the hypothesis that NEH is a response to nonspecific stimuli and may occur in many different clinical settings.

We report a case of "infectious" neutrophilic eccrine hidradenitis who developed papules on the upper arm and trunk. Histological findings revealed vacuolar degeneration and necrosis of epithelial cells in the eccrine sweat ducts and neutrophils that had migrated through ductal epithelium to the lumen. A microabscess was also seen in the eccrine sweat gland coil. Our patient had received no chemotherapy such as cytarabine. Gram-positive cocci were present in the lesional eccrine duct indicating an infective origin of the disease. Human beta defensin-2, one of epithelial antimicrobial peptides, was present in the lesional epidermis and eccrine duct.

Neutrophilic eccrine hidradenitis is a nonspecific clinical reaction pattern classified as a neutrophilic dermatosis that typically occurs in the setting of chemotherapy for hematologic malignant disease. Neutrophilic eccrine hidradenitis more rarely has been reported in association with infectious agents, including Serratia, Enterobacter, Staphylococcus, and HIV. We describe the first case of infectious eccrine hidradenitis occurring in a patient with cutaneous Nocardia infection.

OBJECTIVE: To find out if there is an association between perineal suppurative hidradenitis and Chlamydia trachomatis infection. DESIGN: Open study. SUBJECTS: Seven consecutive patients treated for perineal suppurative hidradenitis during the past three years, and 10 control subjects who were being treated for acute cryptogenic perianal abscesses. MAIN OUTCOME MEASURE: Presence of C. trachomatis detected by direct immunofluorescent staining. RESULTS: All but one patient had serological evidence of C. trachomatis infection. All 10 control subjects failed to react to IgA antibodies to C. trachomatis, and two reacted to IgG antibodies. CONCLUSION: There may be a link between C. trachomatis infection and suppurative hidradenitis, but it is uncertain whether it is a direct cause or a predisposing factor.
